鼻整形の名医に鼻の整形をやるべきか真剣に相談してみました

みなさんこんにちは！友利新です！今日はお隣にいる田中先生がいるんですけど、なんで田中先生に来てもらったかっていうと、以前私、マリリンさんに変身メイクしてもらったじゃないですか。多分ここら辺に出てると思うんですけど、その時にマリリンさんが私を美容師にしてくれたんですよ。みなさん覚えてると思うんですけど、それがもう頭に焼き付いてて、プラス美容師にしてくれた時の鼻、覚えてます？みなさん。私のこの鼻がビョーンという感じ。私もう忘れられなくて入れてるんですけど、ほら見てください、こんな感じになってるよ。ほら全然違うんですよ。でも私多分この鼻になりたくて、マリリンさんとやった時には2時間メイクにかけてて、それはちょっと無理だなって思ったので、じゃあメイクじゃなくて、本当にこの鼻になるんだったら、どんなことをした方がいいのかっていうのを、表参道スキンクリニックでも肌のスペシャリストである田中先生に聞くっていうことで、今日は田中先生に来てもらってます。お願いします。 表参道スキンクリニックはスキンクリニックなので、皮膚科もメインにやってるんですけど、実は美容外科のスペシャリストの先生が多くて、輪郭、エラを削ったりとか、あとは鼻、頬、胸何でもやってて、だから私もうっかりいつもオペラを見てて、こんな綺麗になるんだったら私もやりたいとかってついつい思っちゃうわけですよ。でもなかなかね、カウンセリングを受けに行くとか、美容外科ってハードルが高いし、逆に鼻のオペって、私はね結構見てるから、あれですけど、どんな感じなのかを、今日は詳しく田中先生に聞こうと思うので、まずは先生、自己紹介をお願いします。 表参道スキンクリニックの大阪院で院長をしています、田中克也と言います。経歴としましては、形成外科専門医を持った段階で、こちらの美容クリニック表参道スキンクリニックでお世話になってます。それまでは形成外科として、長崎だったり愛媛だったり、そういった地元を含めたところで形成外科としてやってました。聞いてもらった通り、めちゃめちゃ真面目でしょ。 緊張してるけど、腕は確かで、私はいろんな先生見ていて、やっぱりオペしてもらうんだったら誰にしてもらおうかなって思った時に田中先生を見て、ちょっと鼻お願いしたいなって思ってるんですけど、鼻のオペ増えましたよね。 そうですね。最近すごく増えてて、あれって何なんだろう。やっぱりマスクしてて、顔を取った時に自分の鼻が違うかと思っちゃうのかな。そうですね。たぶん鼻は意外とやっぱり顔の中心なので、目につくと思うんですよ。特に人の鼻は見ないかもしれないんですけど、自分の鼻をたぶん常に見てると、やっぱり気になるところも多いんじゃないかな。 私今まであんまり自分の鼻に興味を持ってなくて、ちょっと若干大きいなとか思ってたけれども、本当にメイクをしてもらってからは、鼻が気になって気になってしょうがないわけ。だから普通アイメイクとかじゃない？肌と肌とかなんですけど、鏡を見たら、あれここがちょっと、これがもうちょっとこうなってとか、こうなってとか、あと自分の写真を見たら、鼻にしか目が行ってないから、そこを一回気になっちゃうとね、ずっと見ちゃうのかもしれない。 最近はどういう相談が多いの？ 鼻はですね、とにかくやっぱり小鼻とか鼻先ってことを言われる方多いんですけど、昔って外人みたいにドーンみたいな、そういうイメージだけど、やっぱり最近はここらへんですよね。そうですね。鼻の高さってやっぱり的によく言うんですけど、高さっていうのは鼻筋の高さを、たぶん昔はプロテーゼを入れて高くしてってことでやってたと思うんですけど、手術の技術がだいぶ進歩したっていうのもあって、鼻先の軟骨をうまく組み立てていって、鼻先を綺麗に作るっていう技術が発達してきたのもあって、今そういった手術が鼻の筋だけじゃなく、鼻先も綺麗にっていうのができてるんだと思いますね。 私やっぱり気になってて、私も医者のはしくれですから、外科ではないですけど、自分がマリリンさんのこの鼻になるとしたら、って考えてて、そしたらやっぱりここをね、ちっちゃくする小鼻の縮小をして、ここを微細形成って言うんですけど、ここにね軟骨入れて、シュッとしたら、マリリンさんになると思うんですけど、どうですか？ 友利先生の鼻は基本的に綺麗なんですよ。本当にいじる必要はないかなってくらいだと思うんですけど、この写真を見させていただくと、違う場所っていうのがいくつかあるかなって思うんですよ。お化粧で作ってる部分っていうのが、たぶんこの一番ハイライトかなと思うんですよね。横から黒く塗ってここに入られることで、筋を作ってると思うんですよ。 そうです。鼻筋のここなんですよね、幅が、こうやって写真に比べると、理想の形に比べると広いのかな。ここね。そうですね。土台の部分ですよね。ここじゃないの？鼻先は綺麗です。バランスはいいと思います。これじゃあ何もしなくていい？ここは。ただ鼻先をもっとスッとさせるのであれば、鼻先もそういうふうに合わせて、シュッとさせてあげるっていうのが、一つういいのかなってところですね。 ここを細くするってどうしたらいいの？簡単な方法だと、もともとのプロテーゼっていうのでも大丈夫。異物を入れるってなんか抵抗あるんだけど。そうですね。プロテーゼは比較的シリコンっていうものでできているので、体に馴染みがいいんですよね。じゃあ昔さ、よくプロテーゼ入れてたら、透けちゃって、ブラックライトで光るとか、そういうのもないっていうのは知ってるでしょ。そうですね。大丈夫と思いました。入れる部位にもよるので、浅いところに行ってしまえば、透けたりグラグラ揺れたりってことがあるので、入れる操作は間違えなければ、そこまで違和感はないかな。 入れたらずっといいの？例えば私今44歳だけど、胃を消して鼻筋綺麗にしてます。プロテーゼ入れました。いつまで入れられる？基本的には半永久でいいと思います。じゃあおばあちゃんになっても、綺麗な鼻でいていいの？もちろん。ただその時のライフスタイルに合った鼻、ご自身の顔に合った鼻になるので、若い時はすっと綺麗な鼻がいいと思っても、年を取った時にそれがいいかどうか、ってところになってくるので、そういったタイミングで入れ替えられる方っていうのは、いますね。ご結婚されるタイミングで、また子供も産もできて、お前高くなくていいな、低くしたいとか、そういった入れ替えっていう、人生のタイミングでの入れ替えですね。 なるほど。もしプロテーゼ入れなかったとしたら、どうしたらいいんですか？やれば土台から変えていくっていう意味で、骨付き幅寄せてですね、鼻の骨を、鼻の骨を？はい、ギュッと？切るんですね、外側の部分を。切ってこうなってるのを、こういう感じにするってこと？そうですそうです。まさにその通り。この鼻の中からやるってことですよね。中からもできますし、外側に少し1ミリとか2ミリくらいの切開は必要なんですけど、この傷跡って目立たなくなるんですか？目立たないと思いますね。このぐらいの傷なので、ニキビ跡じゃないですけど、最初はやっぱり赤みっていうのは、あると思うんですけれども、だんだんだんだん馴染んでくると、分かりにくくなると思います。 でも確かにハイライトを、すっごく入れてるから、確かに鼻筋がすごく細く見えてますよね。そうなんですよ。私大きいんだ。多分土台の形ですよね。ドーム状なのか、尖った感じで高いのか、私の場合はドーム状になってると思うんですよね。日本人には多いんですよね。