皆さんこんにちは。内科皮膚科医の友利新です。 今日は皆さんの寿命が10年以上伸びる、医師がおすすめの5つの習慣をご紹介したいと思います。 もちろん寿命が長くなるって言っても、元気がなく寿命が伸びるということではなくて、健康で楽しく寿命が伸びる5つの習慣というんですかね。医師であれば、大体の人がこの習慣を取り入れていたり意識をしているものなんじゃないかなと思うので、最後までご覧ください。では早速始めていきます。 実はお口の中に歯周病があると、歯周病というのは歯の歯肉、いわゆる歯茎にくっついているじゃないですか。その歯肉、いわゆる歯茎に炎症が起こるものが歯周病ですよね。これ炎症が起こるとどうするかというと、ここから炎症物質というのが出るんですよ。この炎症物質があるので、歯を溶かしたりということで虫歯になったりとか、歯が取れてしまったりとか、歯だけで起こってくれればいいんですが、実はこの歯茎の炎症というのは血液に乗って炎症物質というのが全身に行き渡るということが分かっているんです。 例えばTNF-αというようなサイトカインと呼ばれるものですね。これってね、例えばアトピーとか、あとはリウマチとかでも関与する炎症物質の一つなんですが、そういったものが全身の血管に炎症を起こすんですよ。全身の血管に炎症を起こすと、例えば動脈硬化とか、あとは脳の血管に炎症が起こったりすると、やはり認知症だったりのリスクも上がってきます。あとは動脈硬化が進むと、心筋梗塞を起こしやすくなったりとかいうことも言われています。あとは女性で怖いのは、妊娠中にこの歯周病が悪化したりとか、妊娠はさておいても、動脈硬化、それで心筋梗塞、あとはアルツハイマー、あとは糖尿病が悪化したりということも分かっていますので、そういったことがあると健康寿命が縮まってしまう。 ですから歯ブラシをしましょうっていうことは皆さん分かっていると思うんですが、口腔内を歯ブラシで拭き取ることによって歯ブラシをする習慣はよくあるんですけども、フロス、要は歯間ブラシですよね、そういうものをする習慣がないと。でも歯周病を抑えるには、歯と歯の間のフロスをするほうが歯周病を低下させるということが分かったというデータがあったので、ご飯を食べたら歯を磨いてますよっていう人も、健康寿命を伸ばしたければ、歯ブラシプラス、ぜひデンタルフロス、歯間ブラシをしっかりするようにしてください。 続いて、医師が勧める寿命が10年以上伸びる5つの習慣。2つ目は、寿命を伸ばしたかったら病院ではなく、歯医者さんに行ってください。さっきから歯歯うるさいな、歯医者の回し者かよみたいな感じなんですけど、これ本当に分かっていて、さっきフロスしましょうって言いましたよね。さっきの口腔内の歯周病が悪化すると、先ほど言った全身の疾患、動脈硬化から始まって、いろんな炎症性の病気のリスクが増えて健康寿命を減らしますよって言ったんですけれども、しっかり歯ブラシをしてフロスをしていたとしても、やっぱり溜まってしまうのが歯石と呼ばれる、要は食べかすが取れなくなって、プラークというものになったものが固まって、普通の歯ブラシとかフロスが取れなくなったものが歯石というものなんですね。 この歯石というのは、さっきも言ったように、どんなに綺麗に歯ブラシをしていたりとかフロスをしていても、どうしてもちょっとずつついて、それが溜まっていってしまうんですよ。普通に歯ブラシをしていたらそれは取れません。この歯石というのは歯周病の原因になって、ずっとここに歯石がくっつくことで、さっきお話しした炎症物質とかが出るんですよ。なので歯が痛くなくても、歯医者さんというのはこの歯石を取ってもらうためにできれば1ヶ月に1回、もしくは2、3ヶ月に1回行っていい場所なんですよ。 なんとなく私たち歯が痛かったりとか歯に異常がなければ歯医者さんに行ってはいけないっていうイメージと、あと最近はコロナウイルスの感染症でなんとなく歯医者に行くのちょっと抵抗あるなと思っている方も多いと思うんですけれども、もちろんコロナの感染というのは怖いですけれども、歯医者さんというのはしっかりと対策もされていますし、逆に歯石が溜まってしまって、歯石がこびりついてしまって、そこから炎症物質がどんどん出てしまうということが、長い目で見たときの健康寿命を短くしてしまうということにつながりますので、最初の1つ目の習慣のフロスをするということと合わせて、2つ目はぜひ定期的に歯医者さんに行って歯石を取ってみてください。 続いて医師がおすすめする健康寿命を伸ばす5つの習慣。3つ目は禁煙をしてください。皆さんに散々、タバコのデメリットって知っていると思うので、今更という感じですけれども、改めてここでしっかり話しておきたいんですけれども、やっぱりタバコというのは、体に悪いことはなんとなく皆さんわかっていますよね。有名な表でいうと、30歳でタバコをやめると寿命が10年伸びる、40歳だと9年ぐらい伸びるというようなこともあって、いつやめるかというのもすごく大事なんですけど、その表でいうと、60歳でやめても3年以上は伸びるということもわかっているので、早めにやめるということと、プラス、いつでもいいから吸ってたら今すぐやめるということがすごく大事なんですよ。 でも、タバコの害というのは散々皆さんわかっていて、ニコチンだったりとか、タバコを吸ったときにはまずは口腔内のガンですよね、この辺にタールだったりとか化学物質が入って、それが肺に行くので、肺とか気管の辺のガンが起こったりとか、ということもわかっていますし、それだけじゃなくて、国立がんセンターのホームページを見るとですね、全身のガンに関わっているということもわかっているんですね。例えば、一見関係ないような胃ガンとか膵臓ガンとか、あと食道ガンのリスクも増やしますし、あとはですね、女性でいうと子宮頸ガンだったりとかのリスク、もちろん子宮頸ガンはウイルスということも大きいんですが、タバコを吸うということもリスクファクターにはなっています。あとは膀胱ガンとか、そういった全身のガンのリスクを増やしますということもきちんと検証されています。 いやいや、最近ね、普通のタバコはやめて電子タバコにしたよっていう方もいると思うんですが、皆さん電子タバコと紙タバコの違いって知ってますか?私、宮古島出身なので、葉タバコ農家の人が多いのでちょっと怒られるかなとも思うんですけども、タバコって要は葉っぱなんですよ。そのタバコの葉を乾燥させたものを直接燃やして吸うのが紙タバコですよね。なんですが、加熱式、電子式っていうのかな、ああいう機械に入れて吸うようなものっていうのは、それを直接燃やすんじゃなくて、例えばリキッド状、液体にしてそこに熱を加えることによってタバコを吸うということになる。直接燃やすか、別の形にして熱を加えて吸うかということなんですね。 なので、電子タバコにしたから結構体にいいんですっていうふうに誤解というか思っている方いると思うんですが、結局入っている化学物質とかは結局同じです。まだまだわからないことは多いんですけれども、もしかしたら濃縮されている分体に悪いんじゃないかっておっしゃる先生もいらっしゃるぐらいなので、紙タバコをやめる、そして電子タバコにするっていうふうに思うのであればスパッとやめたほうがいいと思います。 