Hello everyone. I'm Arata Tomori, an internal medicine and dermatologist doctor.
Today, I saw some concerning news again and immediately started filming this video. On July 4th, there was a Yahoo News article titled "Rombu Ryo Considered Retirement After Second Fracture During Hiatus." This is a Sponichi news story, and after seeing it, I'm genuinely worried about Ryo-san and would really like him to get examined as soon as possible, which is why I'm making this video.
This Yahoo News article only has about 40 comments, so honestly, I don't think it's gotten much attention. However, it's a piece of news that I find genuinely concerning, so I want to talk about what worries me about it, as well as why I'm so concerned about Rombu's Ryo-san.
First, since most people probably haven't seen this news, let me explain what it's about. Ryo Tamura from London Boots No. 1 & No. 2 appeared with his partner Jun-san on a Kansai TV regular program he had been away from during his hiatus. During the show, he talked about what he had been going through during his hiatus, saying it was mentally tough but he managed to push through. Then, Non Style's Ishida-san asked, "Honestly, did you ever consider retiring or disbanding?" While his partner Jun-san said no, of course not, Ryo-san admitted that he actually had considered it. The reason he gave was that he had fractured his toe twice last year, which is what's being reported in this news story.
Seeing this, an average person might just think "is that all?" and move on. But looking closely at these two fractures - he fractured his little toe at the end of May and his ring toe in early August. Having two such small fractures within just about three months is something that, from a doctor's perspective, makes me consider various possible conditions.
The reason I think about this is that small fractures like this - fracturing a toe - are actually quite common. In fact, I fractured my own right middle toe by hitting it on a door the year before last. This kind of clumsy accident happens to people. So if it happened just once, I wouldn't think much of it. But the fact that it happened twice in less than three months is the key point here.
Actually, these kinds of small fractures are said to be common in women over 50. Why is that? First, there's decreased muscle strength. When muscle strength decreases, balance becomes impaired. Also, when muscle strength decreases, it affects what's called proprioception - the innate sense that helps maintain posture and instantly lift your foot when you sense a step or uneven ground. This proprioceptive sense gradually deteriorates with age and decreased muscle strength. Another factor is that after age 50, estrogen, a female hormone, decreases due to menopausal symptoms.
Actually, bones maintain a balance between bone formation, which creates new bone, and bone resorption, which breaks down bone, and this balance is what keeps bones strong. Estrogen is said to maintain this balance, and when estrogen decreases, this balance is disrupted. For example, if bone resorption becomes dominant, bones can become brittle. This is why women in the menopausal age group of 50 and above can develop something like osteoporosis, where bones become brittle. Combined with the muscle weakness I mentioned earlier, this makes them prone to these small fractures.
So while we wouldn't normally worry about serious conditions in women over 50, Ryo-san is still in his 40s. Plus, he's male. This makes muscle weakness and hormonal factors related to female hormones less likely explanations. Did you know that Ryo-san's hobby is actually marathon running? Since he participates in various races, muscle weakness is basically not something we'd consider in his case.
So when we doctors hear about someone fracturing bones twice within less than three months under these circumstances, what do we think? This makes us consider a concerning symptom - and that concerning symptom is actually decreased vision. It means his eyesight might be deteriorating.
Conditions that typically cause decreased vision include glaucoma, where increased eye pressure damages the optic nerve, and vision problems caused by diabetes. Another concerning possibility, especially for heavy smokers, involves the sinuses behind the cheeks - specifically the maxillary sinus. When malignant tumors develop in the maxillary sinus, this can also cause vision problems.
So while you might think "it's just a fracture," the fact that this happened in such a short period of time to a young man in his 40s with no muscle weakness, having these kinds of careless minor fractures, really does suggest we should suspect vision problems. So Ryo-san, please don't go to an orthopedic surgeon - instead, please visit an ophthalmologist quickly and get an eye examination.
Now, why am I so worried about Rombu's Ryo-san? Actually, I feel deeply grateful to him, and I've always wanted to thank him if I ever had the chance to meet him. Here's why: I have an older sister, two years older than me, who has been a huge fan of Rombu's Ryo-san since way back. This sister went to dental school and became a dentist, but during her student years, her grades weren't very good, and there was a time when she might have had to repeat a year. At that time, I was working part-time at a club in Shinjuku. Though it was called a club, it wasn't the type where alcohol is served and hostesses greet customers - it was a dance club, and I worked at the reception desk.
Every few months or so, Ryo-san would come to visit. I recognized him as Ryo-san, but since my sister was feeling down and I wanted her to do well in her studies, even though as staff I probably shouldn't have done this, I mustered up my courage and approached Ryo-san. I said, "I'm sorry to bother you, but my sister is a huge fan of yours. She's a student right now and her grades aren't great - could you possibly encourage her?" Ryo-san kindly agreed, and using my phone, he called my sister and told her to do her best. But since this was just a phone call - and remember, this was back in the flip phone era, so we couldn't do FaceTime - he said, "She probably won't believe this is really me, so let's take a photo with your sister too." He took a photo with me, said it was fine to have it developed, and I sent it to my sister.
My sister was truly overjoyed, studied hard, successfully advanced to the next year, and became a dentist. Because of this, I've always wanted to thank Ryo-san someday for what he did.
This might seem like such a trivial piece of news to everyone, but from a doctor's perspective, I do suspect decreased vision, so I hope he goes to see a doctor soon. And on a personal note, since I feel such genuine gratitude toward Ryo-san, I hope this video reaches Rombu's Ryo-san, that he gets examined, and that my long-standing feelings of gratitude reach him as well.
Thank you for watching.
