そこで今日は、蕁麻疹の原因や対策、日常生活での注意点についてお話ししていきます。今悩んでいる方も、まだ一度もなったことがない方も、誰でもかかる可能性がありますので、ぜひ最後までご覧ください。
【蕁麻疹とは】
まず、蕁麻疹という漢字、皆さん書けますか?私も毎回「あれ、どう書くんだっけ」と思ってしまいます。なぜあんなに難しい漢字がついているかというと、実は「蕁麻(じんま)」という草があり、それに触れたときに肌が赤く腫れることから「蕁麻疹」という言葉が生まれたそうです。私自身はその草を見たことがないのですが、昔からある病気ということですね。診療でカルテに書くときも、蕁麻という字がなかなか思い出せず、ひらがなで書くことも多いです。
【蕁麻疹と湿疹の違い】
蕁麻疹と湿疹の大きな違いは、蕁麻疹は数時間程度でパッと消えて、跡を残さないという点です。蕁麻疹が消えた後に茶色い跡が残ったという場合は、それは蕁麻疹ではなかった可能性が高いと、私たち皮膚科医は考えます。
蕁麻疹は、何らかの原因でかゆみを引き起こす物質(ヒスタミン)が皮膚に集まり、血管が広がることで赤く腫れるものです。炎症性の色素沈着を残すようなものではありません。つまり、跡を残すかどうかが、湿疹と蕁麻疹の大きな違いです。
ヒスタミンというかゆみの物質が多く出るため、かゆみを伴うのも特徴です。蚊に刺されたときのようにぷくっと腫れて、かゆみを伴いながら数時間程度で引いていく、というのが蕁麻疹です。
【原因はアレルギーとは限らない】
蕁麻疹がパッと出ると、多くの方は「何かしらのアレルギーだろう」と思われます。外来でも「変わったものを食べた記憶はないんですけど」とおっしゃる方が多いのですが、実は蕁麻疹の原因が食物などのアレルギーであるケースは10%以下と言われています。文献によっては5%前後とも言われており、全体で見るとアレルギーが原因であることは圧倒的に少ないのです。
そのため、「何を食べたかな」と考えたり、アレルギー検査をしても、なかなか原因を特定できないことが多いです。最近は36項目を一度に調べられる血液検査もありますが、それでも何も引っかからないということがよくあります。アレルギーが原因で必ず蕁麻疹が起きるわけではないということを覚えておいてください。
【特発性蕁麻疹が圧倒的に多い】
では何が原因で起こることが多いかというと、「特発性の蕁麻疹」が圧倒的に多いと言われています。「特発性」と聞くとかっこよく聞こえるかもしれませんが、要するに「よくわからない原因で起こる」ということです。原因がはっきりしている蕁麻疹はごく少数だと思っていただいて構いません。
何ということもない日常生活の中で、パッと蕁麻疹が起こってしまうことはよくあります。ただ、話を聞いていると、ストレスが強い、疲れている、寝不足である、といったことが多い傾向にあります。
もう一つの要因は環境因子です。暖かい場所から寒い場所へ、あるいはその逆に移動した際に血管の収縮・拡張が起こり、蕁麻疹が誘発されることがあります。お風呂上がりに出る、涼しいエアコンの効いた部屋から外に出た瞬間に出る、といったように、温度差が誘発因子になることは多いです。心当たりのある方は、なるべく環境(特に温度)が急激に変わらないように気をつけていただくとよいと思います。
原因不明と言われても、食事以外に疲労や寝不足といった要因が多いので、蕁麻疹が出ている方は、数日間の自分のスケジュールやライフスタイルを振り返り、「睡眠不足が続いていないか」「ストレスが溜まっていないか」を考えてみるとよいでしょう。
【慢性化しやすい】
蕁麻疹は長引きやすく、慢性的になる方が多い疾患です。1回きりでパッと出てパッと治まることもありますが、意外と慢性化しやすいものです。慢性化しても、いずれ自然に消えることもありますが、しっかり治療していく必要があります。
【治療法】
基本的な治療は抗ヒスタミン薬(抗アレルギー剤)の内服です。血管からかゆみの原因物質であるヒスタミンが多く出るため、これを抑える薬を服用します。これはクリニックで処方されるものもありますが、花粉症用の市販薬も実は抗ヒスタミン薬であり、蕁麻疹にも効果があります。急に蕁麻疹が出て病院に行けない場合は、こうした市販薬も覚えておくとよいでしょう。
ただし、抗ヒスタミン薬には眠気を誘発するものなど種類や強さに違いがあります。市販薬で効かなかった場合でも、クリニックで処方薬をいろいろ試すことで効く薬に出会えることもあります。「以前この薬を飲んだが効かなかった」ということをきちんと伝えていただければ、別の世代の抗ヒスタミン薬に変更することができますので、どの薬を飲んで効いた・効かなかったかを覚えておくとよいでしょう。
最近では、生物学的製剤という月1回程度の注射による治療法もあります。薬剤費は高めですが、効果を実感する患者さんも多いです。長年抗ヒスタミン薬を飲んでも効果がない方は、一度試してみる価値があります。ただし実施しているクリニックが限られるため、主治医に自分が適応となるか相談してみるとよいでしょう。
これとは別に、以前はステロイドの内服も行われていましたが、副作用もあるため、現在は重症例を除いてあまり行われません。