そうですね、多いと思います。じゃあやっぱりここを、いらない余分なところを、狭くしていくところですかね。でもバランス今はまだいいんで、鼻筋と鼻先は整ってるんですよ。バランスが繋がってるので、別にやらなくていいんですけど、もしここやったらやっぱり鼻筋を、綺麗にしてあげた方がいいような気がしますね。 鼻筋はどうしたらいいですか先生。鼻筋は理想の形から見させてもらった感じで言うと、西洋人の方ってやっぱり高い、鼻先もぐっと上がってくる方が多いので、鼻先を前に出してあげるような形で。前にこういう感じですかね。そうですね。こうしてあげながら、鼻先の丸みをもう少し、鼻筋の細さに合わせて、軟骨をぐっと、伏せていくような形がいいかなと思います。 すごい。そしたらマリリンさんにまた戻れますか。戻れます。戻れますか。ちなみに先生、オペを全部したら、オペの時間ってどれくらいかかるんですか。3時間ちょっとぐらいですかね。3時間。先生にとってそのオペは結構日常。骨を切るっていうのはやっぱり、抵抗ある方多いので、されない方多いですけど、適応のある方はやっぱり一定数いらっしゃいますので、骨切らない手術に関しては日常。鼻先をするだけだったら、プロテーゼと鼻筋を作る、全部セットですることが多いですね。 じゃあ普通の人は骨切りまではせずに、自分の耳の軟骨を鼻先に入れて、長くするのと横を綺麗に、なんか仕上げるっていうことと、必要だったらプロテーゼを入れる。そうですね。それでお時間は、骨切りをしなければどれくらい。あまり変わらない。骨切り自体はプラス30分とか。やる？やらなくて大丈夫です。綺麗なんで。でも本当にね、鼻って大事なのよ。思いません？思います。 私本当にモンテさんと表参道スキンクリニックで、田中先生をはじめ、鼻が上手な先生がたくさんいて、私はオペに入ることはないんですけど、ビフォーアフターでカウンターに入ったりとか、写真を見せてもらったりすると、本当に印象が変わって、すごい可愛くなるから、自分もそうやってメイクしてもらった時に、もちろん目も本当にアイメイクでパチッとさせてもらったんですけど、鼻が素敵すぎて、やっぱり鼻って大事なんだなって。 アジア人の鼻っていうのは基本的に、軟骨がやっぱり柔らかいんですよね。なので潰れる。ダンゴ鼻っていう鼻の人が多いんですよ。なので特にこういった鼻の手術っていうのは、日本人女性の方では、印象も変わる、明るくなりやすい手術かもしれないですね。 分かりました。よかった。そしたら皆さん、ちゃんとオペする時には、「やります」って言って、先生がオッケー出してくれたら、やりたいな。あと夫とも相談して。私のチャンネルではこうやって、私がお悩み相談を田中先生にしたんで、田中先生も鼻だけじゃなくて、輪郭だったりとか、骨切り、眼瞼下垂を削ったり、頬骨を削ったりとかも、すごいいろんな手術をされていて、それをYouTubeで発信されているので、ぜひ田中先生のYouTubeも、見ていただければと思います。概要欄に貼っておきますので、ぜひ見てください。先生、後で終わったら、肉盛りとか。 分かりました。仮にね、仮にね、ちょっとオペ費もいろいろね、済ませていただいて、終わった後でいいんで。 分かりました。はい、ということで皆さん、今日は鼻の相談を、田中先生にしてもらいました。なので、これがどうなるかは、またご報告しますので、また次の動画も、そして田中先生の動画も見てください。では、また次の動画でお会いしましょう。さよなら。

But I probably want this nose, and when I did it with Marilyn, I spent 2 hours on makeup. I thought that was a bit unrealistic, so instead of makeup, if I could actually have this nose, what should I do? As for my background, after obtaining a specialist certification in plastic surgery, I started working at this beauty clinic, Omotesando Skin Clinic. Before that, I worked as a plastic surgeon in my hometown and places like Nagasaki and Ehime. As you can hear, I'm really serious. I'm nervous. You're nervous, but your skills are solid. I've seen many doctors, and when I think about who I want to do my surgery, I see Dr. Tanaka and think, "I'd like to ask about my nose." Nose surgeries have increased a lot, haven't they? Yes, they've increased a lot recently. I wonder what that's about. Maybe people are thinking their noses look different when they take off their masks? Yes, that could be it. Noses are actually quite central to the face, so they stand out. While people might not pay much attention to other people's noses, if you're constantly looking at your own nose, there are probably many things you notice about it. Until now, I haven't really been interested in my own nose. I thought it was a bit large, but since I had the makeup done, my nose bothers me so much. Usually it's about eye makeup or skin, right? But when I look in the mirror, I think, "Hmm, this part is a bit..." and "If this could be like this..." and when I look at my own photos, my eyes go only to my nose. Once you start noticing something, maybe you just keep looking at it. I understand. What kinds of consultations are most common recently? With noses, the most common concerns are small nostrils and the nose