Hello, doctor. Now that it's January, as mentioned in the thumbnail title, let's talk about the 2026 annual beauty schedule you're planning to follow together.
I see. Since it's the beginning of the year, rather than doing beauty treatments haphazardly, it makes sense to set up an annual schedule.
Exactly. This includes beauty medical treatments too, and since morale matters, I thought it might be helpful as a reference. I've actually been asking fellow dermatologists and beauty industry colleagues how they plan their schedules.
Indeed, we haven't really discussed this topic before. So shall we call this "The 2026 Annual Beauty Medical Schedule You'll Follow Together"?
Yes. Let me share what I'm actually struggling with right now. I have oily skin that gets shiny, and I've been fighting pores and sebum for a long time. As that battle has receded a bit—maybe it's my age, since I'll be turning 48 soon—things have finally calmed down, and I'm starting to experience some dryness too. Until now, my focus was on removing excess sebum and dealing with acne scars, but I feel like that journey wrapped up around 2025.
Is dryness something you notice in daily life?
I've noticed that I don't produce as much sebum anymore, and my acne scars have faded somewhat. Of course, I've never once had acne in my life, so I'm still far from having perfectly smooth skin like some people. But I've been thinking, do I really need to keep doing more? I've done things like peels suited to my skin, needle RF treatments like Potenza, and Blessing to a certain degree, so I feel like my more-than-40-year battle with pores has come to a good resting point.
So my priorities have shifted. Even though I tend toward inner dryness, my sebum hasn't completely stopped, so I want to focus on increasing moisture levels first. Then there's sagging and firmness. In the past, my face was puffy and taut like Anpanman's, so I wanted to reduce that excessive firmness, or rather calm it down a bit. So while I wanted the tightening effect, I honestly used to think, "Who on earth wants a puffy, taut face?" But now I understand that feeling.
Noboru-kun, you currently have that wonderfully firm, taut face, and I've come to realize that's actually a symbol of youth. Right now I'm concerned about sagging in that area. As for spots, I've been getting regular laser treatments, so uneven pigmentation isn't a major concern, but it comes back if I stop.
So my three beauty themes for 2026 are: 1) lack of firmness, 2) inner dryness, and 3) improving uneven skin tone. Please think of this as the annual schedule for addressing these through beauty medical treatments.
Having a theme might be a good idea. Doing everything at once would be difficult.
Exactly. It's hard to compare yourself to people who were born with naturally beautiful skin, so rather than comparing with others, it's better to set priorities so you don't get lost. Results don't come from doing something just once—it takes time. Even my pores becoming less of a concern is the result of continuously treating them over a long period.
Some people might think beauty medical treatments are like magic, but they're really not. It's about building a foundation through skincare and then enhancing it—not becoming a completely different person overnight. Since my concerns have shifted this time, I think this could be a useful reference for those with similar concerns.
Also, you can't do beauty treatments every single day—you need downtime and recovery periods. I want to share things from that perspective too.
[January–March]
This is a dry period, right? During this time, I want to thoroughly boost skin moisture levels and strengthen the barrier function, in preparation for when UV rays intensify around April-May. Of course I'll still wear sunscreen, but to strengthen the underlying barrier, moisture and preventing inflammation are key. So I want to do skin-boosting injections to increase moisture. I'll be doing something called Rejuran, and Fill Medical's Densifyll. Rejuran is originally made from salmon testes-derived PDRN, and Densifyll adds hyaluronic acid and other anti-aging ingredients to that. Younger people might be fine with just Rejuran, but for someone my age, Densifyll is probably better. You do it once a month for three months, and then you can leave a six-month gap. So if I do it in January, February, March, by the time UV exposure increases, my barrier function will be in place. PDRN also calms skin inflammation and is even used in burn treatments, so it's great for boosting the skin's potential.
During January-March, if I have time, I'd also like to use a machine that boosts collagen—the InMode Forma available at Noboru-san's skin clinic. It's a radiofrequency device similar to Thermage. What's the difference from Thermage? Thermage is the king of radiofrequency devices—it really tightens the skin. InMode, in a good way, isn't quite as intense, so you can do it every 2-3 months, and it enhances collagen repair, giving the skin a plump, firm appearance. Until now I was always doing Thermage to tighten things up, but since I want to increase collagen this time, I'd like to try InMode. But fundamentally, I want to focus on skin treatments that boost moisture.
