皆さんこんにちは、ともこです。今日は2023年やってよかった美容医療をご紹介したいと思います。 私は美容皮膚科に勤めているので、いろんなことをしています。もちろんスキンケアという日々のケアが大前提なんですが、そこに定期的な美容医療を加えることによって、私は今の自分を保つことができています。 私は2023年時点で45歳なので、美容の世界に入ってからもうほぼ20年近くになります。どんどん進化していって、20代はどちらかというと脱毛やニキビ、毛穴が気になることが多かったので、機械もそんなになかった時代だったこともあり、お顔の脱毛やピーリングなどをしていました。20代後半くらいからIPLが出てきて、そこから30代に向けては美白ですね。シミを作らないようにするとか、色ムラを取るというものにだんだんフォーカスしてきました。 30代になったら、それに加えてタルミの予防。あくまでも30代は予防だったんですが、30代後半になって妊娠・出産をするようになり、シミの中でも肝斑が気になるようになって、肝斑ケアを始めました。あとはタルミとの戦いが妊娠・出産を終えた後から始まりました。 40代になったら、引き続きタルミとの戦いはあるんですが、それに加えて皮膚の薄さですね。皮膚がどんどん張り感を失っていくので、張り感を出しつつ、シワなど色々なものに追いつかなくなってくる。そういったものをスキンケアプラス、クリニックの施術でケアしているわけです。 2023年も新しい機械や新しい薬剤をいろいろ試したので、その中で45歳の今、リアルな美容医療、そして本当にやってよかったと思う美容医療を今日はご紹介したいと思います。 **1つ目:ポテンザ** ポテンザは2023年に表参道スキンクリニックでも導入した、いわゆるニードルRFという機械です。RFというのはサーマクールなどに使われるもので、熱を与えて脂肪層や真皮をキュッと引き締めてくれます。サーマクールはどちらかというと筋膜、筋肉に効いていきますが、RFの機械であるサーマクールは引き上げというよりも引き締めなんですね。丸顔の私は結構引き締めたかったので、RFは30代から始めて、今は自分が勤めているクリニックで患者として初めてサーマクールを30代の時に受けて、それから定期的に受けています。 もちろん表面から当てるサーマクールもすごくいいんですが、ポテンザのようなニードルRFの素晴らしさは、針を刺してそこからRFが流れていくので、表面から当てるよりも奥にしっかりと効いていくところです。 ニードルRFの一番の効果はもちろん肝斑にも良いんですが、私が長年気になっていた毛穴ですね。この距離だと毛穴はそんなにわからないし、メイクしたり光を飛ばしたりすれば気にならないのですが、私はもともとニキビ肌でニキビ跡も多く、毛穴も開いているタイプでした。ただ、例えばフラクショナルレーザーのようなアクティブな施術をやってしまうと肝斑が悪化してしまうこともあります。人からしたら気にしないレベルかもしれないけれど、自分では気になるし、肝斑を悪化させないようにどうしようかなと思っていました。 今まではピーリングなどのケアが中心だったんですが、今回はニードルRFで肝斑を悪化させないようにしつつ、RFの針を入れていくことで引き締めとコラーゲン増殖を促し、さらにポテンザのいいところは薬剤をポンピングで圧を使って入れることができる点です。なので、スキンブースターと呼ばれる、自分のヒアルロン酸などを増やしていくような製剤も一緒に入れながら、私の場合はマックスジェムやジュベルックなどを入れています。 薬剤を入れるバージョン、針管だけを出すバージョン、リジュビネーションをするバージョンで、針を変えたり薬剤を変えたりして全部できるので、私は合計4回ほどやりました。それぞれ薬剤を入れたり熱だけ入れたりリジュビネーションしたりと色々試したんですが、一番毛穴と針管に効いてきたなと感じたので、2023年は本当にポテンザをやってよかったなと思いますし、引き続きやっていきたいと思っています。特に毛穴が気になる方、針管が欲しい方、もう一歩肌をきれいにしたい方にはかなりおすすめです。 **2つ目:スネコス** 覚えているでしょうか。1月に事務所の社長に呼ばれて「お前はオペをしただろう」というオペ疑惑の動画を撮ったと思うんですが、それは何かというと、私は目の周りの皮膚が結構薄くて青くなっていたんですね。社長はたまにしか会わないので、その時メイクをしていなかったこともあり、目の下が青いのを内出血だと思って「脱脂をやったんじゃないか」と疑いをかけられたんです。もちろん脱脂自体は全然いいと思うんですが、私はどちらかというと目の脱脂というより皮膚の薄さに悩んでいたので、そんな風に内出血に見えるくらい皮膚が薄いんだと思った時に、じゃあ何をしようかと思いました。 皮膚の厚みを出すために何をしたかというと、動画を見ていただいた方は覚えていると思うんですが、スキンブースターの一つであるスネコスの注射をしました。「目の下の疑惑」という動画で詳しくお話ししたと思うので、そちらを見ていただきたいのですが、スネコスを1週間に1回、目の下にしっかりと入れました。カクセルほどではないんですが、内出血もして、やった時にはちょっとざわつくというか、正直1週間に1回やるのはハードルが高いかなと思いました。ただ、おかげさまで目の下の薄さ、青い血管が目立たなくなって、メイクで十分隠れるくらいになったし、目の下の張り感がすごく良くなったなと思います。 別件なんですが、薬剤が余ったのでゴルゴラインにも入れてみました。動画で言っていたので覚えている方もいらっしゃると思うんですが、ゴルゴラインにもスネコスを入れたことで張り感が出て、よく見れば薄っすらとゴルゴラインが分かる程度、動画くらいの距離だと分からないくらいのふっくら感になりました。本当にやってよかったなと思います。 本当は年末にもやりたかったんですが、撮影が立て込んでいて今回はできそうもないんですけど、その代わりポテンザで目の周りをリジュビネーションしているので、今年はそこまでやらなくてもいいかなと思っています。ただ、時間ができたら絶対にもう一度スネコスをやりたいと思うくらい、本当にやってよかったと思っています。 **3つ目:ケアシス** 「ケアシスって何?」と聞いたことがない方もいらっしゃると思うんですが、これは導入の機械です。今まで導入というとイオン導入が主な機械だったと思うんですが、ケアシスはイオン導入よりもざっくり言うと、たくさんのいろんな成分を入れられるものになります。 これはエレクトロポレーションという特殊な電気を流すことによって肌の細胞をちょっと緩めるんですね。それはイオン導入と同じなんですが、イオン導入の20倍以上と言われるくらい導入ができます。あと、イオン導入では入れられなかった薬剤、トラネキサム酸なども、しっかり入れることができるんですよ。 イオン導入だとビタミンCや保湿成分が中心なんですが、肝斑などでしっかりとトラネキサム酸を入れたい時や、ポテンザの後などにもケアシスを使っていました。ポテンザの後は高炎症で赤くなったりするので、ケアシスでしっかりとトラネキサム酸を導入して鎮静させるということをやっていました。 通常、導入系の施術はちょっと冷たいんですよ。でもケアシスは温めるという機能も実はありまして、レーザーを当てた後などは冷たいものだけで鎮静させるんですが、例えばハリ感や透明感だけを出したい時には、温めた後に冷やして導入していくということもやります。ダウンタイムも全くないし艶感も出るし、ハリ感ももちろんレーザーなどよりはすぐに効果は切れてしまうんですが、艶感はめちゃめちゃ出るので、1週間に1回とか2週間に1回、明日撮影がある時などのスペシャルケアとしてもいいですし、さっき言ったようにポテンザやレーザーの後にやることでダウンタイムや赤みもかなり軽減されるんじゃないかと思います。 レーザーと組み合わせた時も良かったし、単体でやるのも良かったので、ケアシスはずっとやり続けたいですし、ダウンタイムがある施術をした時には必ずプラスアルファでやっていきたいと思っています。 **4つ目:IPL** これは今までもご紹介したものなんですが、光治療、IPLです。「今更?」と思うかもしれませんが、さっきの年表の話でもした通り、20代後半から30代前半は光治療で美白をやっていて、30代後半になって妊娠・出産を経て肝斑が気になるようになり、肝斑治療に特化していました。光治療は肝斑を悪化させることがあるので、最近はトーニングにシフトしていたんですね。 でもハリ感や透明感というものはトーニングでも出るんですが、久しぶりにIPLをやってみた時に「透明感めちゃくちゃ出るな」と思ったんです。