皆さんこんにちは!友利新です。今日は久しぶりにクリニック帰宅です。というのも、モテさんとスキンクリニックにも私がずっとずっとずっと入れて欲しかった、今話題のニードルRFポテンツァが入りました! こちらがポテンツァっていうニードルRFの機械になります。ニードルRFってどんな機械かというと、RFっていうのは私が愛してやまないサーマクールの機械のRFなんですね。なので、熱を加えて引き締めっていう感じなんですけども、そこにニードルがついていて、針を刺してそこからRFが出て、熱を加えたい層に沿って熱が出るっていう感じになります。例えば毛穴ですね。毛穴ってやっぱり皮脂が多いじゃないですか。なので、皮脂栓の近くにRFを流すことによって皮脂栓の活動をちょっと弱める。それによって毛穴を引き締めたりとか、あとはニキビ跡っていうのは皮膚の奥の方で半根が起こっているので、そこに熱を加えてそこで半根を良くしてくれる。なので、ニキビ跡をフラットにしてくれるっていう感じなんですよ。SNS、YouTubeとか、あとはTikTokとかではニキビとかニキビ跡、あとは毛穴にめちゃめちゃ効くっていう風に拡散されてるんですけども、実はこのポテンツァ、それだけじゃないんですよ。結構悩んでる方も多いと思うんですが、肝斑にも実はこのポテンツァは効果があります。肝斑っていうのは表皮と真皮の間の方の基底膜っていうところでメラノサイトがおかしくなっているって言われているので、そこの基底層のところに針を合わせてそこにRFをやることによってメラノサイトの活動を抑えていく。それによって肝斑が良くなっていくっていうような器具なんですね。なので、思い出してください、皆さん。私はこの辺は本当にもともと皮脂が多くて、鼻とかあとはこの頬っていうこういったところには結構毛穴が気になるし、あともともとニキビがあってニキビ跡が気になるんですね。だけれどもここには肝斑があるから、今までの例えば傷をつけてよくするような毛穴系のダーマペンとかをやっちゃうと肝斑が悪化しちゃうので、ダーマペンはここだけにしかできない。でもこの辺のニキビ跡も気になるって言って、ニキビ跡を取るか肝斑の治療を取るかっていう風に迷ってた方には朗報というか待ってましたみたいな機会なんですね。なので今日は私が毛穴モードプラス肝斑モードでこのポテンツァを照射していって、どういう風に良くなっていくかっていうのを皆さんにレポートしたいと思いますので、最後までご覧ください。ではまずは麻酔を塗っていきたいと思います。はい、塗る麻酔をして今ちょうど30分経ちました。この塗る麻酔はそこまでガッツリ効くわけじゃないんですけども、ちょっとなんか痺れて感覚が鈍くなるような感じになってきました。これから麻酔を取ってさっき言ったみたいにニキビ跡が気になるところと毛穴の場所、肝斑の場所っていう風にマーキングしたいと思います。麻酔を取りました。触ってもなんか変な感じ。じゃあ私の悩みを言います。ここがやっぱりニキビと毛穴が気になる場所なので、あと頬はこうだね。ニキビ跡が結構あるんで。じゃあここもこうですね。でもポテンツァでこういう風に部分当てをするところって実はあんまり少なくって、全顔ねやるところが多いんですけれどもせっかくなんで私は自分的には。この感じですか?はい。ポテンツァは針で刺してRFを流した後に薬剤を入れることができるんですよ。今回はマッスルムっていう薬剤を入れます。これ自体が肌のコラーゲンとかエラスチンを増やしてくれる。熱を加えるだけでも肌の新しい再構築ができるんですけども、この薬剤を入れることでさらにそれをブーストしてくれるので、毛穴の開きだったりとかニキビ跡を改善してくれるっていうものになります。よく糸を入れた後って肌がね、張り感が出るっていうのはそのマッスルムの原料である成分が糸に入っているからということになります。なので糸を入れてなくてもその張り感っていうのが出てくるっていう薬剤がマッスルムなので、今回毛穴モードでしか薬剤は入れられないので、ちょっとそれはちょっと残念なんですが、それを毛穴のところに入れていこうと思います。ではやっていきましょう。はい、やっていきましょう。はい、よろしくお願いします。でもRFって本当に定期的にやるのが一番いいよね。やっぱり私は本当にサーマクールが一番好きなもの。今熱を入れているだけ。針を刺しながら。そんなに痛くないですよね。痛くないね。全然じゃないけど、すっごい痛いかって言われたらそんなに痛くはなかった確かに。でもダーマペンとかよりは痛くないかも。鼻とかなんか全然痛くない。針を刺してそこに熱を加えるから、一番効かせたい場所にRFをいかせるっていうのは画期的だよね。そうですね。毛穴が開いている人っていうのは皮脂腺の働きが活発で皮脂腺が結構大きくなってるんですよね。そこに針を刺して熱を入れるので、皮脂の分泌が少し抑えられるのと、それによって広くなった毛穴っていうのが小さくなるっていうので、毛穴が気になっている人にはおすすめかなって思います。熱を加えているので、血はほとんど出てないですね。これで何回ぐらいやるのがいいんですか?渋谷くん。だいたい1ヶ月から1ヶ月半に1回ぐらいがいいですかね。毛穴の状態とかによりますけど。あとはダウンタイムが今までのダーマペンとかより短いので取り入れやすいっていうのはあると思いますね。これって2、3日なんでしょ?翌日にはお化粧すれば全く分かんないぐらい。おじいさんあると思うんですけど。薬剤入れていきますか。薬剤を入れるときに、これって針を刺して吸引するような感じにして押し入れていくっていうものなので、ダーマペンだと薬剤入れても表面に塗るだけなんですけど、これはちゃんと圧をかけることによって奥まで行かせるっていう感じですね。また痛みが違うの?熱を入れてるので、その感じもあってなのか、全然そこまで痛くないと思います。出血してる?してないです、全然。ちょっと見てみます。あ、結構思ったよりも赤くなるんだね。自分でされてる感じはそんなに痛くないから、結構赤くなってるんだなっていうのは結構びっくりかも。しっかり熱が入ってる感じはします。よし、じゃあこれで毛穴モード終わりですかね。はい。はい、じゃあ今度肝斑モードに変えていこうと思います。お願いします。全然痛くない。さっきは皮脂性のあたりに行くので、結構深く押すんですよ。でも肝斑っていうのは基底膜の方にやるので、そんなに深くないんですよね。なので、もう全く痛くないですね、これは。肝斑って特殊で、メラノサイトが刺激を与えられることによって活発になっているので、メラノサイトの活発になっているのを抑えなくちゃいけないので、今までの光治療だったりとか、トーニングとかでもあんまり良くならなかった肝斑の人には、このニードルRFのポテンツァっていうのはすっごく効果があるっていうふうに最近の研究会には言われているので、こちらをやっていただけるといいんじゃないかなと。はい、ということで全顔終わりました。なんかね、自分で思うのは、そんなに赤みはあるんですけれども、ちょっと低い感じなのかなと思います。