Hello everyone, this is Tomori Nitta. Today I want to expose the truth behind "pelvic dieting." I might get some backlash for saying this, but I think many doctors would wonder, "What exactly is pelvic dieting?" When you look at diet-related books or social media, you often see claims like "tightening your pelvis will help you lose weight" or "an open pelvis makes you gain weight easily," strongly linking dieting with the pelvis. But let me say this clearly: the pelvis and dieting are essentially unrelated.
To begin with, the pelvis doesn't really open or close that much. People often talk about the pelvis "opening," but the only time it truly opens even slightly is during childbirth—and even then, it's only a matter of a few millimeters. So what exactly is the pelvis? It's made of bone. Three bones combine to form the pelvis. The pelvis plays a crucial role in firmly connecting the upper and lower body. Because of this, the pelvis itself doesn't just open or close on its own. If it did, you'd have bigger problems than gaining or losing weight—you might not even be able to walk. That's how sturdy the pelvis is; it's not something that opens from bad sitting posture or closes from doing a particular exercise.
That said, pelvic dieting does seem to work for many people, and this is usually in the postpartum period. People often say things like, "After giving birth, your pelvis is open, so you need to tighten it to return to your original shape." Since I've had three children myself, I'm often asked, "What did you do for postpartum pelvic care?" I know many people have lingering doubts about this, so I want to explain it thoroughly.
During pregnancy, since there's a baby inside, it does seem like the pelvis would need to open for childbirth. Indeed, the pelvis does "loosen" slightly at one point. Rather than saying the pelvis "opens" during pregnancy, it's more accurate to say it "loosens." The ligaments connecting the pelvic bones loosen a bit, which is often described as "opening." This happens to help the baby descend. During pregnancy, a hormone called relaxin is released. Relaxin loosens ligaments and muscles, which slightly loosens the joints of the bones forming the pelvis. This makes it easier for the baby to descend.
However, relaxin disappears immediately after childbirth. So the loosened ligaments quickly return to normal. Even though we say "loosen," it doesn't mean things become loose and wobbly—it simply becomes easier for the baby to pass through. In other words, even if the pelvis loosens slightly during childbirth, it naturally returns to normal on its own if left alone. So tightly binding the pelvis doesn't actually accomplish much.
Some might say, "But doctor, after giving birth my hips got bigger and it became harder to lose weight or return to my original shape." This isn't because the pelvis opened, but because of how muscles were used during pregnancy. For example, during pregnancy, as the belly grows, women tend to walk with a bowlegged stance, which reduces the use of inner thigh muscles and can create a swaying walk that makes the hips appear wider. Also, since abdominal muscles go largely unused for almost a year, they weaken, making it easier for fat to accumulate around the belly.
Bones basically don't move. So no matter how much you push on them, it won't do anything. What actually changes your body shape is muscle. How you use your muscles affects the lines of your legs and face. Tightening the pelvis itself has no effect; what actually helps is stimulating muscles that haven't been used, or loosening tense muscles, which restores proper muscle movement and improves body lines. So visiting a chiropractor or getting a massage isn't meaningless, but tightly squeezing your own pelvis won't make you lose weight.
I don't want you to misunderstand: "tightening the pelvis" itself does not lead to weight loss. During pregnancy, it's more accurate to say the ligaments loosen rather than the pelvis opening, and it will return to normal naturally without doing anything. However, during that time, an imbalance in unused muscles can change your body shape. So stimulating your muscles and maintaining good posture is very important, but it's not about fixing things by squeezing with a pelvic belt.
That said, using a girdle after childbirth to help engage the abdominal muscles, or to help with lower back pain, is a good idea. Think of it not as "tightening" the pelvis but as "supporting" it. Taking care of your pelvis is a good thing, and wearing a girdle after childbirth or getting bodywork done—which I did myself—is good too. But this doesn't mean it's because tightening the pelvis leads to weight loss.
Pelvic care means taking care of the muscles supporting the pelvis—namely the pelvic floor muscles and the muscles around the pelvis. It's definitely not about pushing hard to "fix" it. In particular, if you don't care for your pelvic floor muscles after childbirth, it can lead to issues like urinary incontinence, so pelvic floor exercises are very important postpartum. So rather than thinking "shrink the pelvis to lose weight," think of it as "caring for the pelvic floor muscles."
Up to this point, I've been talking about the postpartum period, but even for those who haven't been pregnant or given birth, there are many girdles and leggings sold with claims like "tightening the pelvis leads to weight loss." I'd recommend viewing products with such claims with skepticism. Of course, wearing compression garments can stimulate muscle movement and reduce swelling, which can result in weight loss. But that's not because the pelvis was tightened—it's simply because swelling was reduced. So please don't be fooled by such claims.
To summarize: the pelvis may loosen during pregnancy, but it doesn't actually open. Whatever loosens during pregnancy will naturally tighten back up once the hormone disappears. So during that period, some support may be needed, but there's no need to push or force anything. However, postpartum pelvic floor care is very important, and using muscles that weren't used during pregnancy can help you return to your original shape more easily, so please try that.
Also, regardless of pregnancy or childbirth, there are girdles and leggings sold claiming to "tighten the pelvis," but I don't think these have much effect. If you feel like you've lost weight or your legs look better, it's likely due to reduced swelling or stimulation of unused muscles—not because the pelvis was tightened. So please view products with claims like "tighten the pelvis and lose weight" with a critical eye.
