皆さんこんにちは、友利新です。今日は2025年上半期、友利新がやってよかった美容医療紹介をしたいと思います。
ということで、のぼり君、今日はクリニックに来ていただいてありがとうございます。今まで美容医療をやってるんですけど、どういうものかっていうのが分からないっていうことがあったので、クリニックでこういう機会でちょっとお話しようかなと思ってやっております。
出会った年から変わらないんですよね。出会った頃と同じ気持ちでやってるってことですね。私のモットーは大きく変えないこと。例えば5年前のYouTubeを見ても今を見ても、カメラの性能が良くなったとか場所とか髪型とかは変わってるけど、あんまり時を感じないよねっていうのが私の美容医療との向き合い方なんですよ。
私はスキンケアと美容医療っていうのをどっちもやっています。ただここで言いたいのは、スキンケアが意味がないとか美容医療一択とかではなくて、美容医療っていうのはアップさせるものなんですよ。スキンケアっていうのはそれをキープして上向きにするもの。だからアップしてキープっていうものをしていくのが、私は美容医療とスキンケアの関係性だと思っているので、スキンケアだけっていうのもいいかもしれないですけれども、どううまく相乗効果でやっていくかっていうことが大事なので、今日は美容医療のお話をしたいと思います。
まず私自身は何が気になるかっていうと、色ムラとかシミがっていうのはそこまでではないですね。子供を産んですぐっていうのは肝斑があったりして、去年一昨年ぐらいまではやっぱり肝斑の治療でトーニングとかもしてたんですけれども、ニキビ跡のボコボコ感だったりとかたるみとの戦いかなとは思っています。なので悩みとしてはトーンアップさせたいっていうところと、肌質ですよね、ニキビ跡だったり皮脂が多い毛穴感みたいなもの、毛穴系、たるみっていうこの3つでやっていこうと思います。
まず色ムラっていうとニキビ跡だったり毛穴のボコボコ感っていうので悩んでいるので、XLVっていう機械をやっています。これは1064っていう波長と532っていう波長を組み合わせてやるレーザーなんですけれども、波長が長ければ長いほど皮膚の奥の方に行くのでコラーゲンとかエラスチンをふっくらさせながら、500台だと赤とか茶色に効いていくので、私みたいにシミの治療とかをしていると黒いものっていうのはなくなってきて、茶色とか赤が最後残るんですよ。そういう意味ではこのXLVっていうものを照射して、だいたい1ヶ月おきか2ヶ月に1回ぐらいはおもすきでXLVはあるので、必ず照射するようにしてます。そうすることによって毛穴も引き締まるしトーンアップが1個進むっていうのはあるのかなと思うので、後でこの機械ですっていうのは見ていただこうかなと思うんですけども、意外と毛穴が気になる男の子とか、あとはニキビが気になる人もXLV効果があるので患者さんにも好評ですし、韓国とかでもXLVを置いているクリニックさん多いんですけども、あんまり日本では置いてないのよ。だから韓国で受けてよかったよっていう人が来てくださったりとか、私がカウンセリングしてXLV進めてよかったですっていう患者さんもいるので、自分自身も体感ができてるしいいなって思ってます。
続いてはたるみ。つらいのよ。
僕も別に今はね27なんですけどたるむじゃないですか絶対。
こんなに20代からその話を聞いてると何やろうかなと考えてる。20代は別に今から何かやるっていうよりは、日焼け止めを塗ってビタミンD飲んで骨を丈夫にしながら光老化をさせないようにするっていうのがあれなので、20代でいろいろやる必要性は私はないと思う。30後半からはやったほうがよくて、私はもう47、来年48だからね。

48はもうやんなきゃじゃん。信じられなくなりますその年齢が。
そういう私が何をしてるかっていうと、去年も言ったかもしれないんですけどもRFとハイフっていうのの組み合わせですね。いっぱい機械あるんですよ。RFの王様はサーマクール、ハイフの王様はウルセラっていう機械があって、特許が外れていっていろんなものが発生してる。
そうなんですね。
だから新しい機械がいいんじゃなくって、特許をうまく侵害しないように新しい機械がどんどんできているっていう感じ。
そういうことなんですね。出てから結構経ってるからダメとかじゃないんですか?