だいたいこの赤みは2,3時間くらいで低いらしいです。私も今日初めてなのであれですけれども、洗顔とかそういったことは基本的にしない。24時間はメイクもしないようにということなので、私明日はお休みですから、外に出ないようにしようかなと。どうしても外に出なきゃいけないのは日焼け止めもちょっと塗れないので、帽子と日傘で対応していただくっていう感じになります。なので今日はもうお風呂も入らず、顔洗わずに寝るっていう感じですね。一応赤みを抑えるために、うちではトラネキサム酸の導入をケアシスっていうものでやっていこうと思います。これからちょっとトラネキサム酸導入していきます。こちらはエレクトロポレーションの機械です。薬剤を経皮的にしっかりと導入するような機械で、痛みとかはほとんどないですね。イオン導入とはまたちょっと違う電気で皮膚のバリア機能をちょっと広げるような感じで入れていくので、今回は赤みを抑えていくためにトラネキサム酸を入れるんですけれども、その他のレーザーとかだったらビタミンCとかそういったものとかも入れていけるので、その時々に合わせて入れていくような感じで、痛みとかはなく、なんかおー気持ちいいなーみたいな感じです。はい、ケアシスも終わりました。どうでしょう、ちょっと赤み、自分の体感的にはヒリヒリ感が少し収まって、赤みも気持ち減ったような気がします。スキンケアは12時間なので、今ちょうど7時なので、明日の朝の7時までは顔も洗わないで、いろんなものはつけずに、お化粧は24時間ということなので、明日は1日お家にいますので、撮っていきたいと思います。では、1日目こんな感じでした。レーザーを当ててちょうど2時間くらいかな、今。もちろん何も塗ってない状態。まあ確かにちょっとだけ赤みありますけれども、ほとんどわかんない感じかなと思います。私帰ってくる時は、もうその頃には若干ひいてる感じがあったので、2時間でこれぐらい。確かにちょっと赤黒い感じはするんですけれども、もう多分ほとんどわからないかなと思います。ケアシスって言って導入もしてもらったので、今日はこのままお顔とかスキンケア何もせずに寝ようと思います。おはようございます。娘がちょっと隣で私の邪魔をしておりますが、次の日になりました。赤みはほとんどない感じがしてますが、どうでしょうか。朝起きて顔を洗って、ちょうど12時間経ったところです。ちょっとだけこうしがたにあるかなぐらいですね。あんなに赤かったとは思えない。いい感じ?うん。どうやってやった?どうやって?痛かった?痛くないよ。こんな感じで一日過ごしてみようと思います。おはようございます。2日目の朝です。まだ何もお化粧してませんが、肌がゴワついてる感じはするんですけれども、赤みとか下感は全くなく、これからお化粧するんですけれども、お化粧したらほぼ何も変わらないぐらいになっているような感じで、全くダウンタイムはないかな。スキンケアも、昨日一昨日はビタミンCとかレチノールとかそっち系はせずに、鎮静系のスキンケアだけにしてますけれども、ほぼ何も表面的にはないですけど、ちょっとゴワっとしてるんですけど、内側から張り感は出ているような気がしてますが、いかがでしょうか。はい、ということでポテンツァを受けて、ちょうど9日分になりました。1週間じゃないの?と思った方。すいません、私ですね、実はポテンツァを受けてから、4日後に学会でシンガポールに行って、戻ってきてから動画を撮っているので、ちょうど1週間目には日本にいなかったので、動画を撮ることができなかったので、本当は1週間後って言いたかったんですけれども、9日目で動画を撮っております。正直翌々日ぐらいまでは、あんまり肌の変化を感じなかったんですよね。赤みとかもなかったし、ザラつきもなかったんですが、ちょうどシンガポールに着いて、その日の夜かな、あれなんか肌がザラザラしてるぞ、みたいな感じに思って、ポロポロ剥けてるわけじゃないんですけれども、確実に顔を洗うときに何かが剥がれている感触がありました。シンガポール、結構エアコンはすごかったんですけども、乾燥してるっていうこともなかったので、お化粧のりが悪いとか不具合があったわけではないです。ただ一つだけちょっとポテンツァの影響だと思うんですけれども、ニキビができました。多分これはやっぱりニードルRFで皮脂腺をちょっとね、刺激をしたっていうところ。もちろん本当は私たち皮膚科なので、それを抑えるように熱を加えてるんですけれども、熱が加えきらなかった場所はちょっと活発になっちゃったのかなと思って、ちょっと鼻の横にポコッとニキビができたのがちょっと嫌だなぁと思ったんですが、そんなに大きくなるというよりは、やっぱり皮脂が毛穴に詰まってポコッと盛り上がるという感じなので、そんなになんか大きく広がること、旅行中にはあんまりなかったですし、そこまで大引くっていう感じにはなってなかったです。まあ2,3日ですかね、シンガポールにいる間はちょっとザラつくなぁと思ったんですが、戻ってきてからは全然肌質全く問題なくって、なんだったらちょうど1週間目にメイクさんにメイクをしてもらうことになったんですね、戻ってきてから。そうしたら、メイクの鈴木さんはなんか肌すごい調子いいんじゃないですか、旅行行かれてたのにみたいな感じだったので、自分の客観的な感じも状態いいなと思ったんですけども、いつも私をメイクしている方が見ても肌のね、調子は良かったんじゃないかなと思います。9日後ですが、こんな感じになってます。どうですかね、9日目で、まあ一応ファンデーションは塗ってますが、私ここらへんの毛穴がすごく気になっていたんですけれども、ファンデーションの毛穴落ちとかもないし、結構私的にはアリかなぁとは思います。ダウンタイムがほぼないのに、肌の張り感だったりとか、毛穴が1回なので全部がなくなるっていうわけではないんですけれども、ちょっとはいいかなぁとは思いますね。なのでダーマペンみたいに本当にダウンタイムがないですから、大体このポテンツァ最低でも5回ぐらいはやれば、この辺のニキビ跡とか、私ぐらいの毛穴だったら良くなるかなと思うので、引き続きやっていきたいなと思います。今回はポテンツァを施術中、そして9日後という風にレビューしてみました。本当にポテンツァっていっぱいね、今やってるクリニック あると思うんですけれども、設定が本当に大事だなぁと思いました。肝斑が気になるモードだったら肝斑モードで、毛穴が気になるんだったら毛穴モードでっていう形で、ちょっと針は変わったりとか、入れる薬剤は変わったりするんですけれども、同じ機械でも本当に設定だったりとかの違いで、効果は違うんだなと思うので、モテさんとスキンクリニックでも、やっぱりたくさんの患者さんをこなしながら、そして私自身もいろいろ文献とか読みながら、いろいろ設定を変えて、皆さんに満足いくような設定の施術にしていきたいなと思いますし、なんといっても私の流れの時になっていた、ボコボコじゃないんだけど光当たった時にちょっとボコついて見えるニキビ跡と毛穴が何とか綺麗になるように、これからも自分の設定も考えていきたいと思います。ということで、ぜひぜひ気になった方はポテンツァについても調べてみてください。ということでまた次の動画でお会いしましょう。さようならー!

