【ニキビ爆速で撃退】皮膚科医友利新の常識が変わった鎮静ケア！このニキビパッチ最強です

みなさんこんにちは。友利新です。今日は結構よく聞かれるニキビパッチについてお話ししようと思います。ドラッグストアとかでもよく見ますよね、ニキビパッチって。貼っておくと早く治りますよとか跡になりませんよみたいな感じで売っているんですね。でも実は私たち皮膚科医からするとやめといた方がいいですっていうふうなことを言っていたし、今でもそう思っている先生の方が大半だと思うんですよね。基本的にニキビっていうのは密閉してはいけないっていうのが私たち皮膚科医の常識だったんですけど、それを覆すようなことが出てきたので、今日はニキビパッチは本当に正しいのか、選ぶ時にどういうふうにしたらいいのか詳しくお話ししたいと思いますので、ぜひ最後までご覧ください。 今までニキビというのは閉塞してはいけないって言っていた一番の理由っていうのが、ニキビの元になるアクネ菌っていうのは空気が嫌いな存在なんですね。毛穴っていうのは奥の方に穴が開いているような状態じゃないですか。そこに皮脂が溜まってニキビ菌が寄ってきて増殖をしてしまって炎症を起こしてしまったっていうのがニキビなんですね。もちろんアクネ菌っていうのは皮脂が好きだから毛穴の奥に行くっていうところもあるんですが、もう一つは空気があんまり好きじゃない嫌気性菌っていう、毛穴の中で増殖しやすいっていうところもあるんですよ。なので空気が嫌いな嫌気性菌を密閉してしまったら増殖しちゃうじゃないですか。だからニキビというのは閉塞してはいけないっていうのが常識だったんですね。 でもいろんなブランドさんからニキビパッチって出てるじゃないですか。聞かれたら私は使わないし、今のお話をしてどうしても使いたいんだったら気をつけてくださいとか、膿んでしまったものにはやらない方がいいんじゃないですかっていうような話をしてたんですね。ただあまりにもいろんなブランドさんが出しているので、なんでなのかなって思ったときに、大体のブランドさんがハイドロコロイドっていうキズパワーパッドに使われているものを使っているところが多かったんですね。これってグレーなのかなって思っている部分があったんですけれども、ハイドロコロイドっていうものを使って密閉するニキビパッチも結構出てきていて、これはと思ったんですよ。 その前にキズパワーパッドってどうやって皆さん傷を治していると思いますか。今までは傷ができたらかさぶたを作って治しましょうっていう、消毒して傷を乾かしてみたいな感じだったんですけど、キズパワーパッドっていうのは湿潤療法っていうもので治しているんですね。傷口から出ている浸出液っていう私たちの体液の中には傷を治す必要なものがあると。ただあまりにもいっぱい出すぎてもいけないから、ハイドロコロイドっていうもので吸収して適度な潤いを保つことによって傷を綺麗に跡ができないように治すっていうような療法のことを湿潤療法というふうに言います。 ただ湿潤療法にも一つ弱点があって、化膿して膿を持っているような傷には貼っちゃいけないっていうのがあるんですよ。菌をキズパワーパッドで密閉してしまうと菌が増殖してしまうので炎症を起こしてしまうっていうところになるんですね。でも最近このキズパワーパッドでおなじみのバンドエイドからニキビパッチが出てたんですよ。これ見た時に、言い方はあれですけど、気でもおかしくなったのかなと思ったんですよ。さっき言ったみたいに湿潤療法はやっちゃダメなものを元祖がやってるってどうしちゃったのかしらっていうふうに思ったんですよ。 いろいろ調べたんですよ。衝撃の事実が分かって。まずなんで顔用を作ったかっていうのは、きっかけTikTokらしいんですよ。アメリカでTikTokerさんが普通のバンドエイドのキズパワーパッドをニキビに貼ったらすごく早く治ったっていうのをTikTokで言ったらしいんですね。それが全世界的にバズって研究者の人はやばいと思ったんだと思うんですよ。ちゃんとなんで治るのかをデータを取ったらしいんですね。クリニカルテストって言って、パッチを貼った時と貼らない時っていうのがどういうふうにしたかっていうのもデータを出してるんですよ。多分これで悪くなるから使わないでくださいねってやる予定だったんじゃないかなと私は思っているんですけど、なんとキズパワーパッドを貼った方が早く治ったっていうデータが出ちゃったらしいんですよ。特に潰れたニキビに対して通常よりも早く治ったっていうのが確認されたらしいんですね。 さらに驚きだったのは、潰れていないニキビも同じように早く治ったっていうようなデータなんですよ。多分これは湿潤療法みたいなことじゃなくて、どうしてもニキビがあると触っちゃうので、それを防ぐことができたからなんじゃないかみたいに考察はされてるんですが、どちらにしても潰れたニキビも密閉されているようなニキビもキズパワーパッドを貼った方が早く治ったっていうクリニカルデータが出たので、アメリカでは一早くハイドロコロイドを使ったニキビパッチが発売されてるんですよね。アメリカではニュートロジーナっていう同じグループ会社のものがあったりして、アメリカでは普通に売ってるんですね。 でもあれって思ったのは、これよく見るとニキビとは書いてないんですよこれ。顔用パッチって書いてあるんですよね。探したらいろんなブランドさんから出てたんですよ。マツキヨさんとか杉薬局とかいろんなところで買ったんですけど、キズを早く治すパッチみたいな感じで。でもよく見ると「動物や人に噛まれた傷、砂など異物が入った傷、かさぶたができている傷、ニキビ、しし虫刺されには使わないでください」って書いてあるわけ。でもこっちには書いてないわけ。なんなんだろうなっていうふうに思ったら、日本らしい法律が見えてきたんですね。 キズパワーパッド的なものっていうのは第二種の医療機器として発売されてるんですよ。それが承認されたのが多分2000年代。その時には「傷があるところには使っちゃいけないからニキビにもダメです」っていうふうなことが書いてあるんですよ。でもこれは書いてないんですよ。なんでだろうと思ったら、化粧品として売ってるらしいんですね。ぶっちゃけて言うと一緒ですよ。もちろんキズパワーパッドの方がハイドロコロイドとしては優秀だと思うので一緒なんですけれども、法律が第二種の医療機器にしているとニキビには禁忌ですって書かなきゃいけないんだと思うんですよ。なので第二種を取ってないからニキビとは書けないけれども、ニキビにダメとも書かなくていいっていうところなんですよね。 何が言いたいかっていうと、少なくともこのバンドエイドさんのやつは早く治るんじゃないかなとは思います。ニキビにはダメですとは書いてあるんだけれども、一応こういうニキビパッチは意外にいいんじゃないかなと思います。ただハイドロコロイドにもランクがあるから、このハイドロコロイドを作っているところがクリニカルデータを出しているかどうかは正直わかんない。でもバンドエイドをやっているところはアメリカだけれどもクリニカルデータは出しているので、日本でもちょっとね、出してもらいたいなとは正直思ってるんですけれども、バンドエイドさんが安心なんじゃないかなというふうには思います。 