ダイエットの3つの嘘について解説します。

皆さんこんにちは、内科・皮膚科医の友利新です。春になってきましたね。だんだん暖かくなってきたので、着るものが薄くなってくるじゃないですか。そうしたら気になるのが、やっぱりボディラインというか体重ですよね。サムネイルにも書きましたけれども、ダイエットというのは、SNS上に「うん?」と思うようなことがいっぱいあるんですよ。なので今日は、医師の目線からダイエットの嘘3つをしっかり解説していきたいと思いますので、最後までご覧ください。
まず1つ目ですが、SNSを見ていたらよくありますよね。「これを飲んで何キロ痩せた」とか「宿便がドーンと出た」とか。あんなの99.9%ではなく、100%嘘だと思ってください。サプリというのはあくまでも栄養補助です。なのでそれだけで痩せるということはあり得ません。例えばカフェインのように代謝を上げるものもなくはないんですけれども、それだけを飲んでもあまり意味がないというか、やはり運動もしなければいけないので、サプリだけを飲んで痩せるということは100%ありません。そこは気をつけてください。
そういうサプリを売っているところは、少し怪しいと思っていただきたいんです。「何キロ痩せました」と書いてあって、微妙に高くもなく安くもない値段だと、1回買ってみたいと思うかもしれません。でも、そこには飲んではいけないような成分が入っていたり、漢方薬の中でも下痢を引き起こすようなものが入っていたり、どういう成分かよくわからないものも多くあります。サプリによる健康被害を消費者庁や国民生活センターに相談する第一位の原因は、やはりダイエットサプリなんですよ。昔、よくわからないダイエットサプリを飲んで亡くなってしまったという事例もあったりします。まずそもそも、サプリを飲んで痩せることはありません。海外から取り寄せるということもやめましょう。
何かを飲んで楽して痩せたいという気持ちはわかります。食欲を抑制したり、油の排出を促したりする薬は存在します。ただしこれは、医師の診断のもと、医師の処方によって日数が制限されて出されるものです。最近はGLP-1という、もともとは糖尿病の薬だったものがダイエットの薬として使われるケースもあります。しかしこれはあくまでも薬ですから、副作用もあります。きちんとお医者さんと相談して処方してもらうものですので、通販などで売っているサプリには絶対に手を出さないでください。
ダイエットの嘘2つ目は、マッサージで痩せるというものです。私もマッサージは大好きです。整体やマッサージに行きますが、それはダイエットを目的にはしていません。もちろん、むくみが一時的になくなることはあります。人間は水分でできていますから、例えばずっと立ちっぱなしだと足がむくんで太く見えることがあります。それをマッサージすることでむくみがなくなって細くなることはあり得ます。しかしマッサージをすることで脂肪が燃焼して、本当に脂肪が少なくなるかというと、そういったデータはありません。
むくみを解消して細く見えるという効果はありますし、そういう目的でマッサージに行かれるのはとても良いことだと思います。ただ、ずっと立ちっぱなしだったり運動しなかったり、塩分の多いものを食べ過ぎていたりすると、一度マッサージで流したとしてもまたむくみます。毎日マッサージに行かなければならなくなってしまいますし、根本的な解決にはなっていません。ですから、マッサージだけで痩せるということはあり得ません。
関連して、最近よく見る「着圧スパッツを履くと痩せる」という宣伝についてですが、スパッツを履いて痩せるわけはありません。ただ、着圧によって血液やリンパの流れを良くしてむくみを防ぐという効果はあります。長時間の移動の際にむくみを防ぐ目的や、就寝時に着圧ソックスを使うといった使い方は有効です。しかし「太ももの間に隙間ができた」というのは絶対に嘘です。そんなことはあり得ません。「〇〇するだけ」というダイエット法、つまりサプリを飲むだけ、マッサージするだけ、着用するだけというものは基本的に嘘だと思ってください。
そして3つ目ですが、これは嘘というよりも、まだ論争が続いているものです。「カロリー制限と糖質制限では、糖質制限の方が痩せる」というものです。カロリー制限とは、食事のカロリーをおおよそ1200kcal程度(その人の運動量によって異なりますが)に抑えるもので、カロリーさえ抑えれば何を食べてもよいという考え方です。一方、糖質制限は炭水化物や糖質を多く含む食品を避け、タンパク質をメインにするもので、糖質さえなければ何を食べてもよいという考え方です。
どちらの方がダイエット効果が高いかというと、糖質制限の方が早く体重が落ちます。これは間違いありません。糖質は脂肪になりやすいので、糖質制限をするとスルスルと体重が落ちていきます。カロリー制限でも体重は落ちますが、スピードとしては糖質制限の方が早いです。
ただし、糖質制限が圧倒的に優れているかというと、実はそうでもなく、賛否両論あります。データによると、コレステロールや中性脂肪は一時的に減るものの、長期的には糖質制限によって中性脂肪などが高くなるのではないかと指摘する専門家もいます。また、糖質制限とカロリー制限を長期間続けた場合、最終的な減量維持効果は同じくらいと言われています。これはなぜかというと、糖質制限はやはり続けるのが大変だからです。ご飯もパスタも甘いものも食べたいですよね。だから途中で挫折してしまうことも多いのです。また、極端な糖質制限でご飯よりも揚げ物を選ぶようになると、逆にコレステロールや中性脂肪が上がってしまうこともあります。
つまり、「カロリー制限だけが良い」も嘘ですし、「糖質制限が良い」というのも嘘なんです。現在最も良いとされているのは、地中海式ダイエットと呼ばれる食事法です。炭水化物はどちらかというと茶色っぽいもの、日本で言えば玄米や胚芽米のような全粒穀物を食べ、良質な油(オリーブオイルなど)を使い、魚介類を多めに取り、豆類・野菜・果物をしっかり取るというものです。この食事法の方が健康的で、長期的に見ても減量を維持できることがわかっています。
何か一つのダイエット法に飛びつきたくなる気持ちはわかりますが、食事は体の基礎です。何も面白くないし、キャッチーでもないのですが、バランスよく食べること、体の炎症を抑えるような食事をすること、加工食品を減らすことが、ダイエットには最も効果的です。
ざっくり言うと、ダイエットは簡単ではありません。食事をきちんと見直して、運動を続けていくということです。だから簡単ではない、ということを今日は皆さんにお伝えしたくてこの動画を撮りました。本当にダイエットしたいなら、1ヶ月に0.5〜1kgを目安に、カロリーも糖質も極端に偏らないバランスの良い食事を取り、心拍数が上がるような適度な運動を継続していただければと思います。

Hello everyone, I'm Arata Tomori, an internal medicine and dermatologist physician. Spring is here, and as it gets warmer, our clothes get lighter — which naturally makes us more conscious of our body shape and weight. As I mentioned in the thumbnail, there is a lot of misleading information about dieting on social media. So today, I'd like to clearly explain three lies about dieting from a doctor's perspective. Please watch until the end.
