皆さんこんにちは、内科皮膚科医の友利新です。今日はサムネイルに書いたんですけれども、美容整形暴露みたいな感じで書いています。前回、私が登録者10万人突破の時にライブをさせてもらったじゃないですか。そこで「先生は何やってるんですか」というコメントをいただいたので、「いろいろやっています」というふうに答えたら、「何をやっているのか詳しく教えてください」ということが来たので、それについてちょっと動画で詳しくお話ししようかなと思っています。
基本的に私は保険診療もしていますけれども、美容診療、美容医療にも従事していますので、養成型だったり美容皮膚科的な施術だったりというのは、むしろ推奨というかいいんじゃないかなと思っている派です。でスキンケアでできるレベルと、やっぱり美容医療に頼らないとできないことって、きちんと明確にあるんですね。例えばシミに関しても、シミがポツンと1個あったら、それはスキンケアではなくって、やっぱりレーザーだとか光治療を当てた方がいいし、あと肝斑とかもそうですよね。あとたるみとか。なので私は化粧品では予防、そして毎日の肌の健康を保つ、でやっぱりこれ治療したいよっていうのは医療に頼っているということになります。
基本的に私が一番最初に行った美容医療っていうのは全身脱毛になります。これ多分医者になってすぐだから24歳ぐらいかな。そう、24歳ぐらいの時に全身の脱毛をして、顔も手足もそうですよね、VIOも全部できるところは全部脱毛をしています。なのでお顔の脱毛に関しては、顔のくすみの改善にもなりますし、あと毛穴も目立たなくなるので、これもすごくおすすめです。脱毛っていうのは1回きちんとやるとですね、ほとんどもう生えてこないんですよ。たまにびっくりするぐらい、あれこんなところに生えてる時ありますけれども、基本的にはもう今私、カミソリとかも使わないし、脱毛を定期的にしているかというとしてないので、もう脱毛を卒業したかなっていうふうに感じています。
でもう一つ、脱毛よりも前にやってたことがあった、それがケミカルピーリングですね。私、実はすっごいニキビ肌だったんですよ。高校とか中学校の頃、同級生に聞くとみんな「あー」って思うと思うんです。本当に思春期の男子ぐらい、なんかもう本当にひどいニキビがすごいいっぱいあって、枕で寝るじゃないですか、ちょっと朝ね、血が枕についているぐらい、すっごいニキビ肌だったんですよ。実はケミカルピーリングが日本に上陸してきた時、治療が入ってきた時に、私も医学生だったので、なんかいい材料になると思ったんでしょうね。皮膚科の先生に「ちょっとニキビ治療の実験、まあ知見というんですけどね、をやってみないか」って言われて、初めて行ったのがケミカルピーリングです。でそこからやっぱり27、8歳ぐらいまでは定期的に、まあひどい時は2週間に1回ぐらい、少し落ち着いてきたなぁと思っても1ヶ月から1ヶ月半に1回ぐらいはケミカルピーリングをして、もちろん医療の場所ですよ、病院でね。それでニキビができないようになったし、たまに今もできますけど、基本的にはニキビ跡だったりとかそういったことも残らずにニキビを治療することができました。
そうだから脱毛、ピーリングも今もたまにはしますけども、もうそれは今あんまり定期的にやってるっていう感じはしないですかね。あと私がやっていることは、やっぱりシミ取りは、例えば本当にあんまり大きいシミがボーンとできることはないんですけれども、たまにあるとレーザーで焼くことはあるし、あとはもう全体的にトーンアップさせたいとか、細かいシミに関しては光治療ですね。フォトフェイシャルとか、あとはBBLって呼ばれるものとか、フォトシルクっていうような光治療で、全体的なくすみを飛ばすというのは今でも定期的に、2ヶ月に1回は必ず当てるようにしています。
でもう一つ、たるみ。やっぱり一番、たるみっていうのは皆さんいつ頃からやったほうがいいかというと、たるみを気にしてから始めたらダメなんですよ。もうたるみを気にして始めちゃったら結構ちょっと遅いというか、機械でやるのはね。なので私は30代になってからたるみ治療はもう始めたかな。たるみ治療にはサーマクールとか、ハイフと呼ばれるウルトラセルとかウルセラとかっていう機械あるんですけれども、何で分けているかというと、サーマクールってどちらかというとアンパンマンみたいな顔がちょっと脂肪が多い人は結構サーマクールがおすすめで、どちらかというとちょっと顔が脂肪があんまり大きくない、ちょっとゲッソリしてるんだけどこの辺垂れてるっていうような人は、そのウルトラセルとかウルセラとか、そういうハイフ系が効くんですね。なので私よく言うんですけど、すごいお顔が丸くてパンパンだったので、私が一番最初に受けたたるみ治療っていうのはサーマクールになっていて、それはもう30代、32歳ぐらいかなから、本当に半年に一遍ぐらい当てていて、やっぱりサーマクール当ててると毛穴もちょっとちっちゃくなるし、もちろんたるみも予防できるんですけど、こうちょっとアンパンマンみたいなこのパーツとしたものが少しずつ収まってくるんですよ。そうすると今度はこの辺が気になってくるので、今ではハイフウルトラセルとかウルセラとかも一緒にダブルで合わせています。でもこれはねだって半年に一遍です。なので妊娠中とかはもちろんしてないですけども、半年に一遍はこのあの、まあリフト系サーマクールなのかハイフなのかっていうのを必ず受けています。
でよく聞かれるのが「ヒアルロン酸をやってますか」って言われるんですが、よく言われるのがほっぺたにヒアルロン酸とか唇にヒアルロン酸入れてますかと言われるんですけど、私ね、ヒアルロン酸、頬にも唇にも入れたこと実はないんですね。そうなんですよ。で別にヒアルロン酸を否定しているわけじゃなくて、入れたことある場所が、このこめかみのところが結構ね、わかるかなちょっとえぐれてたりするんですよ。であのおでこが直線なんですね。だからおでこをちょっと丸くしたりとか、ここのそげているところにちょっとヒアルロン酸を入れると顔が立体的になるっていうことで、今はもう多分吸収されてなくなっちゃったと思うんですけど、ここにほんのちょっとだけ入れたり、あとおでこを丸くするためにほんのちょっとだけ入れたことはありますが、もう多分吸収されちゃってるかなっていう感じで。なのであんまり私、さっき言ったアンパンマン顔を気にしてたから、結構このほっぺたとかに入れるのはあんまりおすすめはしていないですし、私昔からタラコ唇だってすごい小さい頃にいじめられたから、あんまり唇をポテッとさせるのもあんまり好きじゃないので、入れるとしたらやっぱり、まあほうれい線が気になる人だったらほうれい線とかはいいと思うんですが、あんまりこのほっぺたのところはおすすめはしてないです。で私がやったことがあるのはこめかみとおでこですかね。であとなので今もヒアルロン酸は入れてないんですけれども、あと定期的にやっているのがボトックスです。
