今日は、皆さんからリクエストの多かった「妊娠中・授乳中にOKなスキンケア成分」についてご紹介したいと思います。
特に妊娠中はホルモンバランスが大きく変化しますよね。人生の中で一番女性ホルモンが大量に出る時期ということになります。例えば、シミがすごく濃くなったり、肌がすごく敏感になったり、ニキビができやすくなったりということが起こってきます。もちろん全くそういうことが起こらず、お肌がすごく調子がいいという方もいらっしゃいます。
シミやニキビが気になっているときに、どういった成分を使えばいいか迷っている方もいらっしゃると思うので、今日はそういったことを症状別に、こういった成分であれば問題ないよということをお話ししていこうと思います。妊娠中の方も、そしてこれから妊娠を考えている方も、ぜひ参考にしていただければと思います。
【シミ・そばかす・色素沈着について】
まず妊娠中に一番お悩みが多いのが、シミやそばかす、色素沈着がかなり悪化してきたというケースです。それに対して、例えばクリニックでレーザーを当ててもらえませんかという方もいらっしゃるんですが、基本的に妊娠中はクリニックで何か施術をすること、特にレーザー系の施術をするクリニックはほとんどないと思います。なのでこれに関しては、ホームケアで対応していただければと思います。
市販の化粧品を使ったからといって赤ちゃんに何か影響があるということは基本的にはないんですが、美白成分の中で一つ気をつけていただきたいのがハイドロキノンです。ハイドロキノンは以前クリニックで美白剤として処方されていたのですが、今は化粧品にも配合されています。
ハイドロキノンを塗ったからといって赤ちゃんに何か影響があるわけではないので、今使っている方は安心してください。ではなぜ妊娠中にハイドロキノンを新しく使うのは気をつけてほしいかというと、ハイドロキノンはとても効果が高い代わりに、被れたり赤みが出たりする可能性が高い成分でもあるからです。
今まで使っていて問題がなければいいのですが、妊娠して肝斑やシミが濃くなってきたからと新しくハイドロキノンを使い始めると、赤くなったり被れたりする可能性がゼロではありません。そうなったときに使うお薬、例えばステロイドには妊娠中に使用が制限されるランクのものもあるので、そういった意味で新しくハイドロキノンを使い始めることは基本的には避けたほうがいいと思います。
では妊娠中にシミや肝斑が濃くなったときにどういったものを使えばいいかというと、おすすめは3つあります。
1つ目はトラネキサム酸です。妊娠中は私自身もそうでしたが、肝斑が結構濃くなったりします。トラネキサム酸の内服は妊娠中NGなのですが、化粧品として肌に塗ることは問題ありません。特に肝斑が妊娠中に悪化したという方は、ぜひトラネキサム酸を使ってみてください。内服より塗るほうが時間はかかりますが、それでも安心して使えるので、しっかり取り入れてみてください。
2つ目はナイアシンアミドです。「ナイアシンアミドっていろんなものに効くから、なんとなく緩やかなイメージ」という方もいらっしゃると思うんですが、データではナイアシンアミド4%の美白効果が、もしかしたらハイドロキノン4%にも匹敵するのではないかというデータも最近出てきているくらいです。すぐに劇的なビフォーアフターというよりも、コツコツ使っていくとよい成分です。また妊娠中に揺らぎがちな肌荒れも抑えてくれるので、肝斑を予防したい方、妊娠中に美白したいという方はぜひ使ってみてください。
3つ目はアゼライン酸です。初めて聞く方もいらっしゃると思いますが、最近は美容業界でもかなり注目されている成分です。もともとアゼライン酸は、これからお話しするニキビの世界的な治療薬として私たち皮膚科医も使っているのですが、日本ではなぜか認可が下りていません。医療専売品としてロート製薬のクリームがあり、これはアゼライン酸が20%配合されているものです。ニキビ治療にしっかりと使われてきましたが、実はアゼライン酸にはメラニンの生成を抑制する効果もあり、特に治療用の20%配合のものは、ハイドロキノン4%と同じくらいの美白効果があるとも言われています。妊婦さんにもOKな成分ですので、頭に入れておくといいと思います。
20%配合のものは医療専売のロート製薬のクリームしかないのですが、同じロート製薬の通信販売で「AZAセラム」というものにアゼライン酸が入っています。濃度は公表されていませんが、20%に近い、もしくはそれくらいあるのではないかと思うので、気になる方はロート製薬のサイトで「AZAセラム」を探してみてください。
【ニキビ・吹き出物について】
続いてニキビや吹き出物が気になる方向けです。もちろんニキビが気になったら皮膚科に行っていただければ、妊娠中でも使えるお薬を出してもらえると思うんですが、スキンケアでも最適な成分を取り入れたいという方に3つおすすめの成分があります。
1つ目は先ほどもお話ししたアゼライン酸です。世界ではニキビの薬として認められていますが、日本では認められていないので、医療専売品としてロート製薬から出ていたり、あとは「ETVOS(エトヴォス)」という化粧品ブランドから、アゼライン酸誘導体が入ったニキビ・吹き出物用の美容液も出ています。それが「バランシングVCクリアスポッツ」という製品です。アゼライン酸誘導体とビタミンCが一緒になって、ニキビケアやニキビの色素沈着ケアになりますので、クリニックに行きづらい方はぜひETVOSのサイトを見ていただければと思います。
2つ目はグリチルリチン酸ジカリウムです。これは抗炎症成分として皆さんご存知だと思います。もちろんクリニックでもらう抗炎症剤とは基準が違いますが、たまに「グリチルリチン酸ってステロイドと同じような感じですか?使っていると効かなくなりますか?」と聞かれることがあります。全く基準が違いますし、使っているからといって効かなくなるということもないので安心してください。ニキビの炎症がひどいときなど、化粧品でできる抗炎症でしっかりしているものといえばグリチルリチン酸ジカリウムだと思います。これはニキビの炎症だけでなく、シミの元になる炎症も抑えてくれます。いろんな化粧品に入っている成分なので、そちらも安心して使ってください。
3つ目はナイアシンアミドです。実はナイアシンアミドは5%以上配合されていると、余分な皮脂を抑えてくれる効果もあります。なので高濃度のナイアシンアミドはニキビや毛穴を抑えてくれるということもあり、ニキビや吹き出物が気になっている方にもいい成分です。肌荒れ防止成分でもあるので、ぜひ使ってみてください。