tip. But unlike the old foreign-looking image with a big "boom" shape, recently it's more like this area. Yes. People often talk about nose height, and when they say height, they mean the height of the nose bridge. I think in the past they would insert a prosthesis to make it higher, but as surgical techniques have advanced considerably, we've developed better techniques for beautifully arranging the cartilage of the nose tip. So now we can do surgeries that not only affect the nose bridge but also beautifully shape the nose tip. I'm really concerned about this. I'm kind of a doctor too, not a surgeon, but if I think about what it would be like if I became Marilyn's nose like this... I think if we make this area smaller - do a nostril reduction - and do what we call fine tip refinement here, inserting cartilage and sharpening it, you'd become Marilyn. What do you think? Your nose is fundamentally beautiful. I don't think there's really a need to touch it at all. But when I look at this photo, I think there are a few different areas. The part created with makeup, I think is mainly this highlight here. By coloring black on the side and creating definition here, you're creating a bridge line. Is that right? Yes, that's right. It's this nose bridge area here, the width. Compared to this photo and the ideal shape, is it wider? Right here. Yes, this is the base area. Not here? The nose tip is beautiful. The balance is good. So we don't need to do anything here? Well, if you want to make the nose tip sharper, we could match it by making the bridge sharper too. I think that would be nice. How would we make this area narrower? As a simple method, we could use a prosthesis like before. I'm a bit resistant to putting in a foreign object. Yes, I understand. Prostheses are made of silicone, which integrates well with the body. You know, I remember from the old days when people would get prostheses and they'd show through, or they'd glow under a blacklight or something. Is that not a problem anymore? That's right. I think it's okay. But it depends on where we place it. If it goes in too shallow, it can show through or move around, so if the insertion technique is correct, there shouldn't be much sense of foreign material. Once you put it in, does it stay good forever? For example, I'm 44 now, and I had my stomach worked on to make my nose bridge beautiful. I had a prosthesis inserted. How long can you keep it in? Basically, it should be fine semi-permanently. So I can have a beautiful nose even when I'm a grandmother? Of course. But your nose should match your lifestyle at that time and suit your face. While you might want a sharp, beautiful nose when you're young, whether that's still good when you're older becomes a question. So some people do have them replaced. At the timing of marriage, or when you might have children, you might think you don't need it high anymore and want it lower. Those replacements happen at different life stages. I see. If we didn't use a prosthesis, what would we do? We'd change the base itself, narrowing the bone width of the nose bridge. The bone? Yes. Tightening it? We'd need to cut the outside part. Cut