でボトックスはどこに打ってるかっていうと、まず脇。そうなんですよ、脇って汗かくので、脇にボトックスを打つと汗が止まるんですね。なのでやっぱ夏場とか汗かきたくないじゃないですか。なので定期的に脇のボトックスを注射しています。ただあの、妊娠中あと妊活しているときにはボトックス打っちゃいけないので、あと授乳中もなので、今ちょっとお休みしているので、もうそろそろちょっとボトックス再開したいなと思っています。
あと脇だけじゃなくて、今もう多分切れてるんですけど、おでこですね。ここのこうやった時にシワができるのが嫌で、妊娠前出産前っていうのはここに定期的に打ったりとか、あと笑った時にここにできるシワが嫌だなぁと思う時はここに打った時もありますが、妊娠出産があってと授乳期間もあったので、今ほとんど多分、最後に打ったのはもう1年以上前なので、もう全く切れてる状態ですけど、そろそろあそこに打ちたいなと思っている感じです。
あと今はもうかなり前になるんですけれども、やっぱりニキビ跡とか毛穴が気になっていた時には、フラクセルというレーザーを当ててた時期もあります。これね結構ダウンタイム、真っ赤になる時が多いので、なかなかその時期を選ぶんですけれども、フラクセル何回くらいだかな、多分4、5回はやったと思います。30代前半ぐらいかな、もう本当に毛穴とかも、私本当にニキビがひどかったのでニキビ跡を消したいな、特にこの辺のニキビ跡を消したいなと思って、フラクセルを多分5、6回は当てましたね。おかげで毛穴だったりあとニキビ跡は良くなったので、今はフラクセルは打ってないですし、毛穴系の施術は今はもうほとんどしていないです。
まとめると、ちょっと今からやりたいなっていうのと、今やっているものとあるんですが、ボトックスはねそろそろ本当にやりたいです、脇とあとこのここの辺。あそうだ、ボトックスは以前ね咬筋に打ってました。この噛む筋肉、このエラにボトックスを打つと、ボトックスで顔がちっちゃくなるんですよ。なので咬筋にも昔入れていて、この辺をスッキリさせたいというので入れてたんですけども、全然切れてるので、そろそろボトックス脇あと咬筋、だからエラですね、エラ。あとはこの額にボトックスを近々でいきたいなと思っています。
でその後も定期的に今やっているのは光治療ですね。私はやっぱりこうくすみを飛ばしたいので光治療、であとは1年に1回まとめて細かいシミをレーザーでシミを取ると必ずかさぶたになるので、そうするとやっぱりちょっと撮影とかがあったらちょっと難しいので、1日お休みの時にまとめて当てているレーザー、あとはたるみ予防のためにサーマクールかハイフウルトラセルかウルセラどっちかを必ず今はやるようにしています。なんでこれが今私がやっているすべてになります。はい。
「そんなにやってるの」って言うかもしれないんですけど、まあ基本でやってるんですよ皆さん。であんまりそれを言う人がいないだけで、結構私みたいに全部やってる人はいないかもしれないですけど、何かしらをやってる人は多くて、「いや私何もしてないんです、でもこんなに綺麗で」っていう人は、私はなんかちょっと違うかなぁと思って。私は努力もちろん、スキンケアもですけれども、スキンケアあってですよ。そうこれをちゃんと言わなきゃと思うんですけど、1ヶ月に1回クリニックに来るのはもうスペシャルじゃないですか。だけど日頃の毎日のスキンケア、特に日焼け止めを塗る、そして保湿をしっかりする、でビタミンCとかビタミンAとかそういったちゃんと美容成分入れるっていう日頃のスキンケアあっての美容医療なので、それを両輪でしっかり回しているから今これを保っている状態です。
でただ今、本当にメスを入れる気はまだ全然ないんですけども、これからでもそれでもゆっくりですけど絶対に年をとっていくので、今後ももしあの今やっていることでちょっと歯止めが効かないなと思ったら、いろいろな施術はしていきたいし、もしそれをした時には皆さんに話していきたいかなとは思っています。

Hello everyone, I'm Arata Tomori, a physician specializing in internal medicine and dermatology. Today, as written on the thumbnail, I'm going to talk about something like a "plastic surgery reveal." When I did a live stream after reaching 100,000 subscribers, someone commented asking "What have you been doing, Doctor?" I answered "I've been doing various things," and then someone asked me to explain in detail what I've been doing, so I decided to make a video about it.