Hello everyone! This is Toshini Tomi. Today I'm returning to the clinic after a long time. The reason is that Mote-san and Skin Clinic have finally introduced the trending Needle RF Potenza that I've been wanting to try for so long! This is the Potenza machine, which is a Needle RF device. What kind of machine is Needle RF? Well, RF is the same RF as in Thermage, a machine I absolutely love. So it adds heat to tighten the skin, but this has needles attached. You insert the needles, and RF comes out from those points, delivering heat along the layer you want to target. For example, with pores. Pores have a lot of sebum, right? So by directing RF near the sebaceous glands, we can slightly weaken sebaceous gland activity. This causes enlarged pores to shrink. As for acne scars, there's inflammation happening deep in the skin, so by applying heat there, we improve that inflammation and flatten acne scars. This is that kind of device. On social media, YouTube, and TikTok, people spread the word that it's super effective for acne, acne scars, and pores. But actually, Potenza does more than that. Many people struggle with this, but Potenza is actually effective for melasma too. Melasma occurs when melanocytes malfunction at the basement membrane between the epidermis and dermis. By inserting the needle at the basement layer level and applying RF there, we suppress melanocyte activity, which improves melasma. So, remember this about me: my T-zone naturally has a lot of sebum, and I'm concerned about pores on my nose and cheeks. I also have acne scars I'm worried about. But because I have melasma in these areas, if I used devices like Dermapen that create injuries to improve pores, it would worsen my melasma. So I could only use Dermapen in certain areas. Yes, after applying the numbing cream, 30 minutes have passed now. This topical anesthetic doesn't work super strongly, but I'm feeling a bit numb and my sensation is getting duller. I'm going to remove the anesthetic now and mark the areas with acne scars, pores, and melasma like I mentioned before. I've removed the anesthetic. It feels strange even to touch. So let me tell you about my concerns. This area is where I'm concerned about acne and pores, and my cheeks have this. I have quite a few acne scars. So here too. But actually, there aren't many places where Potenza can be used for partial treatment like this—most places do the whole face. But since we're doing this, I personally prefer this approach. Is this the right way? Yes. With Potenza, after inserting the needles and delivering RF, you can also inject medications. This time, I'm injecting a medication called Masteum. This medication increases collagen and elastin in the skin. Heat alone allows skin to reconstruct itself, but adding this medication boosts that process further, improving pore openness and acne scars. You know how skin gets that firm feeling after getting