飲むだけで痩せる薬について解説します。

皆さんこんにちは、内科・皮膚科医の友利新です。段々と体型が気になってきて、ボディの悩みが増える季節ですよね。夏に向かって「少し痩せたいな」と思うのは、多くの方に共通する気持ちだと思います。そんな時によく聞かれるのが、「飲むだけで痩せるお薬ってありませんか?」というご質問です。
もちろん、サプリメントで痩せることは基本的にありません。以前のダイエットに関する動画でもお話ししましたが、ネット広告などで見かける「飲むだけで痩せる」というサプリの99.9999%は嘘だと思います。なぜ100%と言わないかというと、私が全てを試したわけではないからですが、基本的には効果はないと考えています。
サプリには痩せる効果は基本的にないのですが、市販でも手に入るお薬の中に、実は効果があるものが存在します。それは薬局でも購入できるお薬です。「そんな魔法のようなお薬があるんですか?」と思われるかもしれませんが、いくつか知っておくべき点もあります。今日は、市販でも買える痩せる漢方薬について、エビデンスを示しながらしっかりとお話しします。また、どういう人に効果があり、どういう人には向かないかについても説明しますので、ぜひ最後までご覧ください。
その漢方薬とは「防風通聖散(ぼうふうつうしょうさん)」です。「聞いたことない」という方もいらっしゃるかもしれませんが、例えばドラッグストアで「お腹の脂肪が気になる方へ」と書かれた漢方薬(例:ナイシトール)の多くには、この防風通聖散が含まれています。私も内科のメタボリック外来において、適応がある場合にこの防風通聖散を処方することがあります。
では、防風通聖散はどのように効くのでしょうか。これは科学的にエビデンスが明らかになっています。防風通聖散には「麻黄(まおう)」という生薬が含まれており、麻黄にはエフェドリンという成分が含まれています。エフェドリンは交感神経を刺激して代謝をアップさせる効果があります。
詳しく説明すると、エフェドリンは交感神経の末端からノルエピネフリンの分泌を促進します。このノルエピネフリンが、体の熱産生を担う「褐色脂肪細胞」の働きを活発にします。これにより熱産生が高まり、代謝が上がって痩せていくという仕組みです。また、ノルエピネフリンは肥満の原因とされる「白色脂肪細胞」の働きを低下させることも分かっています。つまり、脂肪を燃やす働きを促進しながら、脂肪の蓄積を抑えてくれるという、非常に優れた作用を持つ漢方薬なのです。
さらに、防風通聖散に含まれる「甘草(かんぞう)」や「桔梗(ききょう)」「連翹(れんぎょう)」などの生薬には、代謝が上がった際にそれを抑制しようとする体の働きを抑える効果があります。代謝が上がりすぎると体がそれを抑制しようとしますが、これらの生薬がその抑制をブロックすることで、代謝が上がった状態を維持するサポートをしてくれます。
このように、麻黄が代謝をアップして脂肪の蓄積を抑え、甘草・桔梗・連翹などが代謝が上がった状態を持続させる、という科学的な仕組みが明らかになっています。
実際に飲んだ人と飲まなかった人を比較した研究もあります。肥満の女性を対象に、1日1200キロカロリーの食事療法と、1日約350キロカロリーを消費するウォーキング等の運動療法を組み合わせ、そこに防風通聖散の内服を加えたところ、6ヶ月後に約5キロの体重減少が報告されています。
また、糖代謝異常(糖尿病予備軍)と肥満を併せ持つ方を対象に、食事療法・1日5000歩以上のウォーキング・ストレスマネジメントを実施したうえで、防風通聖散を飲む群とプラセボ(偽薬)を飲む群を比較した研究もあります。12週間後はどちらの群も約5.9キロの体重減少で差はありませんでしたが、24週間後(6ヶ月後)には、防風通聖散を飲んでいた群では平均10キロの体重減少、体脂肪はなんと9.4キロも減少していました。一方、プラセボ群では6.9キロの減少にとどまり、12週時点とあまり変わりませんでした。
この研究から、防風通聖散を食事療法・運動療法と併用することで体重が落ちやすくなり、内臓脂肪も一緒に減少しやすくなることが分かっています。
ただし、ここで重要な点があります。この研究の対象者は「肥満のある女性」および「糖代謝異常と肥満のある方」でした。つまり、普通体型の方が防風通聖散を飲んでも、それ以上に痩せるわけではありません。あくまでも内臓脂肪が多く、肥満傾向にある方が、食事療法・運動療法と組み合わせることで効果が期待できる、という点を理解しておいてください。
BMIが正常範囲(概ね24以下)の方が飲んでも効果は期待できませんし、肥満の方であっても防風通聖散を飲むだけで痩せるわけではありません。食事療法・運動療法と組み合わせることで、体が脂肪を燃やすのを後押ししてくれる、というものです。
また、漢方薬には体質によって向き・不向きがあります。防風通聖散は「実証(じっしょう)」、つまり体格ががっしりしていて、汗かきで、お腹がぽっこり出ているタイプの方に向いています。一方、虚弱体質で冷え性があり、水太りで脂肪が柔らかいタイプの方には、あまり効果がないとされています。
お腹周りの脂肪が気になる方で、ダイエットのサポートをお考えの方は、薬局で防風通聖散を購入するか、肥満外来の医師に相談してみてください。ただし、あくまでも食事療法・運動療法と組み合わせて使用することが大切です。
「飲むだけで痩せる」という魔法の薬は残念ながら存在しません。食事療法・運動療法を組み合わせながら、ダイエットをサポートしてくれるお薬、という認識でご活用いただければと思います。

Hello everyone, I'm Arata Tomori, an internal medicine and dermatologist. As the seasons change, many of us start thinking more about our bodies. As summer approaches, the desire to lose a little weight is something many of us share. One of the most common questions I receive is, 'Doctor, is there a pill I can just take to lose weight?'
