今日は、皆さんからも「詳しく解説してください」とリクエストの多かった「白玉点滴」についてお話しします。他の先生方からは、もしかしたら怒られてしまうかもしれませんが、今日はぶっちゃけて、すべて正直にお話ししようと思います。
白玉点滴は人気がありますよね。受けたことがある方も多いと思いますし、美容クリニックで定番メニューとして提供しているところも多いと思います。
「先生、自分のクリニックでもやっているから、本当のことは言わないんじゃない?」と思われるかもしれませんが、冒頭でも言った通り、今日はすべて正直にお話しします。
皆さん、「白玉点滴」と聞くと、どんなイメージを持ちますか? 打つと白玉のようにもちもちの肌になって、真っ白になる、というイメージがありませんか? そのため、当院に来られる方の中にも、「もともと地黒だから白くなりたい」「K-POPアイドルのように全身真っ白にしたいから白玉点滴を受けたい」という方が結構いらっしゃいます。
しかし、正直にお伝えすると、白玉点滴を打ったからといって、自分の肌がもともとの色以上に白くなることはありません。
「えっ」と思われた方もいらっしゃると思いますが、そのメカニズムについて詳しくお話ししていきます。
そもそも白玉点滴とはどのようなものかというと、クリニックによって配合成分は多少異なりますが、メインとなる成分はグルタチオンです。
このグルタチオンは、日本の製薬会社が薬として承認を得たのは1970年代で、かなり前のことです。どのような経緯で承認されたのか、ご存じない方も多いと思いますが、そもそもは薬物中毒の改善、肝機能の改善、放射線を浴びた際の白血球減少の改善、湿疹などに対する効果が厚生労働省から承認されています。
実は、美白作用があるとは記載されていません。「どういうこと?」と思われるかもしれませんが、「炎症性色素沈着の改善」という効果は記載されています。
「炎症性色素沈着の改善と書いてあるなら、美白と同じじゃないですか」と考える方もいらっしゃると思います。大きな意味では美白に含まれるとは思うのですが、炎症後の色素沈着と、皆さんがイメージする「美白」は少し異なるのではないかと私は考えています。
そもそも「美白」とはどのような基準かというと、紫外線が当たった時にメラニンを作るメラノサイトが刺激を受け、細胞を守るためにメラニン色素を放出し、それが排出されずに残ってしまうとシミになります。紫外線を浴びた後に黒くなる方もいますよね。シミではなく、単に日焼けした状態です。ターンオーバーが促されると、夏に日焼けした肌も9月〜10月頃には徐々に元の色に戻っていきます。
つまり「美白」というと、①蓄積したメラニンを排出して白くなるパターンと、②レーザーなどでシミというメラニンの塊を除去するパターン、この2つがあると思います。
しかし残念ながら、グルタチオンにはそのような作用があるという根拠は、色々調べましたがありませんでした。これははっきりと申し上げておきます。
ですから、「日焼けしてしまったから白くするために白玉点滴を打つ」というのは、少し違うのではないかと思います。「海に行って紫外線をたくさん浴びて黒くなってしまったから、白くするために白玉点滴を受けたい」とか、「もともとの自分の肌色よりも白くなりたい」というのは、正直難しいのではないかと思います。
そもそも、私たちの肌の色は決まっています。紫外線を浴びたり、加齢によって糖化や酸化が進んで肌がくすんでいくということはありますが、ベースの色よりも白くなるということは、白斑症のような疾患がある場合を除いて、基本的にはありません。
ですから、どんなに優れた美白化粧品を使ったとしても、もともと持っている肌の色よりも白くなることはないのです。では「もともと持っている色」とは何かというと、紫外線があまり当たっていない、例えば二の腕やお腹の肌の色のことです。それより白くなることは基本的にありません。
少し話がそれましたが、ここは重要なポイントなので覚えておいてください。
「先生、白玉点滴は白くなるって言われてクリニックで受けたのに、嘘だったんですか?」と思われるかもしれません。しかし、これは皆さんが思っている「白くなる」という基準とは別の観点から、私は白玉点滴を受けた方が良いと思っていますし、自分自身も受けることがあります。
それは何かというと、グルタチオンには先ほど述べた肝機能改善作用に加えて、強い抗酸化作用があると言われています。抗酸化作用というとビタミンCなどを思い浮かべる方も多いと思いますが、紫外線を浴びた時に最初に起こるのも酸化なのです。
ですから私は、白玉点滴は紫外線やダメージを受ける前に、注射や点滴をしておくことで、紫外線によるダメージを受けにくくするための予防的な施術だと考えています。
もう一つは、肝機能を改善することです。美白が気になる方はサプリメントを多く摂取される方が多いと思いますが、肝機能がしっかり改善されると、薬やサプリメントの吸収も良くなることがあります。
そういう意味では、沖縄など紫外線が強い場所へ旅行する前や、しっかりケアしていても紫外線が入り込んでしまう夏場などには、予防としてのケアとして白玉点滴を受けるのは良いことだと思います。そういった説明をした上で、当院でも施術を行うようにしています。
また、シミとは少し異なりますが、火傷の跡はまさに炎症性色素沈着ですので、改善の可能性はゼロではないと思います。ニキビ跡も同様に炎症性色素沈着なので、そういったものにはプラスの効果が期待できるのではないかと思います。
