Hello everyone, this is Nitta from Tomoni. Today we have Dr. Terayama joining us again as our guest. Thank you for being here. We've talked about various things like dental treatments, tooth aging, and whitening, but I realized I forgot to ask something important - how to brush your teeth properly. Let's go back to basics. So today's topic is: Is your brushing technique really correct? I'd like you to teach us the proper way to brush our teeth again. Sure, let's do it. First, as always recommended, I use a toothbrush with a small head that allows for good maneuverability. I start from the upper left back tooth, brush the back molars all the way to the back surface - the sides are important, as my sister always says - so I go front, back, front, back, and work my way to the front teeth. I have a crown on my front tooth, so I'm careful there too. I was born without two lower teeth, so I have a bridge there. Since implants weren't available back then, I was told to be careful not to damage the teeth on either side, so I brush carefully to avoid gaps. Then I go to the right side, upper teeth, front, behind the front teeth, and also brush vertically. I brush the canine area and front teeth vertically too, then horizontally, back and forth to the molars - there's a gap where I had orthodontic equipment attached, so I focus there too. Finally, I go back to the front, then lower, then upper teeth. This seems pretty normal, doesn't it? I do the sides properly, and I switch grip positions several times for the molars. Aren't you doing the lower teeth the same way though? No, I don't. I was told not to do it that way, so I use a separate brush for the lower teeth. That's wonderful. I wonder if it's really that impressive though. One thing that impressed me about your technique is your grip. You're holding it like a pencil, which is already excellent. I think your sister, who's a dentist, taught you that. We tell children to hold it like a pencil too - that's wonderful early education. Most people tend to grip it in their fist. How do you hold it, Noboru-kun? Show me. That would cause plaque buildup. Is this how you hold it? Let me see... isn't this hard to use? I guess it really is about awareness. Like this, right? The basic grip is like holding a pencil. For better control, it might be good to hold it closer to the head. It really is like a pencil. Does it fit in your mouth? Yes, it fits. It's just about the right length. Actually, it doesn't reach that far - see, even when you insert it all the way to the back, there's still room. The shorter you hold it, the less it wobbles. If it's too long, it wobbles more. Since adults should brush with small side-to-side strokes, if it wobbles, the strokes become too big, or conversely, it doesn't make proper contact and doesn't clean effectively. So holding it shorter and brushing in small strokes is better. This is eye-opening! Why are toothbrush handles so long? Right? I wish they'd sell them at this shorter length from the start. That's why in some cases, for women, I recommend children's toothbrushes. I thought they were intentionally made long. Probably because our hands are different in size from Noboru-kun's hands. If Noboru-kun holds it here, it reaches all the way there. That's true, that's true. So I think that's why there's that much handle length. I see, so it's not wasted, this part either. Right. If you place your thumb on the narrow indented part, it fits perfectly. Everyone's toothbrush has that indent. Yes, that's the marker. It makes brushing much easier all at once. My sister only talked about the head size. Like you mentioned earlier, a smaller head is better, right? Yes, definitely. It's said to be about the same size as your thumbnail, so that's just about right. It really is about the same size. For the bristles, compact, thin, three-row brushes are good - this is the standard type, and yours might be a softer one. It says "soft" here. Mine says "regular," so it's different, right? Even within the same brand, there might be both soft and regular versions. Regular means standard hardness - it's gentle enough that both the white part of the teeth and the pink gum area won't feel too much pain when touched. The soft type targets the gums specifically, so it might not be enough for scrubbing the white surface of the teeth. But for adults, it's rare for thick buildup to form on the tooth surface. Since your teeth alignment is fairly stable and your mouth moves during eating, causing friction that helps remove surface stains naturally... so rather than focusing on cleaning the surface... Where should we focus on cleaning then? The curved area along the gum line, the boundary between teeth and gums. Here, right? Yes, exactly, this area. Here too? My sister mentioned that too - the "pocket" area. Exactly. So instead of scrubbing the white part of your teeth vigorously, it should be more like this gentle motion. Oh, so it's not side-to-side? Right, not side-to-side, but more like this angle, this angle. I see, so the surface... this is a major discovery. Maybe you should reconsider your own technique too, doctor. True, there's rarely thick buildup stuck on teeth like that. The buildup is at the edges, right? Exactly. The only thing that sticks that obviously is lipstick. Lipstick, right. In a previous video, we talked about using floss appropriately, and I thought it should be used last, as a finishing touch. Actually, it should be used first. So doing it first means things get messier afterward, right? And that's actually fine? Yes, floss removes the bulk of debris first, and the toothbrush is the finishing touch. So the toothbrush is the finishing touch, not the floss? Right. So brushing afterward helps distribute the toothpaste ingredients thoroughly to every corner. This applies to the brushing angle too - assuming you've already removed debris between teeth with floss or an interdental brush, you should