柏木由紀さんを救いたい ~勝手に診察シリーズ~

今日はとても心配な動画を見つけたので、緊急で撮影しております。今回私が心配しているのは柏木由紀さんです。昨日、柏木さんがご自身のYouTubeチャンネルで動画をアップされていて、その動画のタイトルが「皆さん助けてください（緊急）」というものでした。私の概要欄にもリンクを貼っておくので、ぜひご覧ください。 ざっくり言うと、柏木さんは最近左手が痺れているとのこと。病院に行かれて薬をもらったものの、なかなか良くならないそうです。もう一つは喉の問題で、首から喉にかけての辺りにひどい詰まり感があり、物を飲み込むのがとても苦しいとおっしゃっていました。何もしていないのに首を締められているような感じがするとのことで、「自分はどこかおかしいのでしょうか？医師の方がいれば、どの科に行けばいいか教えてください」という内容の動画でした。 これはとても心配です。コメント欄にもさまざまな意見がありましたが、私も柏木さんにぜひお伝えしたいと思い、この動画を撮っています。柏木さんに届きますように。 ■ 手の痺れについて 私たち医師は「手が痺れる」と言われた場合、大きく2つに分けて考えます。一つは整形外科的なもの、もう一つは脳神経的なものです。神経は脳から伝達されるため、脳神経に異常がある場合に手足の痺れが起こることがあります。 ただ、今回の柏木さんの症状からは、脳神経的な原因ではなく整形外科的な原因が考えられます。柏木さんは親指側（親指・人差し指・中指・薬指の一部）が痺れているとおっしゃっていましたが、これはまさに正中神経が支配している領域です。 考えられる原因の一つ目は、**正中神経麻痺（手根管症候群）**です。正中神経は手根管と呼ばれる狭い部分を通っており、そこに炎症や圧迫が生じると痺れが起こります。特に女性や中高年に多く、寝ている時や明け方に症状が悪化しやすいという特徴があります。柏木さんも「寝ている時に痛くなる」とおっしゃっていたので、非常に合致しています。 また、柏木さんが「右手はOKサインが作れるけれど、左手は作りにくい」とおっしゃっていたことも重要なサインです。これを「ティアドロップサイン（手がドロップサイン）」と呼び、正中神経麻痺の際に親指と人差し指でしっかり丸を作れず、涙の形になってしまう所見です。これは正中神経麻痺の診断において非常に特徴的なサインであり、柏木さんの症状と一致しています。 治療としては、安静にすること、神経の修復を促す薬の服用が基本ですが、症状が進行する場合は手術が必要になることもあります。まずは整形外科を受診されることをお勧めします。 二つ目の可能性として、**尺骨神経麻痺**も考えられます。柏木さんが「小指側も少し痺れているような気がする」とおっしゃっていたこと、そしてゲームをよくされているとのことから、この可能性も否定できません。尺骨神経は薬指の一部と小指を支配しており、肘を長時間曲げた状態でいることで圧迫されやすくなります。肘をぶつけたときにピリピリとしびれる、あの神経です。ゲームやスマートフォンの長時間使用でもなりやすいので、皆さんもご注意ください。整形外科で正中神経麻痺と合わせて鑑別していただけます。 ■ 喉・飲み込みの困難について 私が柏木さんの動画を見て、より心配しているのは実は手の痺れではなく、**嚥下困難（物が飲み込みにくい）** の方です。 柏木さんはサプリメントを飲んだ際にうまく飲み込めなかった気がして、それ以来ずっと喉の違和感が続いているとおっしゃっていました。これが私には気になります。 昨年の外科学会で報告された症例と非常に似ているからです。その症例は、サプリメントを飲んだ後に**食道穿孔（食道に穴が開く）**が起きたというものでした。通常、食道穿孔は鋭利な異物を誤飲した際に起こりますが、大粒のサプリメントが食道の壁にくっついて長時間停滞したことで、食道に傷や穴が生じた症例が報告されています。 その症例の患者さんも、サプリメントを飲んだ後から首に違和感があり、物が飲み込みにくいという症状が続きました。最初に耳鼻科（咽頭・喉頭のファイバー検査）を受けたものの異常なしと言われ、様子を見るように指示されました。しかし症状がどんどん悪化し、最終的に消化器内科で胃カメラ（内視鏡）を行ったところ、食道の穿孔が発見されたのです。緊急手術となりました。 もちろん柏木さんがそうだとは断定できませんが、サプリメントを飲んだ後から症状が始まっているという点が一致しているため、耳鼻科だけでなく、**消化器内科で胃カメラ（内視鏡）を受けて、食道の状態を確認していただく**ことをお勧めします。 ■ 2つの症状の根本的な原因として 一見関係なさそうに見えるこの2つの症状（手の痺れと嚥下困難）が、実は一つの病気から来ている可能性も考えられます。それが**リウマチ性疾患（自己免疫疾患）**です。 特に「**シェーグレン症候群**」という疾患は、唾液腺や涙腺に炎症が起き、口や目が乾燥しやすくなるほか、ものが飲み込みにくくなったり、神経障害による痺れや関節炎が起こることがあります。手根管症候群とシェーグレン症候群が合併する症例を私も複数経験しており、柏木さんの症状がこれに当てはまらないとも言い切れません。 ■ まとめ・柏木さんへのお願い 1. **整形外科**を受診して、手の痺れの原因（正中神経麻痺／尺骨神経麻痺）を鑑別してもらってください。 2. **消化器内科**を受診して、胃カメラ（内視鏡）で食道の状態を確認してもらってください。耳鼻科だけでは食道の奥まで診ることが難しい場合があります。 3. **血液検査**でリウマチ関連の検査を行い、自己免疫疾患が隠れていないかを確認してもらってください。 昨日、私の動画のコメント欄に「先生、柏木さんを助けてあげてください」というメッセージをいただきました。柏木さんはたくさんの方に愛されているアイドルですから、ぜひご自身の体を大切にして、早めに受診されることを願っています。この動画がファンの思いとともに、柏木由紀さんに届きますように。ご視聴ありがとうございました。

I found a very concerning video today, so I'm filming this urgently. The person I'm worried about is Yuki Kashiwagi. Yesterday, she uploaded a video to her own YouTube channel titled "Please Help Me (Emergency)." I'll put the link in the description below, so please watch it. In short, Kashiwagi-san has been experiencing numbness in her left hand lately. She visited a doctor and received medication, but her condition hasn't improved. She also mentioned a second