これを聞いて「美白作用がないなんて」と驚いた方もいらっしゃると思いますが、「美白」という概念を少し見直していただき、白くはならないけれど、黒くなるのを予防したり、物理的に炎症が起きてしまった色素沈着を改善したりすることはできる、ということです。また、肝機能が良くなることで栄養の吸収が良くなるというメリットもあります。
「先生、白玉点滴は意味がありますか、ないですか」と聞かれたら、予防としては意味があるのではないかと思います。
ということで、今日は白玉点滴の実態について正直にお話ししました。これを見て他の先生方から怒られてしまうかもしれませんが、やはり「ないものはない」「できないことはできない」ときちんと伝えた方が良いと思います。白玉点滴には意味がないとは思いませんが、皆さんが期待していたものと違うと感じたとしたら、それは少し悲しいことです。
ですから、自分がどんな目的で白玉点滴を受けるべきかを理解した上で、施術を受けた方が良いと思います。繰り返しになりますが、「白くなりたい」「シミを消したい」という目的ではなく、「紫外線を浴びる前、シミができる前に予防する」「栄養吸収を良くしたい」という目的で受ける、というように考え方を変えていただければと思います。

Today, I'm going to talk about "white ball IV drips" (Shiratama IV drips), a topic many of you have requested a detailed explanation for. Other doctors might get upset with me for this, but as I mentioned at the beginning, I'm going to be completely honest and tell you everything today.
White ball IV drips are quite popular, aren't they? Many of you have probably had one, and many beauty clinics offer it as a standard menu item.
You might think, "Doctor, since you offer this at your own clinic, won't you avoid telling the truth?" But as I said at the start, I'm going to be completely honest today.
When you hear "white ball IV drip," what image comes to mind? Don't you imagine that after receiving it, your skin becomes plump like a white ball and turns completely white? Because of this, many people come to my clinic saying things like, "I have naturally dark skin and want to become white" or "I want my whole body to become white like K-pop idols, so I want to get a white ball IV drip."
However, to be honest with you, simply receiving a white ball IV drip will not make your skin whiter than its natural color.
Some of you might be thinking "What?!" right now, so let me explain the mechanism behind this in detail.
So what exactly is a white ball IV drip? While the ingredients may vary slightly depending on the clinic, the main ingredient is glutathione.
This glutathione was approved as a drug by Japanese pharmaceutical companies back in the 1970s—quite a long time ago. Many of you may not know the background of how it was approved, but originally, it was approved by the Ministry of Health, Labour and Welfare for treating drug poisoning, improving liver function, treating decreased white blood cells from radiation exposure, and treating eczema.
Actually, whitening effects are not listed as an approved use. You might wonder "What do you mean?" but it does list "improvement of post-inflammatory hyperpigmentation" as an effect.
