皆さん、こんにちは。とも美新田です。 今日は緊急でこの動画を撮っています。というのも、悲しい書き込みを見てしまったからです。 私は昔から顔が丸いのが悩みで、アンパンマンのように顔がパンパンになりやすく、そのためにレーザー治療などを受けてきました。ところが先日、「とも美先生、顔がパツパツすぎる。ヒアルロン酸を入れすぎていて、まるで薬の副作用みたいだ」という書き込みを見てしまいました。 正直、少し悲しい気持ちになりました。もちろん私自身の顔立ちがそういうタイプだということは自覚していますし、ヒアルロン酸の注入も症例によっては必要な施術として行っています。ただ、「薬の副作用みたいにパンパン」と言われるのはちょっとショックでした。 そこで今日は久しぶりに「引き締め」の施術を受けようと思います。最近は「引き上げ」のリフトアップ系の施術(ハイフなど)が多かったのですが、今日はしっかりお肉を引き締めるサーマクールを受けてみようと思います。 **サーマクールとは** サーマクールは、熱を与えることで脂肪層まで働きかける施術です。個包装のアメの包装を全部外して、ひとつにギュッと固めるようなイメージで、脂肪の塊を小さくしていきます。 リフトアップ(ハイフなど)とサーマクールの一番の違いは、熱を与える層の深さです。 - リフトアップ系は、皮膚の奥にあるSMAS(表在性筋膜)という層に熱を加えます。SMASがキュッと引き締まることで、フェイスラインのもたつきが持ち上がります。 - サーマクールは、真皮層から脂肪層にかけて熱が加わります。真皮層にはコラーゲンやエラスチンがあるので、それを収縮させる効果があり、さらに脂肪層にも働きかけるので脂肪そのものを小さくする効果が期待できます。 つまり、フェイスラインのもたつきが気になる方はリフトアップ系、顔全体を小さく引き締めたい方はサーマクールが向いています。 さらにサーマクールには、皮脂腺の働きを抑制する効果もあります。私のように脂性肌でテカリやすい人には、小顔効果に加えて、毛穴の引き締めやテカリの改善も期待できます。 **施術の様子** では、施術に行ってきます。 (施術前) このあたりがまだもったりしている感じがするので、この部分を照射していきます。 (着替え・準備) 本当は顔全体に麻酔クリームを塗るのですが、時間がなかったので今回は一部だけ塗ってもらいました。 近藤先生、今日もよろしくお願いします。「ヒアルロン酸を入れすぎているみたいで顔がパツパツ」と言われてしまったので、引き締めのためにサーマクールを久しぶりに受けたいんです。 近藤先生:「もともとそれはお顔立ちですから、パンパンに腫れているという感じは全くないと思いますよ。ただ、定期的にサーマクールをされると、4〜5年後にはかなり差がついてくると思います。」 (麻酔クリームを塗布) 近藤先生:「その傷はどうされたんですか?」 私:「これは子供の頃にぶつけた傷なんです。」 近藤先生:「そうだったんですね。」 (施術開始) ドキドキしますね、久しぶりなので。 近藤先生:「では、まいります。」 (1発目照射) 私:「あ、大丈夫です。そうだった、そうだった。これ、焼かれる感じなんですよね。」 熱いというよりは、奥のほうで焼かれるような感覚です。皮膚の奥にある筋肉の上の筋膜に熱が加わることで、ぎゅっと収縮していくのが分かります。 近藤先生:「口の上は、将来的に縦じわが出るのを予防するために、あらかじめ照射しておきますね。これで3分の1終わりました。左右、見てみましょうか。このあたりがしっかり引き締まって、頬骨のあたりも上がってきていますよ。」 洗顔した時に左右の頬の高さが違うのがよく分かる、と言われました。若干こちら側の方がこけている部分もありますが、これをやることで全体がふっくらと上に持ち上がってくるとのことです。 (施術中の会話) 近藤先生:「最近はハイフが人気ですけど、サーマクールも根強い人気がありますよ。ハイフとは引き締める層が全然違うので、使い分けが大事です。」 私:「サーマクールは若い方にも向いていますか?」 近藤先生:「そんなにたるんでいない方には、引き締め目的でサーマクールがいいと思います。ハイフやウルセラは引き上げがメインなので、まだたるみがない若い方には、引き締めのサーマクールの方が向いていると思いますね。」 (施術は半分終了) 私:「なんだかやれる気がしてきました。」 近藤先生:「熱を上げれば上げるほど効果は高くなりますが、その分痛みも伴います。サーマクールの基本は、ギリギリ我慢できる痛みまで温度を上げることなんです。だから全く痛みを感じないサーマクールは、正直あまり意味がないと思います。逆に痛みが分からないと火傷のリスクもあるので、『痛いけど耐えられる』くらいがちょうどいいんです。奥には80度近い熱が入るので、ある程度の痛みは伴いますね。」 私:「全く痛くないわけではないですが、『もうやめて』というほどではなく、ギリギリ耐えられる痛みです。」 近藤先生:「では、もう少し温度を上げていきましょう。」 (施術終了) 近藤先生:「お疲れ様でした、終わりましたよ。」 私:「正直、圧倒的に痛くなかったです。新しい機械は本当に痛みが少なくて、施術も早いですね。先生が配合や設定をしっかり分かってやってくださるので、もう引き締まった感じがします。」 **施術直後** このあたりがシュッと引き締まった感じが自分でもします。これが施術直後の状態です。ただ、今日から1〜2ヶ月かけてじわじわと効果が出てくるので、1ヶ月後がとても楽しみです。 **1ヶ月後** サーマクールを受けてから1ヶ月が経ちました。 自分としても、このあたりがスッキリしたように感じます。皆さんにも分かりますでしょうか? 顔を洗った時に頬に触れる位置が変わったのが、自分では一番分かりやすい変化でした。手のひらが当たる頬の位置が明らかに高くなっているんです。それだけ顔全体がスッキリと引き締まったのかなと思います。 あと、フェイスラインのあたりもスッキリしたと感じています。もちろんこれからもう少し引き締まっていくと思いますし、もともと顔が丸いタイプなので劇的にシャープになるわけではありませんが、自分としてはかなり満足しています。 もう一つ嬉しかったのは、ハイフが「引き上げ」であるのに対して、サーマクールは「引き締め」に加えて、皮脂腺にも働きかけてくれる点です。テカリを抑えたり、毛穴を引き締めてくれる効果もあるので、肌の調子がとても良くなりました。撮影なのでファンデーションは塗っていますが、最近はメイクさんからも「ファンデーション薄くても大丈夫そうですね」と言われることが増えました。これは嬉しい誤算でした。 今はハイフが大人気で、猫も杓子もハイフという感じですが、私のように顔が丸くて脂性肌、毛穴が気になるタイプの方には、サーマクールもとても良い施術だと思います。私自身、最近顔のパツパツ感が気になっていたので、受けてみて本当に良かったです。 サーマクールとハイフの違いや、それぞれの効果について疑問に思っていた方の参考になれば嬉しいです。 いかがでしたでしょうか。今回はクリニックでの施術体験をご紹介しましたが、普段はコスメや美容情報、健康情報を発信していますので、良かったらチャンネル登録をお願いします。 では、また次の動画でお会いしましょう。さようなら。

Hello everyone, this is Tomomi Nitta. Today I'm filming this video urgently because I came across a sad comment. I've always struggled with having a round face — like Anpanman, my face tends to look puffy — and because of this, I've had laser treatments and other procedures over the years. But recently, I saw a comment saying, "Tomomi-sensei, your face is way too puffy. You must be injecting so much hyaluronic acid that it looks like a side effect of medication." Honestly, that made me a little sad. Of course, I know that my facial structure is just naturally like this, and I do get hyaluronic acid injections when needed as part of certain treatments. But being told it looks like "a medication side effect level of puffiness" was a bit shocking. So today, I decided to get a "tightening" treatment for the first time in a while. Lately I've mostly been doing "lifting" treatments like HIFU, but today I want to try Thermage, which really focuses on tightening the tissue. **What is Thermage?