それ間違いです!正しい爪の切り方

皆さんこんにちは。内科皮膚科医のともこです。今日はですね、外来でも意外と悩みが多い爪についてお話ししようと思います。
皆さん、爪の正しい切り方、知ってますか?
というのもですね、貝印さんといって、例えば爪切りだとか、そういったネイル周りの商品の詳細を出している会社さんのサイトに載っていたんですが、私もそれは確かになと思ったのが、「足の爪の悩みはありますか?」というアンケートでですね、女性はなんと7割近い悩みがあるというふうにお答えが出ているそうなんですね。
もちろんその中の悩みの多くは、いろんな症状、例えば水虫とか、爪がポロポロと剥がれてしまうとか、そういったものもあるんですが、圧倒的に巻き爪が悩みの多くになっています。
実はですね、外来でもこの巻き爪、よく見るんですよ。本当に爪がですね、くるっと巻いてしまって、その巻いた場所が指のところを押してしまって、そこから炎症が起こってしまって、中には膿んでしまってというのも見ていて、痛そうって思うような患者さんをよく見かけたりします。
もちろん皮膚科が全員見ているわけじゃないんですけれども、爪の外来といって、ワイヤーをつけて巻いている爪を広げるというような治療もあります。
ただ、これって治療はできるんですけれども、間違った生活習慣だったり間違った爪の切り方を続けていると、ずっと爪が巻いた状態になって巻き爪が続いてしまいますので、まずは生活習慣を直すことと、正しい爪の切り方を皆さんにシェアしたいなと思って、この動画を撮っています。
まず爪って何であるか分かりますか? なんとなくついていると思うかもしれないんですが、爪の役割というのは、まずは指先、手でも足でも、この指先を守るという役割があります。あと、指の腹側を押す力を、この爪で守っているところがあるんですね。なので、実は爪と指というのは、お互い押し合っているような関係になります。
人差し指だったりすると、この爪のここから向こう側に力がいって、今度は指の方から爪の方に力があって、これが要は押しくらまんじゅうの状態でいるんですね。このバランスがちょうどいいということで、お互いが支え合っています。だから指先に強く何か刺激があっても、この爪がきちっと守ってくれることで、指先を保護してくれています。ですから、ここの指先というのは、細かい作業ができるような設計をされているんですね。すごいお利口さんなんですよ。
じゃあどうして爪が巻いてしまうのかという、巻き爪って手ではあまり見ないというか、聞いたことなくないですか。だいたい巻き爪というのは足で多いものになります。これはなぜかというと、さっき言った押す力のバランスの変化によるものが大きいです。
例えば、小さいお靴を履いていたりとか、女性でつま先っぽが尖った靴を履いているとするじゃないですか。そうすると足の指に特に爪の方向にずっと圧迫、圧力が加わることになります。そうするとこの爪がずっと押された状態になると、通常だったら爪が押す力と指が押す力がプラスマイナスでゼロになるところが、爪のところがずっと大きくなると圧力がグーッとかかってしまうので、横がグーッと力が入ることによって爪が押し続けることになるんですね。そうすると指が巻けてしまって、端っこからどんどん圧がかかっていって爪が巻いてしまうということになるんです。
なので要はどういう生活習慣が悪いかというと、常に足の爪に力が加わっている状態っていうのがあまり良くない。先ほど足の爪に悩みを持つ人っていうのは女性が多かったのは、パンプスを履いたりとか、ちょっと細い靴を履くっていう女性に実は多いんですね。
もう一つは歩き方になります。例えばこれは男性にも多いんですが、ブカブカの靴を履いていて、しっかりと足の指で地面を押して歩いていない、なんとなく足の指が浮いて歩いてしまっているという時には、爪には圧力はかかっていないんですが、逆に指の圧力があんまりかかっていないので、こっちの方が今度はマイナスになってしまうことによって、爪の通常の圧力だけなのに爪がグーッとしているような状態と同じことが起こってしまって、爪が巻いてしまうということなんですね。
なので巻き爪が起こってしまう生活習慣、一つは間違った靴の選び方。二つ目は、しっかりと爪先で地面を押して歩くということをしないで、爪先がちょっと浮いてしまったような感覚で歩いてしまうという歩行の習慣になります。
なのでまずはこの靴を変えたりとか、5本の指に力が入るのに、それこそ5本指ソックスを履くとか、そういったことをしていただきたいんですが、もう一つ大切なのは爪の切り方になります。
皆さん爪はどうやって切ってますか。爪切りで端っこからちょこちょこって切っている方多いと思うんですが、それ間違いです。爪っていうのはまずは真ん中からパチンと切るようにしましょう。真ん中を切った後に角の方はあまり深く切らないということがポイントになります。なぜかというと、深く切ってしまうと圧がかかった時に底が弱くなってぐるっと巻きやすくなるんですね。
もちろん長いままだと底が当たって痛いということはあるので、まずは真ん中の部分を切って、角の部分はほんの少し丸くなるように自然なラウンドになるように切るようにしてください。
ついつい深爪をしてしまうという方もいますが、深爪もダメです。深爪をしてしまうと爪がしっかりと指を抑える力が少なくなってしまうので、綺麗なカットをしていても深爪をしてしまうとまた巻き爪の原因になるので、ついつい深爪をしてしまうという方もそれは絶対にやめるようにしてみてください。