じゃないです。今でも外科の先生だとRFの王様はサーマクールだけど痛いよね。だけどやっぱり効くよね。だから痛くないようにいろいろしていって、いろんなRFの機械が出てくる。ハイフもウルセラはしっかり効果があるよね。だけど痛いし高い。だけれども特許が切れたものを組み合わせていろんなものが出てきてるっていうものなので、RFの系かハイフ系かっていうのを自分がどっちやってるか知らない人もいんのよ。自分でやってるのに。
そうでRFですねって言ったら、ごめんなさい分かんないですとか、これハイフ系ですねって言ったら分かんないとか。逆にハイフ系を何度も重ねてる人もいるわけ。名前が違うから。
そうそう名前が違うから。最低限自分がRF系を受けてるのかハイフ系を受けてるのかは知った方がいいし、サーマクールは半年に一遍なの。強いから痛いのよ。だけどすごい効く。ただこまめにやりたいんだったらダイヤモンドチップっていうものがあるから、ダイヤモンドチップは1ヶ月とか3ヶ月以内で全然大丈夫なので、こまめにやりたいなっていう時にはダイヤモンドチップを打ってて、時間がない時はサーマクールなんだけど、最近は城野のサーマクールが人気すぎてやれる時間がなくて。ここだとお休みの時に来てやってもらったりできるので、サーマクールは実はできてないの。だからこそダイヤモンドチップを1ヶ月か2ヶ月に一遍くらいやってる。これが本当にリアル。でももし時間があってがっつりやりたいなと思ったらサーマクールやりたいなとは思っている。
サーマクールやったらどれくらいで分かるんですか?
1ヶ月後くらいから引き締まってきて上がった感じがする。
なるほど。
自分がどっちをやりたいかっていう感じで決めていただきたい。私はダイヤモンドチップをこまめにやってるっていう感じ。

ハイフ系はずっとやってなかったんだけど、今日おもすきにいるんですけど、城野でソフトウェーブっていう機械が出たの。新しいハイフって言われてて、ソフトウェーブは表面にも効いていくから、たるみ治療機として承認も下りたんだけど、ニキビ跡とか美肌治療っていうのでも効果があるんじゃないかって言われて、ニキビ治療にはもう承認が下りてる。
ダブルでいいですね。
そうなのだから私みたいにたるみも気になってニキビ跡も気になってる人でも、めちゃくちゃいい機械なわけ。城野に入ったんでやったら肌質変わったなっていうのが感じるので。
先生が実感するんですね。
新しい機械って言ったらおもすきのXLV、城野のソフトウェーブ、この組み合わせは最強だなって。
そのうち絶対やります。
まだ早いわよ。最近の学会の発表で、それをやっていくには肌の水分量を上げていかないと、熱を加えるから水分に反応するのよ。水分量が年々低下していったら効きも悪くなるから、肌育注射をして肌の水分量を上げてから機械系を重ねていくっていうのが一番効果がいいっていうのが学会発表があったわ。それもあってから私はやっぱりデンシファイっていうものを入れて、2,3週間してダイヤモンドチップをやってからのソフトウェーブっていう、この流れが肌質と引き締め感っていうのはいいなっていう感じになっております。
なるほど。多分これから下半期も同じ感じでいくと思うんですよね。
ちなみに今回は首もデンシファイ入れて、首のこのシワシワ感っていうのはかなり良くなったかなと思うんで、今まで顔ばっかりやってたんですけども、首にソフトウェーブとデンシファイをやるようになってからは、かなり良くなったんで。顔だけやってると首との差が出てくるので、2025年の上半期は首も集中的にやって差をつけないようにしたかなっていうのと。
定期的にやってるのはボトックスね、おでこ。
動いてるけど動かないように。
あれ?確かに。
眉間にシワが。
あと眉間も。シワを一生懸命やろうとするんだけど、あんまり切れはいけない。
2週間前と全然違いますね。
ボトックスやると顔が怖くなるって言うんだけど、これぐらいの表情は作りながらもできるんで。
全然めちゃくちゃいいですね。
そう多分一生やっていくと思う。半年に1回は必ずやるので、もうおでこ、眉間、目尻は今のところやって。口輪筋って言って、エラとここはたまに、絶対じゃないんだけど歯ぎしりする時があるから、そういう時にはやるし、気になったらやりますけど、もう絶対やるのは額、眉間、目尻っていう感じですかね。
例外っていうかあれなんですけど、脇のボトックス。汗が止まるのよ。
止まるんですか?
