皆さんこんにちは、内科・皮膚科医の友利新です。今日は、体が老化していく原因について解説します。皆さん、体の老化の原因は何かわかりますか?実は、炎症によるものなんです。
炎症というと「えっ、炎症ですか?」と思うかもしれません。炎症と聞いてパッと頭に浮かぶのは、例えば蚊に刺されたり、ニキビができたり、転んで捻挫したりした時に、腫れたり赤くなったりするといったものではないでしょうか。
確かに私たち医者も、炎症の4大特徴として「赤くなる・腫れる・熱を持つ・痛みがある」という4つを挙げます。この4つの特徴がある炎症は「急性炎症」と呼ばれるものです。急性炎症は、転倒による物理的なダメージを治そうとして免疫機構が働いたり、ニキビ菌の増殖に対して体が対処しようとしたり、異物が侵入した際に免疫機構が反応したりすることで起こります。これは体の防御反応ですので、決して悪いことではありません。
しかし、老化を進める炎症として、急性炎症よりもさらに怖い「慢性炎症」というものがあります。慢性炎症の最も恐ろしいところは、自分では全く気がつかないという点です。急性炎症は赤みや腫れ、痛みなどで気づけますが、慢性炎症はじわじわと進行し、「なんとなく体の調子が悪い」と感じる程度にとどまります。
例えば、血管に慢性炎症が起こると動脈硬化を引き起こし、血圧上昇・糖尿病・心筋梗塞・脳梗塞につながります。血管で慢性炎症が起きていることはなかなか自覚できませんが、それがじわじわと続くことで動脈硬化が進み、やがて深刻な病気を招くのです。
見た目の老化も同様です。私がよくお話しする「肌でやってはいけない3つのこと」、すなわち紫外線(日焼け)・摩擦・乾燥は、すべて慢性炎症を防ぐためのものです。紫外線は皮膚の奥でコラーゲンやエラスチンを破壊する慢性炎症を引き起こし、シミやたるみの原因になります。乾燥も長く続けば慢性炎症となり、くすみを招きます。摩擦も積み重なることで慢性炎症となり、色素沈着を起こします。
慢性炎症のわかりやすい例として「歯周病」があります。歯周病は歯と歯肉の間で歯周病菌が炎症を起こすものですが、多くの方が歯医者さんに指摘されるまで自覚症状がありません。放置すると、じわじわと歯肉が後退し、最終的には歯が抜けてしまいます。さらに歯周病菌が出す炎症物質は体中に広がり、妊婦さんでは早産リスクの上昇、糖尿病の悪化、脳梗塞・心筋梗塞のリスク増加などとも関連していることがわかっています。
残念ながら、人間は年を取るだけで体が炎症に傾きやすくなります。つまり、慢性炎症を抑えることこそが老化を止めることと言っても過言ではありません。体の炎症を抑えることは、肌の炎症を抑えることにもつながり、見た目も内側も若々しく保つことができます。
それでは、体の中で慢性炎症が進んでいるかどうかを確認する5つのチェックポイントをご紹介します。
1. **肥満**:体に脂肪が多い状態は、慢性炎症を引き起こす大きな原因になります。
2. **運動習慣がない**:過度な運動も問題ですが、全く運動しないことも炎症を促進します。血流の悪化や栄養の蓄積が慢性炎症のもとになります。
3. **喫煙**:タバコに含まれる多くの化学物質が体を慢性炎症に傾けます。くすみ・シミ・シワ・たるみ(いわゆる「スモーキーフェイス」)や、血圧上昇・動脈硬化の原因にもなります。
4. **便秘がち**:腸内環境は免疫機能に深く関わっており、便秘は腸内環境の乱れ=免疫バランスの崩れを意味し、慢性炎症の背景になり得ます。
5. **過度なストレス**:ストレスホルモンの分泌も慢性炎症の大きな原因です。体型や生活習慣が良好でも、強いストレス環境にいる方は炎症に傾きやすいので注意が必要です。
なお、医療機関では「高感度CRP」という血液検査の指標で慢性炎症を調べることができます。気になる方はかかりつけ医に相談してみてください。

Hello everyone, I'm Arata Tomori, an internal medicine and dermatologist. Today, I'd like to explain the causes of physical aging. Do you know what actually makes our bodies age? The answer is inflammation.
