【頭皮ケア】皮膚科医がオススメの頭皮ケアを教えます

スキンケアについてはいろいろ取り上げてきましたが、頭皮はなかなか扱ってきませんでした。ヘアケアについては何度かご紹介したことがありますが、頭皮は実は非常に特殊な環境にある場所です。夏になると汗で濡れて痒みが出たり、その痒みが汗によるものなのか乾燥によるものなのか分かりにくかったりしますよね。
今日は、なぜ頭皮にこうした症状が起こりやすいのか、それに対してどのようなスキンケア・頭皮ケアをすればよいのか、そしてどんな時に病院に行くべきなのかについてお話しします。よろしければ最後までご覧ください。
■頭皮の特殊性
頭皮は毛で覆われているため、紫外線の影響を顔ほど直接受けません。また、頭皮は顔と比べて皮脂の分泌量が非常に多いことで知られています。頭皮が油っぽくなりやすいのはそのためです。
しかし面白いことに、皮脂は多いのに水分を維持する能力は顔の皮膚よりも低いことが分かっています。つまり「油っぽいのに乾燥しやすい」という特殊な環境なのです。
このバランスが崩れると、皮脂が多すぎても痒みが出ますし、逆に皮脂を取りすぎても乾燥して痒みが出ます。ですから、皮脂は多すぎてもいけないのである程度シャンプーなどでしっかり落とす必要がありますが、取りすぎると今度は乾燥して炎症を起こし、痒みにつながってしまいます。
頭皮に痒みがある場合、「皮脂が多すぎるパターン」か「皮脂を取りすぎて乾燥しているパターン」のどちらかであることが多いです。自分の頭皮がベタついているか、逆にケアをしすぎてかえって痒みがひどくなっていないか、使っているヘアケア剤や生活環境から判断してみてください。
特に若い男性は男性ホルモンの影響で皮脂分泌が多く、頭皮が油っぽくなりがちです。そのため皮脂を取り去るタイプのシャンプーを使うことが多いのですが、それが原因で痒みが出ることもあります。大人になると皮脂は出るのに乾燥もするという悪循環に陥りやすいため、特に男性は頭皮トラブルが起こりやすいことを覚えておいてください。
■病院に行くべき境界線
痒みだけならすぐに病院に行くべきか判断が難しいものです。しかし、頭皮を見て赤くなっていたり、掻いた後にかさぶたのようなものがあれば、必ず病院に行ってください。それは乾燥を通り越して炎症を起こしている状態で、頭皮湿疹などの可能性があります。乾燥によるものか、皮脂によるかぶれ(脂漏性湿疹)なのかは、医師の診断のもとで適切な薬を処方してもらう必要があります。
一方、赤みもなく、少し痒い程度で頭皮の調子が悪いかな、油っぽくなってきたかなという程度であれば、ヘアケアで十分対応できる範囲だと思います。

【皮脂が多くて気になる方向け】
毎日のシャンプーは、できるだけ潤いを残すタイプで皮脂を取りすぎないものを使うのがおすすめです。それでも皮脂の匂いや油っぽさが気になる時は、週に1〜2回、頭皮の皮脂を優しく取り除く専用アイテムを使うとよいでしょう。
おすすめは「イグニス イオ ヘッドクレンジングシャンプー」です。頭皮に直接塗布して洗うタイプのシャンプーで、アミノ酸系の優しい界面活性剤を使用しています。さらにカオリンとベントナイトという2種類の泥成分が配合されており、これらが皮脂や汚れを吸着して落としてくれます。ゴシゴシ洗い流すのではなく、余分な皮脂を吸着して取り除くイメージです。
イグニスは女性向けブランドではありますが、男女兼用で問題なく使えます。男性向け商品にありがちな「洗浄力が強すぎる」ということもなく、汚れはしっかり落としつつ必要な皮脂は残してくれるので、香りも含めておすすめです。

頭皮に化粧水のような保湿剤を使うのは難しいと感じる方も多いと思います。それは頭皮が水分を維持する能力が低いためです。皮膚は基底層から生まれ、ターンオーバーの過程で細胞間脂質やセラミドを生成し、それが肌の水分保持につながります。しかし頭皮はターンオーバーが早く、これらが十分に作られにくい場所です。ですから、単に保湿成分を与えるだけでなく、ターンオーバーを正常化することが重要です。
おすすめは「アトピスマイル 薬用頭皮ローション」です。頭皮に直接なじませるタイプのローションで、ベタつかずスッと浸透します。医薬部外品で、有効成分「ライスパワーNo.11」が配合されています。この成分は乾燥改善効果が認められており、水分を与えるのではなく基底層に働きかけてターンオーバーを正常化し、セラミドや細胞間脂質の生成を促して水分保持能力を高めてくれます。頭皮専用の保湿ケアアイテムはあまり多くないので、乾燥による痒みでお悩みの方にはよくおすすめしています。