ニキビパッチ、私いろいろ見たら、ただのテープみたいなものでハイドロコロイドじゃないのもいくつかあったんですよ。これは湿潤療法にもならないし、まずハイドロコロイドかどうかっていうのもちゃんと見ていただきたいなと思います。あともう一つは、ハイドロコロイドだったとしてもいろんな成分が入っているものがあるんですよ。一見その方が早く抑えてくれそうなんですけれども、そもそもニキビって炎症起こしている時に密閉して肌に触れさせておくっていうことが本当にいいかどうかっていうのは私にはちょっとわかんないなと思うんですよね。シートマスクとかみたいに本当に3分とか5分とかではなくて、ずっと貼ってるものじゃないですか。怖いなと思うので、成分入りのパッチっていうのはあんまりおすすめしないかなと思います。 あとはもう大きさだったりとか、剥がれやすかったらそれも刺激になるじゃないですか。何回も貼らなくちゃいけなくなるので、剥がれにくさっていうところも重視した方がいいかなと思います。あともう一つ、これ一番言っておきたいんですけれども、化膿して感染してじゅくじゅくして痛いようなニキビには絶対貼らない方がいいかなと思います。潰れた後のものだったりとか、ぷくっとしているような状態のものにするのはいいんじゃないかなとは思うので、気をつけて使っていただければかなと思います。 ニキビはね、密閉しちゃダメだし湿潤療法は禁止っていうふうに思ってたんですけど、海外ではねクリニカルデータを取ろうっていうのはすごく面白いなと思いましたし、日本って本当に法律がガチガチだなっていうことと、やっぱりできれば日本でもクリニカルデータを取って堂々とニキビにいいですよとか書ける日が来ればいいなというふうに思います。何でもかんでも貼ればいいというわけではないので、さっき言ったポイントを抑えて自分に合ったニキビパッチ見つけていただければいいかなと思います。私もねニキビに悩んだことがすごくあるので、しっかりとした製品を選んでニキビができてもすぐに治って、ニキビ跡ができないっていうことを皆さんがやっていただければかなと思いました。 はい、ということでいかがだったでしょうか。今日の動画良かったよと思う方はグッドボタン押してもらったりとか、チャンネル登録していただけると嬉しいです。ではまた次の動画でお会いしましょう。さようなら。

Hello everyone, this is Arata Tomori. Today I'd like to talk about acne patches, which I get asked about quite often. You often see acne patches at drugstores, right? They're sold with claims like "apply this and it heals faster" or "it won't leave a scar." But actually, we dermatologists used to say you should avoid them, and I think most doctors still think that way today. Basically, the common knowledge among us dermatologists was that acne should never be sealed up. However, something has come along that overturns that idea, so today I want to talk in detail about whether acne patches are really okay, and how to choose one if you do use them. Please watch until the end. The main reason we used to say acne shouldn't be occluded is that the acne-causing bacteria, C. acnes, dislikes oxygen. Pores are like holes that go deep into the skin, right? Sebum accumulates there, acne bacteria gather and proliferate, causing inflammation—that's acne. Of course, C. acnes goes deep into the pore because it likes sebum, but another reason is that it's an anaerobic bacterium that doesn't like oxygen much, so it tends to multiply easily inside the pore. So if you seal up an anaerobic bacterium that hates oxygen, it will multiply, right? That's why the common wisdom was that acne should never be occluded. But so many brands are releasing acne patches, aren't they? If asked, I personally don't use them, and if someone really wants to use one despite what I just explained, I'd tell them to be careful, or that it's probably better not to use them on pimples that have already become infected with pus. But since so many brands were releasing these, I wondered why, and found that most brands were using hydrocolloid, the same material used in things like Band-Aid's advanced healing pads. I had a feeling this might be a gray area, but hydrocolloid-based occlusive acne patches kept appearing, and I thought, "this is interesting." Before that, do you know how products like hydrocolloid wound pads actually heal wounds? Traditionally, we were taught to let a wound form a scab—disinfect