そして爪を切るタイミングも実はあるんです。爪ってやっぱり硬いじゃないですか。真ん中からきちっと切ったとしても、爪が乾燥していたりすると爪が割れてしまう原因になるんですね。なのでできればお風呂上がりに爪が柔らかくなっている時を狙って爪を切るようにしてみてください。
そしてもう一つ、爪の中には角質、まあ要は垢が溜まっていることがあります。これをしっかり取り除いてから切らないと、これも爪に刺激になってしまうので、できればそういったものをしっかり取り除いてから爪を切るようにしてみてください。
どんなもので切ったらいいかというので、私が愛用しているのはこちらのセットになるんですが、例えばヤスリですね。切った後にヤスリをかけるということもすごく大事なことになります。まず爪をヤスリでケアするときって皆さんこうゴシゴシ左右にしてませんか。これ実はダメなんですね。ヤスリをかけるときっていうのは一定方向にこういうふうにかけるようにしてください。その方が爪に負担もなく、そしてきれいに仕上がることができます。
なので例えば爪を真ん中を切った後に、この角のところを深爪がするのが嫌だなと思うときには、ヤスリでこの部分を丸く削るようにしてみてください。爪の中に角質、垢が溜まってしまうような時には、こういうようなヘラじゃないですけれども、こういったものが実は売ってたりもするので、ちょっと爪のあんまり無理に入れてしまうと傷つけてしまうので、こうやって中の角質をしっかりと取って、そこから清潔にしてから爪を切るということをしてみてみるのもいいかなと思います。
あとは爪を切る頻度になります。特に足の爪っていうのはそんなに伸びないんですね。深爪をするとやっぱり巻き爪になってしまうことありますので、1ヶ月に1回程度で構わないです。なので気になってちょこちょこ切るっていうのも、深爪の原因、そして巻き爪の原因になってしまうので、頻度も気をつけるようにしてみてください。
一般的に足の爪が伸びる速度ってゆっくりなんですね。どれくらいかっていうと1日に0.05ミリ。なので1ヶ月で約1.5ミリしか伸びないんですよ。なのであんまり頻繁に切ってしまうと深爪の原因になってしまうので、実は1ヶ月に1回程度で大丈夫なんです。
もちろんこれには個人差があるので、私結構代謝が良くてぐんぐん伸びるんですっていう方もいらっしゃると思うんですが、ポイントは深爪をしない、切りすぎないということなので、1ヶ月に1度目安にしてそこで自分で調整してみるようにしてみてください。
まとめると、爪っていうのは指を保護してくれる大切な役割をしています。圧がかかりすぎてしまうと爪ってくるっと巻いてしまうという性質がありますので、合わない靴を履かない、そして歩き方もしっかりとつま先をつけて歩くということ。あとは爪の切り方ですけれども、お風呂上がりとかちょっと爪が柔らかくなっているときに垢を少し取り出して、真ん中から切って左右はできればヤスリを一定方向、ヤスリがない場合も角を深く切りすぎないように丸く整えるぐらいにしてください。そして足の指の爪を切る頻度というのは大体1ヶ月に1度目安にしてみてくださいね。

Hello everyone. I'm Arata Tomori, an internal medicine and dermatologist doctor. Today I'd like to talk about nails, which is a surprisingly common concern even in outpatient consultations.
Do you all know the correct way to cut your nails?
Actually, I saw this on a website by Kai Corporation, a company that sells nail clippers and other nail-related products. They had a survey asking "Do you have concerns about your toenails?" and apparently nearly 70% of women answered yes, which I found quite relatable.
Of course, many of these concerns involve various symptoms, like athlete's foot or nails peeling off, but overwhelmingly, ingrown toenails are the most common concern.
In fact, I see ingrown toenails very often in outpatient visits. The nail curls inward, and where it curls, it presses into the toe, causing inflammation. Sometimes it even becomes infected with pus, and I often see patients who look like they're in real pain.
Of course, not all dermatologists treat this, but there are specialized nail clinics that use wire treatments to widen the curled nail.