塗り薬について聞かれることも多いのですが、蕁麻疹は全身に出ることも多く、一つひとつ塗るのは大変ですし、かゆみを抑える塗り薬と蕁麻疹を引かせる薬とでは基準が異なるため、塗り薬は第一選択にはなりません。基本的には内服薬がメインの治療になります。
【日常生活での注意点】
ライフスタイルを見直し、疲れているときや蕁麻疹が出てしまったときはすぐに薬を飲むのもよい対策です。また、蕁麻疹が出るとかきたくなりますが、かいてしまうと引っかき跡が残ってしまうことがあるため、なるべくかかないようにしましょう。
【受診時のポイント】
皮膚科を受診したときには、すでに蕁麻疹が引いてしまっていることがよくあります。症状の経過や跡が残っていないことから蕁麻疹だと判断することが多いのですが、可能であればスマートフォンなどで症状が出ているときの写真を撮っておくと、診断の助けになります。皮疹の出方によっては別の疾患の可能性もあるため、写真があると安心です。
【まとめ】
蕁麻疹はアレルギーが原因であることは少なく、原因不明であることが多い疾患です。今まで出たことがなくても急に起こることがあり、慢性化しやすい傾向もあります。長引くようであれば、早めに皮膚科を受診して診断・治療を受けましょう。治療の基本は内服薬です。原因は不明なことが多いものの、温度差、ストレス、寝不足などが誘発因子となることが多いので、なるべくライフスタイルを整えることがポイントです。

I often receive comments during live streams like "I'm struggling with hives" or "I suddenly got hives, what should I do?" Also, in my actual clinical practice, urticaria is one of the conditions I see very frequently.
So today, I'll explain the causes of hives, how to deal with them, and things to keep in mind in daily life. Whether you're currently struggling with hives or have never experienced them, anyone can develop this condition, so please watch until the end.
【What is Urticaria (Hives)?】
First, do you know how to write "urticaria" in Japanese kanji? Even I sometimes forget how to write it. The reason it has such a complex kanji is that there's a plant called "jinma" (nettle), and touching it causes the skin to turn red and swell—this is where the term comes from. I've never actually seen this plant myself, but it's a disease that has been recognized since ancient times. Even in medical records, I often end up writing it in hiragana because the kanji is hard to recall.
【The Difference Between Hives and Eczema】
A major difference between hives and eczema is that hives typically disappear within a few hours without leaving any marks. If you notice brown marks remaining after the hives have gone away, that was likely not actually hives.
Hives occur when a substance that induces itching (histamine) accumulates in the skin due to some cause, causing blood vessels to dilate and the skin to become red and swollen. It's not something that leaves inflammatory pigmentation behind. So whether or not marks are left behind is the key difference between eczema and hives.
