芸能人の顔が大きくなる理由を解説します。

皆さんこんにちは、内科・皮膚科医の友利新です。今日はサムネイルが気になったかもしれませんが、なぜ芸能人の方で顔が大きくなっていく人がいるのかというお話をしたいと思います。これをずっとしようかなと思っていたんですが、いろんなところからクレームが来そうだなということと、私自身もよくコメント欄で「先生、ヒアルロン酸入れてますか」とか「先生、顔が変わりましたね」と言われるんですよ。それで私が「ヒアルロン酸は入れていません」とお答えしたら、「じゃあ脂肪を注入しているんですか」と言われたりして。やっぱり私の顔がパンパンだから、そう見えてしまうのかなと思いました。最近インターネットでも、芸能人さんが昔は可愛かったのに顔が変わってきて、少し大きくなってきているという話もありますよね。あれはなぜそうなるのかということを、美容に関わる医師として、自分も注意しなければいけないと思いながら、皆さんにお話しします。最後までご覧ください。
まず、美容医療の一つの分野として「注入療法」というものがあります。ヒアルロン酸、コラーゲン、脂肪注入、そしてボトックスなどが含まれます。注入治療の主な対象はシワで、ボリュームを出したり、ボトックスで表情筋の動きを抑えたりすることで治療します。特にヒアルロン酸は、法令線などの深いシワに入れると一瞬でシワがなくなるため、画期的な治療として広まりました。
その後、ヒアルロン酸は進化し、分子量の大きいもの・小さいもの、柔らかいもの・硬いものなど様々な種類が登場しました。硬いヒアルロン酸はボリュームを出すだけでなく、形を作って定着させることができます。例えば顎を尖らせたり、鼻筋を高くしたりするために使われます。
そして、ブラジルの医師が提唱した考え方として、顔のたるみや萎縮に対して硬いヒアルロン酸を入れることで「引き上げる」という概念が広まりました。ボリュームを出すのではなく、骨や脂肪組織の萎縮を補い、つなぎ止めるために注入するという考え方です。特に平面的な顔立ちの日本人には一定の需要があり、チークライン(頬骨周辺)にヒアルロン酸を入れることで、たるみ予防・若返り効果を狙う治療が流行しました。
ここからは私の考察です。ヒアルロン酸注入はとても有効な治療法ですが、「何かを入れる」という行為には注意が必要だと思っています。体重が増えると顔が大きくなるのは脂肪が増えるだけでなく、皮膚も引っ張られて伸びるからです。同様に、ヒアルロン酸を継続的に入れていくと、皮膚が少しずつ伸びていくのではないかと私は考えています。伸びた皮膚はまたたるむ。たるんだらまた入れたくなる。そしてまた皮膚が伸びる——このサイクルを繰り返すことになります。特に美意識の高い方は「チークラインを高く保ちたい」という思いから入れ続けてしまう傾向があります。
私はこれを「ピタッと理論」と呼んでいます。ピッタリしたTシャツも、時間とともに劣化してヨレてきますよね。新しいシャツに着替えればいつでもピッタリを保てますが、皮膚は着替えられません。伸びた皮膚にさらに何かを入れると、また劣化して伸びていく。ピッタリを保とうとするなら、皮膚を手術で切るしかない。つまり、物を入れ続けると皮膚は伸び続け、入れる量もどんどん増えていく——これが顔が大きくなっていく一因ではないかと私は考えています。
私がヒアルロン酸をお勧めするとしたら、法令線、こめかみ(加齢でへこんできた部分)、おでこ、鼻、顎などです。頬骨周辺への注入を希望される患者さんには、上記のような可能性もお伝えした上で施術するようにしています。たるみ治療を希望される方には、サーマクールやHIFU(ハイフ)などをお勧めすることが多いです。
唇についても「ヒアルロン酸を入れていますか」とよく聞かれますが、私はもともとたらこ唇なので入れていません。唇は加齢とともにしぼんでくるため、ヒアルロン酸注入は有効ですが、一度膨らませるとしぼんだ時にもっと入れたくなるという心理が働きやすく、過剰になりがちです。リップブランパーなどのケア用品や、リップライナーで輪郭を取るだけでも唇を美しく見せることができますので、まずはそういった方法を試してみてください。
たるまないためには、日焼け止めをしっかり塗ることが最も大切です。たるみを予防する生活習慣や化粧品も活用しながら、日々を過ごしていただければと思います。

Hello everyone, I'm Arata Tomori, an internal medicine and dermatologist. You might have been curious about today's thumbnail — I'd like to talk about why some celebrities' faces appear to get larger over time. I've been wanting to address this topic for a while, but I was worried it might draw complaints. On top of that, I personally get comments like 'Doctor, have you had hyaluronic acid injections?' or 'Doctor, your face looks different.' When I replied that I hadn't had any hyaluronic acid, someone then asked if I'd had fat injections instead. I suppose my naturally full face makes it look that way. You've probably also seen online comments about celebrities who used to look cute but whose faces have gradually changed and grown larger. As a doctor involved in cosmetic medicine — and someone who needs to be mindful of this myself — I'd like to explain why this happens. Please watch until the end.
