皆さんこんにちは。内科・皮膚科医の友利新です。前回は毛穴に対する5つの誤解をお話ししましたよね。例えば、冷たいお水でパシャッと顔を洗っても毛穴は小さくならないとか、毛穴の数は変わらないとか、皆さんが「え?」と思うことをお話ししたかもしれませんが、今日は皆さんが気になる毛穴ケアについてしっかりとご説明していこうと思いますので、最後までご覧ください。 まず、毛穴には2種類あると思ってください。皮脂が詰まることによって起こる「詰まり毛穴」、そして皮膚がたるむことによって起こる「たるみ毛穴」、この2つになります。年齢でいうと、10代から20代くらいの方は詰まり毛穴が多く、30代後半以降の方はたるみ毛穴になってきます。それぞれについて説明していきます。 詰まり毛穴の原因は、活発な皮脂分泌、つまり油が多いことです。この油が多い原因は、男性ホルモンによるものが大きく占めています。「男性ホルモン?」と思うかもしれませんが、女性にも男性ホルモンはあって、その分泌が多くなると皮脂腺を刺激して皮脂がどんどん出るようになります。男性はもともと男性ホルモンが多いので皮脂が多い傾向にありますが、女性も男性ホルモンが増えると皮脂分泌が過剰になり、毛穴の中に詰まってしまうことになります。 皮脂分泌が過剰になりやすいのはTゾーンと呼ばれるおでこや鼻まわりで、Uゾーンは比較的油が少ないと言われています。10代から20代前半はこのTゾーンとUゾーンの皮脂分泌の差がとても大きく、若いほどその差が開く傾向があります。30代後半になるとその差がぐっと縮まり、40代以降はほとんど変わらないというデータがあります。つまり、年齢を重ねるほど皮脂分泌は低下しますが、若いほど過剰になりやすく、その主な原因は男性ホルモンだと言われています。男性ホルモン以外にも、食生活の乱れやストレスも皮脂分泌を過剰にする原因になります。 さらに、皮脂分泌が多いだけでなく、角質が厚くなることも詰まり毛穴の原因です。皮脂は角質にバリアを張る役割がありますが、角質が厚くなりすぎると毛穴を塞いでしまい、毛穴の中で皮脂が詰まってしまいます。角質が厚くなることと皮脂分泌が過剰になること、この2つが詰まり毛穴の大きな原因です。 では、詰まり毛穴のケア方法について3つのポイントをお話しします。 1つ目は「角質ケア」です。クリニックではピーリングをして角質を薄くしていきますが、ご自宅での使用はやりすぎに注意が必要です。ピーリング剤は医師が肌の状態を見て選ぶデリケートなものです。自宅での日常ケアとしては、酵素洗顔がおすすめです。私が最近使っているのはOMAJI(オマジ)の酵素洗顔で、固形タイプのものが多いです。角質を柔らかく分解する成分が入っており、しっかり泡立てて顔にのせて使います。ただし、ゴシゴシこするのはNGです。泡をのせるだけで十分です。 ポイントは、毛穴が気になる場所に重点的に使うことです。混合肌の方もいらっしゃるので、私はよく「自分の顔の地図を描きましょう」とお伝えしています。鏡を見て、どこがテカテカしているか、どこの毛穴が開いているかを確認して、自分の肌の分布図を把握しておくのです。顔全体に同じケアをするのではなく、例えばTゾーンだけ酵素洗顔を使うといった部分ケアがとても大切です。 2つ目は「皮脂分泌ケア」です。自宅で皮脂分泌を抑えるには、ビタミンCが効果的です。ビタミンCはシミケアのイメージが強いかもしれませんが、皮脂を抑える「脱脂作用」と「抗炎症作用」もあります。皮脂が詰まると「油焼け」と呼ばれる炎症が起こることがあるので、ビタミンCで皮脂を抑えつつ炎症も鎮めることが重要です。 私がおすすめするビタミンC製品は、ピュアビタミンC(アスコルビン酸)配合のOBAGI(オバジ)や、スパトリートメントのリアルCセラム、またはドラッグストアで手軽に購入できるシーラボのVエッセンスローション(APPS=ビタミンC誘導体配合)などです。これらを積極的に取り入れることで、詰まり毛穴の改善や黒ずみの予防にもつながります。 また、ビタミンCは朝に使うとシミの原因になるという誤解がありますが、それは間違いです。朝に使うことで紫外線による酸化ダメージを抑え、日中の皮脂分泌も抑えてくれます。夜に使えば肌の修復力も高めてくれるので、朝も夜もスキンケアに組み込んでください。 3つ目は「UV対策」です。詰まった皮脂が紫外線に当たると酸化(いわゆる「錆び」)が起こり、毛穴が黒ずんでしまいます。毛穴が気になる方は、しっかりとUV対策も行いましょう。 次は「たるみ毛穴」についてです。年齢を重ねると皮膚がたるんでくることで、毛穴がすり鉢状になり、涙形のように縦に伸びた形になります。詰まり毛穴が鼻まわりに多いのに対し、たるみ毛穴は頬など皮膚のたるみが出やすい部分に多く見られます。 実は、詰まり毛穴による炎症がたるみ毛穴の原因にもなることがあります。炎症が皮膚の奥のコラーゲンやエラスチンを破壊してしまうため、たるみにつながるのです。つまり、詰まり毛穴をケアすることは、将来のたるみ予防にもなります。 たるみ毛穴のもう一つの大きな原因は紫外線です。紫外線によってコラーゲンやエラスチンが破壊され、皮膚がたるみます。したがって、たるみ毛穴の予防にも日焼け止めが最も重要です。 すでにできてしまったたるみ毛穴のケアに最も効果的な成分は「ビタミンA」です。レチノールやビタミンA誘導体がこれにあたります。ドラッグストアで手軽に使えるものとしては、ラ ロッシュ ポゼのレダミックR SSがあります。敏感肌の方でも使いやすいレチノール製品です。また、マイルドに作用するビタミンA誘導体「パルミチン酸レチノール」が入った製品としては、私が長年愛用しているエンビロンがあります。プチプラでは、アクアレーベルの化粧水にもパルミチン酸レチノールが含まれており、たるみ毛穴にも効果が期待できます。 まとめると、毛穴ケアは種類や年齢、できる場所によってアプローチが異なります。詰まり毛穴にはビタミンC、たるみ毛穴にはビタミンA、そして共通して大切なのは日焼け止めをしっかり塗ることです。 自分がどのタイプの毛穴なのかを把握し、顔の地図を描いて部分ケアをすることがとても重要です。例えば、鼻は詰まり毛穴ケア、頬はたるみ毛穴ケアというように、部位ごとに使うものを変えることも効果的です。 いかがでしたか?このチャンネルでは、美容や健康について医学の立場からわかりやすくお伝えしています。よかったらチャンネル登録をお願いします。コメントもいただけるととても嬉しいです。では、次の動画でお会いしましょう。さようなら。

Hello everyone. I'm Arata Tomori, an internal medicine and dermatologist doctor. In the last video, I talked about five common misconceptions about pores — for example, that splashing cold water on your face doesn't shrink pores, or that the number of pores you have never changes. Today, I'd like to thoroughly explain pore care, which I know many of you are curious about, so please watch until the end. First, I want you to think of pores as falling into two categories: "clogged pores," which