【乾燥について】
あとは乾燥が出てくることもあります。不安定な状態になるので、そういったときは本当に手持ちのもので全然大丈夫です。セラミドやナイアシンアミド、あとはヘパリン類似物質などを使っていただければと思います。
【シワ改善について】
最後にシワ改善です。一つだけ気をつけていただきたいのがレチノールです。ハイドロキノンと同じように、レチノールを化粧品レベルで使ったからといって赤ちゃんに何か影響があるということはほぼないと言われているのですが、ビタミンA自体は内服だと赤ちゃんに少し影響があるとも言われているので、心配な方はやめておいたほうがいいと思います。その代わりに使うとしたら、シワ改善で有名なのは、ナイアシンアミド、ニールワン、そしてビタミンE誘導体、そしてライスパワーNo.11だと思います。レチノールがちょっと心配という方は、それをお休みして、今お話しした別のシワ改善成分で対応していただければ問題ありませんし、心配はいらないのでぜひ使ってみてください。

Today I'd like to introduce skincare ingredients that are safe to use during pregnancy and breastfeeding, a topic many of you have requested.
During pregnancy especially, hormone balance changes dramatically. It's the period in life when the most female hormones are produced. For example, dark spots can become much darker, skin can become very sensitive, and acne can occur more easily. Of course, some people don't experience any of these issues and their skin condition is actually quite good.
For those of you who are concerned about spots or acne and are wondering what ingredients to use, today I'll go through this by symptom and explain which ingredients are fine to use. I hope this will be helpful for those who are currently pregnant as well as those who are thinking about becoming pregnant.
【About Dark Spots, Freckles, and Hyperpigmentation】
One of the most common concerns during pregnancy is that dark spots, freckles, and hyperpigmentation get significantly worse. Some people ask if they can get laser treatment at a clinic for this, but basically, clinics generally do not perform procedures during pregnancy, especially laser treatments. So I recommend addressing this through home care.
Using over-the-counter cosmetics generally shouldn't affect the baby, but there is one whitening ingredient you should be careful about: hydroquinone. Hydroquinone used to be prescribed at clinics as a whitening agent, but now it's also included in cosmetic products.
Using hydroquinone topically won't affect the baby, so if you're already using it, there's no need to worry. So why should you be cautious about starting hydroquinone during pregnancy? While hydroquinone is very effective, it also has a higher chance of causing irritation or redness compared to other ingredients.
If you've been using it without any issues, that's fine, but if you start using hydroquinone for the first time because your melasma or spots got worse during pregnancy, there's a non-zero chance of experiencing redness or irritation. If that happens, some steroid medications used to treat such reactions have restrictions during pregnancy, so for this reason, I think it's best to avoid starting hydroquinone for the first time during pregnancy.
So what should you use when spots or melasma worsen during pregnancy? I recommend three ingredients.
