【46歳の美容事情】皮膚科医が綺麗になるためにやっていることを紹介します

みなさんこんにちは、友利新です。今日はですね、私46歳皮膚科医がキレイでいるために必ずやっていること7選をご紹介したいと思います。そう、私もですね、もう46になりまして、アラフィフ肌でございます。このYouTube始めてもう4年になるんですよね。この4年間ずっと自分の顔がYouTubeにアップされていて、4年歳を取ったわけなんですけれども、もちろんちょっとお姉さんの階段を上がったなというところもあれば、やっぱりこうやってずっと動画が残るので、あんまり変わらないようにしなきゃっていうふうな思いもあって。私のコンセプトとしては、自分の年齢よりも3、4歳若く見えて、ずっと見ていてもあんまり何か変わらないよねっていうふうに言ってもらえるようなスキンケアだったり、ライフスタイルだったり、あとクリニックの施術だったりをしているつもりなので、今日はそういったことをしっかりとお話ししていきたいと思いますので、ぜひ最後までご覧ください。
ちなみに、こちらが私がYouTubeを始めた頃の一番最初の動画です。2020年の3月の私というか、これね、2019年の12月とか11月ぐらいに撮ったので、厳密には4年とちょっと前なんですけれども、こちらとこちらになります。4年時が経っておりますが、こんな感じになっております。もちろんね、カメラだったりとか、何か照明とか機材が変わっているっていうところもあるんですけど、あんまり変わらないような感じなんじゃないかなと思っております。
ということで、私が変わらないと言われるために続けていること。まず一つ目です。ターンオーバーを正常にする。何それ先生、当たり前すぎないって思った方いらっしゃると思うんですが、よく化粧品の話をしている時に、皮膚のターンオーバーを正常にして肌をね、伸ばしましょいうふうに言うと思うんですよね。なので、ターンオーバーって皆さんも聞き飽きた言葉だと思うんですが、めちゃくちゃ肌には大事になってきます。というのも、ターンオーバーって何かっていうと、肌の一番下の基底膜っていうところから細胞が生まれて、どんどんどんどん成長して、今度は角層となって垢となってポロッと取れるっていうのが、皮膚のターンオーバーなんですね。まあ皮膚の一生なんですよ。
この皮膚のターンオーバーっていうのは、皮膚の細胞が成長していくだけじゃなくて、この皮膚の細胞の成長っていうのは、実は私たちの肌の細胞間脂質だったりとかセラミドだったりとかヒアルロン酸だったりとか、肌の潤い成分だったり肌の艶の成分だったりを作りながら成長していくんですね。ターンオーバーが遅くなると、要はこういう肌の艶感を出すような成分を自分で作れなくなってしまうということもありますし、あとは、ターンオーバーが遅くなると角層がですね厚くなるので、くすみ感が出てしまったりとかということにもなります。
角層が厚くなってくすむだけじゃなくて、どんどん成長していかないので、例えばメラニンを受け渡されてしまった肌の細胞も外に出ていかないので、全体的にもくすんでしまうよということになるんですね。で、悲しいことに歳をとるとですね、このターンオーバーというのは遅くなっていきます。なので、本当に1年1年ターンオーバーっていうのが遅くなっていくっていうことは、それが老化につながって、なんとなく肌の艶感がなくなったりとかくすみ感が出てくるのは、このターンオーバーがうまくいかないというところもあるんですね。
じゃあ、ターンオーバー早くすればいいじゃんということで、例えばピーリングとかをして、角層を薄くして、それでターンオーバーを流すっていうこともポイントなんですけれども、逆に今度は早すぎちゃうと、さっき言った肌の潤いだったりとか艶感に大事な成分を作るのが追いつかない、作っている暇がなく、どんどん成長させられちゃうので、今度は皮むけがしたりとか、ちょっとした刺激で赤みが出たりとかっていう風にするので、ターンオーバーっていうのは遅くてもダメだし早くてもダメなんですね。なので、ターンオーバーを正常にするっていうようなスキンケアをすることがすごく大事になってきます。
じゃあ先生、それ具体的に何なのよって思うと思うんですが、それがですね、よく基底膜ケアをしましょうっていう風な話があると思うんですけれども、例えば成分でいうと、ライスパワーナンバー11だったりとか、資生堂のD-アミノ酸だったり、リノール酸のカプチュールだったりとか、そういった基底膜のケアアプローチをしていくものっていうのは、基本的にはターンオーバーを正常化していくというようなこともあるので、そういったものをしっかりと使っていくようにしています。なので、やっぱりターンオーバーを正常にするっていうことが、肌がなんか元気でいられるっていうことなんですね。自分の肌がしっかりと保湿成分も作れるっていうことなので、それはもう本当にスキンケアをする上では、必ずそこを大事にしています。
続いて2つ目。これは一つ目のことともつながるんですが、そういった肌のターンオーバーを正常化したりとか、色々なスキンケアを入れ込むためにも、そして肌の炎症を抑えるためにも使っているスキンケアのアイテムの一つが、炭酸ケアをすることです。炭酸といっても高濃度の炭酸なんですけども、高濃度の炭酸っていうのは、肌の炎症を抑えたりとか、コラーゲンとかエラスチンの生成を促したりとか、あと私みたいに皮脂が多い人はニキビを作らせなくしたりとか、あとは角層を少し剥離するような作用があるので、肌のターンオーバーも正常化させるし、肌の真皮のコラーゲンケアとかもするし、やっぱり炎症ケアもできるんですね。
