みなさんこんにちは。友利新です。今日はですね今話題になっている薬局で買える抗肥満薬アライについてお話ししたいと思います。結構ね、あの新聞広告だったりとかYouTubeとかネットでも話題になっているこの大正製薬さんから発売になる抗肥満薬アライなんですけれども、飲むだけで痩せるっていう画期的なお薬が発売になったって喜んでいる方、ちょっと待っていただきたいなと思います。というのもですね、アライというものなんですが、薬剤の正式名称はオルリスタッドという成分になります。オルリスタッドっていう成分が60ミリ入っているものがアライというこの抗肥満薬なんですね。実はアメリカでは1998年からアライというものは薬局で買える抗肥満薬として発売になっています。ちなみにですね、私はこのオルリスタッドをクリニックでも使ったことがあるし、自分自身も処方したことがあるのですごく馴染みがあるお薬なんですが、オルリスタッドが60ミリ入っているアライっていうのは薬局で買えるんですが、これが120ミリ入っているお薬がですね、処方箋がいるゼニカルというお薬になります。ゼニカルっていうのを聞いたことあるっていう方もいらっしゃると思うんですが、10年ぐらい前ですかね、に、すごく飲む肥満薬ということで美容外科の中のダイエット外来とかで結構使ったりっていうこともありました。なので私も患者さんに出して痩せた患者さんもいらっしゃるし、実はそうでもない患者さんもいたりとかっていうことがあります。なのでしっかりと適応がある方が飲めばそれなりの効果は得られるのがアライなんじゃないかなと思います。 キジョがですね、これは油をカットするというものになります。なんかそういうとサプリとかでもそういうものはありますよっていうふうに言われるんですが、正直サプリとか、よくね私の顔を使ってね何か痩せるお茶を発売しているところもあるんですけど、そういうものっていうのは99.9%私は効果がないと思っているので、そういうものとは全然違って、このオルリスタッド自体は薬理作用もちゃんと分かっていて、体の中で油を吸収しないようにきちっと働いてくれます。 普通脂肪というのはですね、体に入ってくるとですね、脂肪を分解するリパーゼという酵素が働いて、脂肪酸というものとグリセロールというものに分解して、それで消化管から吸収されるということになります。今回のこのオルリスタッド・アライはですね、脂肪が入ってきた時に、脂肪分解酵素であるリパーゼが体から出ないということになるので、脂肪が多いものを食べても体に吸収されず体の外に出ていくっていうのがこの薬理作用になっています。なので摂取した脂肪の大体20から30%ぐらいが体の外に出るということなので、結構カットしてくれるんですよね。 なんとなくお薬っていうと副作用ですね、がすごく大きいというイメージだと思うんですが、例えば肝臓の障害だったりとか、作用する消化器とかに副作用があるかというと、意外とそれは少ないです。このオルリスタッド自体もあんまり体に吸収されないんですね。なので臓器障害みたいな副作用は少ないというふうに言われています。とはいっても、退社するときに肝臓に負担をかかるということはゼロじゃないので、肝機能障害というのは一部出るんですけれども、その他の臓器障害というのは少ないというふうに言われています。 ここからですよ。ここからはすごい問題で、まあ多分ねYouTubeとかもいろんなところでも言われているんですけれども、さっきね、私はアライ・オルリスタッドを飲んだら脂肪を体の外に出してくれるよっていうふうに言ったんですけども、どうやって出るかというと、もちろん便として出ていくんですよ。よくね、脂肪をカットしますというサプリも便として出していきますって言うんですけれども、そういうものじゃないんですよね。というのも本当に20%から30%くらい外に出るので、油の中に便が混じったものが出るって言ったほうがいいかな。油の中に便が入ってるっていうイメージです。なので油って結構サラダ油みたいな油はサラサラしてるじゃないですか。なので便がですね、肛門から漏れてしまうということが本当によくあります。 なので、ちょっとトイレに行こうと思ったら、この油まみれの便が出ることによってまずは便器がすごい汚れたりとか、この油がですね便が混じっているわけですから、まあ臭いというかそれなりの香りがするので、私はこのアライじゃないんですけれども、ゼニカルというお薬を以前処方した時に、すごいね、患者さんから泣きの連絡が入ったことがあります。その方はヴィンテージのジーンズがすごい好きな方で、ヴィンテージのジーンズをお履きになっていたらしいんですね。ヴィンテージのジーンズって洗っちゃダメって言うじゃないですか。だからその人も大事に大事にそのヴィンテージのジーンズを履かれていたんですが、ゼニカルを飲んだら気がつかないうちに、おならなのかちょっとした時に、ちょっと汚い話なんですけど、その油が混じった便がバーっと出てしまって、その大事なヴィンテージのジーンズにそれがついてしまった。もう匂いもすごいして洗わなきゃいけないけど、でも洗うわけにはいかないジーンズなんだけど、洗わないわけにはいかないみたいな感じで、大事にしていたそのヴィンテージのジーンズがダメになってしまいましたっていうふうなことを言われて、ちゃんと説明したつもりだったんだけどなっていうふうに思ったっていう苦い思い出があります。 お腹がこうやっぱりちょっとね、緩くなるというか、油まみれの便が出るので緩くなるんですけれども、最初の1週間とかはあんまりそれが分からないんですよ。1週間ぐらい経つとそれが分かるので、もし飲み始める方がいるとしたら、1週間ぐらいはちょっとリモートだったりとか、お家ですぐトイレに行けるとか、すぐお着替えができるっていうような状態で飲んだ方がいいのかなというふうに思います。 便通がいい男の人よりも、ちょっと便秘気味の女の人の方が効くなっていう印象だったんですけれども、アライの中に入っているオルリスタッド自体が、油が多い食事をしている人によく聞くんですよ。なので女の人で油が好きっていう人、あんまり実はいなくて、女の人ってどちらかというと炭水化物、甘いものが好きで、その炭水化物で太ってるっていう方が多いんですね。なので炭水化物が多めの食事の人にはあんまり実は聞かないんですよ。揚げ物が好きとか焼肉が好きみたいな人はしっかり聞くんですが、自分が出したい人と適応する人っていうのが結構あんまりマッチングしなかったなっていう印象はあります。 ただやっぱりしっかりと、さっき言った便漏れをおむつだったりとか、あと大きいナプキンみたいなものをつけながら乗り越えた人は、脂肪が減っていくっていうことはあったので、もしアライを飲まれる方っていうのは、そういったところも気をつけていただければいいかなというふうに思います。ただあともう一つポイントはですね、やっぱり油が出ていってしまうので、脂溶性のビタミンが不足することがあります。