However, while this can be treated, if you continue with the wrong lifestyle habits or the wrong way of cutting your nails, the nail will keep curling and the ingrown nail problem will persist. So I wanted to share how to fix your lifestyle habits and the correct way to cut your nails, which is why I'm making this video.
First, do you know what nails are for? You might think they're just there, but nails serve a role in protecting the fingertip—whether on hands or feet. They also help support the pressure pushing against the pad of the finger. So actually, the nail and the finger are in a relationship of pushing against each other.
For example, with your index finger, there's force going from the nail outward, and force from the finger pushing back toward the nail—it's like a pushing match. Because this balance is just right, they support each other. That's why even when there's strong stimulation to the fingertip, the nail properly protects it. This is why fingertips are designed to allow for such delicate work. It's quite a clever design!
So why do nails curl in the first place? You don't really hear about ingrown nails on hands, right? It's mostly on the feet. This is largely due to changes in the balance of pushing force I just mentioned.
For example, if you wear shoes that are too small, or if you're a woman wearing pointed-toe shoes, constant pressure gets applied to the toe in the direction of the nail. When the nail is continuously pressed like this, the balance that would normally cancel out to zero between the nail's pushing force and the finger's pushing force gets thrown off—more pressure builds up on the nail side, causing lateral force that makes the nail keep pushing. This causes the toe to curl, and pressure builds progressively from the edges, causing the nail to curl inward.
So essentially, what lifestyle habits are bad? Constant pressure on the toenail isn't good. Earlier I mentioned that women have more toenail concerns—this is often due to wearing pumps or narrow shoes.
Another factor is how you walk. For example, this is common in men too—wearing loose shoes and not properly pressing your toes into the ground when walking, sort of walking with your toes lifted. In this case, there's no pressure on the nail, but conversely, there's not enough pressure from the toe either, causing a negative imbalance. This creates the same effect as if pressure were being pushed onto the nail even under normal pressure, causing the nail to curl.
So the lifestyle habits that cause ingrown nails are: first, choosing the wrong shoes. Second, not properly pressing your toes into the ground when walking, instead walking with a floating sensation in your toes.
So first, you should change your shoes, or wear five-toe socks to ensure force is distributed to all five toes. But another important factor is how you cut your nails.
How do you all cut your nails? Many people cut little by little from the edges with nail clippers, but that's wrong. You should first cut straight across the middle of the nail. After cutting the middle, don't cut the corners too deep—that's the key point. Why? Because if you cut too deep, the base becomes weak under pressure and it becomes easier for the nail to curl.
Of course, if the nail stays long, the edges might hit something and hurt, so first cut the middle portion, then shape the corners just slightly rounded, following a natural curve.
Some people tend to cut their nails too short, but that's also bad. If you cut too short, the nail loses the strength to properly hold the finger in place, so even with a clean cut, cutting too short can still cause ingrown nails. Please absolutely avoid cutting your nails too short.
There's also a good timing for cutting nails. Nails are hard, right? Even if you cut straight across the middle, if the nail is dry, it can crack. So ideally, cut your nails right after a bath when they're softened.
Also, there's often built-up debris—basically dirt—under the nail. If you don't remove this before cutting, it can also irritate the nail. So try to clean this out thoroughly before cutting.
As for what to use, I personally use this set, which includes a nail file. Filing after cutting is also very important. When filing your nails, do you file back and forth? That's actually wrong. You should file in one consistent direction like this. This puts less strain on the nail and gives a cleaner finish.
So for example, after cutting the middle of the nail, if you're worried about cutting the corners too short, use the file to gently round out that area instead.
When there's debris built up inside the nail, tools like this—not quite a spatula, but similar—are actually sold for this purpose. Just be careful not to force it in too much, as that can damage the nail. Clean out the debris this way, then cut the nail after ensuring it's clean.
Next is the frequency of cutting your nails. Toenails especially don't grow very fast. Cutting too often can lead to over-trimming and cause ingrown nails, so once a month is generally fine. Cutting frequently just because you're bothered by them can also cause over-trimming and ingrown nails, so please be mindful of frequency too.
Generally, toenails grow slowly. How slow? About 0.05mm per day. So in a month, they only grow about 1.5mm. If you cut too frequently, you risk cutting too short, so once a month is actually sufficient.
Of course, there are individual differences—some people might say their metabolism is great and their nails grow quickly. But the key points are not to cut too short and not to over-trim. Use once a month as a guideline and adjust based on your own needs.
To summarize: nails play an important role in protecting your fingers. If too much pressure is applied, nails have a tendency to curl inward. So avoid wearing ill-fitting shoes, and make sure to walk properly with your toes pressing down. As for cutting technique—after a bath when nails are softened, remove a bit of the debris, cut straight across the middle, and file the sides in one direction if possible. If you don't have a file, just round the corners without cutting too deep. And for toenails, aim to cut about once a month.