move the brush gently like a gum massage. So does that mean my vertical brushing technique for front teeth isn't really meaningful? Well, for the back of your front teeth, there are indentations, so adjusting the angle there is totally fine. Like the back of the front teeth? That's completely fine. But even there, small strokes work better than large ones for removing debris. I see. Also, with your previous brushing pattern of doing front then back, it might be better to establish a fixed routine - like going clockwise for the front, then clockwise for the back. Why is it not good to go back and forth between front and back? While brushing, we often zone out and think about other things. I do that too, and later I realize "oh, I might have missed this spot" - like doing the front but not the back. When you touch your teeth with your tongue and feel roughness, you realize you missed a spot. Areas you're consciously aware of, like the back of front teeth that you know you must clean, probably get done properly. But areas like the inside of your upper left molars might get neglected some days. So it's better to establish a fixed sequence. I see - it's like with sunscreen too, where I always say to establish a routine for application, to avoid missing spots, like always starting from the right side - it's the same concept. Even the way you hold the brush is eye-opening. As for bristle types, for beginners I recommend flat, thin bristles, but since everyone's dental situation differs - overlapping teeth, unusually large teeth, wisdom teeth growing at an angle - I want people to use additional tools suited to their specific needs. For your concern about the back of front teeth, there are brushes with differently shaped bristles too. The difference might be subtle, but some have tapered, thin bristle tips that can reach into gum pockets more easily. However, if you use tapered bristles without first mastering the standard brushing motion, it can be like scrubbing with a pin cushion, which is dangerous. So first practice with a standard brush, then move on to intermediate or advanced options as needed - using thinner bristles, different shapes, or firmer bristles for areas that need extra care. Also, considering hygiene, it's recommended to replace your toothbrush about once a month, so it makes sense to try different types. If one feels comfortable this month, you can repeat it, or if it feels lacking, try a different one. And if it doesn't work out, you can always repurpose it for kitchen cleaning. A friend told me they're also good for touching up gray roots. These days, people use toothbrush-like tools for applying gray hair dye too. So even if a brush doesn't suit your teeth, there are many other uses, so feel free to experiment. Is it okay if it's cheap? Cheap is fine. Start simple for practice. Try holding it like this - as for how much pressure to apply, people often ask about that. It's said to be around 150 to 200 grams, but grams are hard to gauge, right? But now everyone has a kitchen scale at home. 150 to 200 grams - let's see how much pressure you're using... About this much. That's quite weak. Maybe so. I think you might be holding it too far back, so if you adjust like this... Right, now it's about 150 or 140 grams. When you do it this way, even light pressure gets close to 150 grams. Also, if you hear a scrubbing sound, that's not good. Loud sounds mean you're brushing too hard, so it should be gentle, one tooth at a time. I've been brushing this whole time but I can't tell the difference. Actually, this is quite thorough brushing at about 150 grams. Wow, I thought everyone brushed the way I usually see people doing - looking rough, like at station restrooms. I thought that was normal. Your gums must be damaged from that. If it doesn't hurt when brushing that hard, it probably means you're not actually brushing the areas that need attention. If you master this technique, your hand gets really tired. That's why people switch to electric brushes... That's exactly why I like electric brushes - manual brushing this thoroughly makes me feel like some kind of inspector, and it's exhausting. So are electric toothbrushes good? They're good once you've mastered manual brushing. Using an electric brush before mastering proper technique is very dangerous. If you keep the scrubbing habit while using an electric brush, you'll damage your gums, so please don't do that. How do you hold an electric toothbrush? With an electric brush, you focus on positioning the head properly... Right, I was worried whether I was holding mine correctly. That's why electric brush handles are thicker like this. Only people who have properly mastered technique should use them safely - otherwise it becomes reckless. People who understand appropriate pressure and have internalized that gentle brushing is effective should use electric brushes. I see. So should children use electric brushes only after mastering manual technique first? For children, since they're going through tooth replacement, their current brushing technique won't necessarily continue the same way into adulthood. So it's more about enjoyment - using electric brushes with apps that make brushing fun. Like earning coins or points? Exactly, there are features like that. The main purpose is to make brushing time enjoyable for kids. That's reassuring. My kids always say they prefer electric brushes, but I still worry, so I do finishing touches too. But when I'm busy, sometimes I just tell them to brush on their own, and I can't always supervise them, so I've been thinking they need to eventually do it independently, which worried me. But some apps have features that identify missed spots and tell kids "you missed here," which can make things easier for parents. And if it tells them "you missed this spot, try again," that eliminates the need for parents to do finishing touches. Thank you for being so considerate. I