Because histamine, the itch-inducing substance, is released in large amounts, itching is a hallmark symptom. Similar to a mosquito bite, the skin puffs up, itches, and then subsides within a few hours—that's hives.
【The Cause Isn't Always Allergies】
When hives suddenly appear, many people assume it must be due to some kind of allergy. In my clinic, many patients say "I don't remember eating anything unusual," but in fact, cases where hives are caused by food or other allergies account for less than 10%. Some literature suggests it's around 5%. Overall, allergic causes are actually quite rare.
This means that even if you rack your brain trying to remember what you ate, or get allergy testing done, it's often difficult to identify the cause. These days, there are blood tests that can check 36 items at once, but even with these, nothing often turns up. Please keep in mind that hives are not necessarily caused by allergies.
【Idiopathic Urticaria Is the Most Common】
So what causes most cases of hives? The answer is "idiopathic urticaria"—by far the most common type. "Idiopathic" might sound like a sophisticated medical term, but it simply means "occurring for unclear reasons." You can consider cases with a clearly identified cause to be quite rare.
Hives can suddenly appear even during completely ordinary daily activities. However, based on what patients tell me, high stress, fatigue, and lack of sleep are common contributing factors.
Another factor is environmental change. Moving from a warm place to a cold place, or vice versa, can cause blood vessels to constrict or dilate, triggering hives. It's common for hives to appear after a bath, or right after stepping outside from an air-conditioned room—temperature changes are frequently a trigger. If this sounds familiar, try to avoid sudden changes in your environment, especially temperature.
Even when the cause is labeled as unknown, factors like fatigue and lack of sleep are common beyond just food. If you're experiencing hives, it might help to reflect on your schedule and lifestyle over the past few days—have you been sleep-deprived? Under stress?
【Hives Tend to Become Chronic】
Hives often persist and become a chronic condition for many people. While some cases appear once and resolve quickly, it's surprisingly common for hives to become chronic. Even chronic cases can eventually resolve on their own, but proper treatment is still important.
【Treatment】
The basic treatment is oral antihistamines (anti-allergy medication). Since histamine, the itch-causing substance, is released in large amounts from blood vessels, these medications work to suppress it. These can be prescribed at a clinic, but over-the-counter allergy medications for hay fever are actually antihistamines too, and they can be effective for hives. If you suddenly develop hives and can't get to a clinic or hospital, it's worth remembering that these OTC medications can help.
However, antihistamines vary in strength and side effects, such as drowsiness. Even if an OTC medication doesn't work, trying different prescription medications at a clinic may help you find one that's effective. If you clearly communicate to your doctor which medications you've tried and whether they worked, we as dermatologists can switch you to a different class of antihistamine. It's helpful to keep track of which medications worked and which didn't.
Recently, biologic therapies—administered via monthly injections—have also become available for treating hives. While the cost is higher, many patients find them quite effective. If you've been taking antihistamines for years without success, it may be worth trying this treatment. However, since not all clinics offer this option, it's best to consult your primary doctor about whether you're a candidate.
Separately, oral steroids used to be prescribed for severe outbreaks of hives, but due to their side effects, they're generally avoided nowadays except in severe cases.
I'm often asked about topical treatments, since this is a skin condition, but since hives often appear all over the body, applying cream to each spot is impractical. Also, the standards for itch-relief creams differ from those for medications that resolve hives, so topical treatments are not the first choice. Oral medication remains the primary treatment.
【Tips for Daily Life】
Reviewing your lifestyle and taking medication promptly when you're fatigued or when hives appear can be helpful. Also, while it's tempting to scratch when you have hives, scratching can leave marks from the scratching itself (rather than from the hives), so try to avoid scratching as much as possible.
【Tips for Visiting a Dermatologist】
Often, by the time patients come to see me, their hives have already subsided. While we can usually diagnose hives based on the patient's description of symptoms and the absence of marks, if possible, it's very helpful to take a photo with your smartphone while the hives are visible. Depending on how the rash appears, this can be crucial for accurate diagnosis, so taking photos is highly recommended for peace of mind.