it and let it dry. But hydrocolloid wound pads use something called moist wound healing. The exudate that comes out of a wound contains substances your body needs to heal. However, since too much of it isn't good either, the hydrocolloid absorbs it and maintains an appropriate level of moisture, allowing the wound to heal cleanly without leaving a scar. This is called moist wound healing therapy. However, moist wound healing does have one weakness: you shouldn't use it on wounds that have become infected and are producing pus. If you seal in bacteria with a hydrocolloid pad, the bacteria will multiply and cause inflammation. But recently, Band-Aid, famous for these wound-healing pads, released an acne patch. When I first saw this, to put it bluntly, I thought, "Have they lost their minds?" As I mentioned, moist wound healing therapy is something you're not supposed to use on infected wounds, so I wondered what was going on with the very company that pioneered it. I looked into it further, and found some shocking facts. First, the reason they created a facial-use patch apparently started with TikTok. In the U.S., a TikToker apparently posted that applying a regular Band-Aid wound-healing pad to a pimple made it heal much faster. That went viral worldwide, and I imagine the researchers thought, "this is a problem," so they properly collected data on why it was healing. They ran clinical tests, comparing what happened with and without the patch. I think they probably intended to prove it makes things worse so people would stop doing it, but the data actually showed that pimples healed faster when the pad was applied. In particular, they confirmed that popped pimples healed faster than usual. What was even more surprising was that unpopped pimples also healed faster in the same way. This probably isn't due to moist wound healing itself, but rather because the patch prevented people from touching their pimples, which is one theory. Either way, since the clinical data showed that both popped and sealed pimples healed faster with the pad, hydrocolloid acne patches were released early on in the U.S. In America, there's also a similar product from Neutrogena, which is part of the same corporate group, and it's sold there as a normal product. But here's the odd thing—if you look closely, these patches don't actually say "acne" on them. They say "facial patch." When I searched, I found various brands selling similar things—I bought some at Matsukiyo and Sugi Pharmacy, among other places—all marketed as patches to heal wounds quickly. But if you read the fine print, it says things like "do not use on animal or human bites, wounds with foreign objects like sand, wounds with scabs, acne, or insect bites." But that warning isn't on the other one. I wondered why, and it turned out to be due to Japan's unique regulations. Products like hydrocolloid wound-healing pads are sold as Class II medical devices in Japan. That classification was probably approved sometime in the 2000s, and at that time, it was written that since you shouldn't