are caused by sebum (oil) buildup, and "sagging pores," which are caused by skin laxity. In terms of age, people in their teens to twenties tend to have more clogged pores, while those in their late thirties and beyond tend to develop sagging pores. I'll explain each type in detail. The cause of clogged pores is essentially overactive sebum production — in other words, excess oil. This excess oil is largely driven by androgens (male hormones). You might be surprised to hear "male hormones," but women also produce androgens. When androgen levels rise, they stimulate the sebaceous glands, causing more and more oil to be produced. Men naturally have higher androgen levels and thus tend to have oilier skin, but when women experience a rise in androgens, sebum production becomes excessive and clogs the pores. One interesting fact: sebum production tends to be highest in the T-zone (forehead and nose area), while the U-zone (cheeks and jawline) produces comparatively less oil. In the teens to early twenties, the difference in sebum production between the T-zone and U-zone is quite large — the younger you are, the greater that difference tends to be. However, by the late thirties, the gap narrows considerably, and data shows that by the forties, there is almost no difference. In other words, sebum production decreases with age, but the younger you are, the more prone to excess sebum you tend to be — and androgens are the primary cause. Beyond androgens, a poor diet and stress can also contribute to excess sebum production. In addition to excess sebum, thickened dead skin cells (keratin buildup) also play a role. Sebum normally forms a protective barrier over the skin's surface, but when the dead skin layer becomes too thick, it can block the pore opening, trapping sebum inside. So the two main causes of clogged pores are: thickened dead skin cells and excess sebum production. Now let's talk about the three key points for treating clogged pores. The first is exfoliation (keratin care). At clinics, we perform chemical peels to thin out the dead skin layer, but doing this at home requires caution — peel solutions are quite delicate, and we as physicians select them carefully based on each patient's skin condition. For daily home care, I recommend enzyme cleansers. The one I've been using recently is from OMAJI; enzyme cleansers often come in a solid powder form. They contain ingredients that gently break down dead skin cells. Be sure to lather them up well before applying to your face — but never scrub. Simply placing the foam on your skin is enough. The key is to focus on the areas where pore congestion is a concern. Since some people have combination skin, I often advise people to "draw a map of your face" — look in the mirror and identify where your skin gets shiny and where your pores are enlarged, then make a kind of distribution chart. Rather than applying the same product all over your face, use this map to guide targeted care. For example, you might use the enzyme cleanser only on your T-zone. Personalized, zone-specific skincare is especially important for pore care. The second point is sebum control. To reduce sebum production at home, the key ingredient is Vitamin C. While Vitamin C is often associated