threads? That's because the ingredient in Masteum is in the threads. So even without threads, you get that firmness from the Masteum medication. Since I can only inject medication in pore mode this time, I'm a bit disappointed about that, but I'll inject it into the pore areas. So let's get started. Yes, let's do this. Yes, please. But RF really is best done regularly. I genuinely love Thermage the most. I'm applying heat now while inserting needles. It doesn't hurt that much, does it? It doesn't hurt. It's not bad, but if you ask if it really hurts a lot, it doesn't actually hurt that much. But it might hurt less than Dermapen. My nose doesn't hurt at all. The fact that we can insert needles and apply heat directly to the target area to deliver RF is groundbreaking. It really is. People with open pores have active sebaceous glands that are quite enlarged. By inserting needles and applying heat there, we can slightly suppress sebum secretion, which causes those enlarged pores to shrink. I'd recommend this for people concerned about pores. Since we're applying heat, there's hardly any bleeding. How many times do I need to do this, Shibuya-kun? I'd say about once every month to six weeks, depending on your pore condition. And the downtime is shorter than with Dermapen, so it's easier to incorporate into your routine. It's only 2-3 days, right? The next day, if you wear makeup, it's completely unnoticeable. I think there might be some redness. Should we inject the medication now? When injecting the medication, we insert the needle and use suction-like pressure to push it in deeply. With Dermapen, even if you inject medication, it just sits on the surface. With this, we apply pressure to make sure it goes deep. Does the pain change when injecting? Since we're applying heat, maybe because of that, I don't think it hurts much at all. Any bleeding? No, not at all. Let me look. Oh, it's redder than I thought it would be. Since it doesn't hurt much when being treated, it's surprising how red it actually got. I can definitely feel the heat going in. Okay, so we're done with pore mode then? Yes. Alright, now let's switch to melasma mode. Please. It doesn't hurt at all. For the sebaceous areas, we go quite deep. But for melasma, we're working at the basement membrane level, so it's not as deep. So it doesn't hurt at all with this one. Melasma is special—melanocytes become active when stimulated, so we need to suppress that activation. With traditional light therapy and toning, melasma often doesn't improve much, but Needle RF Potenza is said to be really effective for melasma according to recent conferences. This would be a good treatment option. Yes, so we've finished the whole face. Personally, I think there's quite a bit of redness, but maybe it's milder than I expected. This redness should fade in about 2-3 hours. Since this is my first time, I'm not entirely sure, but basically, we shouldn't do anything like wash our face. We shouldn't