Hello everyone, this is Tomoari Arata. Today I'd like to talk about scalp care, a topic many of you have requested.
I've covered many aspects of skincare before, but I haven't really addressed the scalp specifically. I've introduced hair care a few times, but the scalp is actually in a very unique environment. In summer, it can get wet with sweat and become itchy, and it's often hard to tell whether that itchiness comes from sweat or dryness.
Today, I'll explain why the scalp is prone to these symptoms, what kind of skincare or scalp care you should do about it, and when you should see a dermatologist. Please watch until the end.
■ Why the Scalp Is Special
The scalp is covered with hair, so it doesn't receive UV damage as directly as the face does. Also, compared to facial skin, the scalp is known to produce much more sebum, which is why it tends to feel oily.
Interestingly, though, despite producing more sebum, the scalp's ability to retain moisture is actually lower than that of facial skin. In other words, it's a unique environment that is "oily yet prone to dryness."
When this balance is disrupted, too much sebum can cause itchiness, but removing too much sebum can also lead to dryness and itchiness. So sebum shouldn't be excessive—it needs to be properly cleansed with shampoo to some degree—but if you strip away too much oil, the scalp becomes dry, leading to inflammation and itchiness.
When the scalp is itchy, it's usually either due to excess sebum or due to dryness from over-cleansing. You can judge which applies to you based on whether your scalp feels greasy, or whether your itchiness is worsening despite (or because of) using strong cleansing products—please assess this based on the hair products you use and your environment.
Young men in particular tend to have more sebum production due to male hormones, making their scalps oilier. Because of this, they often use shampoos designed to strip away oil, but this can actually cause itchiness. As people get older, oil production continues but dryness also increases, creating a vicious cycle—so men especially should keep in mind that they're prone to scalp trouble.
■ When to See a Doctor
Itchiness alone can be hard to judge—it doesn't necessarily mean you need to rush to the hospital. However, if your scalp looks red or if you notice scab-like formations after scratching, you should definitely see a doctor. This indicates inflammation beyond mere dryness, possibly scalp eczema. Whether it's caused by dryness or by irritation from your own sebum (seborrheic dermatitis) needs to be diagnosed by a doctor so you can get the appropriate medication.
On the other hand, if there's no redness and it's just a mild itch or a slightly oily feeling, that's likely something you can manage with hair care products.
■ Recommended Products
[For those concerned about excess oil]
For daily shampooing, it's best to use a moisturizing type that doesn't strip away too much oil. If the smell or oiliness still bothers you, try using a specialized scalp product once or twice a week to gently remove excess sebum.
I recommend the "Ignis Io Head Cleansing Shampoo." It's applied directly to the scalp for cleansing and uses a gentle amino acid-based surfactant. It also contains two types of clay—kaolin and bentonite—which absorb sebum and dirt. Rather than scrubbing everything away, it works by absorbing and lifting away excess oil.
Although Ignis is a brand aimed at women, it can be used by anyone regardless of gender. Unlike many men's products that tend to over-cleanse, this one removes dirt effectively while still leaving necessary oils intact—plus the scent is wonderful, so I highly recommend it.
[For those concerned about dryness]
Many people find it difficult to apply something like facial lotion to the scalp, and that's because the scalp has a lower moisture-retention capacity. Skin is formed from the basal layer, and during the turnover process, it produces intercellular lipids and ceramides that help retain moisture. However, the scalp's turnover is faster, making it harder to sufficiently produce these components. So it's important not just to add moisture, but to normalize the turnover process itself.
I recommend "Atopis Smile Medicated Scalp Lotion." This lotion is applied directly to the scalp and absorbs smoothly without feeling sticky. It's a quasi-drug containing the active ingredient "Rice Power No. 11," which is recognized for improving dryness. Rather than simply adding moisture, it works on the basal layer to normalize turnover, promoting the production of ceramides and intercellular lipids, which enhances the scalp's moisture retention. Since there aren't many scalp-specific moisturizing products out there, I often recommend this to people suffering from dryness-related itchiness.
■ In Summary
The scalp is a very unique area where you shouldn't remove too much oil, but you also shouldn't leave too much oil untouched. Please choose your shampoo carefully.
As a bit of a plug—when I see patients with scalp troubles at my clinic, I often find that many of them are over-cleansing. That's why, like the Ignis product I mentioned, I recommend something that doesn't strip away too much oil. When I helped develop the "Vitalism" shampoo myself, I designed it so that scalp care could be done through daily use.
I think it's best to choose a daily shampoo that focuses on scalp care, and then use special care items like the ones I introduced today when that's not quite enough.