with brightening and dark spot care, it also has a sebum-suppressing (de-greasing) effect and an anti-inflammatory effect. When excess sebum gets trapped in pores, it can cause a condition sometimes called "oil oxidation inflammation," so using Vitamin C to both reduce oil and calm inflammation is very effective. Products I recommend include pure Vitamin C (ascorbic acid) serums such as those from OBAGI or Spa Treatment's Real C Serum, as well as the more affordable Cé-LA-VI V Essence Lotion from DR.CI:LABO, which contains APPS (a Vitamin C derivative). Incorporating these into your routine can help improve clogged pores and prevent blackheads. There is also a common myth that using Vitamin C in the morning can cause hyperpigmentation — but this is incorrect. Using it in the morning actually helps protect the skin from oxidative damage caused by UV exposure throughout the day, and also helps suppress daytime sebum production. Using it at night supports skin repair. I recommend incorporating Vitamin C into both your morning and evening routines. The third point is UV protection. When sebum trapped in pores is exposed to UV rays, it oxidizes — essentially "rusting" — and causes pores to appear dark and blackened. If you're concerned about pores, be sure not to neglect sun protection. Now let's move on to sagging pores. As the name suggests, these form as the skin loses firmness with age, causing pores to become funnel-shaped and appear elongated or teardrop-shaped. Unlike clogged pores, which tend to appear around the nose, sagging pores are more commonly found on the cheeks, where skin laxity is more visible. Interestingly, the inflammation associated with clogged pores can also contribute to sagging pores. This is because inflammation damages collagen and elastin deep in the skin, leading to skin laxity. This means that caring for clogged pores is also a form of preventive care against future sagging. The other major cause of sagging pores is UV exposure, which breaks down collagen and elastin and causes the skin to sag. Therefore, the most important step in preventing sagging pores is consistent and thorough sunscreen use. For treating existing sagging pores, the most effective ingredient is Vitamin A — specifically retinol and its derivatives. A gentle and accessible option available at drugstores is La Roche-Posay's Redermic R SS, which is suitable even for sensitive skin. Another option is a Vitamin A derivative called retinyl palmitate, which works more gradually and mildly. A well-known product containing this is Environ, which I have personally used for many years. Even the affordable Aqua Label lotion from Shiseido contains retinyl palmitate and may be beneficial for sagging pores. In summary, pore care varies depending on the type of pore, your age, and where on the face they appear. For clogged pores, focus on Vitamin C; for sagging pores, focus on Vitamin A — and for both types, consistent sunscreen use is essential. Understanding your pore type and drawing a map of your face to guide zone-specific care is extremely important. For example, you might use clogged-pore care on your nose and sagging-pore care on your cheeks — targeting each area with what it actually needs. I hope you found this helpful!