issue: a strong sensation of something being stuck in her throat and neck area, making it very difficult to swallow. Even when she's not eating or drinking, she feels as though her throat is being squeezed. She asked viewers, "Is something wrong with me? If there are any doctors watching, please tell me which specialist I should see." This is genuinely worrying. I saw many comments on her video, including suggestions about which doctor to visit. I want to share my thoughts with Kashiwagi-san directly, so I'm making this video. I hope it reaches her. ■ About the Hand Numbness When a patient tells us their hand is numb, we doctors generally consider two broad categories: orthopedic (musculoskeletal) causes, and neurological (brain/nerve) causes. Since nerves originate in the brain, abnormalities in the brain can cause numbness in the hands and feet. However, based on the symptoms Kashiwagi-san described, I believe this is most likely an orthopedic issue rather than a neurological one. She mentioned that the numbness is on the thumb side of her hand—specifically the thumb, index finger, middle finger, and part of the ring finger—which corresponds precisely to the area innervated by the median nerve. **Possibility 1: Median Nerve Palsy (Carpal Tunnel Syndrome)** The median nerve passes through a narrow channel called the carpal tunnel. When inflammation or compression occurs in this tunnel, numbness results. This condition is particularly common in women and middle-aged individuals, and tends to worsen during sleep or in the early morning hours. Kashiwagi-san mentioned that her hand hurts while she's sleeping, which matches this pattern perfectly. Another key sign: Kashiwagi-san demonstrated that she can make an OK sign with her right hand, but struggles to do so with her left. This is called the "teardrop sign" (or "hand drop sign")—when the median nerve is affected, the patient cannot fully round the thumb and index finger into a circle; instead, the shape becomes teardrop-like. This is a classic diagnostic sign for median nerve palsy, and Kashiwagi-san's self-demonstration was textbook. Treatment involves rest and nerve-repair medications, but surgery may be necessary if the condition progresses. I strongly recommend visiting an orthopedic specialist. **Possibility 2: Ulnar Nerve Palsy** Kashiwagi-san also briefly mentioned that her little finger feels somewhat numb, and that she plays video games frequently. This raises the possibility of ulnar nerve palsy. The ulnar nerve innervates the little finger and half of the ring finger, and runs along the inside of the elbow—it's the nerve that causes that sharp tingling sensation when you hit your "funny bone." Keeping the elbow bent for long periods (as one does while gaming or using a smartphone) can compress this nerve. This condition is also common in certain occupations, such as carpenters or baseball pitchers. An orthopedic doctor can evaluate both median and ulnar nerve involvement simultaneously. ■ About the Difficulty Swallowing What