Hello everyone, this is Tomoni Arata. Today I want to talk properly about "oral sunscreens" (drinkable UV protection supplements).
Every year around this season, I receive a lot of questions about oral sunscreens, like "Which oral sunscreen should I choose?"
But first, do you all really know what kind of effect oral sunscreens have and what they actually are? Honestly, from my perspective, the name itself is misleading—I really wish it wasn't called "sunscreen." I think many dermatologists probably feel the same way.
The reason is that many people seem to think that just by taking these supplements, some kind of power works from the inside to bounce back UV light, as if you don't need to do anything else. But that's actually not the case.
So as I always say, oral sunscreens don't actually block sunburn. If you ask what they actually do, it's more accurate to say they speed up recovery from UV damage after sun exposure, or slightly reduce the damage caused by UV exposure.
Basically, oral sunscreens contain ingredients derived from things like fern extracts or rosemary, which are marketed as "blocking sunburn from the inside." There are experimental results showing they can prevent redness after sun exposure, but when you carefully verify this, when converted to SPF, it's said to be only about SPF 1 or 2.
So it's not a lie that they provide some UV protection, but when converted to SPF, it's only about 1 or 2. When you hear that, you probably realize that SPF 1 or 2 alone can't fully protect you.
Does that mean they're completely meaningless? Not at all. Most oral sunscreens contain additional ingredients with strong antioxidant properties, such as polyphenols and vitamin A.
The biggest problem with sunburn is that it causes inflammation. When inflammation occurs in the skin, this is transmitted to skin cells—for example, to melanocytes, which then produce melanin pigment. In other words, after damage occurs to the skin, there's a whole cascade of events: the stimulus triggers melanocytes, melanin is produced, and this melanin, which starts as a brownish color, gradually darkens and gets transferred to skin cells. These oral supplements work by blocking part of this cascade.
Another factor is that UV exposure causes not just inflammation but also oxidation. This oxidative state also leads to inflammation. Antioxidant ingredients work by essentially sacrificing themselves to stop this oxidation process. This is called antioxidant activity, and representative examples include vitamin C and astaxanthin.
So essentially, oral sunscreens are meant to be an additional supplementary measure on top of topical sunscreen, hats, sunglasses, parasols, and other forms of physical UV blocking. Physical blocking, of course, means UV rays don't reach the skin at all, so the oxidation and inflammation I mentioned simply don't occur.
However, walking around in a hat, sunglasses, a mask, and long sleeves—all in black—looks a bit suspicious, right? So even with sunscreen and hats, blocking 100% of UV rays is genuinely difficult. In that case, oral sunscreens can be expected to help reduce the inflammation and skin oxidation caused by UV rays that manage to slip through your physical protection and topical sunscreen.
So it absolutely does not mean that if you just apply topical sunscreen, you won't get sunburned, won't develop spots, or your skin will become whiter. Please remember this clearly.
Because of this, I don't think there's a single dermatologist who takes ONLY oral sunscreen supplements—I'd say 99.9%, though I'll leave a small margin just in case, but realistically it's 100%. That's how misleading the name "oral sunscreen" really is.
Of course, many people do take oral sunscreens, but there's no doctor who takes ONLY oral sunscreen. Many doctors properly apply topical sunscreen, wear hats, use parasols, AND take oral sunscreen as an additional supplement. But there's absolutely no one who takes only oral sunscreen without using any other sun protection measures. Please don't make this mistake.
However, when I look at social media, I see many manufacturers marketing these products with messages like "it's okay even if you forget to reapply," "perfect for areas you can't apply sunscreen to, like behind your ears," or "great for when you go swimming and can't apply sunscreen because of the water—this oral sunscreen is for you." I think this kind of marketing is a bit misleading.
That's why I have a lot of trust in manufacturers who clearly state, "Please use this in addition to topical sunscreen, not instead of it—this is a supplementary supplement."
For example, here's a product called "White Shield" from Astalift. It clearly states "helps protect skin from UV rays." It says "oral care," but notably does NOT say "oral sunscreen."
I think Astalift is a genuinely trustworthy manufacturer. I'm sure they probably wanted to write "oral sunscreen," but because they're a company that does proper research and development, they avoided that phrasing and instead wrote "helps protect skin from UV rays"—implying that you should properly apply sunscreen, and this product helps support that. I find this to be a very conscientious approach.
This product contains the astaxanthin I mentioned earlier—of course it does, since it's from Astalift. As I mentioned, astaxanthin has very strong antioxidant properties, so it can remove the reactive oxygen species created by UV-induced damage. Since astaxanthin has high antioxidant activity for removing these reactive oxygen species, we can say this ingredient is properly included. It also seems to contain lycopene and other polyphenols with strong antioxidant properties. Of course, besides Astalift, there are other oral sunscreens—or rather, these kinds of "after-care" products—available too, but please, everyone, don't be misled by the term "oral sunscreen."
As I mentioned with the Astalift example, the real meaning is "helps support skin protection from UV stimulation." So it absolutely does not mean this product alone blocks sunburn. But that doesn't mean it's meaningless either—please apply sunscreen properly and use oral sunscreens or these kinds of "drinkable protective care" products as an additional supportive measure.
So, how was that? I'll keep repeating this message persistently every single year, so whenever anyone asks you about this, please help spread the correct message: "Of course you should apply sunscreen, and on top of that, you can also take oral sunscreen." I really appreciate your cooperation in spreading this correct information.