really want people to be able to do oral care without stress. It's an everyday thing after all. Conversely, some people brush three times a day. I wanted to ask about that too - for example, my breakfast is just a drink, so I just rinse afterward, or sometimes brush if I have time, but since I don't eat solid food, I brush when I wake up and rinse, so it's about two or three times. But there are days I can't brush much, so it becomes just once a day, which worries me sometimes. What do you think? For people with generally good oral hygiene, occasionally brushing just once doesn't make a huge difference. However, I definitely want people to brush at night - not brushing at night leaves an uncomfortable feeling, right? That's true. So as long as you make sure to brush thoroughly at night, there shouldn't be major problems. However, plaque - what's called biofilm - starts forming after about 8 hours, so even without eating meals, drinking beverages with certain ingredients or sugar, or snacking, causes buildup and increases cavity risk. So after eating or drinking, at minimum, vigorously rinsing your mouth with water helps. So even just rinsing helps flush out food debris that feeds bacteria? Exactly, that's the important first step. Also, maintaining an environment where buildup doesn't accumulate easily means you don't need to be overly strict about it. But many women who only have yogurt or vegetable juice for breakfast often skip rinsing too, and I see lots of early-stage cavities in such cases. When people consult me saying "I seem very prone to cavities," I take dietary surveys, and often find they drank something sugary but didn't brush afterward. So should you rinse every time you eat or drink something? Yes, you should rinse. The frequency depends on each person's situation - people who've been told by their dentist that they have high cavity risk should definitely brush more often. Even if it's not immediately after, is it okay to rinse about 2 hours later? Yes, that's fine, it depends on the situation. But if it means scrubbing vigorously and roughly, it's better not to brush at all in that case. So in that case, just rinsing is enough? Yes, just rinsing, or removing food stuck between teeth and rinsing is sufficient. I see, maybe I should carry a travel toothbrush after all - it might come in handy for quick touch-ups. Next, let's check what's in your bag - I'm sure you have one. I'll add one starting tomorrow. I always have a toothbrush at the clinic and at the office, so there's rarely a time I don't have one. But we didn't have one in Korea, remember? Right, we didn't. In Korea, hotels don't provide toothbrushes. That's surprising. They're sold separately there. I see, that's why we had to go buy one at Olive Young. But I already knew that. You knew? These days, toothbrushes are sold almost everywhere, but are hotel toothbrushes not bad to use? Since I travel for business and stay at various hotels, I actually collect different hotel toothbrushes as a secret hobby. I gather them thinking "this one's from such-and-such hotel" and analyze them. Recent hotel toothbrushes have really improved. Which hotel's was good, in your opinion? Comfort Hotel's was good. That's a business hotel chain, but it's not necessarily true that luxury hotels have better ones. Sometimes they're stylishly designed though. Stylish! Sometimes even made of wood, right? Right, luxurious-looking, but whether they clean effectively is another matter - the toothbrush itself has improved, but the toothpaste isn't always great. I personally have preferences for toothpaste flavor, so I always bring my own. I understand completely. You're quite advanced in this. I just don't like using unfamiliar ones. Though I do end up using the small hotel ones sometimes. Really? Isn't that not enough in the morning? If it's just for a couple of uses, it's fine. There are times you have to make do though. So I sometimes end up using my own. I think it's good to bring toothpaste suited to your own preferences too. I'm sure you're the same way, doctor - you consciously bring your toothbrush, toothpaste, and floss. I get really frustrated if I don't have floss. "Oh no, I forgot it!" - does that happen to you too, doctor? Yes, it does. I usually have about three in my pouch, but sometimes I forget the whole pouch. I totally understand - I was surprised once too. I went on an overnight trip with a small bag, and when I was about to shower, I took off my clothes and realized I didn't have it. I stopped the shower, got dressed again, but then I thought - I have body wash, I thought I packed shampoo too, so maybe I could use body soap on my face as a last resort, but no, I really didn't want to, so I went out to buy some on my way back. Exactly, I understand completely. It's so frustrating - like "I forgot my floss!" I have the exact same experience - when I arrive at accommodations and take off everything ready to shower, I realize "oh no, I don't have it" and rush out to buy some. Right, you'd go buy some. Compared to the past, oral care products have become much more common even at convenience stores. True, they definitely sell floss now. They do. So it really is that kind of era now - everyone's more health-conscious. How long do you brush for, doctor? Sometimes I use it in the evening and brush for a while without really noticing the time, but at night I usually use an electric brush, so about 3 minutes. That's a relief - I was worried you'd say 30 minutes! With manual brushing instead of electric, it might take about 10 minutes. I only brush longer if I feel something's not clean enough. I guess brushing time varies for everyone. Of course it would for a professional like you - it's surprisingly deep, isn't it? This should give you a general idea, but if there's anything unclear, feel free to ask again. Understood! So today, so many revelations kept coming out... Wait, why are you laughing?