なのでビタミンAとか、ビタミンDとか、あとビタミンEとかですね、その辺の肌にもすごく重要なビタミンだったり、特にビタミンDっていうのは免疫機能だったりとか、骨の代謝っていうことにもすごく関与しているので、とりあえず脂溶性のビタミンをしっかり取りながら飲むっていうことは、すごく大切なことなのかなと思います。 あともう一つ、これはですね、痩せている人がさらに痩せたいっていうわけじゃなくて、ちゃんと肥満という診断がついている人が飲むお薬になります。もちろん薬局に行ってもちゃんとアライの処方できる薬剤師さんの面接があってお話をしたりとか、きちっとこのアライを飲むべき方なのかというスクリーニングが起こります。ざっくり言うと18歳以上ということがポイントです。あと腹部お腹の周りがですね、男性だと85センチ以上、女性だと90センチ以上で、BMIが25から35以内の方っていうふうになります。あとは糖尿病がないかとか、脂質異常症がないかとか、あと高血圧がないかとか、いろいろチェックがあるので、その辺をクリアした人が、薬局でこのアライというお薬を買って飲むことができるというふうになるので、例えばBMIが正常値だったりする人が、ダイエットの目的で飲むものではなくて、肥満の治療薬ということになりますし、ダイエット目的で飲むにはちょっと、油の便が出るとかっていうリスクの方が、私は大きいなと思うので、決してそのダイエット目的で飲むっていうものではなくて、本当に肥満で悩んでいる方で、ちゃんと食事療法だったりとか運動療法やっても、なかなか痩せられない人の後押しに、副作用もあるっていうことをしっかり分かった上で、飲むっていうことがポイントかなというふうに思います。 1998年からこのアライ発売されてるっていうふうにお話ししたんですが、それがね日本で発売されるのが、この2024年の4月なんですよね。大正製薬さんが販売できますよっていう交渉をして、ちゃんと日本で発売権を得たのが2009年なんですね。もちろん日本人用に副作用がないかっていうのを、いろいろ調べるんですけれども、日本の創薬のスピード感って結構遅くて、だから10年とか15年ぐらいかかるのが、それでも早い方なんですよ。2009年だから15年と考えたら、日本の基準からしては普通なのかなと思うんですけども、でも私たちからすればちょっと遅いのかなと思うので、やっぱりその辺のスピード感っていうのが、ちょっと一つの考えるものになるのかなというふうに思います。 大事なことはこのアライだけを飲んで、やっぱり痩せるというわけではないんですね。しっかりと運動だったりとか、その他の食事を気をつけるってこともすごく大事なので、生活水準が変わってなければ、お薬を飲まなければ、また同じように太っていくわけじゃないですか。だからこのお薬っていうのは補助だというふうに考えて、そもそも生活習慣を変えなければ、肥満っていうのは治らないっていうことをちゃんと念頭において、こういったお薬はあくまでも補助というふうに考えていただければいいなと思いますし、あと絶対に痩せてる人とか、ダイエット目的では使わないようにしてください。 はい、ということで今日は4月から薬局で買えるようになる肥満治療薬のアライについて説明しました。何度も言うんですけど、飲むだけで簡単に副作用もなくっていうのは、なかなかないので、しっかりと食事の習慣とかを変えるようにしてみてください。ということで今日も最後まで見ていただいて、ありがとうございました。今日の内容良かったよという方はですね、グッドボタン押していただいたり、チャンネル登録していただけると嬉しいです。ではまた次の動画でお会いしましょう。さようなら。

Hello everyone. This is Arata Tomori. Today, I'd like to talk about Alli, an anti-obesity medication that's available at pharmacies and is currently a hot topic. This anti-obesity drug released by Taisho Pharmaceutical has been the subject of newspaper advertisements and online discussions. Some people are celebrating the release of this revolutionary weight-loss medication, but I'd like them to wait a moment. Alli contains a component called orlistat. Alli is an anti-obesity medication containing 60mg of orlistat. Actually, in the United States, Alli has been available as an over-the-counter anti-obesity medication since 1998. I have personal experience using orlistat at my clinic and have prescribed it myself, so it's a medication I'm very familiar with. While Alli with 60mg of orlistat is available at pharmacies, a version with 120mg requires a prescription and is called Xenical. Some of you may have heard of Xenical. About 10 years ago, it was used quite often in diet clinics at cosmetic surgery facilities. I've prescribed it to patients, and some did lose weight, while others didn't. So I think if people take Alli who have the proper indications, they can get reasonable results. Alli works by blocking fat absorption. Some might say that supplements also do similar things, but honestly, I believe 99.9% of supplements claiming to cause