Hello everyone, this is Dr. Riara. Today I want to share with you the truly frightening topic of "hidden keratin plugs" (kakusen), along with the latest research findings and how to deal with them, so please watch until the end.
When you hear "keratin plugs," you probably think of concerns like enlarged pores and the blackheads that come with them. You might have assumed this was mainly an issue for your teens through your 20s and early 30s, settling down by your late 30s and 40s. But actually, it's become clear that these "hidden keratin plugs" cause significant damage to the skin regardless of age.
So what does this research show? Keratin plugs are originally a mixture of protein and sebum. Excess sebum is released from the sebaceous glands, and inside the pores—just like on the skin's surface—the stratum corneum sheds through the turnover process. This mixes together to form the keratin plug. In other words, it's made of sebum and protein.
When a lot of sebum comes out, you might think of how oil left sitting around eventually oxidizes—old oil turns brownish, for example. It's long been known that sebum left for a long time also oxidizes. This was thought to cause blackheads in the pores, so the conventional wisdom was to properly remove excess sebum.
However, it turns out that when a keratin plug oxidizes and inflammation occurs, the effect doesn't stay contained to that one spot. The oxidized, inflamed keratin plug acts as a trigger point—like pressing a switch—that causes oxidation to spread to neighboring keratin plugs like a chain of falling dominoes.
So what happens to the skin as a result? Since oxidation causes browning, an overall dullness appears. Until now, dullness was thought to be caused by a thickened stratum corneum, where melanin-containing cells that should normally be shed through turnover fail to be released properly, disrupting the turnover cycle.
As countermeasures, approaches like peeling to thin the stratum corneum, or helping melanin-containing cells shed properly—through oral supplements like L-cysteine, or turnover-promoting ingredients like 4MSK in inner care and skincare—have been recommended. But even when doing all this, if you still feel an overall dullness, it turns out this oxidation of keratin plugs is occurring, causing dullness and reducing radiance.
Isn't that scary? Just one keratin plug—since sebum covers the entire face—means that once oxidation starts there, it can spread across the whole face. Thinking back, people with a lot of shine from sebum do tend to have somewhat dull-looking skin. I myself had a lot of sebum shine, so achieving radiance was always a personal challenge. As I got older and started facing my skincare more seriously, I thought I understood things a bit better, but that dullness that appears when wearing a mask or when sebum surges—seeing this latest research made me realize, "So THIS was the cause."
Since this happens in your teens and 20s, you'd think, "Then let's properly care for keratin plugs." Of course, the nose and T-zone are prone to keratin plugs in your teens and 20s. But there's another discovery: keratin plugs on the cheeks. While the T-zone and nose are the most noticeable spots for keratin plugs, they actually form on the cheeks too. It's hard to tell if there are keratin plugs on your cheeks, right? This is called a "hidden keratin plug."
At what age do you think these hidden keratin plugs start increasing? I was surprised myself when I saw the data—compared to the nose, these hidden cheek keratin plugs dramatically increase starting in your 40s. No wonder skin loses its radiance as we age! This isn't just an issue for people with excess sebum production—it's related to disrupted skin turnover, and the fact that sebum oxidizes even without excessive production. That's why it's important not only to promote turnover but also to care for the keratin plugs themselves. However, removing too much sebum can cause dryness, so rather than suppressing sebum, the key is to gently remove hidden keratin plugs that have already formed, while also suppressing oxidation.
Who conducted this research? It was Kao. Kao has long had "keratin plug decomposition technology" that they've incorporated into various brands, and now they've finally released an upgraded version of this technology in new products. Here are the two products.
This is the EST Clarifying Gel Wash facial cleanser. And this is the makeup remover, EST Clarifying Cleansing Milk.
Until now, this keratin plug decomposition technology for hidden keratin plugs was only included in facial cleansers, but this time it's been added to the makeup remover as well. So you can do thorough keratin plug care both morning and night. I think it can be used from your 20s, though it's a slightly more mature-oriented brand. Keratin plug care tends to bring to mind images of over-cleansing leading to dry, flaky skin, but this keratin plug decomposition technology doesn't just pluck out the plug—it works from the inside to loosen it up until it bursts, so it removes plugs gently.
Beyond containing the latest keratin plug decomposition technology, as I mentioned, once inflammation occurs and the oxidation switch is triggered, it spreads to neighboring areas. To stop this, the products contain tocopherol, an antioxidant ingredient. Not only does it break down and eliminate the keratin plugs that are the origin point of stratum corneum oxidation, but tocopherol also suppresses oxidation in areas of the stratum corneum where the switch has already been triggered and dullness has occurred, bringing back radiance from within. This is a slightly different approach to brightening care than before.
Brightening care that you'd normally associate with toners and lotions is now being achieved through cleansing products—I found that an interesting concept.
This EST facial cleanser is actually a gel type that doesn't foam. It has a moisturizing, skin-clinging texture. As I've mentioned before, patting is different from rubbing, so you don't need to worry just because it doesn't foam. When you massage it onto your skin like this and rinse with a little water, your cheeks feel like they've shed a veil, and your complexion instantly brightens. Yet it doesn't feel like it's stripped away too much. It's like it gently removes impurities and then puts on an antioxidant veil.
The other product, the cleansing milk, has a texture like this—though it's called a milk, it's actually more gel-like. This also felt very gentle on the skin, and it removed makeup quite well.
Delivering active ingredients effectively to the skin is important, but if the stratum corneum on the skin's surface itself is oxidized and darkened, radiance will decline from there. That's why preventing oxidation of the stratum corneum is such a crucial point. Moisturizing plays a role in this too, and since sebum oxidizes due to factors like UV exposure, moisturizing is important, and UV protection will also be necessary as we head into this season.
If you feel like your skin looks dull or murky, it might actually be these hidden keratin plugs on your cheeks triggering oxidation in the stratum corneum. So this year, I want to get through summer with three pillars: care for stratum corneum oxidation, using brightening serums and lotions with real whitening effects, and proper sun protection.
How was that? I really came to realize that keratin plugs are problematic whether you have too few or too many, no matter your age. Let's all take proper care of them.
This year, various brightening and whitening skincare products are coming out again. This time I introduced Kao's products, which focus on stratum corneum oxidation, but there are other products coming from interesting research that have been released one after another since March, and I hope to share those with you too. If you enjoyed this, please subscribe to my channel. And if there's a topic you'd like me to dive deeper into, please feel free to leave a comment.
See you in the next video. Goodbye.