あとは、タバコっていうのは自分だけじゃなくて、例えばお子さんがいる人だったら副流煙といってね、タバコから出る煙で相手の健康被害も出てきてしまいます。もちろん電子タバコだと副流煙は出ないですけれども、自分が吸って吐いたときに、吐いた息の中にはやっぱりこういう副流煙と同じような化学物質が入っていますので、副流煙といってタバコからヒューッと出るものはないかもしれないけど、自分の呼気の中には含まれているので、害は少なくはなりますけれども100%じゃないということは覚えておいて、自分の健康、そして家族の健康を守りたければ必ず禁煙をしてください。 続いて、医師がおすすめする健康寿命を10年以上増やす5つの習慣。4つ目はメンタルを整えることになります。今まで私、全身の例えばメタボリックもそうだし動脈硬化もそうだし、がんとかも増えてしまいますよ、だから口腔環境をきれいにしましょう、禁煙しましょうという話をしましたよね。もちろん日本人の一番の死因ってやっぱり悪性腫瘍といってがんなんですが、40代までで見るとですね、実はがんの次に多いのが自死といわれるものになります。 これに関してはもちろん経済的なものだったり社会的な不安とかということもありますが、やはりそこにはメンタルというものが大きく関係しています。皆さん何となくうつ病とか聞いたことあると思うんですけれども、うつ病は心の風邪だ、なんか頑張って治そうみたいな感じかもしれないんですけれども、実は心の風邪というよりはどちらかというと今までは誰でもなるからという意味で心の風邪だと思っていたんですね。 これも一つ深掘り動画を撮ろうかなと思うんですけれども、簡単に説明すると脳の伝達物質が低いよということで言われてたんですが、最近はそれだけじゃなくて脳の神経自体が壊れてしまう、要は器質的というか、例えば骨折みたいなものです。骨折したら骨が壊れてしまいますよね。それと同じようにいろんなストレスを加えられて脳の細胞が一時的に壊れてしまう。なのでその壊れるものを修復しなければいけないんじゃないかというふうにも言われているんですよ。 この修復をするため、メンタルをコントロールするために何が必要かというと、大事なのは睡眠になります。だから皆さんよく寝ましょうということはメンタルケアというか睡眠をとりましょうでしたね。だけどやっぱりメンタルケアをするためにはしっかり睡眠をとることなんですよ。不安があったりとかちょっとうつ症状がある人はしっかり休んでくださいって言うんですけども、その休み方ってすごく大事で、なんとなくぼーっとするっていうこともそうですけども、しっかり眠るっていうことが大事なんですよ。 寝ている間にやはり脳細胞っていうのは修復をされていくっていうことがあるので、実はメンタルとかうつ症状に悩んでいる人っていうのは80%くらい不眠症を併発しているんですね。逆の視点から見ると寝れないって感じている人っていうのは、そうじゃない人に比べて2倍以上そういうメンタルだとか、そういううつ症状っていうのが起こってきやすいっていうことも分かってきます。なのでやっぱり寝るっていうことはとてもとても大切なことなんですね。 これって私たち大人だけではなくて子どもも一緒です。特に子どもっていうのは成長期において睡眠っていうことはとても大事になってきていて、睡眠っていうのは年代によって必要な時間が決まっています。赤ちゃんってずっと寝てるじゃないですか。あれって成長に必要。そして体の機能を落ち着かせるとか安定させるっていうためにも睡眠がとても必要なんですね。なので生まれた時の新生児って24時間の中の20時間ぐらい寝てるんですよ。子どももだいたい10時間ぐらいは寝なきゃいけないっていうことが健康的には言われていますし、徐々に短くなっていて年配の方って何とか朝起きるのが早く感じるじゃないですか。なのでそういった人って睡眠時間が短くなっているからなんですよ。だから睡眠をとりましょうっていうのはしっかりと自分の年齢にあった睡眠をとりましょうということになります。 寝るタイミングとかっていうのもすごく大事になってくるので、それはまた睡眠という動画でまた撮りたいと思いますけれども、健康寿命をしっかり伸ばしたければメンタルケアとして睡眠をしっかりととるように心がけてください。 健康寿命を10年以上伸ばす5つの習慣最後は発酵食品を毎日摂ることになります。これは皆さんもよくわかっている方も多いと思うんですが、発酵食品っていうのは腸内環境を整えてくれます。腸内環境なんで健康寿命に関係あるのって思っている人は少ないと思うんですけれども、もちろん腸っていうのは消化吸収をして、そこで吸収された栄養が全身に行くわけですよ。どうしても腸炎とか腸内環境が悪くて、そこで消化吸収がしっかりされないと全身にしっかりとした栄養はいかなくなるんですね。それだけじゃなくて、やはり腸内環境が悪いと、大腸がんのリスクが増えたりとかいうことも言われています。 そしてあと最近は腸というのは消化吸収をしている場所だけではなくて、いろんなホルモンを出していて、それが体の機能を調節しているということがわかってきています。代表的なものというと、幸福ホルモンとも言うんですが、セロトニンというホルモンが出て、それがうつ症状とかメンタルがちょっと下がり気味という方には、それが効果を発揮してうつ症状から離脱できるということも言われています。なので、便秘という方は、少しメンタル的にも下がってきているということもわかっていますから、皆さん、必ず毎日発酵食品を摂りましょう。 発酵食品というと、毎日納豆を食べようとか、大好きなヨーグルトを毎日食べようと思っているかもしれないですが、それではないんですね。発酵食品というのは、いろんなものを食べるということが大切です。例えば、ヨーグルトというのは動物性だったり、そういうものが多いですよね。そうじゃなくて、キムチとかぬか漬けは植物性のものになるじゃないですか。なので、いろんなものを摂るということで、いろんな種類の発酵食品の中には菌がいて、その菌がお腹の中で悪玉菌とか日和見菌を善玉菌に変えていったりとか、悪玉菌が増えていくものを抑えてくれるわけですよ。 一つの発酵食品だけを食べ続けるというのもダメなわけではないんですが、なるべく多くの発酵食品、キムチだ、納豆だ、ぬか漬けだ、ザワークラウトとかいろいろありますよね。そういったものを毎日毎日摂るということ、変化をさせながらいろんな種類を摂るということが腸内環境を整えるということにすごく大切なので、いろいろな種類の発酵食品を摂るということを忘れないでください。 まとめると、健康寿命を10年以上延ばしたければ、まずはフロスしましょう。歯と歯の間の汚れをしっかりと取ってくださいね。そして、どうしてもそれでも取れない歯石は必ず歯医者さんに行って取るようにしましょう。歯医者さんというのは虫歯があるから行くわけじゃないんです。そういう歯石を取るためにも行きます。なので、健康になりたければまずは歯医者に行ってください。病院にも行ってって言いたいところですが、歯医者さんのほうが明らかにいいと思います。予防のためには。 そして3つ目は電子タバコも紙タバコも今すぐやめてください。あなた自身の健康だけではなくて、家族のために紙タバコも電子タバコもしっかりとやめるようにしましょう。そして4つ目はメンタルケアのためにしっかり睡眠をとりましょう。休んでねというのはしっかり寝ましょうということとイコールだと思ってください。