Basically, I do insurance-based medical treatments, but I'm also involved in cosmetic/aesthetic medicine. So procedures like anti-aging treatments or cosmetic dermatology procedures are something I actually recommend and think are good. There's a clear distinction between what can be achieved through skincare and what requires cosmetic medical treatment. For example, with age spots, if you have even one distinct spot, that's not something skincare alone can fix - you really need laser or light therapy for that. The same goes for melasma, and sagging skin too. So I use cosmetics for prevention and to maintain daily skin health, but when I want to actually treat something, that's when I rely on medical treatment.
The first cosmetic medical treatment I ever had was full-body hair removal. This was probably right after I became a doctor, so around age 24. Yes, at around 24, I did full-body hair removal - face, arms, legs, and VIO area, everywhere that could be treated. Regarding facial hair removal, it also improves dullness and makes pores less noticeable, so I really recommend it. Once you properly complete hair removal, hair barely grows back. Occasionally I'm surprised to find a stray hair somewhere, but basically now I don't even use razors, and I'm not doing regular hair removal treatments anymore, so I feel like I've "graduated" from hair removal.
There was something I did even before hair removal - chemical peeling. Actually, I had really severe acne-prone skin. If you ask my classmates from high school or middle school, they'd probably go "Oh yeah..." I really had acne like a teenage boy going through puberty - so much acne that when I slept on my pillow, there would sometimes be blood on it in the morning. That's how bad my acne skin was. Actually, when chemical peeling first came to Japan as a treatment, I was a medical student at the time, and probably thought it would make good material for learning. A dermatologist asked me if I wanted to try some acne treatment as a kind of clinical experience, and that's how I first tried chemical peeling. From then until about age 27-28, I did it regularly - during bad periods, about every two weeks, and even when things calmed down a bit, about once every month to month and a half. Of course, this was at a medical facility, at a hospital. Thanks to that, I stopped getting acne, and even though I occasionally get some now, I was able to treat my acne without leaving scars.
So hair removal and peeling - I still do them occasionally now, but I don't feel like I'm doing them regularly anymore. Other things I do: for spot removal, I rarely get a really large spot suddenly appearing, but when I do, I sometimes use laser to burn it off. For overall tone-up and fine spots, I use light therapy - things like photofacial, BBL, or Photosilk-type light treatments to eliminate overall dullness. I still do this regularly now, making sure to get it done once every two months.
