老化を進める〇〇ストレスについて解説します。

皆さんこんにちは、皮膚科医の友利新です。今日は老化を進める「〇〇ストレス」について詳しくお話しするので、最後までご覧ください。
老化を進める丸々ストレスと書きましたが、皆さん何だと思いますか?そう、実はこれ「糖化ストレス」になります。
糖化ってなんだそりゃ、みたいに思った方いらっしゃると思うんですね。糖化よりも皆さんが聞き慣れているのは「酸化」だと思うんです。酸化というのはざっくり言うと、体が錆びつくことですよ、ということで、その酸化を抑えるような食品、抗酸化成分を取りましょうというのが皆さんなんとなくわかっていると思うんですね。
酸化はもちろん老化を進める原因のストレスではあるんですね。酸化ストレスといって。でも最近もっと怖いということがわかってきたのが、この糖化ストレスです。
なぜ最近わかってきたかというとですね、酸化っていうのは人間が生きていたら必ず起こるんですよ。なぜかというと、人間は酸素を吸って生きているでしょ。だから酸素を吸っていると必ずこの活性酸素というものが生まれて、この活性酸素っていうのが体を酸化させてしまうんです。
要は酸素を吸っていると必ずなるのが酸化。だから人間の体には酸化を防ぐような防御機構というのもいくつかあるんですね。酸化を防ぐような抗酸化作用がある食べ物というのも結構多いんですよ。例えば紫外線を浴びるということも酸化ストレスを押し進めることになるので、そういう紫外線が強くなってきたら緑黄色野菜、緑の野菜を多く摂ったりとか、そういうふうに人間というのは酸化を抑える体の機構もそうですし、食べ物も結構あったんです。
一方、糖化ストレスっていうのは実はこの何十年かで人間の体の脅威になってきたストレスになるんです。それはなぜかっていうと、これも酸化と同じように人間には必ず糖化ストレスっていうのはずっと昔から加わってきたんですよ。これは、人間が食品などを食べると、それをエネルギーに変える時には糖が必要じゃないですか。エネルギーを作る時に糖をやっぱり分解していくんですね。この糖を使ってエネルギーを作るから、必ず糖化の最後に糖化最終生成物ができるんですね。この最終生成物がAGEsと呼ばれるんですけども、これがタンパク質にくっつくことによってタンパク質を変性させる、というのが糖化なんです。
今まで私たち人間っていうのは、そんなに食事がいっぱい食べれて、ずっとお腹がいっぱいいる状況っていうのは、ここ何十年なわけですよ。昔はやっぱり人間も飢餓の時代が長かったんですね。要は、こんなに飽食の時代じゃなかった。だから酸化を抑えることはある程度今までもねずっと酸素を吸って生きてきたから対応できていた。だけれども体はですね、糖が過剰にある状態を抑えるっていう防御システムがめちゃくちゃ弱いんです。
あとはですね、この糖化っていうのは食べること、血糖値が上がることで起こってくるので、血糖値が上がりやすい食べ物が今の簡単に手に入る食品には多いので、糖化っていうのは現代人はまさにもう脅威中の脅威になっているんです。
なぜこの糖化が老化を押し進めているかっていうとですね、まず私たち食品を取ったら、その糖を代謝してエネルギーを作るって言ったじゃないですか。エネルギー以上の糖を取ってしまうと余分な糖が残るわけですよ。この余分な糖もそれが分解されると、さっき言ったAGEsっていうものになるんですね。このAGEsっていうものが少なかったらいいんですけども、このAGEsが過剰になるとタンパク質が結びついてしまうんですね。そのタンパク質をこのAGEsが変性させて、黄色く変色させてしまうようなことを起こしてしまうのが糖化になります。
そしてもっと怖いのはですね、ある特定のタンパク質には「レセプター・オブ・AGEs」というスイッチを押すような受容体というものがあるんですね。これにこのAGEsがくっつくと、これが慢性炎症を起こすサイトカインっていうものを作ってしまうんですよ。ここで炎症を起こしてください、っていうシグナルが出ちゃうっていうことなんですよ。慢性炎症が起こるっていうことは、動脈硬化を起こしたりという老化を進めてしまうんですね。
なので糖化も実は慢性炎症を体中に引き起こしてしまう怖い怖いストレスになるんです。例えばそれが皮膚に起こると、コラーゲンやエラスチンに慢性炎症が起こるということで、シワ・たるみが起こるとか。あとAGEsっていうのは肌を茶色っぽくするということがあるんですね。これを詳しく言うとタンパク質をカルボニル化っていうんですけれども、ちょっと難しいことは置いておいて、タンパク質を黄色く変性させるということになるので、肌の透明感がなくなって、ちょっと黄色くくすんでしまうということになります。
あとはそれが目に起こったら白内障の原因になったりとか、あとは筋肉で起こると筋肉が炎症を起こしてしまって、筋肉が減少してしまうサルコペニアというようなものになってしまったりとか。とにかくあらゆる臓器で糖化が起こってしまうと老化が進んでしまう。見た目も老けるし、体の中にも炎症が起こって、いろんな疾患を起こしてしまう可能性があるということになります。
ではこの糖化ストレスを抑えるにはどうしたらいいかというと、5つポイントがあります。
まず1つ目が、よく噛んでゆっくり食べるということになります。そんなこと?と思うかもしれないんですが、もちろん余分な糖分ができるということもあるんですが、急激にバーンと血糖値が上がると糖化ストレスは高くなると言われています。なので血糖値が急に上がるというのは早食いをしてしまうとなってしまう。なので早食いをしないように、ゆっくり食べるということをしてください。