The first lie is diet supplements. You've probably seen posts on social media claiming things like 'I lost X kilograms with this product' or 'I expelled a huge amount of waste.' Please consider those claims to be 100% false — not 99.9%, but 100%. Supplements are nutritional aids, nothing more. It is simply not possible to lose weight from them alone. While some ingredients like caffeine can slightly boost metabolism, taking them without exercising won't get you far. There is absolutely no supplement that will make you lose weight on its own.
You should also be skeptical of companies selling such products. A product claiming you'll lose weight, priced at a tempting mid-range point, might seem worth trying once. But these products may contain harmful ingredients, laxative herbal compounds, or components that are difficult to verify. In fact, the number one source of supplement-related health complaints received by consumer protection agencies is diet supplements. There have even been cases of people dying after taking unknown diet supplements. The bottom line is: no supplement will make you lose weight. And please do not order supplements from overseas, either.
The desire to lose weight easily by taking something is understandable. Medications that suppress appetite or reduce fat absorption do exist — but these are prescribed by a doctor, under medical supervision, with limited duration of use. Recently, GLP-1 medications, originally developed for diabetes, have been used in some weight-loss treatments. However, these are medications with side effects and must be used under proper medical guidance. Please never purchase supplements sold online or through mail order claiming to do the same thing.
The second lie is that massage can make you lose weight. I personally love massages — I regularly visit chiropractors and massage therapists. But I don't go for weight loss purposes. Massage can temporarily reduce swelling. Since the human body is largely made up of water, standing for long periods can cause fluid retention in the legs, making them look puffy. Massaging can relieve that puffiness and make legs look slimmer temporarily. However, there is no scientific data showing that massage burns fat or reduces fat tissue.
Massage is great for relaxation and reducing puffiness, and going for those reasons is perfectly fine. But if you continue standing all day, not exercising, or eating salty foods, the puffiness will return. You'd need daily massages just to maintain the effect, which is not a solution to the root problem. Massage alone will not make you lose weight.
On a related note, I often receive requests to promote compression leggings or spats that claim to make you slimmer. There is no way that wearing compression spats will make you lose weight. However, compression garments can help improve circulation and prevent fluid retention, which is genuinely useful — for example, during long-haul travel or while sleeping. But claims like 'a thigh gap appeared after wearing these' are absolutely false. Please be skeptical of any diet method that promises results simply from doing just one thing — taking a supplement, getting a massage, or wearing a garment.
The third point is not exactly a lie, but rather an ongoing debate: the claim that low-carbohydrate dieting is superior to calorie restriction for weight loss. Calorie restriction means limiting your total daily calorie intake — roughly around 1,200 calories depending on your activity level — and allowing any foods as long as the calorie target is met. Low-carb dieting, on the other hand, eliminates or minimizes carbohydrates and sugars, focusing primarily on protein, and allows most foods as long as carbs are avoided.
In terms of speed of weight loss, low-carb dieting does produce faster results. This is true. Because carbohydrates are easily stored as fat, cutting them out leads to rapid initial weight loss. Calorie restriction also leads to weight loss, but more gradually.
However, whether low-carb dieting is definitively superior is debatable. Some research suggests that while cholesterol and triglyceride levels initially drop on a low-carb diet, they may rise again over the long term. Furthermore, studies that have tracked both approaches over extended periods found that the long-term weight maintenance outcomes are roughly the same. This is largely because low-carb diets are hard to sustain. People naturally want to eat rice, pasta, and sweets, leading many to give up partway through. Additionally, extreme low-carb dieting that favors fried foods over rice can actually raise cholesterol and triglyceride levels.
So it's not accurate to say calorie restriction alone is best, nor is it accurate to say low-carb dieting is best. The most well-supported dietary approach is the Mediterranean diet. It emphasizes whole grains (in Japan, think brown rice or germ rice), healthy fats like olive oil, plenty of seafood, legumes, vegetables, and fruits. Research shows this approach is healthier and more effective for sustaining long-term weight loss.
I know it's tempting to jump on the latest diet trend, but food is the foundation of your body. It may sound boring and unexciting, but eating a balanced diet, choosing anti-inflammatory foods, and reducing processed foods is the most effective approach to weight loss.
Simply put, dieting is not easy. It requires genuinely reassessing your diet and maintaining regular exercise. That's the message I wanted to share today. If you truly want to lose weight, aim for 0.5 to 1 kilogram per month. Eat a balanced diet without extreme restrictions on either calories or carbohydrates, and incorporate exercise that raises your heart rate. Consistency is key.
