マスク肝斑についてお話ししようと思います。
今年は絶対に肝斑が悪化する患者さんが多くなるというか、すでに多くなっているような傾向があるので、その理由を3つ、そして解決策までお話ししようと思っています。ぜひ最後までご覧ください。
まず、肝斑というシミ、ご存知ですか?シミっていろいろあるんですけど、一番有名なのはそばかす。あとは老人性の色素斑と呼ばれるものですよね。そばかすというのは生まれつきというか若い頃からポツポツとあるような小さくて小型のものですよね。老人性色素斑というのはいわゆるこれが皆さんが持っているシミですよ。見た目に見えやすいものですよね。それがお顔に1個か2個ある。
ただ肝斑というものは、そういったそばかすとか老人性色素斑と呼ばれるシミとは違って、実は大体30代以降に出てくる、対称性に出てくるシミで、目の下から頬骨、頬のあたりにかけてぼやーっとした全体的にシミというか色素が出てくるものになるんですね。遅発性両側性太田母斑というものもあるんですが、それとの違いは、その遅発性両側性太田母斑はお鼻にもあったり、あとおでこにもあったりするんですが、基本肝斑というのはほっぺたの方、両頬にあるという、それも対称性にあるというものがポイントになってきます。ただこれって私たち医者でも見た目に見えやすいものですよね。なかなか鑑別が難しかったりとか、そばかすだけありますよとか老人性の色素斑だけありますよとか、肝斑だけありますよという人は基本いないんですよ。大体が混ざっていると思ってください。
これがまたベースの話。肝斑に戻りますが、肝斑というのは先ほども言ったように、大体30代以降の女性に多くて、少し年齢がいっていくと逆に収まってきて、そこからは悪化するということはなぜかというと、女性ホルモンが関係があるというふうに言われているんですね。女性ホルモンとか紫外線が悪化要因の1つなんですよ。30代から40代、50代って妊娠・出産をしたりとか、後は更年期があったりとか、結構女性ホルモンがアップダウンするときになりますから、そういう意味ではそういった時期に悪化しやすいというふうに言われています。後はそういう意味では、ピルを内服している人、ピルというのは女性ホルモンを補充したりするというか、外から入れていくものになるので、ピルを飲んでいたりという人も悪化の原因になりやすいと言われています。
以前のピルの動画でもお話ししましたけれども、私自身はピルが日本で、定容量のピルが解禁されてからずっと飲んでいたことを肝斑で悩んでいましたというか、いますという方が正しいかな。そういったこともあって、私自身も肝斑をすごく気を付けているんですね。
じゃあ、妊娠とか出産とか、ピルは置いておいても、妊娠とか出産とかって結局、自分の人生においてシミが嫌だから妊娠・出産しなかったらそういうわけじゃないじゃないですか。じゃあ肝斑というのは年齢的に仕方がないのかというと、先ほど私、肝斑が悪化する原因って1つの紫外線だよって言いましたよね。これはもう何となく分かると思うんですが、それ以外に実は3つ、また肝斑を悪化させる原因というものがあります。
その3つ、何かというと、1つ目が摩擦、2つ目が乾燥、そして3つ目がストレスになります。1つ1つお話ししていこうと思うのですが、摩擦というのはもちろんゴシゴシお顔を洗ったりとか、メイクの時にゴシゴシ洗ったりとか、ゴシゴシしたりというような、自分では摩擦だと思っていないような日々の肌を触るということも摩擦になります。これが摩擦による摩擦熱が起こることによって、小さい炎症ですけど肌に刺激が行くんですね。そうすると日焼けをしているのと同じように炎症性の色素沈着ではないですけど、そういったものが起こって肝斑が悪化するというふうに言われています。なので結構、マッサージしている人に限って肝斑が多かったりする傾向もあります。
もう1つ、乾燥になります。乾燥って思うかもしれないのですが、乾燥していると皮膚のバリア機能が低下しちゃうんですね。皮膚のバリア機能が低下すると何が起こるかというと外的刺激が入りやすくなってきます。この外的刺激が入ってくることによって皮膚に炎症が起こります。この炎症が摩擦と同じように肝斑を悪化する原因になってくるんですね。なのでやっぱり乾燥させないようにするということは、肝斑を悪化させないという意味ではとても大事なことになってきます。
最後にストレスです。ストレスは関係ないでしょうと思うかもしれないんですけれども、ストレスで交感神経優位になってくると皮膚の水分量が低下したりとか、あとは皮膚のバリア機能が低下すると言われているんですね。もちろんそうすることによって、また先ほど言った乾燥が起こってきたりとか炎症が起こりやすくなってくるということになるので、ストレスが高くなって乾燥が起こったり、バリア機能が低下して、それで肝斑もひどくなるというような負の連鎖。また肝斑が悪化してバリア機能が低下して、痒みが起こってくるとなると、またその肌状態が悪くなるとストレスになってくるという悪循環が起こってしまうということが分かっています。
肝斑が悪化するといったときに、マスクが結構ポイントだよと言ったじゃないですか。マスクというのは、この肝斑を悪化させる摩擦、乾燥、ストレスというのを結構与えるものなんですよ。言わずと知れてマスクをしていると、ずっとこのちょうど肝斑ができるあたりがマスクが当たって常に常に擦れているんですよ。擦れている。だって皆さんマスクをつけたときに、ファンデーションがついているところで、だいたいこの辺でしょ?ファンデーションがついているということは、ここが常に擦れていて、擦られている、イコール摩擦をされているんですよ。なので、もちろんしっかり紫外線対策をしていても、ずっとずっとこの肝斑があるところに炎症が起こっている。
あと、マスクの肌荒れのときにもお話ししましたけれども、マスクをしていると自分の呼気というの、吐く息ですね、喋ったりとかするときに出る呼気というのには水蒸気が含まれています。その水蒸気がずっとここに蔓延していると、お顔がふやけた状態になって、お顔がふやけた状態になると、お肌のバリア機能の要である細胞間脂質というのが、細胞と細胞をくっつけているセメントみたいな役割のところがちょっと弱くなって出ていってしまうんですね。それによって乾燥が起こってしまうということになるんです。