うつというのは風邪だよ、ではないんです。脳の神経が壊れているということもあります。これを修復するには睡眠がめちゃくちゃ大切です。なのでしっかり寝るようにしましょう。これについてはまた深掘りします。 そして5つ目はいろんな種類の発酵食品を食べてください。いろんな菌があなたの腸内環境を助けて、そしてあなたの健康を助けてくれるはずです。 Hello everyone. I'm Arata Tomori, an internal medicine and dermatologist doctor. Today I'd like to introduce 5 doctor-recommended habits that can extend your lifespan by 10 years or more. Of course, when I say lifespan extension, I don't mean extending your life without vitality—rather, these are 5 habits for extending your healthy lifespan happily and healthily. I think most doctors incorporate these habits or are conscious of them, so please watch until the end. Let's get started right away. Actually, if you have gum disease (periodontal disease) in your mouth—gum disease is inflammation that occurs in the gingiva, the gum tissue that attaches to your teeth. When this inflammation occurs, inflammatory substances are released. Because of these inflammatory substances, teeth can dissolve leading to cavities, or teeth can fall out. It would be fine if this only happened in the mouth, but actually, this gum inflammation is known to spread inflammatory substances throughout the body via the bloodstream. For example, there's something called TNF-α, a type of cytokine. This is one of the inflammatory substances also involved in conditions like atopic dermatitis and rheumatism, and such substances cause inflammation in blood vessels throughout the body. When this happens throughout the body's blood vessels, it can lead to things like arteriosclerosis, and when inflammation occurs in brain blood vessels, the risk of dementia also increases. Additionally, as arteriosclerosis progresses, it's said to increase the likelihood of heart attacks. Also, what's frightening for women is that gum disease can worsen during pregnancy, and aside from pregnancy, it's known to be associated with arteriosclerosis, heart attacks, Alzheimer's, and worsening diabetes. When these things happen, your healthy lifespan shortens. So, I think everyone knows they should brush their teeth, but while brushing the inside of your mouth is a common habit, many people don't have the habit of using floss, meaning interdental brushes. However, there's data showing that flossing between teeth is more effective at reducing gum disease than just brushing. So even if you're someone who brushes your teeth after every meal, if you want to extend your healthy lifespan, please make sure to use dental floss and interdental brushes in addition to brushing. Next, doctor-recommended habit #2 for extending your lifespan by 10+ years: If you want to extend your life, go to the dentist, not just the hospital. I know I keep talking about teeth and you might think I'm some dental industry shill, but this is genuinely understood. I just mentioned flossing, right? I explained that when oral gum disease worsens, it leads to systemic diseases starting with arteriosclerosis and increases the risk of various inflammatory diseases, shortening your healthy lifespan. But even if you brush thoroughly and floss, what still accumulates is something