【コロナ対策】家庭でできる重症化予防

今日も皆さんが気になっているコロナウイルス感染症についてのお話をしたいと思います。
巷では感染予防ということが言われていますけれども、科学的にエビデンスがあるもの、一番はやはり手洗いになります。
もうこれはね、再三うちの動画でもお話ししていると思うんですが、それとは別に意外と皆さんが盲点になっているもう一つの系があるんです。口の中のケアになります。
2019年、日本大学歯学部の研究員の報告なんですけれども、適切な口腔内のケアをすることで、歯周病菌が低下するとインフルエンザのウイルス量も減少するということが分かっているんです。
これは口腔内のケアを怠るとですね、歯周病を起こす細菌が増えてしまいます。歯周病菌というのは、プロテアーゼというタンパク分解酵素を出します。これは口の中の粘膜を破壊してしまうんです。
口の中の粘膜というと、ウイルスが入ってきたときに一番最初の防御壁になる場所になるんです。なので、この粘膜が破壊されてしまうと容易にウイルスが入ってきて付着しやすくなってしまうんです。これによって少量のウイルスでも感染が引き起こされてしまう可能性が高くなるんです。
また、歯周病菌が発生するプロテアーゼというものを誤って飲んで肺の中に入ってしまうと、肺胞という肺の器官も壊してしまいますので、さらに肺炎のリスクが高くなってしまいます。
また、東京医科歯科大学のグループの研究によると、口腔内のケアを老人ホームで比べてみたところ、実は口腔内のプロである歯科衛生士さんが口腔内のケアをしたグループの方が、インフルエンザの発症が10分の1に軽減したという報告もあるんです。
このようにインフルエンザを軽減させるということは、コロナウイルスも軽減させる可能性はとても大きいと思われます。
実はこの歯周病菌というのは血流に乗って全身に広がっていきます。そして糖尿病だとか動脈硬化をさらに悪化させる要因になるということが近年わかってきているんです。
最近話題になっているコロナウイルス、80%の人は軽症で済むんですが、残りの20%の人が残念ながら悪化すると言われています。そしてこの悪化する、重症化する引き金となるものが年齢もありますけれども、持病持ちの方ということは皆さんご存知ですよね。
この持病の多くが糖尿病だとか動脈硬化を伴う疾患だと言われています。先ほどもお話しした通り、歯周病菌というのは全身を流れて糖尿病やこの動脈硬化を悪化させる原因の一つにもなります。
なので、もともと持病がある方も歯周病が悪化することで、さらにこの糖尿病だとか動脈硬化が悪化してしまうということにつながるのです。そこにコロナウイルス感染症が入ってきてしまうと、通常であれば軽症で済むものも重症化しやすくなります。
ですので、健康な人もウイルスが増殖しないためにも口腔ケアは大切ですし、持病がある方はその持病を悪化させないためにも口腔ケアを怠らないようにしましょう。

Hello everyone. I'm Arata Tomori, an internal medicine and dermatologist doctor.
Today as well, I'd like to talk about the coronavirus infection that everyone is concerned about.
It's often said that infection prevention is important, and the one measure with the most scientific evidence is definitely handwashing.
I've talked about this repeatedly in my videos, but aside from that, there's another area that surprisingly many people overlook—oral care.
According to a 2019 report by a researcher at the Nihon University School of Dentistry, proper oral care reduces periodontal disease-causing bacteria, which in turn has been found to reduce influenza virus levels.
When oral care is neglected, bacteria that cause periodontal disease increase. These periodontal bacteria release an enzyme called protease, which breaks down protein.
This protease destroys the mucous membrane inside the mouth. The oral mucous membrane serves as the first line of defense when viruses enter the body. So when this membrane is damaged, viruses can easily enter and attach more readily. This increases the likelihood that infection can occur even with a small amount of virus.
Also, if the protease produced by periodontal bacteria is accidentally aspirated into the lungs, it can damage the alveoli (the air sacs of the lungs), further increasing the risk of pneumonia.
Additionally, according to research by a group at Tokyo Medical and Dental University, when comparing oral care in nursing homes, it was found that the group whose oral care was performed by dental hygienists—oral care professionals—had a tenfold reduction in influenza incidence.
Since reducing influenza in this way is possible, it's highly likely that oral care could also help reduce coronavirus infection.
Actually, these periodontal bacteria travel through the bloodstream and spread throughout the body. In recent years, it has become clear that they are a factor that worsens diabetes and arteriosclerosis.
With the coronavirus that's been in the news lately, 80% of people experience only mild symptoms, but unfortunately the remaining 20% become severely ill. As you all know, the triggers for this severe progression include age, but also having underlying health conditions.
Many of these underlying conditions are said to involve diabetes or arteriosclerosis-related diseases. As I mentioned earlier, periodontal bacteria travel throughout the body and can be one of the causes that worsens diabetes and arteriosclerosis.
So, for people who already have underlying conditions, worsening periodontal disease can further aggravate diabetes and arteriosclerosis. When coronavirus infection occurs on top of this, cases that would normally be mild can become more likely to turn severe.
Therefore, oral care is important for healthy people too, in order to prevent the virus from multiplying, and for those with underlying conditions, please don't neglect oral care in order to avoid worsening those conditions.
