皆さんこんにちは。内科皮膚科医の友利新です。今日はですね、今年の夏は心配なマスクによるシミの発生についてのお話をしようと思っています。そう、今年は怖いマスク色素沈着です。もちろんシミの原因というのは基本的には紫外線になります。紫外線が皮膚に当たることによって、皮膚の細胞のDNAが傷つかないように、肌の細胞がですね、メラノサイトという色素細胞に「ちょっと傷がつきそうだから、ちょっと助けてって」言うと、そのメラノサイトが、メラニン色素という色素を出して、肌の細胞のこれが核。DNAが入っている核としたら、この上にですね、日傘を差すように、メラニン色素を置くっていうものが、まずスタートになります。これがずっと定着してしまうことがシミになるんですね。なので、まずスタートは紫外線が当たって、肌に炎症が起こった。大変だっていうことがポイントなんですよ。なので、シミというのはそれが原因なんですね。 じゃあマスクをしていたら、なんでシミになるの?だってマスクしていると、紫外線をブロックしてくれているような気がするから、シミができることって少ないんじゃない?って思っている方、いらっしゃると思うんですが、それは大きな間違いです。肌というのは以前ね、動画でも言いました、バリア機能というものがありますよね。バリア機能はどういったものでなっているかというと、角質という細胞があって、そこに皮脂があって、それでバリア機能というものがなっています。バリア機能があると外的刺激も入ってこないし、あと内側の水分も出ていかないということがあるんですね。なんですけども、マスクをずっとしていると、実は自分が出ている呼気、息ですよね。その中に水蒸気、水分が含まれています。これ一瞬だけだったら、肌を潤しているような感じになるんですけれども、それがずっとある状態だと、要はお水の中にずっと手を入れておくような状態になります。そうすると手にシワシワっと、ふやけてきますよね。このふやけた状態ってどういうことかというと、この角質と角質、これをブロックだとすると、その間を埋めている、セメントの役目をしている、細胞間脂質と呼ばれるものがあるんですが、それが水にふやけることによって、この細胞間脂質というのが流れ出てしまうんですよ。細胞間脂質が流れ出てしまうと、角質と角質の細胞がくっつけなくなるので、バリア機能自体がだんだん低下していって、隙間が開くわけですから、その内側の水分がどんどん出ていきやすくなってきて、乾燥肌になってきます。 皮膚というのは乾燥すると炎症を起こしてしまうんですね。そこに物理的なマスクの摩擦という炎症がまた加わります。なのでバリア機能が低下して炎症が起こる、プラスマスクの物理的な炎症。これは日焼け後起こっているのと同じ炎症だと肌は認識するんですね。そうすると肌を守らなくちゃといって、やはり同じようにメラニンを作ってしまうことになるんです。 もう一つ今話しているシミというのは老人性色素斑といって、皆さんができるシミですけれども、肝斑といってアジア人の女性に多くて、女性ホルモンの影響を受けやすいと言われている、よく左右対称にできるベタっとあるようなシミがあるんですけれども、この肝斑というのは紫外線の影響もあるんですけれども、実は物理的な摩擦によっても悪化するというふうなことが分かっています。なので元々肝斑がある人はちょうどマスクをするあたりに肝斑ある人多いですから、その乾燥だけじゃなくて摩擦によってまた肝斑も悪化するし、肝斑というのは意外と眠っているというか、表に出てきていない方も大勢いるんですね。なんですが常に摩擦を起こることで、肝斑が起きてしまうというか表面に出てきてしまうという方も、今年は多いんじゃないかなと心配しています。 なので今年はよりシミができやすい状態なんだということを念頭に置いて、日焼け止めをしっかり塗るということはもちろんですけれども、スキンケアの中に炎症を抑えるという効果のものを入れ込んでほしいんですよ。今年はマスクもしているしあまり外出をしないから、美白ケアはしなくていいよと思っているかもしれないんですが、美白と書いてある化粧水だったり美容液だったりという美白ラインというものには、だいたいこの抗炎症作用のある、例えばビタミンCなのかトラネキサム酸なのか、そういったものが何かしら入っています。あとはメーカーさんの秘伝のタレの抗炎症作用があるものが必ず入っているんですね。 なので今年の夏こそ必ず美白ケアのものを何かしら取り入れていただければいいかなと思います。美白ケアのおすすめの美容液だったりは、以前の動画でも出ていますので、そちらを参考にしていただければいいかなと思っています。 まとめると今年は日焼けだけじゃなくて、マスクによるバリア機能低下の炎症、そしてマスクの摩擦による炎症で、シミ・肝斑が悪化しやすい。なので今年こそ必ずスキンケアに美白ラインを置くようにしてください。以前このチャンネルでもマスクが感染対策のためには有効なんじゃないかという動画を上げました。その時にはもちろん感染がどんどん増加している状態だったんですが、今はそれとはまたちょっと違うフェーズに入ってきているので、必要な時と必要じゃない時というのをご自身でしっかりと判断をして、こまめに感染が飛沫を飛ばしたりしないなとか、飛沫が飛んでこないなという場所では、こまめに外して肌のためにはこまめにマスクを外すということも必要なんじゃないかなと思っています。 いかがでしたか?このチャンネルではこのように美容や健康について皆さんに、わかりやすくお話をしているチャンネルになりますので、よかったら登録してみてください。では次の動画でお会いしましょう。さようなら。チャンネル登録よろしくお願いします!