もう一つ、ずっとマスクをしていると結構ストレスになっちゃったりするじゃないですか。今ちょっと感染が増えてきているので絶対マスクは必要だとは思うんですが、ずっと日中マスクをつけているって精神衛生上あまりよろしくないし、だからといって外していくと結構非難されたりとかもするので、そういったいろんな社会的なこともあって、ストレスが今年はマスクをつけるということに多いんじゃないかなと思いますので、去年の春からずっとこの摩擦と乾燥とストレスというのがずっとピークに来ているので、ここでまたマスクをつけていると、また乾燥、悪化してきている患者さんが結構多いんですよ。
なので皆さん、マスクをすることでもしかしたら乾燥が悪化していて、なんとなく顔がくすんでいるとか新しいシミができてきたなというのは、もしかしたらこのマスクによる乾燥かもしれません。
じゃあ先生もマスク外せないし、乾燥悪化する一方なのって思うじゃないですか。なんですが一応乾燥を治療するレーザーというものもあります。なのでクリニックで施術をするときは必ずそういった肝斑用のレーザー、トーニングをするようにしてください。
あとは肝斑にはありがたいことに内服薬と塗り薬というものもあるんですね。それが有名なトラネキサム酸というものになります。それは今、薬局でもトラネキサム酸、トランシーノとかという名称で売っていたりするので、このトラネキサム酸とビタミンCを3ヶ月から半年くらい内服をする。あとはトラネキサム酸が入っているような化粧品を使うということもとてもポイントになってきます。
最後はやはりストレスをためないということはとても大切なことになってきますので、ちょっと人がいないところだったりとかはマスクを外してみたりとか、あとはスキンケアでテクスチャーがいいものとか、あとは香りがいいものとか、自分がスキンケアをして楽しいなと思う時間を作っていただいて、それによってストレスを軽減していただければとてもうれしいかなと思います。
まとめると、30代以降から多くなってくる肝斑というシミ、悪化の原因は女性ホルモン紫外線もあるんですけれども、それ以外だと摩擦、乾燥、そしてストレスになります。今年はずっとマスクをしていますから、この摩擦と乾燥とストレスが大きく加わってきます。なので日ごろのスキンケアが大事になってきます。日ごろのスキンケアでは保湿をしっかりして、プラス肝斑に効くといわれるトラネキサム酸、そしてビタミンCをスキンケアでも取り入れていくようにしましょう。そして内服でもトラネキサム酸、ビタミンCの内服を続けるようにしましょう。あとは摩擦をしないように、スキンケアはごしごし洗ったりしないように気をつけてください。最後はストレスをためないように、ご自身が楽しいと思うようなストレス発散のものを見つけていただければ、そういったものを見つけていただけるといいかなと思います。

I'd like to talk about mask-related melasma.
This year, patients with worsening melasma will definitely increase—or rather, this trend is already happening—so I'll explain three reasons for this, and also share solutions. Please watch until the end.
First, do you know about melasma, a type of pigmentation? There are various types of spots (shimi). The most well-known is freckles. There's also what's called senile pigmented macules (age spots). Freckles are the small, pinpoint spots that appear from a young age, congenitally. Senile pigmented macules are what most people think of as typical age spots—easily visible, usually one or two on the face.
However, melasma is different from freckles or senile pigmented macules. It typically appears after your 30s, symmetrically, as a hazy, overall pigmentation spreading from under the eyes across the cheekbones and cheeks. There's also something called late-onset bilateral Ota's nevus, but the difference is that this type can also appear on the nose or forehead, whereas melasma basically appears on both cheeks symmetrically—that's the key point. However, even for us doctors, this can be hard to distinguish visually. It's rare for someone to have only freckles, or only senile pigmented macules, or only melasma—usually these are mixed together.