You might be surprised to hear that. When most people think of inflammation, they picture things like mosquito bites, pimples, or a sprained ankle that becomes swollen and red. As doctors, we also define inflammation by four cardinal signs: redness, swelling, heat, and pain. Inflammation that displays all four of these signs is called acute inflammation. It occurs when the immune system responds to physical trauma, bacterial overgrowth (as with acne), or foreign substances entering the body. Because it is a protective response, acute inflammation is not inherently bad.
However, the type of inflammation that drives aging is a far more insidious form known as chronic inflammation. The most frightening thing about chronic inflammation is that you cannot feel it at all. Unlike acute inflammation, which announces itself with redness, swelling, and pain, chronic inflammation smolders quietly, manifesting only as vague feelings of being "not quite right."
When chronic inflammation occurs in blood vessels, for example, it leads to arteriosclerosis, which in turn raises blood pressure, promotes diabetes, and increases the risk of heart attack and stroke. You would never notice that your blood vessels are inflamed, yet this silent process advances steadily over the years until serious disease strikes.
The same principle applies to visible aging. The three things I always tell patients to avoid for their skin—UV exposure, friction, and dryness—are all about preventing chronic inflammation. UV rays penetrate deep into the skin and trigger chronic inflammation that destroys collagen and elastin, causing age spots and sagging. Prolonged dryness becomes chronic inflammation that leads to a dull complexion. Repeated friction accumulates into chronic inflammation that causes hyperpigmentation.
A relatable example of chronic inflammation is periodontal disease (gum disease). Bacteria between the teeth and gums cause inflammation that most people don't notice until their dentist points it out. Left untreated, this slow-burning inflammation gradually erodes the gum tissue and eventually leads to tooth loss. What's more, the inflammatory substances released by periodontal bacteria spread throughout the body and have been linked to increased risk of preterm birth in pregnant women, worsening of diabetes, and higher rates of heart attack and stroke.
Unfortunately, the aging process itself causes the body to become more prone to inflammation. This means that simply growing older—even with a healthy lifestyle—tilts the body toward a state of chronic inflammation. It is no exaggeration to say that controlling chronic inflammation is the key to controlling aging. Reducing inflammation inside the body simultaneously reduces inflammation in the skin, leading to both better health and a more youthful appearance.
Here are five checkpoints to help you assess whether chronic inflammation may be at work in your body:
1. **Obesity**: Excess body fat is one of the most significant drivers of chronic inflammation. Maintaining a healthy weight is essential.
2. **Lack of exercise**: While excessive exercise can also be inflammatory, a completely sedentary lifestyle impairs circulation and allows metabolic byproducts to accumulate, fueling chronic inflammation.
3. **Smoking**: The many chemicals in cigarettes beyond nicotine push the body into a state of chronic inflammation, contributing to dull skin, age spots, wrinkles, and sagging (known as "smoker's face"), as well as high blood pressure and arteriosclerosis.
4. **Chronic constipation**: The gut plays a critical role in immune regulation. Constipation signals a disrupted gut environment, which disturbs immune balance and may serve as a sign of underlying chronic inflammation.
5. **Excessive stress**: Stress hormones are a major contributor to chronic inflammation. Even people who are not overweight, do not smoke, and exercise regularly can be highly susceptible to inflammation if they are under severe or persistent stress.
For those who want a medical assessment, doctors can measure a blood marker called high-sensitivity C-reactive protein (hs-CRP), which specifically indicates low-grade chronic inflammation. If you are concerned, consider asking your doctor about this test.
Controlling chronic inflammation inside the body is the foundation of true anti-aging—for both your health and your appearance. I encourage everyone to be mindful of chronic inflammation and make lifestyle choices that keep it in check.
