皆さんこんにちは。内科皮膚科医の友利新です。今日はですね、マスク時代に増えた美容医療ベスト5を皆さんにお話ししたいと思っております。
ちょっと前から話題にはなってたと思うんですけれども、マスクを今するのがもうスタンダードじゃないですか。だからほぼほぼマスクを皆さんしていて、マスクを外す機会って、まあお家の中だったりとかも本当に限られた時間だと思うんですよね。なのでマスクで顔の下半分が覆われているので、今やれる美容のオペだったり施術があるっていうことを考えている方が結構実はいらっしゃって、私が勤めている美容クリニックにも結構患者さんがいらっしゃっています。なのでこのマスク時代に増えた美容医療5つを詳しくお話ししていこうと思います。
ではまず一つ目が、ボトックスになります。ボトックスといえば私もこのYouTubeチャンネルで「ボトックスやってますよ」「美容整形暴露じゃないけどボトックスやってますよ」ってのはもう再三言っていて、前回はここの顎ですね、顎をもうちょっと「まだできないんですか?」ってすると、ここに梅干しみたいなシワができるじゃないですか。それを止めたいので、今もだって力が入らないんですけれども、ここにボトックスを打ちました。でそれが反響だったのか、結構うちのクリニックにも顎のボトックスをしてくださいっていう方だったり、あとマスクをしているからこの眉間のシワがすごく目立っているような気がして、この眉間のシワを止めてくださいとか。あとは私も打ってますけども、おでこはもうちょっと動くようになってきましたけれども、ピシャッと止めるようにはしてなくて、しっかり表情が作れるようにぐらいはしてるんですが、シワにならないようなボトックスを打っているので、こんな感じで、まあマスクをしていると顔の上半分が目立ってしまうから、この眉間とおでこにボトックスを打ってくれっていう患者さんが結構増えています。
ボトックスっていうと表情が全然できなくて仮面みたいな顔になるからちょっと心配だっていう方いらっしゃるんですけれども、もちろんこの顎のところはしっかり入れているのでまだちょっと動きにくいっていうか、こんな顔にしようとして、あえてここにシワを作るような表情することはないので、ここにはしっかり打ってますけれども、例えばおでこはやっぱり「あー」ってびっくりしたり「んー」って表情を作るじゃないですか。なので私はボトックス打ってますけれども表情が作れるように打ってるんですね。なのでボトックスっていうとなんか仮面みたいになるっていうイメージはなくて、打つ場所によってしっかり効かせるところと、そこまでじゃないところを決めることによって表情もできるし、気になるシワを止めるっていうことができるので、結構そういうお話をYouTubeとか話したりとか、あとはインスタとかで話すと結構患者さん多くて、なのでマスク時代結構ボトックス増えています。
続いて2つ目が、おでこのヒアルロン酸になります。これもですね、マスクで覆われているから顔の上半分が気になるということで、韓国の女優さんとかって結構おでこがデコパチで丸くて、なんか赤ちゃんみたいにつるっとした可愛いおでこの女優さんが流行っているので、そういったこともあってこのおでこを丸くする、ちょっと出っ張らせるんですかね、こう丸くするっていう施術が実は密かに流行っています。
私もちょっと興味があるんですが、私はそこまで入れようと思ってないんですけれども、やっぱり若い子では結構ここにしっかり丸く入れていくっていうのが流行っていて、これもですねヒアルロン酸でできることがあるんですね。でだいたいヒアルロン酸っていうのは半年ぐらいで吸収されていくので、「あれおかしいな」と思ってもそれは吸収されていきますから、ちょっと入れてみてこういう丸い可愛いお顔にしたいという方は結構増えています。
ダウンタイムなんですけれども、あの見えないところから注入するので、傷というか注入口ということもほとんどわからないですし、腫れも、例えばヒアルロン酸入れすぎたかなっていう感じで1日目とかはちょっとやっぱり腫れたりする。腫れるって言ってもちょっと入れすぎたかなぐらいですね。でも2日目ぐらいからはそれも落ち着いてはきますので、あんまりなんかダウンタイムもなく、あとあんまりおでこってそんなに皆さん注目してないじゃないですか。だけどちょっと注入するだけでかなり印象が変わるんですね。おでこが丸くなると優しい感じになったりとか、あとちょっと若返って見えるんですよ。なのであの、なんかちょっとメイク変えたみたいな感じで人にはすごくバレないんだけれども、なんとなくいい可愛い印象に変わるということで、結構マスク時代人気になっている施術になります。
続いて3つ目がシミ取りになります。もうねやっぱりシミってこのフェイスラインだったりとか、この目の周りだったりねそういったところに多いじゃないですか。シミっていろいろな種類があるので、もちろん1回でかさぶたにして取るというものもあれば、何回かレーザーだったり光を照射しなきゃいけないということもあるんですけれども、やっぱりですねちょっと目立つ大きなシミっていうのは、そこをちょっとある程度かさぶたにして、それが取れた時にはちょっとまだ色素沈着があるので、日差しに当たるのをやめて美白剤を塗ってっていうようなことを繰り返すんですが、やっぱりマスクをしているとそのかさぶたであるダウンタイムもそんなに気にならないし、かさぶたが取れた後にちょっと茶色っぽく色素沈着をしていても、きちんと美白剤も塗ることができるし、まあ通勤もできるということで、結構マスクの中のシミ取りっていうのは増えてきています。
でもう一つ、私が結構YouTubeでマスクで摩擦して肝斑がひどくなりますよということをお話ししたということもあると思うんですが、やっぱりマスクをしていると肝斑が悪化したりとか、摩擦でシミが悪化するということが多いので、それを予防するというために、大きなシミがなくても光治療だったりとかトーニングとかでそのくすみを飛ばすようなシミ改善というのを受ける方が結構多くなっています。
続いて4つ目はたるみ治療になります。これもかなり最近増えているんですが、マスクをしていると人ってこの顔の下半分が結構緩むっていうことを皆さん言うと思うんですがそれは確かにあると思うのと、あとマスクをしているとこの顔の下の部分って皆さん想像するじゃないですか、初めて会った方でなんとなく顔の上とは同じぐらいかなと思うんですが、これでパッて取った時にかなりたるみがあるとびっくりするんですよね。あと自分もマスクをつけている顔に慣れると、なんとなくマスクを取った時にかなりたるんで見えるということもあります。なのでマスクをして隠しているからこそ、マスクを取った時にこのたるみがすごく気になるんですっていう方が多くて、うちのクリニックではハイフと呼ばれる、要はここをグーッと上げるような治療ですね。プラス、ハイフは上に引き上げるのに対して、サーマクールっていうのはギュギュギュッとちっちゃくするんですね。なのでこのボリュームをちっちゃくしたいっていう、それで脂肪とかが多いからたるむ感じの人はギュッとちっちゃくするようなサーマクール、たるみを上げるっていうのはハイフでやるんですね。なのでギュッと小さくするとか上げるっていう機械を組み合わせてたるみ治療をしたいという方が増えています。
