皆さんこんにちは、友利新です。急に寒くなってますよね。秋がなくなって急に寒くなって、寒暖差が激しくなってくると、やっぱり起こってくるのは体調不良ですよね。周りにも風邪をひいていたりとか体調不良になっちゃう方が多くて、よく聞かれるのが「お医者さんって風邪ひいた時に何してるんですか」っていうところ。なので今日は私たち医師が風邪をひいた時にやることとやらないことっていうのをしっかりとお話ししていきたいなと思います。ぜひ最後までご覧ください。
風邪っていうのはウイルスなんですよね。コロナとかもコロナウイルスじゃないですか。ばい菌が入ってなるっていうよりはウイルスになるんですね。風邪のウイルスっていっぱいあって、喉が痛くなったりとか鼻水が出たりとか、悪寒がして熱が高くなったりっていうようなことになります。
風邪を一発で治す薬っていうのは存在しないんですね。風邪を一発で治しますよっていうような動画があったりとか薬があるって言われたら、それ嘘だと思っていただいていいです。風邪薬一発で治るものが出たらノーベル賞って言われるぐらい、本当に難しいんですよ。今日お話しすることっていうのは、これをやったからといってピタッと治るわけじゃないっていうところだけ覚えておいてください。
では私たち医者がですね、風邪をひいた時にやらないこと。それは抗生物質を飲むことです。抗生物質出すこともあるんですよ。飲むこともあると思います。だけれど基本的にはすぐ抗生物質飲もうっていう風に医者はならないです。最初にお話ししたように風邪っていうのはウイルスから起こるんですよ。抗生物質は細菌を殺すものになります。飲んだとしてもウイルスには効かないんですよ。逆に腸内環境荒らしてしまったりとか、細菌が入って肺炎とかになった時には抗生物質を乱用しちゃうと効きづらくなっちゃうというところもあるので、風邪では抗生物質は飲まないです。
患者さんにも逆に出したくないなって私は思ってます。だから患者さんが「風邪ひいたんで抗生物質を出してください」って言うと、私は結構断ります。出してた時期もあるんですが、それはウイルス感染がひどくなった後に出してた時期はあるんですが、耐性菌といって効かなくなるような菌を生まないように、腸内細菌のことだったりとかを考えて、基本的には出さないという方向になっているので覚えておいていただきたいなと思います。
私たち医者が風邪をひいた時にやらないこと、もう一つが総合感冒薬、いわゆる風邪薬を飲むことです。これも意外かと思うんですけども、風邪薬って風邪を治しているわけじゃなくて、風邪から起こる諸症状を抑えるんですよ。鼻づまりとか喉が痛いとか咳が出るとか、それを総合的に抑えているんですよね。もちろん風邪薬飲んでる方もいらっしゃるとは思うんですが、風邪薬としてのパッケージというよりは、風邪の中でも喉が痛いなと思ったら喉の炎症を抑えるお薬だったりを飲むとか、あと咳がひどいなと思ったら咳を止めるお薬を飲むとか、それぞれに合ったお薬を飲むようにするんですよ。それぞれの症状のお薬は飲みます。だけれども総合感冒薬みたいなものっていうのは基本的には飲まないですよね。覚えておいていただけるといいかなと思いますし、風邪薬っていうのは根本的に治しているわけじゃなくて対症療法なんですよ。だから万遍なくの対処じゃなくてピンポイントの対処の方がしっかりと効いていくので、熱が辛いんだったら熱を下げるお薬を飲んだ方がいいですし、喉が痛いっていうのが辛いんだったら喉の炎症を取るようなお薬を飲むっていう方がいいので、絶対風邪薬を飲まなくちゃいけないっていうふうには思わないでいてほしいなと思います。
続いて私たち医師が風邪をひいてやらないこと、3つ目は点滴です。風邪ひいてだるいんで点滴をちょっと打ってほしいっていう方もいらっしゃるんですけれども、点滴っていうのも有効な場合があります。それはどういう時かというと、すっごく熱が高いとかお口からお水を飲むことができない状態で脱水になっているという時には点滴をすることがあるんですけれども、点滴っていうのは体の水分バランスを整えているっていうところになっていて、点滴をした方がすぐに効いていくみたいなイメージがありますけれども、自分で呼吸できるっていう人はお水をしっかり飲んだりとか、経口補水液だったりとか糖分が入っているスポーツドリンクみたいなものを飲むっていうところでも脱水っていうのは抑えられますので、点滴をするっていうことはないですね。
ということで先にやらないことをお話ししたんですが、正直風邪ってほっとけば治るんですよ。この薬を飲んだから治ったみたいに思いがちなんですけれども、風邪の動画も出したので見ていただければいいと思うんですが、大体1週間から2週間ぐらいすれば症状って収まっていくので、対症療法することによって自分の免疫で治していくっていうところになるので。
