シミ・シワ増加の原因になるすっぴんの罠について解説します。

みなさん、こんにちは。内科皮膚科医の友利新です。
みなさん、すっぴんで過ごしてませんか?それ、シワやシミの原因になるんです。
最初から怖いことをお話ししましたけれども、最近マスクもしてるから「すっぴんの方がいいんじゃない?」ということで、すっぴんで過ごしているとか、あとは「お化粧とかをすると肌に悪いから私すっぴんなんです」っていう方。実はそれっていうのは、お肌にはすっごく悪いっていうことが分かっているんです。
すっぴんでいるっていうことは、日焼け止めとかファンデーションを塗らないですよね。「先生、どうせ紫外線でしょ」っていうことだと思うんですが、もちろん紫外線もダメです。だけどね、もっと怖いものが最近では肌に降り注いでいるんですよ。それが大気汚染になります。タバコの煙や黄砂など、大気中に蔓延しているものになります。これ自体は年々増えてきていて、特に今の時期は黄砂が東京とか日本列島各地に降り注いでいると思います。私も朝起きると黄色い靄がかかったようなことがあるんですが、それは黄砂なんですね。
あと最近注目されているのがPM2.5になります。分かりやすく言うと、2.5マイクロメートル、つまり1ミリの1000分の1くらいの大きさなんですね。それでもよく分かんないという方は、髪の毛の断面の直径の、だいたい30分の1ぐらいの大きさの粒子のことをPM2.5というふうに呼びます。
これは皮膚科だけではなくて、内科の分野でもこれを吸うことによってアレルギー症状が出て、気管支喘息の方が悪化したりということがよく言われているんですが、これはもう皮膚科の中でもすごくクローズアップしてきて、実はこのPM2.5だったり大気汚染というのが、肌のシミとかシワの原因になるということが近年分かってきているんです。
特に少し前から言われているのはシミですね。PM2.5だったり大気汚染物質がお肌に付着していると、そこに活性酸素が発生してしまうんですね。活性酸素が起こってくると、肌に炎症が起こるようになるので、肌のバリア機能が低下して、痒みが起こったりというのは分かると思うんですが、この炎症は日焼け、UVと同じように、メラノサイトに「メラニンを作ってくれ」という指令を出してしまうんですね。そうすると実はシミの原因になってきてしまいます。
そして最近もっと怖いことが分かったのは、このPM2.5だったり大気汚染が実はシワにも影響するということが、最近のポーラさんの研究で分かったんです。ポーラさんというとシワの原因がですね、好中球という、傷に集まってくる血液物質がですね、好中球エラスターゼというものを出すことで真皮を溶かしてしまって、シワができるという研究を長年やっていて、それをブロックするのがニールワンという物質まで開発しています。
シワ研究をしているポーラさんがですね、2021年の3月に発表したデータによると、実はこの大気汚染物質、PM2.5だとかが肌に付着するとですね、実は好中球がですね皮膚に遊走ってくる、つまり集まってきやすくなるということが分かったんです。好中球がいっぱい集まってくると、「ここに傷があるんじゃないか」と勘違いをしてしまって、そこで好中球エラスターゼを出してしまう。それによって真皮の繊維がですね、溶かされてしまってシワが深くなってしまうそうなんです。
きちんとしたデータも出していて、2016年の年間の平均大気汚染が73μg/立方メートルの地域と、34μg/立方メートルの地域を選んで、定められたシワのグレードを測るテストをそれぞれ行った結果、大気汚染物質が高いところに住んでいた女性の方が、シワのグレードが圧倒的に高かったという有意さも出ています。
この大気汚染、怖いことに年間の大気汚染の平均というものが出てるんですが、だいたい1月ぐらいからだんだん上がってきて、ピークは4月と5月なんですよ。そう、今が一番ピークの時。6月になると梅雨もあってちょっと下がって、7、8と少し高くなって冬にかけて少し下がっていくんですけれども、基本的にはこの時期、圧倒的に多くなってくるということを思っていてください。
もう少し踏み込んでいくと、この大気汚染物質がついているときにUVが当たるとですね、「シミを作ってください」という炎症物質が出るということも分かっていて、シワもシミも両方濃くなってしまう可能性があるんです。
これはですねロレアルグループの実験なんですけれども、そのまんますっぴんの状態でPM2.5がついてしまったお肌というのと、日焼け止めを塗っていてPM2.5がついてしまったお肌を比べて、同じクレンジングで洗った時に、どれだけPM2.5が肌からきちんと洗い流されているかという実験を行ったところ、なんと日焼け止めを1枚塗っていて、そこにPM2.5がついたお肌の方が綺麗に取れているという実験データもあるんですね。
なので皆さん、なんとなく日焼け止めとかファンデーションを塗っていると、お肌に悪いからといってすっぴんで過ごしていると、日焼けだけじゃなくてこの大気汚染物質だったりPM2.5がお肌につくことによって、好中球が湧いてきて好中球エラスターゼを出してしまったり、あとは紫外線が当たった時にメラノサイトに「シミを作ってください」という同じような指令が肌に行くことによってシミが濃くなってしまう、シミができてしまうということになるので、特にこの黄砂だとかPM2.5が多い4月、5月というのは、しっかりと日焼け止めを塗ってメイクをして、すっぴんで過ごさないように心がけてください。

Hello everyone. I'm Arata Tomori, an internal medicine and dermatologist doctor.