2つ目が、さっきから言っているように急激に血糖値が上昇しないようにするのを防ぎたいので、炭水化物だけを食べるということはやめてもらうということ。例えば丼物とかパスタとかだけだと、これ炭水化物、要は糖分ですよ。それだけを食べると急激に血糖値が上がるので、例えばやっぱり定食みたいなものにして、ご飯は後から食べるというような、急激な血糖値を上げるような食べ方をしないようにする。
3つ目がそれと似ているんですけれども、お野菜とか汁物とかから食べるということですね。これも急にお腹が空いているときに炭水化物、ご飯とかパスタとかを食べると急激に血糖値が上がってしまうので、お野菜から食べると、食物繊維というのは急激な血糖値の上昇を抑えるということがわかっていますので、お野菜から先に食べるようにしてください。
4つ目が適度な運動をすることです。要はエネルギーを消費すれば、余分な糖というのを糖化ストレスになる前に消費するということになりますから、適度な運動を毎日心がけてください。ただ「適度な」なので、例えば今日から毎日ジョギングするぞ、マラソンするぞというと、今度は酸化に繋がってしまいますので、「適度な」というのがポイントです。だからお話しして歩けるぐらいのゆっくりのウォーキングとかということが一番おすすめになっています。
最後は睡眠不足をやめるということです。これ関係なくないと思うかもしれないんですけれども、実はですね、睡眠不足になると代謝が落ちる、要はエネルギーをうまく使えなくなってしまうということになるので、そうなると余分な糖ができてしまって、その糖化ストレスというのが助長されてしまうということになります。なのでゆっくり寝る、質の良い睡眠を取るということは、糖化を抑えるという意味でもすごく大切になるので、必ず質の良い睡眠を取るように心がけてください。
まとめるとですね、酸化も怖いけど糖化も怖いということ。糖化というのはここ数十年で私たちの体に脅威をもたらしているストレスになります。この糖化ストレスというのは、余分な糖分がタンパク質にくっつくことで炎症を起こしてしまうので、要は余分な糖分を取らないようにする。余分な糖分を取りすぎると肌がくすんだり黄色く変性したりするので、肌に起こると、シワ・たるみだけじゃなくて肌のくすみ感も出てしまうということ。そしてあらゆる臓器で炎症を起こすと、あらゆる臓器で老化を進めてしまいますので、この糖化というのが老化を進めるということを頭に置いて生活習慣を整えるようにしてください。

Hello everyone, I'm dermatologist Arata Tomori. Today I'll explain in detail about the "〇〇 stress" that accelerates aging, so please watch until the end.
I wrote "the stress that accelerates aging," but what do you think it is? Yes, this is actually "glycation stress."
Some of you might be thinking, "What on earth is glycation?" I think what you're more familiar with than glycation is "oxidation." Oxidation, simply put, means that the body rusts, so you probably vaguely understand that you should take foods that suppress oxidation, that is, antioxidant components.
Oxidation is certainly a stress that causes aging—it's called oxidative stress. But recently, it has become clear that something even scarier exists: this glycation stress.
Why has this become clear recently? Oxidation is something that inevitably occurs as long as a human is alive. Why? Because humans breathe oxygen to live. So when you breathe oxygen, reactive oxygen species are inevitably produced, and these reactive oxygen species oxidize the body.
In other words, breathing oxygen inevitably leads to oxidation. That's why the human body has several defense mechanisms to prevent oxidation. There are also quite a lot of foods with antioxidant properties that help prevent oxidation. For example, exposure to UV rays also promotes oxidative stress, so when UV rays become stronger, people tend to eat more green and yellow vegetables. In this way, humans have both bodily mechanisms and foods to suppress oxidation.