wear makeup for 24 hours, so since I'm off tomorrow, I'll try to stay home. If I absolutely have to go out, I can't apply sunscreen either, so I'll use a hat and umbrella instead. So today I'm not bathing or washing my face, just going to bed like this. To suppress the redness, I'm going to do a tranexamic acid treatment with electroporation at the clinic. But once I arrived in Singapore that night, I thought, "Hey, my skin feels rough," like really rough. It wasn't peeling off in flakes, but when I washed my face, I definitely felt something shedding. Singapore had strong air conditioning, but my skin didn't feel particularly dry, and my makeup application wasn't bad or anything. But there was one thing I think was from Potenza—I got a pimple. I think it's because the Needle RF stimulated the sebaceous glands. Of course, as dermatologists, we apply heat to suppress that, but in areas where the heat wasn't strong enough, they might have become a bit more active. So a little pimple popped up on the side of my nose, which was disappointing. But rather than getting larger, it's more like sebum got trapped in the pore causing a little bump, so it didn't really spread much, and during the trip it wasn't really a big deal. For maybe 2-3 days while I was in Singapore, I thought my skin felt rough, but once I got back, there was absolutely no issue with my skin texture. In fact, around the one-week mark after I got back, I had a makeup artist do my makeup. The makeup artist Suzuki said, "Your skin looks great, even after traveling," so from an objective perspective, my skin seemed to be in good condition. I think the person who always does my makeup probably thought my skin was in good shape too. On day 9, my skin looks like this. What do you think? On day 9, I mean I have foundation on, but I was really concerned about pores in this area, and my foundation doesn't sink into the pores or anything. Personally, I think it's pretty good. Even with barely any downtime, I can feel the firmness and improved pores—not that one session makes everything disappear, but it's definitely an improvement. So without downtime like Dermapen has, I think if I do Potenza about 5 times minimum, the acne scars and pores like mine should improve quite a bit. So I want to keep doing it. This time I've reviewed Potenza during the treatment and on day 9. I really think the settings are so important with Potenza. There are a lot of clinics doing it now. If you're concerned about melasma, you use melasma mode; if you're concerned about pores, you use pore mode. The needles and medications change a bit, but even with the same machine, the effects differ based on settings. So at our clinic too, while seeing many patients, I'm reading literature and trying different settings to provide treatments that satisfy our patients. And most importantly, for my own concerns—acne scars and pores that look a bit bumpy in certain lights—I want to work on settings to make them smoother.