it like this to make it like this? Yes, that's exactly right. We'd do this from inside the nose. We can do it from inside, but we'd need a small cut of about 1 or 2 millimeters on the outside. Would this scar be noticeable? I don't think it would be. With a scar this small, though not like acne scars, there would initially be redness, but as time goes on, it becomes less noticeable. But certainly you're putting in a lot of highlight, so your nose bridge looks very narrow. That's right. I have a big one. It's probably the shape of the base. Whether it's dome-shaped or pointed and high, in my case I think it's dome-shaped. Is that common in Japanese people? Yes, I think it's quite common. So the excess should be narrowed here? But your balance is still good right now. The nose bridge and tip are aligned. Since the balance flows together, you don't necessarily need to do anything. But if we did do this, I think it would be good to also beautify the nose bridge. How should we handle the nose bridge, Doctor? Looking at the ideal shape, Western people typically have higher noses with the tip angled upward more. So we'd bring the tip forward somewhat. Like this? Yes, like this. While doing that, we'd take the roundness of the nose tip and shape the cartilage to match the fineness of the nose bridge more sharply. That would be good. Wow. So could I become Marilyn again? You could. Really? By the way, Doctor, if we did the entire operation, how long would it take? About 3 hours or so. 3 hours. For you, is this operation pretty routine? Many people are resistant to cutting bone, so many don't do it, but there's definitely a certain number of people who are candidates for it. Surgery without cutting bone is routine for me. If we just do the nose tip, we usually do the prosthesis and nose bridge creation all as a set. So for regular people, without bone cutting, we'd insert cartilage from their own ear into the nose tip to lengthen it and make the sides clean with some kind of finishing, and add a prosthesis if needed. Yes. And for the time, if we don't do bone cutting? Not much different. The bone cutting itself would add maybe 30 minutes. Would we do it? No, you don't need to. You have a beautiful nose. But honestly, the nose is really important, don't you think? I do think so. I really do see a lot at Omotesando Skin Clinic, where Dr. Tanaka and many other doctors are skilled with noses. While I don't go into surgery myself, when I look at before and afters or see photos, the impression really changes and they become so cute. When I had makeup done like that myself, of course my eyes were really made to pop with eye makeup, but the nose was so nice that I realized how important the nose really is. Asian noses basically have softer cartilage, so they tend to collapse. Many people have what we call "dango noses." So this kind of nose surgery might be a procedure where the impression changes more easily for Japanese women and brightens up more. I understand. That's good. So everyone, when you're going to have surgery, if you say "I'll do it" and the doctor gives the okay, I'd like to do it. Plus I'll consult with my husband.