そう、夏になったらやる。Tシャツ着てさ、テンション上がったりすると脇が汗ばんでくる。衣装とか着させてもらった時に濡れちゃうのが嫌なので、夏になったら脇のボトックスはやってるし、もうちょっとしたら皮脂が出てくるのでマイクロボトックスをやろうかなって。
でもこれって多分7月8月だから下半期になっちゃうんだけど。
実は上半期の方がいい?まだやってないんですけど、上半期としてやりたいなと思ってるっていう感じですかね。
汗系で打つとその分他から出るって聞いたことあるんですけど。
出てるっていうのはあるんだけど、全体にまばらになるじゃん。ここ抑えたから顔だけバッと出るとかじゃなくて、全体的にまんべんなくっていう感じ。
この競争って。
ケツだけ出るとか顔だけ出るとかじゃなくてっていう感じかなと思ったんで。

Hello everyone, this is Arata Tomori. Today I'd like to introduce the aesthetic medical treatments I've been glad I did in the first half of 2025.
So, Nobori-kun, thank you for coming to the clinic today. I've been doing aesthetic medical treatments all along, but there was a sense that people don't really understand what they involve, so I thought I'd take this opportunity here at the clinic to talk about it a bit.
It hasn't changed since the year we met, has it. So you're saying you're doing this with the same feelings as when we first met. My motto is not to change drastically. For example, if you look at my YouTube videos from 5 years ago compared to now, the camera quality has improved, and the locations and hairstyles have changed, but you don't really feel the passage of time — that's my approach to aesthetic medicine.
I do both skincare and aesthetic medical treatments. What I want to say here isn't that skincare is meaningless or that aesthetic medicine is the only option — aesthetic medicine is something that raises your baseline up. Skincare is what maintains that and keeps it trending upward. So doing both — raising up and maintaining — is, I believe, the relationship between aesthetic medicine and skincare. Doing skincare alone might be fine too, but how well you create a synergy between the two is important, so today I want to talk about aesthetic medical treatments.
First, what concerns me personally — uneven skin tone and spots aren't that big of an issue anymore. Right after having my child I had melasma, so up until about last year or the year before I was doing toning treatments for melasma, but now I think it's more of a battle against acne scar bumpiness and sagging. So my concerns are: wanting to brighten my skin tone, skin texture issues like acne scars and pore-related oiliness/pore concerns, and sagging — these three things is what I'm focusing on.
For uneven tone, I'm troubled by acne scars and pore bumpiness, so I do a machine treatment called XLV. This is a laser that combines a 1064 wavelength and a 532 wavelength. The longer the wavelength, the deeper it penetrates into the skin, plumping up collagen and elastin, while the 500-range wavelength works on red and brown tones. For someone like me who's been treating spots, the black pigmentation goes away first, and brown and red tend to remain at the end. In that sense, I irradiate with this XLV, roughly once every month or every two months — since Omosuki has XLV, I make sure to get it done. Doing this tightens the pores and also advances the tone-up by one step, I think. I'll show you the machine later, but interestingly, men who are concerned about pores, and people concerned about acne, also see effects from XLV, so it's popular among patients. Many clinics in Korea have XLV too, but it's not really available much in Japan. So people who received it in Korea and were happy come to see me, and there are also patients who I recommended XLV to during consultations who were glad they got it — so I feel the effects myself and think it's great.
Next is sagging. It's tough.
Well, I'm 27 now, but I'm definitely going to sag eventually, right?
Hearing about this stuff from your 20s already, makes me wonder what to do. In your 20s, rather than starting treatments now, I don't think there's a need to do a lot — it's more about applying sunscreen, taking vitamin D to strengthen your bones, and preventing photoaging. From the late 30s onward, it's better to start treatments. I'm already 47, turning 48 next year.
You don't look it.
At 48 you really have to start doing something. It's hard to believe that age.
So what am I doing — I may have mentioned this last year too — it's a combination of RF and HIFU. There are so many machines. The king of RF is Thermage, and the king of HIFU is Ulthera. As patents expire, various other devices keep appearing.
I see.
So it's not that newer machines are better — it's more that new machines keep being developed in ways that avoid infringing on patents.
I see, so that's how it is. Since it's been a while since these came out, doesn't that mean they're outdated?
No, not at all. Even now, surgeons consider Thermage the king of RF, but it hurts, right? But it really works. So in order to make it less painful, various RF machines have been developed. HIFU — Ulthera really has strong effects too, but it's painful and expensive. But by combining elements whose patents have expired, various other devices have come out. Some people don't even know whether they're getting RF or HIFU treatments, even though they're the ones receiving them.