called tartar (calculus)—essentially, food debris that can't be removed becomes plaque, which then hardens into something that regular brushing and flossing can't remove. That's tartar. As I mentioned, no matter how thoroughly you brush or floss, tartar gradually builds up little by little. Regular brushing simply cannot remove it. This tartar becomes the cause of gum disease, and as it continuously sticks to your teeth, it releases the inflammatory substances I mentioned earlier. So even if your teeth don't hurt, the dentist is a place you should go, ideally once a month or every 2-3 months, specifically to have this tartar removed. We tend to have this image that we shouldn't go to the dentist unless we have tooth pain or some dental abnormality, and recently, many people also feel hesitant about visiting the dentist due to COVID-19. Of course, COVID infection is scary, but dentists have thorough safety measures in place. On the other hand, if tartar accumulates and becomes firmly stuck, continuously releasing inflammatory substances, this will shorten your healthy lifespan in the long run. So, along with habit #1 of flossing, habit #2 is to please visit the dentist regularly to have tartar removed. Next, doctor-recommended habit #3 for extending healthy lifespan: Quit smoking. I think everyone already knows the disadvantages of smoking repeatedly, so this might feel redundant, but I want to reiterate it clearly here. Smoking is something everyone vaguely knows is bad for health. According to a famous chart, quitting smoking at age 30 extends lifespan by 10 years, and quitting at 40 extends it by about 9 years. When you quit matters a lot, but according to that same chart, even quitting at 60 extends lifespan by more than 3 years. So quitting early is important, plus, whenever you are now, if you're smoking, quitting immediately is extremely important. The harms of smoking are well understood by everyone—nicotine and so on. When you smoke, first there's oral cancer risk since tar and other chemicals enter this area, then it goes to the lungs, causing lung and tracheal cancers. But that's not all—looking at the National Cancer Center's website, smoking is known to be involved in cancers throughout the body. For example, seemingly unrelated cancers like stomach cancer, pancreatic cancer, and esophageal cancer risks are increased. Also, for women, cervical cancer risk—of course, cervical cancer is largely attributed to viral infection, but smoking is also a risk factor. Additionally, bladder cancer and other cancers throughout the body have their risks increased, and this has been thoroughly verified. Now, some people might say they've switched from regular cigarettes to e-cigarettes recently. Do you know the difference between e-cigarettes and paper cigarettes? I'm from Miyako Island, where there are many tobacco leaf farmers, so I might get scolded for this, but tobacco is essentially a leaf. Regular cigarettes burn the dried tobacco leaf directly for smoking. However, heated tobacco products or e-cigarettes work differently—instead of burning