皆さんこんにちは、友利新です。今日は田中先生に聞く第2弾です。今日はたるみ治療について、皮膚科と外科の立場からいろいろ話したいと思っていて。というのもね、先生、今年は美容業界というかスキンケアのトレンドってあるんだけど、今年の秋冬はたるみとか肌の張り感。たるみっていうのはなかなか化粧品では難しいって言われることもあるから、張り感っていうふうに言って、やっぱり肌に張りを持たせて、要はたるみを予防しようとか、っていうのがトレンドだなと。すごいですね。そうだから、そういうふうなクリームだったりとか、そういうものも結構出ていて、スキンケアでね、そういったものはある程度は予防だったりとかっていうのをやりつつ、でもやっぱり治療したいってなるじゃないですか。そうですね。そういう人から結構言われるので、今日はね、本当に皮膚科の立場と本当にオペをしている外科の先生の立場からたるみとか張りケアについてを聞いていきたいと思っているので、先生お願いします。 まずやっぱり皮膚科っていうと、今はもう皆さんね、たるみって言ってもハイフ。ハイフっていうのは難しい話を抜きにすると、曇ったメガネの原理だから、熱を与えると表面には何もないんだけど、奥の方の筋肉に熱が当たって、焼かれる感じ。そうすると、よく言うのがね、お肉って焼くと縮むから、それでキュッと上に上がりますよ、っていう話をね、よくしますね。でもそれを言うと、焼肉で焼くと硬くなる。そうですね。そう、イメージ育ちそう、硬くなるから、顔が動かなくなるとか、将来悪いことが起きるんじゃないかみたいなことを言っていらっしゃる先生も、中にはいることはいるわけ。私はそれを思うことがあるんだけれども、でもこれは皮膚科の立場で、私たちは表面だけを見ているので、実際にほら、フェイスリフトとかで、正直剥がしたりとか、奥の方を見ている先生から外科の立場から、たどうですか。 そうですね。確かにハイフがすごく流行っているので、手術で言うと、フェイスリフト。やっぱり剥がしていく時の剥がしつきを釣り上げる手術で多いんですけど、そういった際にやっぱり、皮膚の下のところを見た時に、やはりハイフを重ねている方は、やってない方に比べると、硬くなっている印象はありますね。 硬くなっているってことは悪いことなの? 悪くはないです。悪いっていうか、硬くなっているっていうことで、手術する時には、少し僕たちがやりにくい、っていうところがあるかと思うんですが、ある意味、組織自体が硬くなるっていうことは、ちゃんとしっかり効果が出ている、ハイフの効果が出ていると思うので、それによって引き上げ、っていう効果が出てるんじゃないかなと、はある意味、実感しています。 硬くなるとかって言うと、なんとなくネガティブなイメージだけれども、やっぱり私たち人間って、生きているだけで、組織っていうか、顔の肉だったりはどんどんどんどん柔らかくなっていく。だからこそ下に落ちていくわけじゃない。そうですね。それを抑えようとして、ある程度硬くするというか、元に戻して、しっかり組織をさせているっていうイメージで、じゃあいいのかな。 いいと思いますね。やはり、その硬くなっている分、やっぱり引き上がるってことになりますので、やっぱりハイフはいいんだろうなと、気をつけている側からも思いますね。 よかった。なんかそこをね、私はやっぱり表面というか、皮膚のことだけだし、自分自身もハイフをやっていて、実感してるし、だけれども、本当にじゃあ、外科で見た時のことって、やっぱり外科の先生に聞いてみたいなって思ってたので、それはちょっと安心しますね。じゃあ安心してやっていい? 大丈夫です。ただ、僕たちが苦労するだけです。 その時にはよろしくね。 頑張ります。 もう一つ、私はその皮膚科の立場からはじゃあ、ハイフはもう本当にたるみ治療としてはやってもいいし、その機械にもよるけど、大体半年とか3ヶ月とか、1ヶ月に1回、っていうハイフシャワーみたいなのもあるので、それはクリニックによってハイフってね、本当にいっぱい種類があるんですよ。だからハイフやってきたんですけど、って正直言われてもどの機械なのかな、で全然効果も違うし、あとぶっちゃけ、当てる人のスキルはやっぱりあるので。ハイフが1回やってみて、あんまり効果なかったなー、って思ったら、そのハイフの機械が何だったのか、っていうのは聞いてみてもいいかもしれないですよね。 そうですね。エステでハイフしてきました、っていう方は、やはり1ヶ月に1回、ってすごい短期間で、周期的にやっていいって言われました。大丈夫なのかな、って、こういう医療ハイフしてると思ったりもしますし、やはり効果ってね、機械の違いなんだろうなと。機械によって違うから、なんかハイフというのはもうスマートフォンみたいなもんで、ギャラクシーなのか、iPhoneなのか、はたまた何か別のなのか、っていう感じなので。ハイフという総称だと思って、機械の名前、それこそウルトラセルなのか、ウルセラなのか、いろんなものがあるので、そういうものをちょっと聞いてきていただけると、私たちとしてはありがたいかなと思いますね。そうですね。 あともう一つ、ちょっとそろそろやろうかなと思ってるんですけど、糸よ。 あ、なるほど。たるみといえば糸ですよ。 なるほどですね。先生、どうか、私、ちょっと相談してもらって、何本くらい入れたらいいかな。 いや、もうそろそろ。そろそろ。いや、全然綺麗なんですけど、そろそろ。 というフェイスラインですよね。はい。フェイスラインと糸リフトされる方はここ。やっぱり、私、やっぱり、こっちあるからこれがほら、可愛くなるじゃん。ほら、ほら、可愛くなった。ほら、いつもやさ、いつもこうやって、ああ、こうやったら可愛くなるのに、いつもやったようなこうしたらどうしたいです。 そうじゃん。この下にどうしても、もしかすると下りてくるので、そういったものを引き上げる、っていう意味で糸リフトなんですよね。なので、特にフェイスラインだったり、このマリオネットライン(高齢者の真下の方)、この辺りの引き上げには、やっぱり、こういいんじゃないかなと思う。 何本。何本。 私のクラスだと、私のクラスというか、44歳。場所で考えてもらってもいいと思うんですけど、やはりこのフェイスラインまで上げていくと思うと、5本。 5本いっちゃう?どういう感じかな。 面で考えてもらうんですけど、線と面なんですけど、線を引いていきながら、この辺の面を上げていく、ってイメージなので、1本で釣り上げたらおかしいから、点だけど、それがちゃんと扇が面になるように。要は、これをこういうふうに上げるような感じで上げるってことね。 そうですね。上げるところにどこに視点を持っていくか、だとか、どこを引き上げるか、っていうのはちょっとポイントがあるかなってところで。 じゃあ、5本。5本ね。 ぐらいあれば十分かなと。だから、パーツで考えた時に、例えば、ここだけ上げるとか、であれば、少なくて済むと思うんですよ。それで、もっと法令線の方までしっかり引き上げたいです、ってなってくると、前の方にも行かなくていけないので、その分本数が増えてくるかな、っていうところですね。 せっかくやるなら。 せっかくやるなら。でも、前ですけどね。でも、ほら、スケジュールの問題とかもあるじゃない。