内服やケアシスでのトラネキサム酸導入、トーニングもある程度やって、自分ではあまり目立たないくらいまで肝斑が改善していたので、久しぶりにトーニングとIPLを両方受けた時にすごくいいなと思いました。 相変わらずトーニングもしているんですが、トーニング一辺倒じゃなくて、IPLとトーニングを一緒にやるというのが今の私の中でのマイブームです。そうするとかなり透明感が出るようになってきたので、今までもやっていたことなんですが、改めて「IPLって素晴らしいな」と思って、透明感を出すため、美白ケアのためにまた取り入れていきたいと思っています。 **サーマクール** これもいつもやっていることなんですが、やっぱりサーマクールですね。ニードルRFももちろんいいんですが、引き締めという点ではサーマクールがすごく良くて、どんな施術よりも私は配布(筋膜)より断然サーマクール派です。配布はここ1年やっていないんですが、サーマクールは半年に1回ペースでやっていて、これから先も2023年だけじゃなくずっとやってよかったと思いながら続けていくんだろうなと思っています。 そのほか、ボトックスもおでこや顎に打つこともあります。これは継続しているのでまあいいかなと思うんですが、やったことといえばボトックスくらいですかね。あとは今話したものが全てかなと思います。 もちろんこれらをやれば底上げはしてくれるんですが、何度も言うようにベースは日々のスキンケアです。日々のスキンケアをしながらの美容医療だと思うので、美容医療をやるからスキンケアをおざなりにするというのは違うかなと思っています。いいバランスでやっていければいいですし、2024年はまた新しい美容機器などを試してみたいと思うので、また2024年もご紹介できたらと思います。 ということで今日は「2023年やってよかった美容医療」をご紹介しました。今日の内容が良かったと思う方はグッドボタンを押していただいたり、「これも先生やってないの?」とか「これも良くない?」というものがあればコメント欄に書いていただけると嬉しいです。 ということで、また次の動画でお会いしましょう。

Hello everyone, this is Tomoko. Today I'd like to introduce the aesthetic medical treatments that I'm really glad I did in 2023. I work at a cosmetic dermatology clinic, so I do all sorts of things. Of course, daily skincare is the essential foundation, but by adding regular aesthetic medical treatments on top of that, I'm able to maintain how I look now. As of 2023, I'm 45 years old, so it's been nearly 20 years since I entered the beauty industry. Things have kept evolving. In my 20s, I was mostly concerned about hair removal, acne, and pores, and since there weren't as many machines back then, I mainly did facial hair removal and peeling. From my late 20s, IPL came onto the scene, and heading into my 30s I gradually focused more on whitening — preventing spots and evening out skin tone. In my 30s, on top of that, I added sagging prevention. That was purely preventive in my 30s, but in my late 30s, after going through pregnancy and childbirth, melasma among the various spots became a concern, so I started melasma care. Also, the battle against sagging began after finishing pregnancy and childbirth. In my 40s, the battle against sagging continues, but on top of that, skin thinness became an issue. As skin keeps losing its firmness, I need to restore firmness while also dealing with wrinkles and other things that keep piling up. I've been addressing these with a combination of skincare plus clinic treatments. In 2023 as well, I tried out various new machines and new formulations, so today, at 45, I want to introduce the real aesthetic treatments I've tried and the ones I'm truly glad I did. **1. Potenza** Potenza is a so-called needle RF machine that was introduced at Omotesando Skin Clinic in 2023. RF is the technology used in things like Thermage — it applies heat to tighten the fat layer and dermis. Thermage tends to work more on the fascia and muscles, but as an RF machine, Thermage is more about tightening than lifting. Since I have a round face, I really wanted tightening, so I started RF treatments in my 30s. In fact, I received Thermage for the first time as a patient at the very clinic where I now work, back in my 30s, and I've been getting it regularly ever since. Of course, surface-level Thermage is great too, but what's wonderful about needle RF devices like Potenza is that because a needle is inserted and RF energy flows from there, it works more effectively deep within the skin compared to surface application. The best thing about needle RF is, of course, that it's good for melasma too, but for me, it was really effective for the pores I'd been concerned about for years. At this distance, my pores might not be that noticeable, and with makeup or lighting adjustments they're not really an issue, but I originally had acne-prone skin with a lot of acne scars and open pores. However, doing something more