directly, they turn it into liquid form and apply heat to it for smoking. So it's a matter of burning directly versus converting to another form and applying heat. So, some people misunderstand that switching to e-cigarettes is much better for health, but the chemical substances contained are ultimately the same. There's still much unknown, but some doctors even suggest that concentrated forms might be worse for health. If you're thinking of quitting paper cigarettes and switching to e-cigarettes, I think it's better to quit completely instead. Also, smoking isn't just about yourself—for example, if you have children, secondhand smoke from cigarettes causes health damage to others as well. Of course, e-cigarettes don't produce secondhand smoke, but when you exhale after inhaling, your breath still contains chemical substances similar to secondhand smoke. So even though there's no visible smoke coming out like with regular cigarettes, these substances are still contained in your breath. While the harm may be reduced, it's not 100% eliminated, so please remember this. If you want to protect your own health and your family's health, please quit smoking completely. Next, doctor-recommended habit #4 for extending healthy lifespan by 10+ years: Managing your mental health. I've talked about how metabolic issues, arteriosclerosis, and cancer risks increase, so we should clean up our oral environment and quit smoking. Of course, the leading cause of death for Japanese people is malignant tumors, cancer, but looking at people up to their 40s, the second most common cause of death is actually suicide. Of course, this involves economic and social anxiety factors, but mental health is significantly related to this as well. Everyone has probably heard of depression, and you might think of depression as "a cold of the heart"—something everyone gets and you should just try hard to recover from. Actually, rather than being like a common cold, this expression was used in the sense that anyone can get it. I'm thinking of making a deep-dive video on this topic separately, but simply put, it was previously explained as low levels of brain neurotransmitters. But recently, it's understood that beyond that, brain nerves themselves can actually become damaged—it's more structural, like a fracture. When you break a bone, the bone becomes damaged, right? Similarly, various stresses can temporarily damage brain cells. So it's said that we need to repair this damage. What's needed to repair this and control your mental state is sleep. So when people say "sleep well," that's essentially the same as saying "take care of your mental health." To properly care for your mental health, you need to get proper sleep. For people with anxiety or mild depressive symptoms, we tell them to rest properly, but how you rest matters greatly—it's not just about zoning out, but actually sleeping properly that's important. While sleeping, brain cells are being repaired, so about 80% of people struggling with mental health issues or depressive symptoms also have insomnia. Conversely, people who feel they