ただやっぱりハイフとかサーマクールでもちょっと間に合わないというか、すごくたるみ切ってしまうという方は、ちょうどマスクをしているので、結構ダウンタイムがある——ハイフとかサーマクールはほとんどダウンタイムがないんですね——なんですけれども、ちょっと腫れてもいいよ、それでもしっかりとこのマスクの間にギュッと上げたいっていう人は糸を入れる方も結構増えています。糸を入れると引きつった顔になるんじゃないかっていう方もいらっしゃるんですが、最近の糸っていうのはすごくナチュラルに効くことも多いので、意外とハイフやってやっぱりもうちょっと綺麗にしたいという欲が出て、それで糸を入れるっていう方も結構多くなっています。
じゃあ糸を入れるんだったらハイフとか意味ないかっていうと、それはまたちょっと違って、柔らかい皮膚を持ち上げるのとちょっと引き締まった厚みのある皮膚を持ち上げるのでは全然違うんですね。言うなればお豆腐を持ち上げるのって結構大変じゃないですか。でも油揚げを持ち上げるって結構楽というか、糸で引き上げるってわかりますよね。なのでそういう皮膚の厚みというかこう収縮させて、厚みを持たせて引き上げるのがハイフとかサーマクールなので、もちろん糸をするまではいかなくても継続的にハイフとかサーマクールをすることで皮膚の支持組織をしっかりして、いつかもうどうしてももうこれでは無理だっていう時に向けて、糸に向けてやるっていうこともいいかなと思って、私もちょっとハイフとかサーマクールでうまく、もうちょっとそろそろ無理だなと思ったらそっちに移行するために、今は肌の支持組織をしっかりさせるためにもハイフとかサーマクールを私もやっております。
続いて5つ目が、これはかなり上級者になるんですが、鼻の手術になります。皆さんね、美人って目が二重とか目がパッチリしてるってイメージだと思うんですが、実はお顔の印象を変えるのは鼻なんですね。なので美人と言われる女優さんのお顔を思い出してもらうと、あまり主張しすぎない、大きすぎてもないし、鼻の穴が見えているとか鼻の位置が横幅が大きいとか、そういう主張のない鼻なんですね。もちろん高さとかもあるんですが、忘れ鼻と言われるぐらい鼻があまり主張しない方が多いんですよ。
なので上級者になってくると、目じゃなくて鼻の印象を変えて美人度アップさせたいという方がいて、でも鼻のオペって結構終わった後に大体の方がここを切ってビョンって開けるので、ここにちょっと傷ができたりするので、やっぱりマスクで隠している時間が増えるんですよ。なので前はマスクがスタンダードじゃない時っていうのは「飲んでマスクしてるの?」って言われちゃうことがあるんですが、今って「なんでマスクしないの?」って言われる時代じゃないですか。だから心置きなく鼻のオペをしたいという方が増えているらしくて、特にうちのクリニック、表参道スキンクリニックではですね、鼻の上手い先生が男の先生何人かいるので、昔はプロテーゼを入れて鼻の高さを出したいっていう方多かったんですが、最近は鼻の鼻尖といって、ここちょっと尖らせたりとか幅をちょっとちっちゃくしたりとか、あとここの鼻中隔というのをちょっと出すっていうような鼻が流行っていて、それってプロテーゼみたいな異物を入れなくても自分の耳の耳介軟骨と呼ばれる耳の軟骨ですることができるので、プロテーゼを入れていると年を取るとそれがちょっと石灰化しちゃったりすることがあるんですが、自分のものだとそれが安心ということで、結構鼻のオペされる方増えてます。ダウンタイムは、抜糸するのに1週間ぐらいで、この赤みがわからなくなるのはまあ数ヶ月かかるんですが、マスクをしているとあんまりわからないし、まあわざわざここをじっと見る方ってなかなかいらっしゃらないと思うので、さっき言ったみたいにお顔の印象ってすごく変わるんですね。ここの高さを出さなくても、この鼻尖だけ変えてもお顔が華やかに見えるというか。私はビフォーアフターの写真を見せてもらうのでわかるんですが、通常そこまでビフォーアフター見ないと、すごくはもうわからなかったとしても、すごく美人度が上がるんですよね。これもおでこのヒアルロン酸と一緒で、あれなんかちょっとメイク変えたとか痩せたんじゃないって言われて、すごく美人度がアップされる。もちろんこれは本当に美容整形という形になるんですが、結構マスク時代増えているものになります。

Hello everyone, I'm Arata Tomori, a physician specializing in internal medicine and dermatology. Today I'd like to talk about the top 5 cosmetic treatments that have increased during the mask era.
This has been a topic of discussion for a while now, but wearing masks is now the standard, right? So almost everyone wears masks, and the opportunities to take them off—like at home—are really limited. Because the lower half of the face is covered by masks, quite a few people are actually considering cosmetic procedures or treatments they can do now, and I've seen many such patients at the cosmetic clinic where I work. So today I'll talk in detail about these 5 cosmetic treatments that have increased during the mask era.