Are you doing this every month?
You do it once a month, and if you do that for three months, the effect lasts for six months. So if I finish in March, the calculation is that my skin's barrier function will remain elevated until around September. That coincides with when summer UV exposure is strong, so having my skin's repair capacity elevated during that time seems really beneficial, so I want to do it.
[April–May]
As UV rays get stronger, I don't want to switch on the genes that produce melanin. I want to do light therapy—specifically a machine called BBL, and one called XLV for reducing redness. There are various light therapy machines, but BBL has data showing it acts on the genes that determine skin color, suppressing melanin synthesis. Traditionally, light therapy has been thought of as removing existing spots, but BBL has shown effects in preventing the melanin-producing switch from being triggered in the first place. So I want to do this in April-May, maybe starting around June, so that by July-August when UV is strong, the switch won't get triggered.
[June]
Serious sagging treatment. This means Ulthera and Sofwave. Among HIFU devices, I consider Ulthera to be the absolute top, working on the muscle layer, while Sofwave thoroughly treats the upper dermis layer. Combining them lifts the skin while smoothing out wrinkles, so I want to schedule this for June.
Can people just decide to do Ulthera and Sofwave on their own whenever they're concerned?
If sagging is due to fat, Thermage-type treatments are probably better. If it's sagging without firmness, InMode-type treatments might be better. If there's excess skin, Sofwave is probably better. If you want to lift from the muscle layer, HIFU—especially Ulthera—is probably better. It's not that everyone in their 20s or 30s should do all of these, but for me, with sagging muscles, sagging skin, and diminishing collagen—being an adult is tough—I want to do HIFU and Sofwave, and ideally layer on some Thermage too. If InMode-type treatments already give enough firmness, I might skip Thermage—that really depends on consulting with my own skin.
[July–August]
Since UV exposure is high, this is a defensive period. I'll use treatments like tranexamic acid delivered via electroporation, a device available at clinics that creates tiny channels in the skin to deliver ingredients, especially after sun exposure. I'll do this regularly during this time.
[September]
Since July-August involves heavy UV exposure, the stratum corneum tends to thicken. So starting around September, I want to support skin turnover. I'll do moisture-retentive peels like AquaPeel, along with a firming treatment called Genesis, applying a bit of heat to the skin. September seems like a good time for this.
[October–December]
Dryness returns during this period, so since the skin-boosting injections last about six months, I'll do another round. And in December, I'd like to get a facelift.
What's your thinking on the facelift? Have you already decided to go through with it?
Yes, it's decided.
Does it really make that much of a difference?
Seeing how beautiful patients become afterward, yes. As for timing, I'm considering it based on work schedule, but I'm hoping to do it this year. Of course, cutting and stitching the skin is the ultimate anti-sagging treatment, but if you don't maintain firmness and even skin tone along the way, it can end up feeling like you're just doing skincare in preparation for an eventual facelift.
If you do it in December, let me know! We should film it—both the process of the facelift and the recovery period afterward.
Skincare during that time would be really important too, right? It's definitely different.
Yes, completely different. You can't touch your face, and you basically soak your face in a bowl of toner—it's that gentle. Originally I thought I'd do it at 50, but I don't think I can wait that long. I might as well do it now. I probably should get my neck done first, but since facelifts require general anesthesia, I'd like to have everything done at the same time. It would be nice if it could all be finished in one go.
It's not that convenient, though. December is really more of a dream—it would be great if I could do it—but everything else I've talked about is quite realistic, so I hope it's helpful as a reference. As I always say, beauty medical treatments aren't magic, and there's always downtime involved. Whether you actually do everything I've talked about today depends on assessing your own skin type and condition. Also, proper skincare to build a solid foundation is essential—without that base, treatments won't have much effect. Like I always say, if your baseline is 1, multiplying by 2 only gets you to 2. But if your baseline is 5, multiplying by 2 gets you to 10. Because you raise the baseline, the beauty treatments that boost it become much more effective. So if you're interested in beauty medical treatments, I recommend getting a consultation.
So, how was that?
Planning things out in January really is important.
Yes. Let's check our results together in December.
Thank you very much for today.
Thank you.