** Thermage is a treatment that applies heat that reaches down to the fat layer. Imagine unwrapping individually wrapped candies and compressing them all into one — that's kind of the image of how it shrinks fat deposits. The main difference between lifting treatments (like HIFU) and Thermage is the depth of the layer being targeted with heat. - Lifting treatments target the SMAS (superficial musculoaponeurotic system) layer deep beneath the skin. When the SMAS tightens, it lifts sagging along the jawline. - Thermage targets the dermis and fat layer. The dermis contains collagen and elastin, which contract with heat, and the fat layer itself is also affected, helping to reduce fat volume. In short, if you're concerned about sagging along the jawline, lifting treatments are better suited. If you want to make your whole face smaller and tighter, Thermage is more appropriate. Thermage also has the effect of suppressing sebaceous gland activity. For people like me with oily skin prone to shine, this means it can help not only with facial slimming but also with tightening pores and reducing shine. **The Treatment** Now let's go ahead with the treatment. (Before treatment) This area still looks a bit puffy, so I'll have this part treated. (Getting ready) Normally numbing cream is applied to the whole face, but since we didn't have much time today, only part of my face was treated with it. Dr. Kondo, thank you for having me again today. Since I was told my face looks puffy "like I've had too much hyaluronic acid," I wanted to get Thermage done for tightening. Dr. Kondo: "That's really just your natural facial structure — I don't think it looks swollen at all. Me: "It's not completely painless, but it's not unbearable either — right at the edge of what I can tolerate." Dr. Kondo: "Then let's raise the temperature a bit more." (Treatment complete) Dr. Kondo: "Great job, we're all done." Me: "Honestly, it was far less painful than I expected. The new machines really do hurt less, and the treatment goes by quickly too. Since the doctor really understands the settings and technique, I can already feel my face tightening up." **Right After the Treatment** I can really feel this area looking tighter and more defined. This is what it looks like right after treatment. The results will gradually develop over the next one to two months, so I'm really looking forward to seeing how it looks after a month. **One Month Later** It's been a month since I got Thermage done. I feel like this area has become noticeably slimmer. I wonder if you all can tell the difference too? The clearest change for me was noticing where my cheek touches my hand when washing my face — the point of contact is now clearly higher than before. That tells me my whole face has become tighter and more lifted. I also feel like my jawline area has become more defined. Of course, there's likely still more tightening to come, and since I naturally have a round face, it's not like it's become dramatically sharp — but overall, I'm very satisfied with the results. One more thing that made me happy: while HIFU is about "lifting," Thermage is about "tightening" — plus it also affects the sebaceous glands. It helps control shine and tighten pores, so my skin condition has improved a lot. I am wearing foundation for filming, but lately my makeup artist has even told me, "You probably don't even need much foundation anymore." That was a nice surprise. HIFU is incredibly popular right now — it feels like everyone's doing it. But for people like me with a round face, oily skin, and visible pores, Thermage can be a wonderful treatment option too. I'm really glad I got it done, especially since I had been self-conscious about my puffy-looking face lately.