So when I say "oh that's RF," they say "sorry, I don't know," or when I say "this is HIFU," they say they don't know. Conversely, some people repeat HIFU treatments over and over under different names.
Right, because the names are different.
Exactly, the names are different. At minimum, you should know whether you're getting RF or HIFU treatments. Thermage is done once every six months — it's strong, so it hurts, but it's very effective. But if you want to do treatments more frequently, there's something called Diamond Tip. Diamond Tip is fine to do within 1 to 3 months, so when I want to do something more frequently, I use Diamond Tip, and when I don't have time, I do Thermage — but lately Thermage at Shirono has become so popular that I can't find the time to get it done. Here, I can come in on my days off and get it done, so I actually haven't been able to do Thermage. That's why I do Diamond Tip about once every 1-2 months instead. This is the real situation. But if I had the time and wanted to go all out, I do want to do Thermage.
How soon can you tell after doing Thermage?
About a month later, you start feeling tightened and lifted.
I see.
I want you to decide based on which one you want to do. I do Diamond Tip frequently, that's my approach.

It feels warm.
What kind of pain is it?
It feels like being burned.

I hadn't been doing HIFU treatments for a while, but today, here at Omosuki — Shirono released a new machine called Soft Wave. It's being called the new HIFU. Soft Wave also works on the surface layer, so it's been approved as a sagging treatment device, but it's also said to be effective for acne scars and skin treatment in general — it's already been approved for acne treatment.
Great, it's dual-purpose.
Exactly, so for someone like me who's concerned about both sagging and acne scars, it's an amazing machine. Since Shirono introduced it, I feel like my skin texture has changed after using it.
So you feel it yourself, doctor.
If I had to name the newest machines, it would be Omosuki's XLV and Shirono's Soft Wave — this combination is the strongest.
I'll definitely try this eventually.
It's still too early for you! According to a recent conference presentation, in order to do this treatment effectively, you need to raise the skin's water content first, because heat reacts with water. If water content decreases year by year, the effectiveness decreases too. So doing a skin-boosting injection first to raise water content, then layering on the machine treatments, was presented as the most effective approach at a conference. Because of that, I now get something called Densify injected, then after 2-3 weeks I do Diamond Tip, followed by Soft Wave — this flow gives the best results for skin texture and tightening, I feel.
I see. I think you'll probably continue with the same approach in the second half of the year too.
By the way, this time I also got Densify injected into my neck, and the wrinkled texture on my neck has improved quite a bit. Until now I'd only been treating my face, but since I started doing Soft Wave and Densify on my neck too, it's improved a lot. If you only treat the face, a gap develops with the neck, so in the first half of 2025 I focused on the neck intensively too, to avoid that gap.
What I do regularly is Botox — on the forehead.
So it moves but doesn't move too much.
Oh, that's true.
There are wrinkles between the eyebrows.
Also between the eyebrows. I try hard to make wrinkles but it doesn't quite work.
It's completely different from two weeks ago.
They say Botox makes your face look scary, but I can still make expressions like this.
That's really great.
Yeah, I think I'll probably keep doing this for life. I do it without fail once every six months — forehead, glabella, and crow's feet are the areas I'm currently doing. There's also the masseter — the jaw area and here — I do it sometimes, not always, but when I grind my teeth, in those cases I get it done, or when it bothers me. But the ones I absolutely always do are forehead, glabella, and crow's feet.
As an exception, there's also underarm Botox. It stops sweating.
It actually stops it?
Yes, I do it when summer comes. Wearing a T-shirt, when you get excited, your underarms start to sweat. When wearing costumes for shoots, I don't want them getting wet, so I do underarm Botox once summer arrives. And a bit later, since oil production increases, I'm thinking of doing micro-Botox too.
But that's probably July or August, so that would be the second half of the year.
Actually, is the first half better? I haven't done it yet, but I'm thinking I want to do it in the first half.
I've heard that if you inject in the sweat glands, the sweat comes out more from elsewhere instead.
There is some of that, but it becomes spread out more evenly overall. It's not like suppressing it here makes it suddenly appear only on the face — it becomes evenly distributed overall.
That competition, huh.
It's not like it comes out only on the buttocks or only on the face — it's more evenly distributed, I think.