The first is tranexamic acid. During my own pregnancy, my melasma got noticeably darker too. While oral tranexamic acid is not allowed during pregnancy, applying it topically through cosmetics is fine. If your melasma has worsened during pregnancy, please try using tranexamic acid. Topical application takes longer than oral medication, but it can be used safely, so please incorporate it into your routine.
The second is niacinamide. Some of you might think niacinamide seems mild since it's said to work on so many different things, but data suggests that 4% niacinamide's whitening effect may actually be comparable to 4% hydroquinone. Rather than producing dramatic before-and-after results immediately, it's an ingredient that works well with consistent use over time. It also helps calm the skin instability that tends to occur during pregnancy, so if you want to prevent melasma and whiten your skin during pregnancy, please try niacinamide.
The third is azelaic acid. Some of you may be hearing about this for the first time, but it's an ingredient that has been gaining a lot of attention in the beauty industry recently. Azelaic acid was originally used as a globally recognized acne treatment, which we dermatologists use—I'll talk more about acne later—but for some reason it hasn't been approved in Japan. There is a medical-grade cream from Rohto Pharmaceutical containing 20% azelaic acid, which has been used extensively for acne treatment. It turns out azelaic acid also has an effect of suppressing melanin production, and the whitening effect of the 20% treatment-grade formula is said to be comparable to 4% hydroquinone. It's a pregnancy-safe ingredient, so it's good to keep in mind.
The 20% formula is only available as the medical-grade Rohto Pharmaceutical cream, but Rohto also sells a mail-order product called "AZA Serum" that contains azelaic acid. The concentration isn't disclosed, but I think it's fairly close to 20%, or possibly around that amount, so if you're interested, please check Rohto's website for "AZA Serum."
【About Acne and Breakouts】
Next, for those concerned about acne or breakouts. Of course, if you're concerned about acne, I recommend visiting a dermatologist, who can prescribe medications safe to use during pregnancy. But for those who want to incorporate the best ingredients into their skincare routine as well, I have three recommendations.
The first is azelaic acid, which I mentioned earlier. It's recognized as an acne medication worldwide but not approved in Japan, so it's available as a medical-grade product from Rohto Pharmaceutical, or from the cosmetic brand ETVOS, which offers a serum for acne and breakouts containing azelaic acid derivatives. That product is called "Balancing VC Clear Spots." It combines azelaic acid derivatives with vitamin C to help with acne care and post-acne hyperpigmentation. If it's difficult for you to visit a clinic, please check out ETVOS's website.
The second is dipotassium glycyrrhizate. Many of you probably know this as an anti-inflammatory ingredient. Of course, the standards are different from anti-inflammatory medications you'd get at a clinic, but I sometimes get asked, "Is glycyrrhizic acid similar to steroids? Will it stop working if I use it regularly?" The standards are completely different, and using it regularly won't make it stop working, so there's no need to worry. When acne inflammation is severe, dipotassium glycyrrhizate is one of the most effective anti-inflammatory ingredients available in cosmetics. It suppresses not only acne inflammation but also the inflammation that leads to dark spots. Since it's included in many cosmetic products, please feel free to use it with confidence.
The third is niacinamide. Interestingly, niacinamide at concentrations of 5% or higher can help suppress excess sebum. This means high-concentration niacinamide can help with acne and pores, making it a great ingredient for those concerned about acne and breakouts. It's also an ingredient that helps prevent skin irritation, so please try using it.
【About Dryness】
Dryness can also occur during pregnancy. Since your skin becomes more unstable, you can simply use whatever products you already have on hand. Ceramides, niacinamide, and heparinoid are all good options to use.
【About Wrinkle Improvement】
Finally, for wrinkle improvement. There's just one ingredient you should be cautious about: retinol. Similar to hydroquinone, using retinol at cosmetic-grade levels is said to have almost no effect on the baby, but vitamin A itself is said to have some effect on the baby when taken orally, so if you're concerned, it's best to avoid it. As alternatives, well-known wrinkle-improvement ingredients include niacinamide, Nile One (ニールワン), vitamin E derivatives, and Rice Power No. 11. If you're worried about retinol, you can take a break from it and use these other wrinkle-improvement ingredients instead—there's no need to worry, so please give them a try.
So today, I've covered ingredients that are safe to use during pregnancy, as well as ones you should be a bit cautious about, organized by concern. Pregnancy is truly a sensitive time in many ways, and if you look online, you'll see all sorts of claims like "alcohol is bad" or "fragrance is bad." Basically, ordinary cosmetics sold in stores should be fine to use, but if you're concerned, please avoid those ingredients and use products that give you peace of mind.