あの、針のリスクを先生、お願いします。 そうですね。やっぱり、針を刺す施術なので、どうしても内出血、っていうリスクがあるかなと思います。内出血は、もっと、出る人、出ない人がいるので、肌が荒れたりだとか、両側で、まあ、両方出ない。全く腫れません、でした、って方も、やっぱり出てきますね。そこが分かれるところで、それが出たら、やはり1週間から2週間ところは、紫になって、黄色くなって、抜けていく、ってあ、いう。 でも、それは、さ、コンシーラーっていうのがあるから。 確かにそうですね。それはコンシーラーで何とか。ほら、画面上は、さ、何もないように見えるから。あとは、あとは、例えば、引き連れですよね。人が引き連れたえくぼ。えくぼみたいな感じで出る方がいらっしゃるんですけど、そういったものも、1ヶ月ぐらいですかね。様子見ていただいて、治ることがほとんどですかね。 腫れたりはどうなんですか。 腫れも、そこまでひどくないと思いますね。例えば、私が今日やりました。いつYouTube撮れる。もしかしたら内出血次第かな、って。内出血、じゃあ、本当にコンシーラーでカバーしたら、1週間後いけると思います。 じゃあ、バレずにいける? やる時は、もちろんYouTubeでやろうかなと思うけど、一応何かあって思った時には、いける? やるんですか? いや、もう年末、先生、スケジュール合わせたいなって。 いや、どうしても、って言うのはダメですけど。 本当? いや、いや、本当ですか? それはもう、YouTubeで、こんな感じよ、っていうのは、体当たりなくて。私はまだもう、ちょっと後かなと思うけど、先生がやるんだったら、YouTubeで。 わかりました。その時には頑張ります。 じゃあ、あともう一つ。これもよく言われるんですけど、糸入れたら、例えば、さっきのハイフじゃないけど、固くなって、将来的に何か起こるんじゃないか、とか。だって、糸って、1回入れただけで、一生持つわけじゃなくて、大体1年から2年3年持って、3年ぐらいでしょ。2、3年。 そうですね。 そうすると、じゃあ、2年置きとか、3年置きに入れました、ってなると、何かネガティブなことだったり、外科的には、ありますか? そうですね。基本的に、今流行っている糸っていうのは、溶ける糸でやっているので、1年2年あたりで溶けてしまう。溶けるんですけど、そこに瘢痕(はんこん)が入った後の傷ができるから、効果としては持続しますよ。そういった形で言われているとか、整理されていることが多いと思うんですけど、やはり、手術して開けてみると、やっぱり、糸って結構残ってたりだとか。 残ってるんだ。 そういった瘢痕(はんこん)があるのはやっぱり、よく見ますね。やはりフェイスリフトって最終手段、って形になることが多いので、まず糸リフトで開けていったんだけど、年齢的にもそろそろフェイスリフト、っていう今までの一般的な流れがありますので。そういった流れで手術した場合には、やはりあります。問題なのが、糸が通っている場所なんですよね。結構開けてみると、結構深いところを通っている糸とか、も結構見てて。だって見えてないもんね。結構危ないなって思う。大丈夫で良かったね、って思うような瞬間も、なくはないかな。 やっぱり、解剖を知っている先生のところで入れた方が、やっぱり。 そうよね。だって、私も自分の糸を入れると思ったら、ちゃんと解剖を知って。私の顔の筋肉だったり、顔面神経だったりいろんなものを知っている人にやってもらいたいなと思うので。そういう先生にやってもらう、っていうのが一番。 そうですね。あとはやっぱり、通っているのはしょうがないんですけど。それと、通っていたりすると、やはり剥がしにくい。手術はしにくくなるので、やはり、限界の負担が増えている、というところはありますけどね。 でも、先生なら大丈夫でしょ。 頑張るしかないです。 じゃあ、やっぱり、私、今44歳だから、フェイスリフトはやっぱり、50。どうですかね。 まず、糸リフトを今年やるとして。 じゃあ、次、2年後。44でやるでしょ。46。じゃあ、また6で、やるでしょ。48でやって、じゃあ、50歳でいける? Hello everyone, this is Arata Tomori. Today is the second installment of "Ask Dr. Tanaka." Today, I want to discuss sagging skin treatment from both a dermatological and surgical perspective. You know, this year there are trends in the beauty industry and skincare. In fall and winter this year, the trend is addressing sagging skin and skin firmness. Since sagging skin is often difficult to address with cosmetics alone, we talk about firmness, trying to give skin elasticity and essentially prevent sagging. That's the trend. First, when we talk about dermatology, when people mention sagging skin, they often mean HIFU (High-Intensity Focused Ultrasound). Setting aside the technical explanation, HIFU works on the principle of foggy glasses - you apply heat and while the surface remains unchanged, the deep muscle layer gets heated and contracted. We often say meat shrinks when cooked, so it tightens upward. However, when people hear this, they think of grilled meat becoming hard, causing concerns that the face might become stiff or have future problems. However, from a dermatology perspective, we only see the surface. I'd like to ask you, as a surgeon who actually performs procedures and sees deeper layers - from your surgical perspective, what do you think? Well, since HIFU is very