インフルエンザワクチンを打っていないとSNS上でよく見られます。患者さんから「ワクチンを打ちましたか?」と聞かれることもありますし、「打っていいんですね?」と確認される方もいます。これはきちんと説明しておかなければいけないと思い、この動画を撮っています。
最初に言っておくと、私自身、そして私の家族——6歳、4歳、1歳の子どもたち、主人、近くに住んでいる義理の父・母——全員のインフルエンザワクチンを接種しています。私が勤めている3つのクリニックでインフルエンザワクチンを打つかどうかのアンケートを取ったところ、私が把握している範囲では医療従事者はほぼ全員接種しています。
なぜSNS上にインフルエンザワクチンを打たない医療従事者が多いように見えるのかというと、インフルエンザワクチンに対する誤解があり、それが都市伝説化しているのではないかと思っています。
その誤解の一つが「重症化を防ぐだけで、発症は防げない」という認識です。皆さんご存知の通り、インフルエンザワクチンは打ったからといって100%発症を防げるわけではなく、「発症したときに重症化を防ぐから打っておきましょう」という説明がされてきました。しかし実際のデータを見ると、インフルエンザを発症した人の中でワクチンを打っていない人の方が多く、ワクチンを打った人は約60%の発症を防御できているということが示されています。きちんとしたデータを見ることで、インフルエンザの発症をしっかりと抑えているということは明らかです。
今年は新型コロナウイルスも流行しており、発熱した際にどちらの疾患か判断しにくい状況です。インフルエンザワクチンを打っているかどうかが、診断や治療方針を決める上で今年は特に重要なポイントになってくると思います。ワクチンを打っていない場合、新型コロナかインフルエンザか判断が難しくなりますし、両方に同時に感染するとさらに重症化するリスクがあります。
また、「ワクチンを打つと自閉症になる」という説は誤りであることが明らかになっていますので、ご安心ください。
水銀についての懸念もよく聞かれますが、水銀には2種類あります。公害で有名になったメチル水銀は体外に排出されるまでに約2年かかるため、少量でも体に蓄積しやすく害が出ます。一方、インフルエンザワクチンに含まれるエチル水銀は約4日で体外に排出され、1か月もすれば完全に体外に出ていきます。エチル水銀の安全な摂取量は体重1キログラムあたり4マイクログラムと定められており、例えば体重50キロの人であれば20マイクログラムが基準となります。インフルエンザワクチン1回分に含まれるエチル水銀は1〜4マイクログラムであり、これは1週間の食事で摂取する量よりもはるかに少なく、お刺身を食べる程度と大きく変わりません。水銀の害を過度に心配するよりも、インフルエンザに罹患して重症化する方がはるかにリスクが高いと言えます。
さらに、インフルエンザワクチンは自分自身を守るだけでなく、全員が接種することで集団免疫として社会全体を守る役割も担っています。ワクチンを接種できない体質の方や免疫不全の方を守るためにも、接種できる方は積極的に検討していただければと思います。

It is common to see posts on social media claiming that healthcare workers do not get the influenza vaccine. Some of my patients ask me, "Did you get the vaccine?" while others ask, "Is it actually okay to get it?" I felt it was important to address this properly, which is why I made this video.
First of all, I want to be clear: I personally have been vaccinated, as have all of my family members — my children aged 6, 4, and 1, my husband, and my in-laws who live nearby. I also surveyed the three clinics where I work, and as far as I know, nearly all of the healthcare workers there have received the influenza vaccine.
The reason it may appear on social media that many healthcare workers skip the flu vaccine is, I believe, due to misconceptions about the vaccine that have taken on an urban legend quality.
One of those misconceptions relates to the idea that the vaccine only "prevents severe illness" rather than preventing infection altogether. As many of you know, the flu vaccine is not 100% effective at preventing infection, and it has often been explained as something you get "to prevent severe illness if you do get infected." However, when you look at actual data, among people who develop influenza, a greater proportion are unvaccinated, and the vaccine has been shown to prevent infection in approximately 60% of cases. The data clearly shows that the vaccine significantly reduces the likelihood of developing influenza.
This year, with COVID-19 also circulating, it is difficult to distinguish between the two illnesses when a patient develops a fever. Whether or not a patient has received the flu vaccine is becoming a particularly important factor in making diagnostic and treatment decisions this year. If a patient hasn't been vaccinated, it becomes harder to determine whether they have COVID-19 or influenza, and being infected with both simultaneously carries an even greater risk of severe illness.
I also want to reassure you that the claim that vaccines cause autism has been thoroughly debunked.
Another common concern involves mercury. There are actually two types of mercury. Methylmercury — the type associated with pollution-related diseases — takes nearly two years to leave the body, which means even small amounts can accumulate and cause harm. Ethylmercury, on the other hand, which is used as a preservative in some influenza vaccines, is excreted from the body in about four days and is completely eliminated within a month. The safe intake level for ethylmercury is 4 micrograms per kilogram of body weight — so for a 50 kg person, that's 20 micrograms. A single dose of the influenza vaccine contains only 1 to 4 micrograms of ethylmercury, which is far less than what you might consume in a week through diet — not much different from eating sashimi. The risk of worrying too much about mercury in the vaccine is far outweighed by the risk of getting influenza and developing serious complications.
Furthermore, the influenza vaccine is not just about protecting yourself. When everyone gets vaccinated, it creates herd immunity that protects society as a whole — including those who cannot receive vaccines due to medical conditions or immune deficiencies.