can't sleep are more than twice as likely to experience mental health issues or depressive symptoms compared to those without sleep problems. So sleep truly is extremely important. This applies not just to us adults but to children as well. Especially for children during their growth period, sleep is extremely important, and the amount of sleep needed varies by age. Babies sleep constantly, right? That's necessary for growth, and sleep is also very necessary for calming and stabilizing bodily functions. So newborns sleep about 20 out of 24 hours. It's said that children need about 10 hours of sleep for good health, and this gradually decreases with age—elderly people tend to wake up earlier in the morning, which is because their required sleep time has become shorter. So getting proper sleep means getting the amount of sleep appropriate for your age. Timing of sleep is also very important, so I'd like to make another video specifically about sleep, but if you want to properly extend your healthy lifespan, please make an effort to get proper sleep as part of your mental health care. The last of the 5 habits for extending healthy lifespan by 10+ years is consuming fermented foods daily. Many of you probably already understand this well, but fermented foods regulate your gut environment. You might think gut environment has little to do with healthy lifespan, but of course, the intestines digest and absorb nutrients, which then go throughout your body. If you have intestinal inflammation or poor gut environment, digestion and absorption won't work properly, and nutrients won't be properly delivered throughout your body. Beyond that, poor gut environment is also said to increase the risk of colon cancer. Also, recently it's been discovered that the intestines aren't just a place for digestion and absorption—they also release various hormones that regulate bodily functions. A representative example is serotonin, also called the "happiness hormone." This hormone is released and is said to help people with depressive symptoms or lowered mental states recover from depression. So people with constipation are also known to have somewhat lowered mental states. Therefore, everyone should definitely consume fermented foods daily. When you think of fermented foods, you might think of eating natto every day or your favorite yogurt every day, but that's not quite right. What's important with fermented foods is eating a variety of different types. For example, yogurt tends to be animal-based, right? In contrast, kimchi and pickled rice bran vegetables are plant-based. So by consuming various types, the different bacteria in various fermented foods work in your gut to convert bad bacteria or neutral bacteria into good bacteria, or suppress the increase of bad bacteria. It's not necessarily bad to eat just one type of fermented food continuously, but consuming as many different fermented foods as possible—kimchi, natto, pickled rice bran, sauerkraut, and various others—while varying and rotating through different types is very important for regulating your gut environment.