That's the baseline. Now back to melasma. As I mentioned, melasma is common in women in their 30s and beyond, and as women get a bit older, it tends to calm down. Why does it worsen during that specific age range? It's said to be related to female hormones. Female hormones and UV exposure are two of the aggravating factors. During your 30s, 40s, and 50s, you may experience pregnancy, childbirth, or menopause—times when female hormone levels fluctuate significantly. In that sense, melasma tends to worsen during these periods. Also, women taking birth control pills—since pills supplement or introduce female hormones from outside the body—are also said to be prone to worsening melasma.
As I mentioned in a previous video about birth control pills, I myself had been taking the pill continuously since low-dose pills were approved in Japan, and I've been struggling with melasma—or rather, I still am. Because of this, I personally pay close attention to melasma.
Now, setting aside the pill, pregnancy and childbirth aren't things you'd avoid just because you don't want spots on your skin—that's not how life works. So is melasma just something you have to accept due to age? Earlier I mentioned that one cause of worsening melasma is UV exposure. That much makes intuitive sense, but there are actually three more factors that worsen melasma.
Those three are: first, friction; second, dryness; and third, stress. Let me explain each one. Friction includes obviously scrubbing your face hard when washing, or rubbing vigorously during makeup application, but also everyday touching of your skin that you might not even recognize as friction. This friction generates frictional heat, causing minor inflammation and irritation to the skin. This is similar to what happens with sun damage—it's said to trigger something like post-inflammatory pigmentation, worsening melasma. That's why people who frequently massage their face tend to have more melasma.
The second factor is dryness. You might not expect it, but dryness weakens the skin's barrier function. When the skin barrier is compromised, external irritants can penetrate more easily. This causes inflammation in the skin, and just like friction, this inflammation becomes a cause of worsening melasma. So preventing dryness is very important for preventing melasma from worsening.
Finally, stress. You might think stress isn't related, but when stress causes sympathetic nervous system dominance, it's said to reduce skin moisture levels and impair the skin's barrier function. This in turn leads to the dryness and inflammation mentioned earlier, creating a negative cycle where increased stress causes dryness, which weakens the barrier function, which worsens melasma. Furthermore, when melasma worsens and the barrier function declines, itching can occur, and this worsened skin condition itself becomes a source of stress—creating a vicious cycle.
Now, when it comes to melasma worsening, I mentioned that masks are a key factor. Masks contribute significantly to the friction, dryness, and stress that worsen melasma. Obviously, when you wear a mask, the exact area where melasma appears is constantly being rubbed by the mask. Think about it—when you wear a mask, foundation typically rubs off in this area, right? The fact that foundation comes off here means this area is constantly being rubbed—meaning it's experiencing friction. So even if you're diligently protecting against UV rays, this area where melasma exists is continuously experiencing inflammation.
Also, as I mentioned in the video about mask-related skin problems, when you wear a mask, your breath—the air you exhale while breathing or talking—contains water vapor. When this water vapor lingers inside the mask, your face becomes essentially waterlogged. This waterlogged state weakens the intercellular lipids, which act like cement holding skin cells together and are crucial to the skin's barrier function. This causes these lipids to be lost, resulting in dryness.
One more thing—wearing a mask constantly can also become quite stressful. Given the current rise in infections, I understand masks are absolutely necessary, but wearing a mask all day long isn't great for your mental well-being. Yet, if you take it off, you might face criticism from others. Due to these social factors, I think wearing masks has become a significant source of stress this year. Since last spring, friction, dryness, and stress have all been at their peak, and continuing to wear masks now means many patients are experiencing worsening dryness and melasma.
So everyone, if you're wearing a mask and experiencing dullness in your skin or noticing new spots forming, it might actually be due to dryness caused by the mask.
You might think, "But doctor, you can't remove your mask either, so won't the dryness just keep getting worse?" Well, there are actually laser treatments available for treating dryness. So when receiving treatment at a clinic, make sure to get laser toning treatments specifically designed for melasma.
Fortunately, there are also oral medications and topical treatments for melasma. The most famous one is tranexamic acid. It's now sold at pharmacies under names like tranexamic acid or Transino. Taking tranexamic acid along with vitamin C orally for about three to six months, and also using skincare products containing tranexamic acid, are both very effective approaches.
Lastly, managing stress is extremely important. Try removing your mask when you're in places without people around, and make time to enjoy your skincare routine—choose products with textures and scents you love. Creating an enjoyable skincare experience can help reduce stress.
To summarize: melasma is a type of pigmentation that becomes more common from your 30s onward. While female hormones and UV exposure are contributing factors, friction, dryness, and stress are equally important causes. Since we're all wearing masks constantly this year, these three factors—friction, dryness, and stress—are significantly amplified. That's why daily skincare is so important. Make sure to moisturize thoroughly in your daily skincare routine, and incorporate tranexamic acid and vitamin C, both known to be effective against melasma, into your skincare products. Also continue taking tranexamic acid and vitamin C orally. Additionally, be careful not to create friction—avoid scrubbing your skin harshly during skincare. Finally, find stress-relieving activities that you genuinely enjoy to avoid accumulating stress.