私たち医者が風邪をひいた時にやること、ズバリ睡眠をしっかりとることです。風邪っていうのは自分の免疫力で退治するしかないんですよね。なので風邪をひいたらゆっくり寝る、これが一番風邪を早く治す方法だと思います。お医者さんの中には忙しいから無理してっていう方もいらっしゃると思うんですが、食べれるんだったら食べた方がいいですし、しっかりと水とかも飲んで温かくして体温を上げてゆっくりするっていうことが一番の治療法っていうことを私たちはわかっているので、とにかく寝るっていう、ゆっくりするっていうところになると思います。
あとは咳がひどい時には蜂蜜っていうのも実は有効だって言われていて、咳をする方に咳止めと蜂蜜を舐めさせたところ、実は咳を止めるっていうのはあんまり有意差がなかったっていうようなデータもあるぐらいなので、もちろん1歳以下のお子さんには絶対舐めさせちゃいけないんですけれども、咳がひどかったら蜂蜜を舐めてみてくださいみたいなことを言う先生もいらっしゃいますし、私自身もこの時期になるとマヌカハニーっていうものを舐めますけれども、ある程度有効なんじゃないかなと思うので、しっかりとやっていただければかなと思います。
あとゆっくり体温を上げて寝てくださいって言ったんですけど、体温を上げるにはやっぱり漢方薬って有効だったりするんですね。葛根湯っていうものはエフェドリンと言って交感神経を活発にして体温を上げていくというところがあるので、風邪の引き始めに葛根湯を飲むっていうのはそれで理にかなっているんですよ。逆に熱が上がりすぎちゃったら葛根湯ってあまり意味がないので、引きそうかもっていう時に温かいお湯と一緒に葛根湯を飲むっていうのは私たち自身は結構やってたりはします。私も常備していて、体が寒いなぁと思った時には飲むということをしているので、葛根湯は常備しておくっていうのもいいんじゃないかなというふうに思います。
今日は私たち医師が風邪を引いた時にやらないこととやることっていうのをご紹介してみました。風邪っていうのは万能薬はございません。一発で治るっていう方法はなく、自分の免疫で打ち勝つしかないのでゆっくり休むことなんですね。風邪かなと思ったら体を温めてゆっくり寝て、余計なお薬は飲まない。必要だったらピンポイントのお薬を対症療法として飲む。もちろん本当に熱が高いとか別の症状ができたら必ず病院を受診してください。

Hello everyone, this is Arata Tomori. It's suddenly gotten cold, hasn't it? With autumn seemingly disappearing and the sudden cold, as temperature fluctuations become more severe, what tends to happen is people getting sick. I see a lot of people around me catching colds or feeling unwell, and I'm often asked, "What do doctors actually do when they catch a cold?" So today, I want to thoroughly explain what we doctors do and don't do when we catch a cold. Please watch until the end.
Colds are caused by viruses, you know. COVID is also a coronavirus, right? It's not so much about germs getting in as it is about viruses. There are many cold viruses, and they cause things like sore throats, runny noses, chills, and high fevers.
There's no medicine that cures a cold in one shot. If you see a video or hear about a medicine claiming to cure a cold instantly, you can consider that a lie. If a cold medicine that cures instantly were ever developed, it would be Nobel Prize-worthy — it's genuinely that difficult. Please keep in mind that what I'm about to talk about today won't make your cold disappear instantly just by doing it.