aggressive like a fractional laser can worsen melasma. Something others might not even notice can still bother me, so I was thinking about how to address it without worsening melasma. Until now, my main approach was peeling and similar treatments, but this time, with needle RF, I could avoid worsening melasma while the RF needles provided tightening and stimulated collagen production. What's great about Potenza is that it can also pump in serums using pressure. So I combined it with so-called "skin booster" treatments that boost your own hyaluronic acid, using products like Maxgen and Juvelook. There are versions where you inject serum, versions that just create needle channels, and rejuvenation versions — you can change the needles and serums accordingly, so it's very versatile. I did it a total of four times, trying different combinations of serum injection, heat-only application, and rejuvenation. I felt it was most effective for pores and needle marks, so I'm really glad I did Potenza in 2023, and I plan to continue. It's especially recommended for people concerned about pores, those who want smaller pore appearance, and those who want just one more step toward clearer skin. **2. Sunekos** Do you remember? Back in January, I was called into a room by my agency's president, and I made a video about the "surgery suspicion" — he thought I'd had surgery. What happened was, the skin around my eyes is quite thin and looks bluish. Since he rarely sees me, and I wasn't wearing makeup that day, he mistook the blueness under my eyes for bruising and suspected I'd had lower eyelid fat removal surgery. Of course, I think fat removal itself is totally fine, but what I was actually struggling with wasn't under-eye fat so much as skin thinness. Realizing that my skin was thin enough to look like a bruise, I thought about what I could do about it. To add thickness to the skin, what I did — as those who watched the video probably remember — was get injections of Sunekos, one of the skin boosters. I talked about this in detail in the video about the "under-eye suspicion," so please check that out, but I got Sunekos injected properly under my eyes once a week. It wasn't quite as intense as Rejuran, but there was some bruising, and honestly, doing it once a week felt like a pretty high hurdle. But thanks to that, the thinness under my eyes and the visible blue vessels became less noticeable — enough that makeup could fully cover it — and the firmness under my eyes improved a lot. As a side note, since I had leftover product, I also injected some into my tear troughs (Golgo lines). I mentioned this in the video too, so some of you might remember — putting Sunekos into the Golgo line area added firmness, so that if you looked closely you could faintly see the line, but at video-viewing distance it looked plump enough that you couldn't really tell. I'm really glad I did it. I actually wanted to do it again at the end of the year, but since I have a packed filming schedule, it doesn't look like I'll be able to this time. Instead, I've been doing rejuvenation around the eyes with Potenza, so I think that should be enough for this year. But I'd definitely like to do Sunekos again once I have time — I'm really glad I did it. **3. Careisys** Some of you might be thinking "What's Careisys? Never heard of it." This is an introduction (delivery) device. Until now, ion introduction (iontophoresis) has been the main machine for this kind of delivery, but Careisys, put simply, allows you to deliver a lot more of various ingredients compared to iontophoresis. This works through something called electroporation, a special electrical current that loosens up the skin cells a bit — similar to