Good evening. Since watching Dr. Tomori's videos, I've been able to gain proper knowledge about skin care, and for the first time recently I went to a clinic to have my acne looked at. I used to think acne wasn't worth seeing a doctor for, so I'm glad I worked up the courage to go. **Question:** I recently saw a video on YouTube where a dermatologist said that to treat acne at home, you should heat a needle with a lighter to sterilize it and then pierce the pimple to make a hole. Is that actually recommended? I wouldn't really recommend that. Whether you can truly sterilize a needle that way is not 100% certain, and even if the needle is heated, it's quite difficult to prevent the pus from spreading to the surrounding area. --- Good evening. What do you think about facial esthetics treatments? I had one recently, but I was quite bothered by the friction from the hand massage and the wiping steps. I started to worry that it might actually be putting more stress on my skin. It really depends on the course and the esthetician. Some estheticians are very mindful of skin friction, while others pay little attention to it. I don't often get facial esthetic treatments myself, but the esthetician I saw about two or three years ago was very skilled. I think it really comes down to the individual esthetician's expertise. --- Is it effective to apply face masks or serums while bathing? I'm not sure whether the open pores improve absorption, or whether the products just wash away with sweat and become less effective. I've seen both opinions and can't figure out which is correct. Also, is it better to cleanse or shampoo after the pores have opened up from the heat? Applying face masks or serums while bathing is actually said to be counterproductive. As you mentioned, products can wash away along with sweat, and the idea that pores open and close and absorb ingredients through that mechanism is not really supported. If you do it simply because you don't have time otherwise, that's fine, but doing it with the intention of improving absorption doesn't have much merit. Similarly, using steam to make cleansing easier before washing your face doesn't make a significant difference. Using a facial steamer to improve blood circulation might have some benefit, but beyond that, the effect is limited. --- Good evening. Due to severe atopic dermatitis in the past, even though my condition has improved significantly, I still have severe hyperpigmentation on my neck that remains quite dark. Is there any way to restore it to my original skin tone? I've been diligently moisturizing. I would suggest incorporating brightening or whitening skincare products. Since your atopic dermatitis has improved, you should be able to use vitamin C products without irritation. Additionally, using products that promote skin turnover and boost skin metabolism—such as niacinamide—would be beneficial. This year, ETVOS released a wonderful product combining niacinamide and ceramide in one formulation. Niacinamide not only helps with wrinkle improvement but also has brightening effects—it promotes skin turnover to help release pigment and generally revitalizes the skin. Combined with ceramide, ETVOS's Ultimoist Lotion is an excellent product. Applying it to the neck area may be very effective for post-inflammatory hyperpigmentation. --- I'm 26 and have shallow nasolabial folds around my mouth that I'm concerned about. What skincare products or beauty treatments would you recommend? For skincare, I would recommend POLA's Wrinkle Shot or Shiseido's Vital Perfection Wrinkle Lift for wrinkle improvement. Regular Elixir products are also perfectly fine. Whether you choose a retinol-containing product or POLA's Wrinkle Shot, apply it carefully to the nasolabial fold area. If you are open to non-surgical procedures, a thread lift is likely to give the most aesthetically pleasing result. --- Good evening. I have a question about lip care. I usually apply Vaseline to my lips three times a day after brushing my teeth and also after bathing, but the skin peels off when my lips are wet. I've tried Vaseline, lip packs, and lip serums multiple times a day with no improvement. What should I do? If your lips are cracking, there may be some inflammation involved. I would recommend visiting a dermatologist to check whether you have angular cheilitis or a similar condition, and to get a prescription for medication to reduce the inflammation. --- Good evening, and you look lovely today as always! I've recently gotten into saunas, but are they bad for the skin? There are sauna hats for hair protection—should I also wrap my body in a towel? And should I moisturize my face before going in? In a dry sauna especially, the face tends to become very dry. If you can moisturize beforehand, that's a good idea. After the sauna, make sure to rehydrate and apply moisturizer thoroughly to both your face and body. --- Thank you for always sharing such helpful information. After watching your videos, I started using a vitamin C serum, but I'm getting small red bumps on my cheeks. I've tried APPS and the Spa Treatment serum you recommended, but the same thing happens. I'm in my 30s and really want to incorporate vitamin C—is there any way to do so? You may simply have a skin type that doesn't agree with vitamin C, so if you're getting those bumps, it might be best to stop using it. To figure out whether you're reacting to the vitamin C itself or to another ingredient in the product, I'd suggest trying sample sizes of various different products to narrow it down. --- Good evening, Doctor. I'm 58 years old. The signs of aging have become more pronounced, and I'm starting to feel like skincare products alone can't keep up. I don't want to have any cosmetic procedures, but can skincare products actually slow down aging? Absolutely. Think of it this way: skincare products slow down aging, cosmetic procedures stop it or reverse it. It's never too late to start, so please make it a habit to avoid sun exposure, avoid friction, and avoid dryness. In addition, incorporating anti-aging ingredients such as retinol, vitamin C, and niacinamide into your daily skincare routine can make a meaningful difference. --- Last Thursday I burned my forehead with hair tongs. I went to a dermatologist and was prescribed a steroid ointment and Sinал (tranexamic acid/vitamin C supplement), and it's healing well. When I go outside, I loosely cover it with a bandage to protect it from the sun. Are there any skincare products I should use to prevent hyperpigmentation going forward? Once any itching or discomfort has completely resolved, go ahead and use brightening products such as vitamin C, bisabolol (Chamomilla ET), and kojic acid. --- Good evening, Dr. Tomori. I've read that retinol can cause "vinyl skin," and I'm worried. I use Elixir's Wrinkle Cream every day. I looked it up but couldn't quite understand what "vinyl skin" means, and it made me anxious. Is it bad to use retinol every day? This is a common concern. The term "vinyl skin" has a negative connotation, but I think it refers to a glossy, tight-feeling skin texture. It's true that with high-concentration retinol—such as ZO Skin Health products or those that cause peeling—the skin can temporarily feel thinner and more fragile. However, once you get through that phase, skin thickness returns, so it's a normal part of the process. That said, I don't think it's necessary to use such high concentrations in the first place. Gradually introducing retinol little by little is the better approach. Products from brands like Heals, Clinique, C'Lab, and of course Elixir are not concentrated enough to cause peeling, so using them consistently in small amounts is entirely appropriate—and it's what I do myself. --- Good evening, Dr. Tomori. I'm a 28-year-old male with combination skin. I'm bothered by blackheads on my nose. For the purpose of exfoliation, how often per week should I cleanse? Is an oil-type cleanser better? I work from home so I don't apply sunscreen or anything similar. For exfoliating just the nose, I'd recommend using a cleansing oil on that area one to two times per week. Rather than applying it to your whole face, first apply the cleansing oil just to the nose, and then follow up with an enzyme-based facial wash. --- Good evening, Doctor. I'm in my 40s and every month as my period approaches, I get painful pimples that I can feel when I touch them. I've been going to a dermatologist and was prescribed BPO, but it hasn't really helped. After researching, I thought that controlling hormone balance with the pill, or Melsmon injections, might be helpful. What treatment would you recommend? For someone in their 40s, I would suggest progesterone therapy rather than the combined oral contraceptive pill. The pill contains both estrogen and progestogen, but there are also progesterone-only options. Since women in their 40s are approaching perimenopause, a combined pill may not always be the best choice for hormone balance. I'd recommend consulting a gynecologist near you and asking about progesterone therapy specifically. Thank you for watching!