On the other hand, glycation stress is actually a stress that has become a threat to the human body only in the past few decades. Why is that? Like oxidation, glycation stress has always been present in humans since ancient times. This is because when humans eat food and convert it into energy, sugar is needed. When making energy, sugar is broken down. Because energy is made using this sugar, glycation end products are inevitably formed at the end of this process. These end products are called AGEs, and glycation is the process by which these AGEs attach to proteins and cause them to denature.
However, the situation where we humans can eat so much food and always have a full stomach has only existed for the past few decades. In the past, humans went through long periods of famine. In other words, it wasn't such an era of abundance as it is now. So, to some extent, the body has always been able to cope with oxidation since we've always been breathing oxygen to live. But the body's defense system against having excessive sugar is extremely weak.
Also, this glycation occurs from eating and from blood sugar rising, and many easily accessible foods today tend to raise blood sugar levels. So glycation has truly become a threat among threats for modern people.
Why does this glycation accelerate aging? Well, as I said, when we eat food, we metabolize the sugar to create energy. But if we take in more sugar than we need for energy, excess sugar remains. When this excess sugar is broken down, it becomes the AGEs I mentioned earlier. It would be fine if there were only a small amount of these AGEs, but when AGEs become excessive, they bind to proteins. These AGEs denature the proteins, causing them to change color to yellow—this is what causes glycation.
And what's even scarier is that certain proteins have receptors called "Receptor for AGEs" that act like switches. When AGEs attach to these receptors, they produce cytokines that cause chronic inflammation. This sends a signal saying "cause inflammation here." Chronic inflammation occurring means it can lead to arteriosclerosis and other conditions, accelerating aging.
So glycation is actually a very, very scary stress that causes chronic inflammation throughout the body. For example, when this happens in the skin, chronic inflammation occurs in collagen and elastin, causing wrinkles and sagging. Also, AGEs tend to make things brownish. To explain in more detail, this is called carbonylation of proteins, but setting aside the difficult details, it causes proteins to turn yellow, resulting in a loss of skin transparency and a yellowish, dull complexion.
Also, if this occurs in the eyes, it can cause cataracts, and if it occurs in the muscles, it causes inflammation in the muscles, leading to muscle loss known as sarcopenia. In any case, if glycation occurs in any organ, aging progresses. You age in appearance, and inflammation occurs inside the body, potentially causing various diseases.
So what can we do to suppress this glycation stress? There are five points.
First, chew well and eat slowly. You might think, "Is that really it?" But of course, this also relates to excess sugar production—it's said that when blood sugar spikes suddenly, glycation stress increases. Blood sugar rising suddenly happens when you eat too fast, so make sure to eat slowly instead of eating quickly.
Second, as I mentioned, to prevent blood sugar from rising sharply, avoid eating only carbohydrates. For example, if you eat only rice bowls or pasta, that's essentially just carbohydrates, meaning sugar. Eating only that causes blood sugar to spike rapidly, so try having something like a set meal instead, and eat the rice last, avoiding eating patterns that cause sudden blood sugar spikes.
Third, similarly, eat vegetables or soup first. This is because if you suddenly eat carbohydrates like rice or pasta when you're hungry, your blood sugar will spike rapidly. It's known that eating vegetables first, due to the fiber content, suppresses sudden increases in blood sugar, so please eat vegetables first.
Fourth, get moderate exercise. In other words, if you use up energy, you can consume excess sugar before it turns into glycation stress, so try to incorporate moderate exercise daily. However, the key word is "moderate"—if you suddenly decide to jog or run marathons every day starting today, that would lead to oxidative stress instead. So "moderate" is the key point. Something like a leisurely walk where you can still have a conversation is most recommended.
Lastly, avoid sleep deprivation. You might think this isn't related, but actually, sleep deprivation lowers your metabolism, meaning you can't use energy efficiently. This results in excess sugar being produced, which promotes glycation stress. So getting good quality sleep is extremely important not just in general, but specifically for suppressing glycation, so please make sure to get good quality sleep.
To summarize, both oxidation and glycation are scary. Glycation is a stress that has become a threat to our bodies in just the past few decades. This glycation stress causes inflammation because excess sugar binds to proteins, so essentially, you should avoid taking in excess sugar. If you take in too much sugar, your skin can become dull or change to a yellowish color, so when this happens to the skin, it causes not just wrinkles and sagging, but also dullness. And since inflammation in any organ accelerates aging in that organ, please keep in mind that glycation accelerates aging, and work on adjusting your lifestyle habits accordingly.