First of all, supplements will not make you lose weight. As I discussed in a previous video on dieting, I believe that 99.9999% of the 'just take this and lose weight' supplements advertised online are simply false claims. The reason I don't say 100% is that I haven't personally tested every single one, but fundamentally, I believe none of them work.
That said, while supplements are ineffective, there are actually some over-the-counter medications that do have a real effect on weight loss — medications you can buy at a pharmacy. You might be thinking, 'Really? A magic pill like that actually exists?' Well, there are some important nuances to understand, so today I'd like to explain the evidence behind one herbal weight-loss medicine available over the counter, and discuss who it works for and who it doesn't. Please watch until the end.
The herbal medicine I'm referring to is called Bofutsushosan (防風通聖散). You may not have heard the name, but if you've ever been to a drugstore and seen a product like Naishitol, marketed for people concerned about abdominal fat, that product most likely contains Bofutsushosan. I myself prescribe Bofutsushosan at my metabolic syndrome clinic when patients meet the appropriate criteria.
So how does Bofutsushosan work? Its mechanism is scientifically well understood. Bofutsushosan contains a herb called Mahuang (ephedra), which contains a compound called ephedrine. Ephedrine stimulates the sympathetic nervous system, boosting metabolism.
More specifically, ephedrine promotes the release of norepinephrine from sympathetic nerve endings. Norepinephrine activates brown adipose tissue (brown fat), which is responsible for generating body heat. This increases thermogenesis and raises metabolic rate, leading to fat burning. Additionally, norepinephrine has been shown to suppress the activity of white adipose tissue — the type of fat associated with obesity. In other words, Bofutsushosan simultaneously promotes fat burning and inhibits fat storage, making it a remarkably well-designed herbal formula.
Furthermore, other herbs in Bofutsushosan — such as Glycyrrhiza (licorice root), Platycodon (balloon flower root), and Forsythia — help counteract the body's natural tendency to suppress elevated metabolism. When metabolism rises, the body attempts to bring it back down as a regulatory response. These herbs inhibit that suppressive mechanism, helping to sustain an elevated metabolic state.
In summary: Mahuang (via ephedrine and norepinephrine) boosts metabolism and suppresses fat accumulation, while Glycyrrhiza, Platycodon, and Forsythia help maintain that elevated metabolic state by blocking the body's counter-regulatory response.
There are also clinical studies comparing outcomes between people who took Bofutsushosan and those who did not. In one study involving obese women who followed a 1,200-calorie diet combined with exercise (such as walking to burn approximately 350 calories per day), the addition of Bofutsushosan resulted in an average weight loss of approximately 5 kilograms after 6 months.
In another study involving individuals with impaired glucose metabolism (prediabetes) and obesity, participants followed a diet plan, walked at least 5,000 steps per day, and received stress management support. They were then divided into two groups: one taking Bofutsushosan and one taking a placebo. After 12 weeks, both groups showed similar weight loss of approximately 5.9 kilograms — not surprising, since both groups were following diet and exercise regimens. However, after 24 weeks (6 months), the Bofutsushosan group had lost an average of 10 kilograms, with body fat decreasing by 9.4 kilograms. The placebo group, by contrast, lost only 6.9 kilograms — barely more than at the 12-week mark.
This research demonstrates that combining Bofutsushosan with diet and exercise leads to significantly greater weight and body fat loss, particularly in terms of visceral fat.
However, there is an important point to keep in mind. The participants in these studies were obese women and individuals with both glucose abnormalities and obesity. This means that people of normal weight should not expect to lose additional weight simply by taking Bofutsushosan. The medicine is intended to support weight loss in people who are overweight or obese, particularly those carrying excess visceral fat, and it must be used in conjunction with diet and exercise.
For those with a normal BMI (roughly 24 or below), taking Bofutsushosan will not produce meaningful weight loss. And for those who are obese, taking Bofutsushosan alone — without dietary and lifestyle changes — will not be sufficient. Think of it as a supplement to, not a substitute for, diet and exercise efforts.
It's also worth noting that in traditional Kampo (Japanese herbal) medicine, different formulas are suited to different body types. Bofutsushosan is best suited for individuals with a robust constitution (known as 'jitsucho' in Japanese) — those who tend to be solidly built, sweat easily, and carry a protruding belly. It is generally considered less effective for those who are more delicate, tend to feel cold, retain water, and have softer fat deposits.
If you're concerned about abdominal fat and looking for support in your weight loss journey, Bofutsushosan is available at pharmacies, or you can consult with a physician at an obesity or metabolic syndrome clinic. Just remember: it works best as a complement to dietary changes and regular physical activity.
There is unfortunately no magic pill that causes weight loss on its own. Rather than looking for a shortcut, think of Bofutsushosan as a supportive tool that, combined with proper diet and exercise, can help your body burn fat more effectively.