worried me more than the hand numbness, however, is the **dysphagia** (difficulty swallowing). Kashiwagi-san said that after taking a supplement, she felt she hadn't swallowed it properly, and has had a persistent sensation of something being stuck in her throat ever since. This caught my attention immediately. The reason is that it closely resembles a case report I encountered at last year's surgical conference. That case involved **esophageal perforation**—a hole in the esophagus—caused by a supplement. While esophageal perforation is typically associated with accidentally swallowing sharp objects, the reported case involved a large supplement tablet that adhered to the esophageal wall, remained there for an extended period, and caused a wound and subsequent perforation. The patient in that case also experienced persistent throat discomfort and difficulty swallowing after taking a supplement. She first visited an ENT (ear, nose, and throat) doctor, who used a fiber-optic scope but found nothing abnormal—because the scope typically doesn't reach deep enough into the esophagus. She was told to wait and see, but her symptoms gradually worsened. She was eventually referred to a gastroenterologist, who performed an upper endoscopy (gastroscopy) and discovered the esophageal perforation. Emergency surgery was required. I'm not saying this is definitely what's happening to Kashiwagi-san. But the fact that her symptoms began after taking a supplement is a notable parallel. I recommend she visit not only an ENT, but also a **gastroenterologist for an upper endoscopy** to inspect the condition of her esophagus. ■ Could Both Symptoms Share a Common Root? These two seemingly unrelated symptoms—hand numbness and difficulty swallowing—may actually stem from a single underlying condition: a **rheumatic (autoimmune) disease**. In particular, **Sjögren's Syndrome** is an autoimmune disorder that causes inflammation of the salivary and lacrimal glands, leading to dry mouth and dry eyes, difficulty swallowing, peripheral nerve disorders (causing numbness), and joint inflammation. I have personally seen several cases where carpal tunnel syndrome and Sjögren's Syndrome coexisted. I sincerely hope this is not the case for Kashiwagi-san, but it's worth ruling out. ■ Summary and Recommendations for Kashiwagi-san 1. **Visit an orthopedic specialist** to evaluate the hand numbness and determine whether it is median nerve palsy (carpal tunnel syndrome), ulnar nerve palsy, or both. 2. **Visit a gastroenterologist** for an upper endoscopy to examine the esophagus. An ENT scope alone may not be sufficient to visualize the deeper esophagus. 3. **Get a blood test** for rheumatic markers to rule out an underlying autoimmune disease. Yesterday, I received a comment on my own video from someone saying, "Doctor, please help Kashiwagi-san." It was clearly written by one of her devoted fans. Kashiwagi-san is an idol loved by so many people. I truly hope she takes care of herself and seeks medical attention as soon as possible.