Another thing is sagging skin. When should everyone start treating sagging? You shouldn't wait until you're worried about sagging to start - if you start worrying about it and then begin treatment, it's kind of already too late, at least for machine-based treatments. So I started sagging treatments once I hit my 30s. For sagging treatments, there are machines like Thermage, and HIFU devices called Ultherapy or Ulsera. The way to differentiate them is: Thermage is more recommended for people whose faces are like Anpanman - a bit fuller with more fat - while Ultherapy or Ulsera type HIFU treatments work better for people whose faces don't have as much fat, more gaunt-looking but with sagging in certain areas. I often say this because my face was really round and full, so the first sagging treatment I received was Thermage, starting from my early 30s, maybe around 32, and I've really been getting it about once every six months since then. When you get Thermage, your pores get smaller too, and of course it prevents sagging, but that Anpanman-like fullness gradually goes down. Then this area starts to bother me, so now I combine both HIFU treatments like Ultherapy or Ulsera together with it. But this is only once every six months. Of course I don't do it during pregnancy, but once every six months I definitely get either the lifting treatment - Thermage or HIFU.
I'm often asked "Do you get hyaluronic acid injections?" People often ask if I get hyaluronic acid in my cheeks or lips, but actually, I've never had hyaluronic acid injected in my cheeks or lips. That's right. It's not that I'm against hyaluronic acid - the areas where I have had it are my temples, which are somewhat sunken, you can probably tell. And my forehead is quite flat/straight. So to round out my forehead a bit, and to fill in that sunken area, injecting a little hyaluronic acid there makes the face more three-dimensional. I think it's probably already been absorbed by now, but I did get a tiny bit injected there, and also a tiny bit in my forehead to round it out, but I think it's probably been absorbed already. So because I was conscious about that Anpanman-face issue I mentioned, I don't really recommend injecting into the cheeks. Also, I was bullied for having thick lips ("tarako lips") since I was very young, so I don't really like making lips puffier either. If I were to inject somewhere, for people concerned about nasolabial folds, that area would be fine, but I don't really recommend the cheek area. What I have done is temples and forehead. So I don't currently have hyaluronic acid in me, but something I do regularly is Botox.
Where do I get Botox? First, underarms. Yes, that's right - underarms sweat, so injecting Botox there stops the sweating. Especially in summer, you don't want to sweat, right? So I regularly get Botox injections in my underarms. However, you can't get Botox during pregnancy or when trying to conceive, and also during breastfeeding, so I've been taking a break, but I'm thinking it's about time to restart Botox soon.
Besides underarms, this is probably worn off by now, but my forehead too. I dislike the wrinkles that form here when I make this expression, so before pregnancy and childbirth, I used to get it injected here regularly, and also for the wrinkles that form here when I smile - I've had it injected there too when I found those wrinkles bothersome. But with pregnancy, childbirth, and the breastfeeding period, it's probably been over a year since my last injection, so it's completely worn off now. I'm thinking it's about time to get it done there again.
Also, this was quite a while ago now, but when I was concerned about acne scars and pores, there was a period when I used a laser called Fraxel. This one tends to have a lot of downtime with redness, so it's hard to choose the right timing, but I think I did Fraxel maybe 4-5 times. This was probably in my early 30s - since my acne was really severe, pores and acne scars especially bothered me, particularly in this area, so I wanted to get rid of the acne scars. I probably did Fraxel about 5-6 times. Thanks to that, my pores and acne scars improved, so now I don't do Fraxel anymore, and I've mostly stopped pore-related treatments.
To summarize, there are things I want to start doing now and things I'm currently doing. For Botox, I really want to get back to it soon - underarms and this area. Oh right, I used to also get Botox in my masseter muscle. Injecting Botox into this chewing muscle, into the jaw area, makes the face smaller. So I used to get it in my masseter to slim down this area, but it's completely worn off now, so I'm thinking of restarting Botox soon - underarms, masseter (jaw area), and I also want to get Botox in my forehead again soon.
After that, what I continue to do regularly is light therapy. I really want to eliminate dullness, so I do light therapy, and once a year I get all my fine spots removed together with laser, since laser spot removal always results in scabbing, which makes things difficult if I have photo shoots coming up, so I schedule it for when I have a day off. And for sagging prevention, I make sure to always do either Thermage or HIFU (Ultherapy or Ulsera). So this is everything I'm currently doing. Yes.
You might think "Wow, you're doing that much?" but everyone does this as a baseline - it's just that not many people talk about it openly. There might not be many people doing as much as I am, but many people are doing something. When someone says "I'm not doing anything, but look how beautiful I am," I feel like that's not quite right. Of course I put in effort, including skincare - but it's skincare PLUS this. I really should mention this properly: coming to the clinic once a month is a special treatment. But it's built on the foundation of daily skincare - especially applying sunscreen, moisturizing thoroughly, and incorporating proper beauty ingredients like Vitamin C and Vitamin A. Cosmetic medical treatment exists on top of that daily skincare foundation. I'm maintaining my current condition by properly running both of these together like two wheels.
Right now, I have no intention of going under the knife at all, but going forward, since I'll definitely continue aging slowly no matter what, if I ever feel like what I'm currently doing isn't enough to keep things in check, I'd like to try various other procedures, and if I do, I'd like to share that with all of you.