Some of you might think, "If it says 'improvement of post-inflammatory hyperpigmentation,' isn't that the same as whitening?" In a broad sense, I think it could be considered part of whitening, but I believe post-inflammatory pigmentation and the "whitening" that most of you are imagining are somewhat different things.
So, what exactly defines "whitening"? When skin is exposed to UV rays, melanocytes—the cells that produce melanin—are stimulated and release melanin pigment to protect the skin cells. If this melanin isn't expelled and remains in the skin, it becomes a spot/blemish. Some people's skin darkens after UV exposure without necessarily forming spots—that's simply tanning. As skin turnover progresses, the tan you got over summer vacation gradually fades back to your original skin color by around September or October.
So, "whitening" essentially involves two patterns: (1) excreting accumulated melanin to become whiter, and (2) removing melanin clusters (spots) using treatments like lasers.
Unfortunately, after researching thoroughly, I found no evidence that glutathione has either of these effects. I want to state this clearly: it does not.
Therefore, using a white ball IV drip to "whiten" skin that has tanned from sun exposure isn't quite the right approach. If you've been to the beach, gotten a lot of UV exposure, and your skin has darkened, wanting a white ball IV drip to make it white again—or wanting to become whiter than your original skin tone—is, honestly, not really achievable.
To begin with, our skin color is essentially fixed. While things like UV exposure and aging (through glycation and oxidation) can cause the skin to become dull, becoming whiter than your baseline skin color simply doesn't happen—except in cases of diseases like vitiligo.
So no matter how excellent a whitening cosmetic product you use, your skin will never become whiter than your natural, original color. What is this "original color," then? It's the color of skin that hasn't been exposed to much UV, such as the skin on your upper arms or stomach. You generally can't become whiter than that.
I've gone off track a bit, but please remember this key point.
You might be thinking, "Doctor, I was told the white ball IV drip would make me whiter, and I got it at a clinic—was that a lie?" However, from a different perspective than the "becoming whiter" standard most people have in mind, I do think there's value in receiving this treatment, and I get it myself sometimes.
What is this value? Glutathione, in addition to the liver-function-improving effects I mentioned earlier, is said to have strong antioxidant properties. When you think of antioxidants, vitamin C probably comes to mind first, but oxidation is actually the very first thing that happens when skin is exposed to UV rays.
So personally, I think of the white ball IV drip as a treatment—whether by injection or IV drip—that you receive before UV exposure or damage occurs, to make your skin more resistant to UV damage.
The other benefit is improved liver function. Many people concerned about whitening tend to take a lot of supplements, and when liver function improves, the absorption of medications and supplements can also improve.
In that sense, I think it's perfectly fine to use this as preventive care—for example, before traveling to a place like Okinawa where UV exposure will be strong, or during summer when UV rays inevitably get through even with careful sun protection. After explaining this to patients, we do offer the treatment at our clinic.
Also, though slightly different from spots/blemishes, burn scars are a clear example of post-inflammatory hyperpigmentation, so there's definitely potential benefit there. Acne scars are also a form of post-inflammatory hyperpigmentation, so I believe there could be positive effects for these conditions as well.
Some of you might be shocked to hear "there's no whitening effect," but I'd like you to reconsider the concept of "whitening." While it won't make you whiter, it can help prevent darkening and can improve pigmentation caused by physical inflammation. Additionally, improved liver function can lead to better nutrient absorption.
So if you ask me, "Doctor, is the white ball IV drip meaningful or not?"—I'd say it does have value as a preventive measure.
So today, I've been completely honest about revealing the truth behind white ball IV drips—something that might get me in trouble with other doctors. But I believe it's important to clearly say what doesn't work doesn't work, and what can't be done can't be done. I don't think white ball IV drips are meaningless, but if you had different expectations and now feel disappointed to learn the truth, that would make me sad.
So, I think it's best to understand exactly what purpose you're getting a white ball IV drip for before receiving it. Again, rather than thinking "I want to become white" or "I want to erase my spots," think of it instead as "prevention before UV exposure or before spots form" or "to improve nutrient absorption"—please shift your mindset in that direction.