use it on wounds, you also shouldn't use it on acne. But this new patch doesn't say that. I wondered why, and found that it's being sold as a cosmetic product instead. Honestly, they're essentially the same thing—though of course the original wound pad is probably a superior hydrocolloid—but because the law requires Class II medical devices to explicitly state that acne use is contraindicated, and since this new patch hasn't obtained that classification, it can't say "for acne," but it also doesn't have to say "not for acne." What I want to say is that, at the very least, I think this Band-Aid product probably does heal things faster. It says not to use it for acne, but I think these kinds of acne patches might actually be surprisingly good. That said, hydrocolloid quality varies, so honestly, I don't know for certain whether every hydrocolloid manufacturer has run clinical tests. But since the company behind the original wound pad brand in the U.S. has published clinical data, I honestly wish Japanese companies would do the same. Still, I do feel that products from that brand are relatively trustworthy. When I looked at various acne patches, I found some that were just plain tape and not actually hydrocolloid at all. Those won't provide moist healing benefits, so please make sure to check whether a product is actually hydrocolloid-based. Another point is that even hydrocolloid patches sometimes contain various added ingredients. At first glance, that might seem like it would help calm things down faster, but honestly, I'm not sure whether it's really a good idea to seal in extra ingredients against inflamed skin for a long time. Unlike sheet masks, which you only leave on for three to five minutes, these patches stay on your skin continuously. That feels risky to me, so I wouldn't really recommend patches loaded with extra ingredients. Also, pay attention to size, and whether it peels off easily—if it does, that itself becomes an irritant, since you'd have to keep reapplying it. So durability against peeling is another important factor. And here's the most important point I want to emphasize: please absolutely avoid using these patches on pimples that are infected, oozing, and painful. It's fine to use them on pimples that have already popped or on ones that are just raised bumps, so please use them carefully with that in mind. I used to firmly believe that acne should never be occluded and that moist wound therapy was strictly forbidden for it, but I find it fascinating that clinical data was actually gathered overseas to test this. It also made me realize just how rigid Japan's regulations can be, and I do hope that one day Japan will also gather clinical data so that products can openly state that they're good for acne. Not just any patch will do, so please keep the points I mentioned in mind and find an acne patch that suits you. I've struggled with acne quite a lot myself, so I hope everyone can choose a solid, well-tested product so that even when a pimple appears, it heals quickly and doesn't leave a scar.
