皆さんこんにちは。内科・皮膚科医の友利新です。今日は、皆さんからよく聞く「顔のマッサージ器具を使うのはどうですか?」という質問についてお答えしようと思います。コロコロローラーなど、たるみを取りますよというようなマッサージ器具がたくさんありますよね。私も実は持っていたりもします。
こういったマッサージ器具は、昔からありますよね。以前は「肌に刺激を与えることで皮膚のコラーゲン産生が促されてハリがアップする」「たるみを予防できる」というふうに言われてきました。しかし医学的に見ると、物理的に肌に刺激を加えることで肌の線維芽細胞がコラーゲンを産生するというデータはありません。
また、「引き上げることで顔がリフトアップするのではないか」と思う方もいると思いますが、実は持ち上げるだけで顔がリフトアップするというほど簡単なものではありません。むしろ、強く一生懸命マッサージ器を使うと逆効果になります。「優しくやってください」とお伝えしているにもかかわらず、「強くやった方が持ち上がる」「強くやった方がハリがアップする」と思い、力いっぱいゴリゴリとやってしまう方がいますが、あれは全くのNG行為です。
なぜかというと、肌を支えている「支持組織」というものがあり、この組織に強い力を加えると、ぷちぷちと切れていってしまうのです。支えている組織が切れてしまうということは、当然ながら肌が下に下がってきます。つまり、マッサージ器具を強く使えば使うほど、顔のたるみが起こってくるのです。
もう一つ、韓国などのマッサージで「カッサ」というものがありますね。特殊な器具で顔をこすり、顔が真っ赤になるくらいまで行って「すっきりする」と言われていますが、あれも支持組織を傷める可能性があるだけでなく、強い摩擦を起こしてしまいます。クレンジングの回でもお伝えしましたが、肌に摩擦を与えると炎症が起こります。それは日焼けと同じで、炎症が終わると「炎症後色素沈着」が必ず起こります。その結果、せっかく美白化粧品を使っていても、ゴシゴシすることでシミができてしまったり、この刺激によって生じるシミの一種である「肝斑(かんぱん)」がひどくなったり、顔全体がくすんでしまうことになるのです。
では、顔のマッサージが全部ダメかというと、そうではありません。腕の良いエステティシャンの方は、支持組織を壊さないよう、肌に摩擦を与えないような絶妙なタッチで施術してくれます。ですので、そういったプロの方に任せることは問題ありませんし、器具を使う場合も優しいソフトタッチにする、やり過ぎないことを心がけていただければと思います。
ただ、むくみを取ったりリフトアップに最も簡単で効果的な方法は、実は「笑顔を作ること」「表情を動かすこと」なのです。笑ったり笑顔を作ったりすることで、マッサージと同様にむくみを改善する効果があり、その刺激によってコラーゲンやエラスチンを産生する効果もあることが分かっています。
高いマッサージ器具を購入することも良いですが、もし余裕がない方は、笑顔でしっかりと効果を上げていくことをおすすめします。マッサージ器具が全て悪いわけではなく、使い方次第です。ただ心配な方は、マッサージ器具を使うよりも、よく笑う・笑顔にする・口角を上げるだけでも、リフトアップ効果やコラーゲン産生促進効果が得られますので、いつも笑顔で表情豊かな生活を送るようにしてみてください。

Hello everyone. I'm Toa, a physician specializing in internal medicine and dermatology. Today, I'd like to answer a question I often receive from viewers: 'Is it okay to use facial massage devices?' There are many massage tools on the market—such as rolling face rollers—that claim to reduce sagging. I actually own some myself.
These kinds of massage devices have been around for a long time. In the past, it was said that stimulating the skin would promote collagen production and improve firmness, or that it could prevent sagging. However, from a medical standpoint, there is no data showing that physically stimulating the skin causes fibroblasts to produce collagen.
Some people also wonder if lifting the skin with a device will result in a lifted appearance. In reality, it's not that simple—just lifting the skin does not lead to a lifted face. In fact, using a massage device too forcefully can be counterproductive. Despite being advised to use gentle pressure, many people tend to think 'the harder I press, the more it lifts' or 'the more pressure I apply, the firmer my skin will become,' and end up pressing very hard. This is absolutely the wrong approach.
The reason is that there are support structures in the skin that hold it up. When excessive force is applied, these structures can break apart little by little. Once the supporting tissue is damaged, the skin naturally begins to sag. In other words, the harder you use a massage device, the more facial sagging you may cause.
Another example is 'Gua Sha,' a massage technique popular in Korea and other parts of Asia, where a special tool is used to scrape the face until it turns red, supposedly leaving it feeling refreshed. However, this not only risks damaging the support structures, but also causes significant friction on the skin. As I mentioned in my cleansing video, friction causes inflammation in the skin—just like sunburn. After the inflammation subsides, post-inflammatory hyperpigmentation inevitably occurs. As a result, even if you're using brightening skincare products, scrubbing your skin can lead to the formation of dark spots, worsen a type of pigmentation called melasma, or cause overall dullness in the face.
That said, facial massage is not entirely bad. Skilled estheticians use a precise, delicate touch that avoids damaging the support structures and does not create friction on the skin. So having a professional perform facial massage is perfectly fine. If you use devices at home, make sure to apply only gentle, soft pressure and avoid overdoing it.
However, the simplest and most effective method for reducing puffiness and achieving a lifting effect is, surprisingly, smiling and making facial expressions. By smiling and moving your facial muscles, you can improve puffiness just as massage does, and research shows that this stimulation can also promote the production of collagen and elastin.
While purchasing a high-quality massage device is a valid option, if that's not feasible, I recommend focusing on smiling to achieve similar benefits. Massage devices are not inherently bad—it all depends on how you use them. But if you're concerned, simply smiling more, lifting the corners of your mouth, and making expressive faces throughout the day can provide both a lifting effect and promote collagen production. Try to live a life full of smiles and rich facial expressions.
As for what to do with the facial devices you already own—try using them on your neck to relieve stiffness, on your body, or to gently massage your scalp. This can help improve blood circulation, so please make use of them in those ways.