So, the first thing we doctors don't do when we catch a cold is take antibiotics. We do prescribe antibiotics sometimes, and we do take them ourselves at times, but basically, doctors don't jump to "let's take antibiotics right away." As I mentioned earlier, colds are caused by viruses. Antibiotics work by killing bacteria. Even if you take them, they won't work against viruses. On the contrary, they can disrupt your gut environment, and if bacteria enter and cause something like pneumonia, overusing antibiotics can make them less effective. So we don't take antibiotics for colds.
As a doctor, I'd actually rather not prescribe them to patients either. So when a patient says, "I have a cold, please give me antibiotics," I usually decline. There was a period when I did prescribe them, but that was after a viral infection had worsened. Considering the risk of creating resistant bacteria — bacteria that no longer respond to antibiotics — and the impact on gut bacteria, the general policy now is not to prescribe them. Please keep that in mind.
Another thing we doctors don't do when we catch a cold is take general cold medicine. This might surprise you, but cold medicine doesn't actually cure the cold — it suppresses the various symptoms caused by the cold. It comprehensively suppresses things like stuffy nose, sore throat, and coughing. Of course, some people do take cold medicine, but rather than taking an all-in-one package, if you have a sore throat, you should take medicine that reduces throat inflammation, and if you have a bad cough, take medicine that suppresses coughing — matching the right medicine to each symptom. We do take medicine for each specific symptom, but we generally don't take all-in-one cold medicines. Please remember this — cold medicine doesn't fundamentally cure anything; it's symptomatic treatment. So rather than a blanket approach, targeted treatment works more effectively. If a fever is bothering you, it's better to take fever-reducing medicine, and if a sore throat is bothering you, it's better to take medicine that reduces throat inflammation. So please don't think you absolutely must take cold medicine.
Next, the third thing we doctors don't do when we have a cold is get an IV drip. Some people want an IV because they feel sluggish from a cold, but IVs are only effective in certain situations — specifically, when you have a very high fever or can't drink water orally and are becoming dehydrated. An IV works by adjusting your body's fluid balance. While there's an image that IVs work instantly, if you're able to breathe normally on your own, drinking plenty of water, oral rehydration solution, or sugar-containing sports drinks can also prevent dehydration. So we don't get IV drips.
Now, having talked about what we don't do, honestly, colds heal on their own if you leave them alone. People tend to think "I got better because I took this medicine," but as I mentioned in my other video about colds, symptoms usually subside within about one to two weeks. Symptomatic treatment just supports your immune system while it does the actual healing.
So, what we doctors actually do when we catch a cold — simply put, it's getting plenty of sleep. A cold can only be defeated by your own immune system. So when you catch a cold, sleep well — this is the best way to recover quickly. Some doctors push through and work despite being sick, but if you can eat, you should eat, drink plenty of water, keep warm to raise your body temperature, and rest. We know this is the best treatment, so basically it comes down to sleeping and resting as much as possible.
Also, honey is actually said to be effective for severe coughing. There's data showing that when children with coughs were given either cough medicine or honey, there wasn't much significant difference in cough suppression. Of course, you should never give honey to children under one year old, but some doctors do recommend trying honey if you have a bad cough. I myself take Manuka honey during this season, and I think it's reasonably effective, so I'd recommend trying it.
I also mentioned sleeping while raising your body temperature — and for that, herbal medicine (kampo) can actually be effective. Kakkonto contains a compound similar to ephedrine that activates the sympathetic nervous system and raises body temperature, so taking kakkonto at the very onset of a cold makes logical sense. On the other hand, once your fever has already risen significantly, kakkonto doesn't do much good. So when you feel like you might be catching a cold, we often drink kakkonto with warm water. I keep it stocked at home and take it whenever I feel a chill coming on, so I'd recommend keeping kakkonto on hand as well.
Today, I introduced what we doctors do and don't do when we catch a cold. There's no miracle cure for a cold. There's no way to cure it instantly — you can only rely on your own immune system, so resting is key. If you think you might have a cold, keep your body warm, sleep well, and avoid unnecessary medications. If needed, take targeted medicine as symptomatic treatment. And of course, if you have a very high fever or develop other symptoms, please be sure to see a doctor.