popular now, regarding surgical procedures like facelifts where we lift skin, I've noticed that those who have undergone repeated HIFU treatments have harder skin compared to those who haven't. That's my impression. Because the tissue becomes firmer, that might sound negative, but we humans naturally experience progressive tissue softening just from living. That's why everything sags. By applying some firmness, we're essentially restoring tissue condition, which should be fine, right? Yes, I believe that's correct. Since the tissue becomes firmer, it results in better lifting effects. From a treatment perspective, HIFU seems beneficial. I've also experienced HIFU personally and can confirm this. I was hoping to hear from a surgeon about what happens deeper in the tissue, so I'm relieved. So it's safe to do? It's fine. We surgeons just have to work a bit harder. From the dermatology perspective, HIFU is definitely good for sagging skin treatment. Depending on the machine, it's typically done every three to six months, or even as a monthly "HIFU shower" at some clinics. Since there are many types of HIFU devices, when someone says they've done HIFU, you might wonder which device they used, as effects vary significantly. Also, the skill of the person administering it matters. If you try HIFU once and don't see much effect, it's worth asking which device was used. True. Those who've had HIFU at esthetics clinics were told they can do it monthly, which seems like a short interval. When I consider medical-grade HIFU, I sometimes wonder if that's appropriate. Effects definitely vary by machine. HIFU is like smartphones - whether it's Galaxy or iPhone or something else. So think of HIFU as a general term, and there are specific machine names like Ultrasound Cells or Ulthera. It would help if you asked which specific device was used. Another thing I'm considering soon is thread lifting. Ah, I see. When talking about sagging, threads are essential. Maybe I should consult you about how many threads I need. Well, it might be soon. Actually, it's still a bit early. Regarding the jawline, when I look in the mirror and adjust it like this, it looks so much cuter. This area sags naturally, so thread lifting helps lift it. Particularly for the jawline and marionette lines below the mouth, threads work well. How many threads? For someone my age - 44 years old - considering the location, to lift to the jawline, I'd say about five threads. Five? What's the approach? Think of it as filling a surface rather than a line. While drawing lines, we lift the area in a fan-like pattern so one thread wouldn't look right. It's more about creating a cohesive surface lift. So about five threads should be sufficient. If you're only lifting one specific area, fewer might work. But if you want to lift up to the laugh lines more significantly, you'd need threads in the front as well, increasing the count. Since we're doing it, let's plan ahead. In two years I'd be 46, then at 48, and at 50? You're very strategic. But then you're counting