weight loss are ineffective. Some companies even use my face to promote such products. However, Alli is completely different. Orlistat has a well-understood mechanism of action and works effectively to prevent fat absorption in the body. Normally, when fat enters the body, an enzyme called lipase breaks it down into fatty acids and glycerol, which are then absorbed through the digestive tract. With orlistat in Alli, when fat enters the body, the lipase enzyme doesn't get released from the body, so even if you eat fatty foods, they won't be absorbed and will be eliminated. Approximately 20-30% of ingested fat is eliminated, which is a significant cutback. People often think medications have serious side effects, but regarding liver damage or effects on the digestive system, there are surprisingly few side effects. Orlistat itself isn't much absorbed by the body, so organ damage side effects are rare. That said, there's no zero risk of liver burden during elimination, so some liver function abnormalities can occur, but other organ damage is uncommon. Now, here's the major issue that's probably being discussed on YouTube and other platforms. Earlier, I mentioned that orlistat helps eliminate fat from the body, but the question is—how does it come out? Of course, it comes out as stool. While some supplements claim to eliminate fat through stool, this is different. Because 20-30% really does exit the body, it's more accurate to say oily stool comes out. Imagine oil mixed into stool. Since oil, like salad oil, is quite slippery, stool can leak from the anus. This can really happen frequently. So if you think you just need to use the toilet, oily stool may leak out, which dirties the toilet and has a notable odor. I personally didn't experience this with Alli, but I once prescribed Xenical to a patient, and I received a tearful call from them. This patient loved vintage jeans and wore them regularly. As you know, vintage jeans shouldn't be washed. So they carefully maintained these vintage jeans, but after taking Xenical, they didn't realize when the oily stool leaked out—through a fart or just slightly—and soiled their precious vintage jeans. The smell was terrible, they needed to wash them, but they couldn't wash vintage jeans. It was a terrible situation, and those carefully maintained vintage jeans were ruined. They said they thought I'd explained it properly, but it became a bitter memory. Your stomach becomes looser because oily stool comes out. However, in the first week, you might not really notice this. Around the week mark, it becomes apparent. So if you start taking this medication, I'd suggest using it during about a week when you can work remotely, stay home with easy toilet access, or quickly change clothes. I noticed that people with better bowel movements than men—women with somewhat constipation—seemed to benefit more. Orlistat in Alli works particularly well for people