First is Botox. Speaking of Botox, I've mentioned repeatedly on this YouTube channel that I get Botox myself—not that this is a cosmetic surgery exposé, but I do get Botox. Last time, I talked about my chin—when I try to do this expression like "can't you still do it?", it creates wrinkles like pickled plums here. To stop that, since I can't apply force there anyway, I got Botox injected here. Maybe because of that response, quite a few patients have come to our clinic asking for chin Botox, or saying that because they wear masks, their frown lines between the eyebrows seem to stand out more, asking to have those stopped. As for my forehead, it's become a bit more mobile now, but I don't make it completely frozen—I keep enough movement to make expressions, while still getting Botox that prevents wrinkles from forming. So because masks hide the lower half of the face, the upper half becomes more noticeable, which is why more patients are asking for Botox in the forehead and between the eyebrows.
Some people worry that Botox makes your face expressionless, like a mask, but of course this area on my chin is done firmly so it's still a bit hard to move—but I never intentionally make an expression that would create wrinkles there anyway, so it's fine to treat firmly there. But for the forehead, you do make expressions like being surprised or thinking, right? So I get Botox but in a way that still allows expressions. The image of Botox making you look like a mask isn't necessarily true—by deciding where to apply it firmly and where to be more moderate, you can still make expressions while stopping the wrinkles that bother you. When I talk about this on YouTube or Instagram, I get a lot of patient interest, so Botox has definitely increased during the mask era.
Second is forehead hyaluronic acid injections. Since the upper half of the face is more noticeable because it's not covered by masks, and Korean actresses often have this round, baby-like, smooth forehead that's become trendy, there's actually a quietly popular treatment to round out and slightly project the forehead.
I'm personally interested too, though I don't plan to get much done myself, but among younger people, getting this area filled in nicely and rounded is quite trendy. This can be done with hyaluronic acid. Since hyaluronic acid generally gets absorbed within about six months, even if you think "that's odd" afterward, it will be reabsorbed, so quite a few people are trying it to achieve this cute, round-faced look.
As for downtime, since the injection is done from an area that isn't visible, you can barely notice the injection site itself. There might be some swelling—like feeling "maybe I put in too much"—on the first day, but that kind of swelling settles down by the second day. Also, people don't pay that much attention to foreheads normally, but just a little injection can significantly change your impression. A rounder forehead gives a gentler feel and can make you look younger. So it's like changing your makeup slightly—people won't really notice you did anything, but somehow your impression becomes cuter, which is why this treatment has become popular during the mask era.
