Should morning and night skincare be different? Please tell me the key points on how to change them.
I don't think you necessarily need to change things much, but for example, some products like retinol can only be used at night. Depending on the product, some retinols are fine to use both morning and evening, but if the instructions clearly say "only use at night" or "for nighttime use," then night is best for those. Basically, I don't think you need to make big changes. However, more than people realize, due to wearing masks and using less skincare in the morning, skin dryness reportedly peaks around 3 or 4 pm, so I'd say you should do your morning skincare just as thoroughly as your evening routine.
Wearing sunscreen causes skin irritation and makes me prone to acne, so I can't apply it every day. Should I just look for a milder product or one that suits my skin? Do you have any recommendations?
Basically, for people with sensitive skin, I recommend La Roche-Posay. Some products just don't suit certain people—finding skincare that works for you is really like finding a compatible partner, so if you can get samples or trial products, it's best to try them out. That said, wearing sunscreen is essential for preventing photoaging and for anti-aging, so please make sure to wear it and find something that suits you.
I tend to get acne especially in the T-zone and around my mouth, and even after it heals, new acne keeps appearing. Is there any way to improve this? Also, I'm thinking of buying the ADDICTION concealer releasing on September 3rd—Dr. Tomori, have you tried it?
Yes, I saw the ADDICTION one. The color really has strong pigmentation, so if you want to fully conceal something and blend the color precisely, it's good. I felt it was a bit more artistic compared to the Recolte one from before, but it was really good.
As for acne in the T-zone and around the mouth, the T-zone naturally has more oil, and around the mouth, mask-wearing is also a factor, so rather than just skincare, I think it's better to have it checked at a clinic. But if you want to prevent it through skincare, as I always say, using non-comedogenic skincare products would be good.
I had spot removal with a PICO laser two weeks ago. To prevent rebound pigmentation, I'm applying 4% hydroquinone, but after researching, I found information saying hydroquinone is carcinogenic, which scared me. Do you prescribe or use hydroquinone yourself?
Yes, I use it. There is some scary information written about hydroquinone, but it has been used for many years, so unless you use it long-term—like for decades—it's generally not something to be too afraid of. Using it for just two weeks isn't going to cause any carcinogenic effect. I prescribe it in the clinic, use it in cosmetic products, and use it myself as well. This is a common thing in science: the possibility isn't zero, but it's mostly fine. So while the carcinogenic risk isn't zero, I've never seen it happen and it's quite rare—but saying "zero" isn't accurate because there's no such thing as absolute zero in science or medicine. So when you research it, you might find such things written, and that's not a lie either. But it's a matter of probability—it's not zero, but I don't think it's something to worry about.
I don't have this yet, but some people—even young people—get vertical wrinkles above their lip when eating, like older women do. What should I be careful about to prevent these wrinkles?
This area around the mouth is anatomically normal in everyone. But wrinkles forming in that puckered area happen because collagen and elastin decrease, and the epidermis becomes thinner. So it's good to properly incorporate vitamin A to add firmness—not just the hyaluronic acid in the epidermis, but also promoting the production of hyaluronic acid in the dermis with vitamin A-type ingredients to create that plumpness.
Good evening. Do you have any simple, recommended diet methods?
I talk about this in my video about fasting/hunger diets—basically, to give your body rest, avoid foods that cause a rapid rise in blood sugar. So basically being careful about not eating just one type of food or just rice bowls is probably the best approach.
I'm considering a GLP-1 diet. I'm a woman, 169cm, 85kg. I'm unsure about the effectiveness and whether to choose oral medication or injections. My goal is to lose 20kg, which I expect will take 6 months. It's an expensive treatment so I'm hesitant—how effective is it? Would you recommend this treatment?
First, GLP-1—to put it bluntly—keeps food in the stomach longer, slowing down the digestive process, so you stay full without needing to eat. Previously, appetite suppressants worked differently—interestingly, even if you're full, if there's a cake you like in front of you, you'll eat it because your brain remembers liking it. But with GLP-1, even if something looks delicious, since you're genuinely full, you won't feel like eating it. It's good if you use it while also controlling yourself and exercising, but please always use it under a doctor's supervision. Recently oral medications have become available too, so if you're resistant to injections, starting with oral medication might be a good option.
I'm a 27-year-old woman with oily skin. After coming home exhausted, I often just quickly wipe off my makeup with cleansing sheets and fall asleep without doing any further skincare. But my skin condition the next morning is better when I only use the cleansing sheet, compared to when I do a thorough skincare routine after a bath. My "thorough" skincare routine is still fairly light since I have oily skin. I know cleansing sheets can cause friction and aren't great, but they seem to suit me, so I'm conflicted. Please advise.
Honestly, I don't use cleansing sheets much myself, but if you're gently pressing rather than rubbing harshly, I think that's fine. Skincare isn't medicine, so if it suits your skin and you feel good doing it, then that's perfectly fine.
I'm concerned about clogged pores and blackheads on my nose and cheeks and wanted to get a Dermapen treatment, but my dermatologist advised against it due to my atopic dermatitis. Are there effective pore-prevention methods for people with atopic dermatitis? I currently do enzyme cleansing about once or twice a week and also use vitamin C serum as prevention.
If you currently have minimal symptoms, or if you had atopic dermatitis in the past but not now, it might not be impossible as long as you follow up with proper aftercare. So it really depends on your current symptoms. For blackheads, something like HydraFacial might work well. I'd recommend having it checked whether it's currently not possible due to active symptoms, or whether it won't be possible in the future either. If you're properly removing dead skin cells and using something that suppresses sebum production, it should definitely improve, so please continue your current care routine.
I was concerned about pigmentation on my cheeks and had PhotoBright and Genesis treatments, but the spots didn't disappear at all. I was told it might be ADM (Acquired Dermal Melanocytosis) and I'm debating whether to get treatment. How many sessions would it take before it's no longer noticeable?
It depends on the person. ADM is more like a birthmark-type pigmentation rather than a regular spot—it's deep like a birthmark and can improve with laser treatment, but it's an annoying type of pigmentation that can come back. It really varies by person, but I'd say at minimum 3-4 laser sessions would be needed.
Good evening, doctor. I've started getting itchy when wearing necklaces. Is metal allergy something that can't be cured? Also, should I get a prescription from a doctor instead of using over-the-counter anti-itch medication?
Yes, rather than over-the-counter medication, it's better to get a prescription from a doctor. Recently there's jewelry designed to be hypoallergenic even for people with metal allergies—I've seen it come up on Instagram, so please look into that if you're interested. If you still get itchy even with that, definitely—while over-the-counter medication is fine too—I think it's best to get it properly checked once.
Good evening, doctor. I took an ID photo today and noticed my sagging skin. I'm debating whether to get thread lift treatment given my mask-wearing lifestyle. Are there any clinics with skilled thread lift treatments?
For thread treatments, of course, Omotesando Skin Clinic has skilled doctors, so I'd recommend going there. Also, there's a doctor who used to work in Omotesando who has opened their own clinic—Stella Clinic in Akasaka is also good.