Hello everyone. I'm Arata Tomori, an internal medicine and dermatologist. Today, I'd like to talk about mask-induced hyperpigmentation, a concern for this summer. That's right, mask pigmentation can be scary. Of course, the main cause of blemishes is ultraviolet radiation. When UV rays hit the skin, skin cells recognize the risk to their DNA and signal melanocytes, the pigment cells, saying "we need help." In response, melanocytes produce melanin pigment and place it like an umbrella over the cell nucleus containing DNA. This is the first step. When this persists and becomes fixed, it forms a blemish. So, the starting point is UV exposure causing skin inflammation, which is the key point. That's why blemishes are caused by UV rays. So why do you get blemishes when wearing a mask? You might think that since the mask blocks UV rays, you shouldn't get blemishes. But that's a major misconception. As I've mentioned in previous videos, skin has a barrier function. This barrier function is composed of the stratum corneum cells and sebum, which provides protection. With a healthy barrier function, external irritants cannot penetrate, and internal moisture doesn't escape. However, when wearing a mask continuously, your breath contains water vapor and moisture. If this were temporary, it would seem to moisturize your skin. But in a continuous state, it's like keeping your hand submerged in water all the time. This causes the hand to become wrinkled and swollen. What does this swelling mean? Between the stratum corneum cells, there are intercellular lipids that act like cement. When these lipids become swollen with water, they wash away. When intercellular lipids are lost, the stratum corneum cells can no longer stick together, so the barrier function deteriorates and gaps form. As a result, internal moisture escapes more easily, and you develop dry skin. When skin becomes dry, it becomes inflamed. Add to this the physical friction from the mask, which causes additional inflammation. Your skin recognizes this as the same inflammation that occurs after sunburn. Then, to protect itself, your skin produces melanin in the same way, causing blemishes. Another type of blemish I want to mention is senile lentigines, which is the common blemish. But there's also melasma, which is common in Asian women and easily influenced by female hormones. It appears as flat, symmetrical patches on both sides. Melasma is influenced by UV radiation, but it's also known to worsen due to physical friction. So people who already have melasma often have it right where the mask sits, and not only will the dryness make it worse, but the friction will also worsen the melasma. Many people have latent melasma that hasn't surfaced, but continuous friction can activate it and bring it to the surface. I'm concerned that more people might experience this this year. So, keeping in mind that this year is more prone to blemish formation, you should not only apply sunscreen thoroughly, but also incorporate anti-inflammatory ingredients into your skincare routine. You might think that since you're wearing a mask and not going out much this year, you don't need whitening care. But whitening skincare products like toners and serums contain anti-inflammatory ingredients such as vitamin C or tranexamic acid. Manufacturers also include their secret ingredient with anti-inflammatory effects. So this summer, be sure to incorporate some whitening care product into your routine. I've mentioned recommended whitening serums in previous videos, so please refer to those. In summary, this year, blemishes and melasma are more likely to worsen not only from sun exposure, but also from barrier function degradation caused by mask-wearing and friction from the mask itself. So this year, be sure to add a whitening skincare line to your routine. Previously, I posted a video on this channel about how masks might be effective for infection control. At that time, infections were rapidly increasing. But now we've entered a different phase, so you should make your own judgment about when masks are necessary and when they're not.