eating high-fat meals. Actually, women don't often prefer fatty foods. Women tend to prefer carbohydrates and sweets, and often become overweight from carbohydrate consumption. So it's not very effective for people eating high-carbohydrate meals. It works well for people who love fried food or grilled meat. There was often a mismatch between people I wanted to prescribe it to and those it was suitable for. However, people who endured the embarrassment of stool leakage by wearing adult diapers or large pads did experience fat reduction. If you take Alli, please be mindful of this. Another important point is that since fat exits the body, fat-soluble vitamins can become deficient. Vitamins A, D, and E are important for skin health. Vitamin D particularly affects immune function and bone metabolism, so taking sufficient fat-soluble vitamins while using this medication is important. One more thing: this isn't for thin people wanting to be even thinner. It's a medication for people diagnosed with obesity. Of course, when visiting a pharmacy, a qualified pharmacist will conduct an interview and screening to determine if you're appropriate for Alli. Roughly speaking, you must be 18 or older. Additionally, your abdominal circumference should be 85cm or more for men and 90cm or more for women, with a BMI between 25 and 35. There are various checks for diabetes, dyslipidemia, high blood pressure, and more. Only people who pass these criteria can buy and take Alli at pharmacies. For example, people with normal BMI shouldn't use it for diet purposes—it's a medication for obesity treatment. For diet purposes, I think the risk of oily stool outweighs the benefits. It's not meant for diet purposes. It's supplementary support for people truly struggling with obesity who have tried diet and exercise therapy but can't lose weight, understanding the side effects involved. Although Alli has been available since 1998, it was released in Japan in April 2024. Taisho Pharmaceutical negotiated sales rights and obtained permission to sell in Japan in 2009. Of course, they investigated whether there would be side effects for Japanese people, but Japan's drug development pace is quite slow. Even 10-15 years is considered fast. From 2009 to now is 15 years, which is normal by Japanese standards, but it seems slow to us. This speed difference is worth considering. Crucially, just taking Alli doesn't lead to weight loss. Exercise and dietary attention are very important. If your lifestyle doesn't change, you'll gain weight again without the medication. Think of this medication as supplementary. Obesity won't be treated without changing lifestyle habits fundamentally. This medication should only be considered supplementary support. And absolutely avoid using it if you're not obese or for diet purposes. So today, I've explained Alli, the obesity treatment medication available at pharmacies starting in April.
