皆さんこんにちは、内科・皮膚科医の友利新です。今日は、最近手を洗う機会が多くなって手荒れをしてしまうので、ハンドクリームを教えてくださいというコメントが多かったので、症状別でおすすめするハンドクリームをご紹介したいと思います。 実は手というのは汗はかきやすいのですが、肌を守る皮脂と呼ばれる脂が出る皮脂腺は、手のひらにはないんですね。なので、手を洗えば洗うほど皮脂がどんどん落ちていってしまって、乾燥に傾いてしまいます。乾燥してしまうと外的刺激で痒みが出てしまったり、カサカサしてしまったり、最終的にはパックリ割れてしまうということも起こってきます。 なので、手を洗ったらハンドクリームを塗るという習慣をつけてもらいたいんですね。そうすることでエイジングケアや、さらなる手をきれいに見せることもできます。今日は症状が出てしまっている方に、ドラッグストアや通販でも買える医薬部外品や薬剤をご紹介しようと思いますので、最後までご覧ください。 【1. ワセリン】 まず、軽い症状として手が乾燥しているだけで、特に切れたり痒みがないという方に一番おすすめで、刺激が少ないのはワセリンになります。ワセリンは皮膚に膜を張ってくれて、水分が外に出ていくのを防いでくれるので、これ以上乾燥をさせないためにはバッチリです。私たち皮膚科でもパッチテストをするときに使ったりする、ほとんどアレルギーがないものなので、敏感肌の方にも安心して使っていただけます。ただ、やはりベタつきがあるので、塗った後に何かを触るときには少し注意が必要です。ワセリンは安心して全身に使えるので、一本持っておいてもいいかと思います。 【2. 尿素入りハンドクリーム】 乾燥もあるけどカサカサしている、なんとなく皮膚が熱くなっているという方におすすめなのが、尿素入りのハンドクリームです。尿素は角質を柔らかくして水分を入れ込む働きがあるので、ゴワゴワしていたりカサついていたりする方におすすめです。ただし、尿素が高濃度の場合は皮膚に刺激になることがあり、傷があると刺激を与えてしまいます。カサカサ感はあるけれど傷はないというときに使うようにしてください。尿素は10%程度がベストです。このハンドクリームには尿素以外にもヒアルロン酸も入っており、保湿効果も期待できます。 【3. セラミド入りハンドクリーム（キュレル）】 次に、カサカサ感よりも一歩進んで、少し手に炎症があるという方におすすめなのが、セラミド入りのキュレルのハンドクリームです。セラミドは皮膚のバリア機能を司る成分で、乾燥を改善してくれます。さらに、消炎作用のある有効成分であるアラントインが入っているので、ひびや赤切れを防いでくれます。少し進んだ乾燥肌の方にはとてもおすすめです。 【4. ヘパリン類似物質入りクリーム（ウルンラップ ヘパインクリーム）】 乾燥もあるけど少し痒みもあるという方におすすめなのが、ヘパリン類似物質が入ったウルンラップ ヘパインクリームです。ヘパリン類似物質は血行を促進してくれて、皮膚の水分量をアップさせてくれるので乾燥肌にとても向いています。さらに、グリチルリチン酸ジカリウムという抗炎症作用のある成分も入っているので、乾燥と軽い痒みがある方におすすめです。 ここまでの3製品（尿素・セラミド・ヘパリン類似物質配合）はすべて医薬部外品です。乾燥に加えて症状がある方はこちらを使うようにしてください。ただし、お薬ではないので、傷がある場合は使用しないほうがいいです。 これ以上の症状がある場合は、本来であれば皮膚科を受診していただきたいのですが、湿疹と呼ばれるような皮膚の病気の症状になってきますので、皮膚科で適切な薬をもらうのが理想です。ただ、今回はドラッグストアや通販で買える医薬品もご紹介します。 【5-A. ヘパソフト（医薬品）】 かゆみが主な症状でかゆみを止めたいという方には、ヘパソフトがおすすめです。この医薬品には、抗ヒスタミン剤であるジフェンヒドラミンと、鎮痒剤であるクロタミトンという2つのかゆみ止め成分が入っています。さらに保湿成分としてヘパリン類似物質も含まれているので、乾燥＋かゆみを止めたいという方にはこちらがおすすめです。 【5-B. メディクイック（医薬品）】 かゆみだけでなく、赤みや亀裂がひどいという方にはメディクイックがおすすめです。メディクイックには、皮膚科でも使われているステロイドの一種であるプレドニゾロン吉草酸エステル酢酸エステルが配合されており、炎症を速やかに抑える効果が高いです。ただし、ステロイドには強さの段階があり、本来は医師が診断してその方に合った強さのステロイドを処方するものです。このメディクイックにはミディアムクラスのステロイドが入っており、場合によってはより強いものが必要なこともあります。この薬剤を使っても1ヶ月以上症状が改善しない場合は、必ず皮膚科を受診するようにしてください。 いかがでしたか？最近は手を洗う機会が多いため、乾燥やそれ以上の症状が出てしまう方も多いと思います。乾燥がある場合は医薬部外品で対処し、それ以上の症状があれば医薬品を使って様子を見て、症状が進んでしまったら必ず皮膚科を受診するようにしてください。 このチャンネルでは美容や健康について発信しておりますので、よかったらチャンネル登録してみてください。では、また次の動画でお会いしましょう。さようなら。

So I'd like to introduce hand creams organized by symptom. The hands tend to sweat easily, but the palms actually lack sebaceous glands — the glands that produce the skin's protective sebum (oil). This means that the more you wash your hands, the more sebum is stripped away, leaving the skin prone to dryness. Dryness can then lead to itching from external irritants, flaking, and eventually painful cracking. That's why I'd like everyone to develop the habit of applying hand cream after washing their hands. This not only helps with anti-aging but also keeps your hands looking beautiful. Today, I'll be introducing quasi-drug and pharmaceutical products available at drugstores and online for those who are already experiencing symptoms, so please watch until the end. 【1. Vaseline (Petrolatum)】 For those with mild symptoms — just dryness, without any cracking or itching — the most recommended and least irritating option is Vaseline (petrolatum). Vaseline forms a protective film over the skin, preventing moisture from escaping, making it perfect for stopping further dryness. We dermatologists also use it in patch testing, and it causes almost no allergic reactions, so even those with sensitive skin can use it with confidence. The downside is that it can feel greasy, so be careful about touching things after applying it. Since Vaseline is safe for use all over the body, it's worth keeping a tube on hand. 