Everyone, are you spending time without makeup? That can actually cause wrinkles and spots.
I started with something scary, but recently, since we're all wearing masks, some people think "maybe going bare-faced is better" and go without makeup, or say "putting on makeup is bad for my skin so I go bare-faced." Actually, it's now known that this is very bad for your skin.
Going bare-faced means you're not applying sunscreen or foundation. You might think, "Doctor, it's about UV rays, right?" Of course, UV rays are bad too. But there's something even scarier that has recently been raining down on our skin - air pollution. This includes cigarette smoke, yellow dust (kosa), and other things prevalent in the atmosphere. This itself has been increasing year by year, and especially this time of year, yellow dust is falling across Tokyo and all over Japan. I sometimes notice a yellowish haze when I wake up in the morning - that's the yellow dust.
Another thing that's been getting attention lately is PM2.5. To put it simply, it's about 2.5 micrometers, or 1/1000 of a millimeter. If that's still hard to understand, PM2.5 refers to particles about 1/30th the diameter of a cross-section of a human hair.
This isn't just a dermatology issue - in internal medicine, it's also said that inhaling this can cause allergic symptoms and worsen conditions like bronchial asthma for some people. But dermatology has really been focusing on this too, because it's been discovered in recent years that PM2.5 and air pollution can actually cause skin spots and wrinkles.
Spots have been talked about for a while now. When PM2.5 or air pollutants adhere to the skin, it generates reactive oxygen species. When reactive oxygen species occur, inflammation happens in the skin, which weakens the skin's barrier function and can cause itching, as you might know. This inflammation, just like sunburn from UV rays, sends a signal to melanocytes to "produce melanin." This then actually becomes the cause of spots.
And something even scarier was recently discovered: PM2.5 and air pollution actually affect wrinkles too, according to recent research by POLA. Speaking of POLA, they've been researching for years how neutrophils - blood substances that gather at wounds - release an enzyme called neutrophil elastase, which dissolves the dermis and causes wrinkles. They've even developed a substance called Nir1 (Niru-one) that blocks this process.
According to data published by POLA, who researches wrinkles, in March 2021, it was found that when air pollutants like PM2.5 adhere to the skin, neutrophils become more likely to migrate to the skin - meaning they gather there more easily. When lots of neutrophils gather, the body mistakenly thinks "there must be a wound here," and releases neutrophil elastase there. This apparently dissolves the dermis fibers, deepening wrinkles.
They've published solid data too. They selected an area with an average annual air pollution level of 73μg per cubic meter in 2016, and another area with 34μg per cubic meter, and conducted tests measuring standardized wrinkle grades in each. The results showed that women living in areas with higher air pollution had significantly higher wrinkle grades.
What's scary about this air pollution is that when you look at the annual average, it gradually rises from around January, peaking in April and May. Yes, right now is the peak time. It drops a bit in June due to the rainy season, rises slightly again in July and August, then decreases toward winter - but basically, this time of year sees overwhelmingly high levels, so please keep that in mind.
Going a bit further, it's also known that when UV rays hit skin that already has these air pollutants attached, inflammatory substances that trigger "make spots" are released, meaning both wrinkles and spots can potentially become worse.
This is from an experiment by the L'Oréal Group: they compared skin that had PM2.5 attached while completely bare-faced versus skin with sunscreen applied that had PM2.5 attached, then washed both with the same cleanser to see how thoroughly the PM2.5 was washed away from the skin. Surprisingly, the data showed that skin with a layer of sunscreen applied, even with PM2.5 attached, was cleaned more thoroughly.
So everyone, if you vaguely think that applying sunscreen or foundation is bad for your skin and go bare-faced, not only sunburn but also these air pollutants and PM2.5 attaching to your skin can cause neutrophils to gather and release neutrophil elastase, or when UV hits, send similar signals to melanocytes to "create spots," which can deepen existing spots or create new ones. So especially during April and May, when yellow dust and PM2.5 are abundant, please make sure to apply sunscreen and wear makeup, and try not to go bare-faced.