First, one branch of cosmetic medicine is called 'injection therapy,' which includes hyaluronic acid, collagen, fat injections, and Botox. The primary targets of these treatments are wrinkles — either by adding volume to fill them in, or by using Botox to suppress facial muscle movement. Hyaluronic acid in particular became a groundbreaking treatment because injecting it into deep lines like nasolabial folds could instantly erase them by plumping up the area.
Over time, hyaluronic acid technology evolved to include a wide range of products — high and low molecular weight, soft and firm formulations. Firmer hyaluronic acid can not only add volume but actually sculpt and maintain a shape. For example, it can be used to sharpen the chin or raise the nose bridge.
A concept popularized by a Brazilian doctor involved using firm hyaluronic acid not just for volume, but to 'lift' areas affected by age-related bone and tissue atrophy. The idea was to inject it strategically to support and anchor sagging tissue rather than simply fill it. This became particularly popular in Japan, where flatter facial features made cheek augmentation via hyaluronic acid a sought-after treatment to lift the cheekline and create a more youthful appearance.
Now, here is my own analysis. While hyaluronic acid injections are genuinely effective, I have concerns about the long-term implications of repeated injections. When we gain weight, our faces don't just get bigger because of added fat — the skin stretches too. I believe the same principle applies to repeated filler injections: as you keep adding volume, the skin gradually stretches. Stretched skin sags again, which makes you want more filler. More filler stretches the skin further. This cycle repeats. People with high aesthetic standards may keep injecting to maintain a lifted cheekline, unknowingly perpetuating this cycle.
I call this my 'Fitted T-Shirt Theory.' A brand-new fitted T-shirt looks great, but over time it stretches and loses its shape. You can simply buy a new shirt — but you can't replace your skin. Once the skin stretches from repeated filler injections, the only way to truly restore a tight appearance is through surgical skin removal. In other words, continuously injecting filler causes the skin to keep expanding, requiring ever-increasing amounts — and this, I believe, is one reason why some people's faces grow larger over time.
If I do recommend hyaluronic acid, I tend to suggest it for the nasolabial folds, temples (which hollow with age), forehead, nose, and chin. For patients who want cheek injections, I explain these potential consequences before proceeding. For those seeking lifting treatments, I more often recommend Thermage or HIFU (High-Intensity Focused Ultrasound).
Regarding lips — I'm frequently asked if I've had lip fillers, but my naturally full lips are just how I was born. Lips do thin with age, and hyaluronic acid injections can help. However, once lips are enhanced, there's a psychological tendency to want more volume as they settle, which can lead to an overdone appearance. I encourage patients to try lip plumping skincare products or simply use a lip liner to define the contour before opting for injections.
Above all, the single most effective way to prevent sagging is consistent use of sunscreen. Alongside good lifestyle habits and anti-aging skincare, daily sun protection can make a significant difference.