【2. Urea Hand Cream】 For those who have dryness and rough, scaly skin, or a feeling of warmth in the skin, I recommend a hand cream containing urea. Urea softens the stratum corneum (outer layer of skin) and helps draw moisture in, making it great for rough or flaky skin. However, high concentrations of urea can be irritating to the skin, and it should not be used on broken skin, as it will cause stinging. Use this only when the skin is rough but unbroken. A urea concentration of around 10% is ideal. The cream I'm introducing also contains hyaluronic acid for added moisturization. 【3. Ceramide Hand Cream (Curel)】 For those who are a step beyond simple dryness and are experiencing some inflammation in the hands, I recommend Curel's ceramide hand cream. Ceramide is the key component that governs the skin's barrier function and helps improve dryness. This product also contains allantoin, an active ingredient with anti-inflammatory properties, which helps prevent chapping and cracked skin. It's highly recommended for those with more advanced dry skin. 【4. Heparin-like Substance Cream (Urunrap Heparin Cream)】 For those who have dryness accompanied by some itching, I recommend the Urunrap Heparin Cream, which contains a heparin-like substance. This substance promotes blood circulation, which significantly increases the skin's moisture levels, making it very effective for dry skin. Additionally, it contains dipotassium glycyrrhizate, an ingredient with anti-inflammatory properties, making it ideal for those with both dryness and mild itching. The three products above (urea, ceramide, and heparin-like substance) are all quasi-drugs (医薬部外品). If you have dryness plus additional symptoms, please use one of these. However, since they are not prescription medications, avoid using them on broken or wounded skin. For symptoms more severe than what these can address, I would ideally recommend seeing a dermatologist, as these symptoms may indicate a skin condition such as eczema (湿疹). However, I'll also introduce some over-the-counter pharmaceutical products available at drugstores. 【5-A. Hepasoft (OTC Pharmaceutical)】 For those whose primary symptom is intense itching, Hepasoft is a great option. This pharmaceutical product contains two anti-itch ingredients: diphenhydramine (an antihistamine) and crotamiton (an antipruritic agent). It also contains a heparin-like substance for moisturization. For those dealing with both dryness and itching, this product is highly recommended. 【5-B. Mediquick (OTC Pharmaceutical)】 For those suffering not only from itching but also from significant redness and cracking, I recommend Mediquick. It contains prednisolone valerate acetate, a type of steroid also used in dermatology, and is highly effective at quickly reducing inflammation. However, steroids come in various strengths, and ideally a doctor should evaluate and prescribe the appropriate strength for each individual. The steroid in Mediquick is classified as medium strength (mid-potency), and some patients may require a stronger formulation. If your symptoms do not improve after using this product for more than one month, please be sure to visit a dermatologist. How was that? With more frequent handwashing these days, many people are experiencing dryness and even more advanced symptoms. For dryness, try a quasi-drug product; for more severe symptoms, use an OTC pharmaceutical and monitor your condition; and if symptoms progress significantly, please make sure to see a dermatologist.