iontophoresis, but it delivers 20 times more or so compared to iontophoresis. Also, it can properly deliver ingredients that couldn't be delivered with iontophoresis, like tranexamic acid. With iontophoresis, it's mainly vitamin C and moisturizing ingredients, but when I want to properly deliver tranexamic acid for melasma, or after Potenza treatments, I've been using Careisys. After Potenza, there's some inflammation and redness, so I use Careisys to properly deliver tranexamic acid and calm the skin. Usually, delivery-type treatments feel a bit cold. But Careisys actually has a warming function too — after laser treatments, you calm the skin with cooling alone, but when you want to bring out firmness or radiance specifically, you warm it up first and then cool it down while delivering the ingredients. There's absolutely no downtime, and it brings out a lot of glow. The firmness effect fades faster than with lasers, of course, but the glow effect is really amazing, so it's great as a special care treatment once a week or every two weeks, or when you have filming the next day. Plus, as I mentioned, combining it with treatments like Potenza or laser afterward seems to significantly reduce downtime and redness. It was great combined with laser, and great on its own too, so I want to keep doing Careisys, and whenever I do a treatment with downtime, I definitely want to add it as an extra step. **4. IPL** This is something I've introduced before, but it's phototherapy — IPL. You might think "why now?", but as I mentioned in the timeline earlier, from my late 20s to early 30s I did phototherapy for whitening, then in my late 30s, after pregnancy and childbirth, melasma became a concern, so I focused specifically on melasma treatment. Since phototherapy can worsen melasma, I had recently shifted toward toning treatments. But while firmness and radiance can also come from toning, when I tried IPL again after a long time, I thought, "Wow, this brings out so much radiance!" With oral medication, tranexamic acid delivery via Careisys, and a fair amount of toning, my melasma had improved to the point where it's barely noticeable even to myself. So when I did both toning and IPL together after a long break, I thought it was really great. I'm still doing toning as always, but rather than relying on toning alone, doing IPL together with toning has become my current personal trend. Doing so brings out noticeably more radiance, so even though I'd done this before, I was reminded once again that IPL is wonderful, and I want to incorporate it again for bringing out radiance and for whitening care. **Thermage** This is also something I always do, but it's Thermage. Needle RF is great, of course, but in terms of tightening, Thermage is fantastic — more than anything else, I'm definitely more of a Thermage person than a HIFU (fascia-lifting) person. I haven't done HIFU in the past year, but I do Thermage about once every six months, and I think I'll keep doing it going forward, not just in 2023, feeling glad every time that I did it. Other than that, I also get Botox in my forehead and chin sometimes. Since this is ongoing maintenance, I feel like that's fine as is — I guess Botox is the only other thing. I think everything else I've talked about covers it all. Of course, doing these treatments provides an extra boost, but as I keep saying, the foundation is daily skincare. Aesthetic medical treatments should be added on top of daily skincare, so I don't think it's right to neglect skincare just because you're doing medical aesthetic treatments. I hope to keep a good balance, and in 2024 I'd like to try new aesthetic devices as well, so I look forward to sharing more with you again in 2024.