on me to handle it well. Of course. OK, now I feel like I can do this. Don't worry, you'll be fine. Regarding risks, since we're inserting needles, bruising is a possibility. Some people bruise more than others, with some having no swelling at all. If bruising occurs, it typically goes from purple to yellow over one to two weeks. But with concealer, you can cover it well. Some experience "dimpling" like dimples, which usually resolves in about a month. Swelling isn't usually severe. If you did it today, could you film YouTube tomorrow? It depends on bruising. With good concealer coverage, I'd say you could do it within a week. So I could do it without anyone noticing? When you do it, you could film for YouTube, but if something happens, could you fit me in? Are you really going to do it? Well, maybe around year-end when schedules align. One more thing people often ask - if threads are inserted, won't they harden like HIFU and cause future problems? But threads don't last forever - they dissolve in one to three years, usually about two to three years. That's right. So if you replace them every two to three years, is there any concern from a surgical standpoint? Current popular threads dissolve in one to two years, creating scar tissue that maintains the lifting effect. While it's commonly explained this way, when we open the skin during facelifts, we often find threads still remaining. They're still there? Yes, the scar tissue persists. Since facelifts are typically the final procedure, patients usually start with thread lifts, and when aging progresses further, they move to facelifts. In such cases, threads are indeed present. The problem is where the threads are located. We sometimes find threads running through very deep structures. Since you can't see it from the outside, it can be concerning. I've had moments where I'm grateful everything turned out okay. It's important to have threads inserted by someone who understands anatomy. Right. If I were getting threads, I'd want someone who understands my facial anatomy and facial nerve structure. Working with such a specialist is ideal. Also, if threads are present, they can make surgical planes harder to navigate during facelifts, increasing surgical difficulty. But you'll manage fine, right? I'll do my best. So, at 44, should I aim for a facelift around 50? First, let's do thread lifting this year. Then at 46, 48, and 50? That's quite a detailed plan. But then you'll take good care of me? Of course. OK, I feel more confident about this now. Don't worry, you'll be fine. Anyway, whether or not you must do this, the approach is mild treatment - maintaining current condition plus slight lifting. Humans naturally experience progressive tissue softening with age. While hardening tissue sounds negative, even when trying to firm it up, human tissue continues to soften. The goal of giving heat through HIFU is to maintain tissue integrity. When that's no longer sufficient, we add thread lifting across a broader area. When that's not enough, we do a facelift.