Third is spot/blemish removal. Spots tend to appear a lot along the jawline or around the eyes, right? There are various types of spots—some can be removed in one session by scabbing them off, while others require multiple laser or light treatments. For particularly noticeable large spots, the process typically involves creating a scab, and after it comes off there's still some pigmentation, so you need to avoid sun exposure and apply whitening treatments repeatedly. With masks on, that downtime from having a scab isn't as noticeable, and even if there's some brownish pigmentation after the scab falls off, you can still properly apply whitening treatments and go to work as normal. So spot removal under masks has been increasing.
Also, I've talked on YouTube before about how masks can worsen melasma due to friction, and how friction can worsen spots in general. To prevent this, more people are getting light therapy or toning treatments to reduce dullness and improve spots, even without having large visible spots.
Fourth is sagging (lifting) treatments. This has increased a lot recently too. People say that with masks on, the lower half of the face tends to sag more, and I think that's true. Also, when wearing a mask, people tend to imagine that the lower half of someone's face is probably similar to the upper half, so meeting someone for the first time and then seeing them without a mask can be surprising if there's significant sagging. Also, once you get used to seeing your own face with a mask, taking it off can make the sagging look more pronounced than you remembered. So because the mask hides it, people become very concerned about sagging when they remove the mask. At our clinic, we use HIFU, which is a treatment that lifts the skin, and in addition, Thermacool, which tightens and shrinks the skin. So for people with more fat or volume causing sagging, Thermacool tightens things up, while HIFU is used to lift sagging skin. So more people are combining these machines—one that tightens and one that lifts—to treat sagging.
However, for people whose sagging is too severe for HIFU or Thermacool alone, since they're already wearing masks anyway (even though HIFU and Thermacool have almost no downtime), more people are opting for thread lifts, accepting some swelling, to firmly lift things up during this mask-wearing period. Some worry that thread lifts will make their face look overly tight, but recent threads tend to work in a much more natural way. Interestingly, many people who do HIFU end up wanting even more improvement and then get thread lifts as well.
So does that mean HIFU is pointless if you're going to get threads anyway? Not at all—lifting soft skin versus lifting firmer, thicker skin are completely different processes. It's like the difference between lifting tofu versus lifting a thin fried tofu pouch (aburaage)—lifting the pouch with thread is much easier. So treatments like HIFU and Thermacool work by contracting and thickening the skin's supportive tissue, making it firmer. Even if you don't proceed to threads, continuing HIFU or Thermacool regularly strengthens the skin's support structure, preparing you for the day when you might eventually need threads. I do this myself too—continuing HIFU and Thermacool now to strengthen my skin's support structure, in preparation for transitioning to threads someday when needed.
Fifth, this is quite an advanced-level treatment—it's nose surgery. Many people think of double eyelids or big round eyes when they imagine a beautiful face, but actually, the nose is what really changes the overall facial impression. If you recall actresses considered beautiful, their noses usually don't stand out too much—not too large, nostrils aren't visible, and the width isn't too wide. It's a nose that doesn't draw attention to itself. Height matters too, but many beautiful noses are the type people might forget entirely because they're so well-balanced.
So for more advanced cases, people want to change their nose rather than their eyes to boost their overall attractiveness. However, nose surgery usually involves an incision that leaves some visible marks afterward, so people appreciate having more time to hide it behind a mask. In the past, if you wore a mask when it wasn't the norm, people might ask "are you sick or something?" But nowadays, it's more like "why aren't you wearing a mask?" So people feel freer to get nose surgery without worrying about it being noticed. Especially at our clinic, Omotesando Skin Clinic, we have several skilled male doctors specializing in nose surgery. In the past, many people wanted implants to increase nasal height, but recently, procedures focusing on the nasal tip—making it sharper or narrower—or adjusting the nasal columella have become more popular. These can be done using the patient's own ear cartilage instead of foreign implants. With implants, there's a risk of calcification as you age, but using your own tissue feels safer, so more people are opting for this type of nose surgery. Downtime involves about a week until stitches are removed, and the redness can take a few months to fully fade, but with a mask on, it's hardly noticeable, and people rarely stare closely at someone's nose anyway. As I mentioned, changing the nose can dramatically alter someone's facial impression—even without changing the height, just adjusting the tip can make the face look more striking. I see before-and-after photos, so I notice the difference clearly, but even people who don't look closely at before-and-afters will still notice that the person looks noticeably more attractive. Similar to the forehead hyaluronic acid, people might just think "did they change their makeup?" or "did they lose weight?" without realizing surgery was involved, yet their overall attractiveness increases significantly. Of course, this falls under proper cosmetic surgery, but it's definitely something that has increased during the mask era.