通常、炭酸というといろんなものは浮かぶと思うんですが、炭酸を入れ込むときのポイントはやっぱり、炭酸がしっかりイオン化されているかどうかということがポイントになってきます。イオン化されるような製剤が作れているのは、やっぱりカオグループの製品が一番信頼がおけるかなと思うので、エスト、だったりとか、ソフィーナIPだったりとか、アルブランだったりとか、そういったものの炭酸コスメだったらいいかなと。あと、めぐりっていう炭酸のミストを作るようなものだったりするのも、イオン化した製剤が肌に行くので、そういったところを使う。それによってさっき言ったようなこともできますし、あと炭酸を入れることで、めぐりが良くなるんですよね。なので、顔色が良くなったりとか、むくみケアだったりもするので、何か一つは炭酸コスメを入れていただきたいなと思いますし。
例えば、角質ケアだったりとか、ニキビケアだったりは、炭酸の洗顔とかでもできるので、そういったものもいいかなと思うんですけれども、炎症ケアだったりとか、コラーゲンケアだったりとかっていうのは、高濃度の炭酸コスメじゃないと難しいので、その辺のちょっと住み分けがあるんですけれども、自分の肌のためには毎日高濃度の炭酸ケアを取り入れております。
続いて3つ目は、ビタミンCとレチノールを毎日使うです。これはもう本当に4年前からずっと、私は言っていて、ビタミンCっていうのはもう、しっかり使いましょう、朝晩使ってください。レチノールっていうのも、肌のターンオーバーを流して正常化したりとか、あとはエイジングケアには、すごいいいですよというのも、再三言ってきて、今では何か当たり前になって、よく朝はビタミンC、夜はレチノールみたいな感じになってきたと思うんですけれども、私はどちらかというと、朝はビタミンCもレチノールも使ってほしい派なんですけれども。
特にビタミンCは、抗酸化作用もすごく高いですし、あとは美白で、皮脂が多い人にとっては、皮脂を抑制したりとかも、本当にマルチに使えます。ただ、間違っていっちゃいけないのは、濃度が濃いからいいというわけではないということです。もちろん、濃度が濃いのが肌に合っている方もいれば、やっぱり低濃度のものが肌に合っているという方もいらっしゃるので、ポイントは毎日使うということなんですよね。なので、毎日使うということは、高濃度がちょっと刺激になってたら、使いづらいので、低濃度でも毎日毎日こまめに使うということがポイントになってきますし。
そうするとあとは、長く使うから、すごい高いものっていうのも、なかなか難しいかなと思うので、使い続けることが大事ということがまず念頭において、高濃度じゃなくてもいいので、毎日使えるような製剤を選んでください。ただ、ビタミンCというのは本当に抗酸化作用が強いので、酸化しちゃったりするんですよね。だから、ビタミンC単体でいるっていうよりは、例えば、私がいつも使っている、このドクターKのK-シールドエッセンスなんですが。
ビタミンCって、自分が身代わりになって、酸化しているものを還元といって、元に戻してあげるんですね。なので、自分自身が酸化しちゃうんですよ。なので、じゃあこの酸化しちゃったビタミンCを、今度は還元して正常化するために、グルタチオンだったりとか、いろんなものを入れているという、ビタミンサイクルをしているので、そういったものを使う。ビタミンC単体でもいいんだけれども、ビタミンCのこの抗酸化作用が、ずっとサイクルで生むような製品を使っていただくと、濃度が高くなくても、ずっとビタミンCの効果が得られるかなと思います。
そして、レチノール。これもですね、濃度じゃないんですよ。高濃度のレチノール配合みたいになっているところも、すごく私は悲しくて。レチノールっていうのは、人に合う合わないっていうのは本当、個人差があります。高いパーセンテージのものを使っても、赤みとか皮むけとかは出ない人もいれば、本当に低濃度でも赤みが出る人がいるので、これもビタミンCと同じで、低濃度のものでもいいので、長く使うということがポイントになってきます。
私もほとんど赤みとかが出ないようなものを、ゆるーくゆるーく使っています。レチノールもレチノールの誘導体だったりとか、ピュアレチノールとか、っていろいろあるので、どのものでもいいと思うんですよね。それを長く続けるっていうところがポイントなので、これって濃度が少ないから意味がないかも、ではなくって、ずっと使い続けるっていうことがポイントになっているので、そこは間違えないようにしてください。
続いて4つ目は、定期的にレーザーを照射することです。これはね、もう本当に、私は職業柄かもしれないんですけれども、私が一番最初にレーザーを当てたのはですね、多分顔の脱毛のレーザーだと思うんですが、20代後半ですね。そこからレーザーって、どんどん進化していって、光治療が出てきて、その後はピコレーザーが出て、とか、あとはニードルRFが出て、本当に革新的なものがたくさん出ているんですね。
よく、何とかっていうレーザーはどうですか、とか、何とかっていうレーザー新しくなったんですけど、どうですか、みたいに、いろんな新しいレーザーを聞かれるんですけれども、正直言うと、私はどのレーザーもいいと思いますって言ったら、なんかざっくりしてるんですが、基本的には、熱を加えてメラニンを破壊するとか、あとは赤みを撃退するとか、あとは熱を加えることによって、コラーゲンとかエラスチンを増やしていくっていうものなので、熱を与えるっていうことになるんですよね。
タルミケアのレーザーがいいのはもちろんなんですが、そうじゃないレーザーでも、やっぱり年単位で当てていくことによって、肌っていうのは、やっぱり熱を定期的に加えることによって、タルミケアにもなりますので、私自身は、どんなに長くても3ヶ月以上は開けないですね。なので、1ヶ月から3ヶ月以内に何かしらのレーザーを当てていくっていうことをずっとしているので、それもこの4年間も、もちろんずっとやってきたことなので、変わらないね、って言ってもらえるような一つになっているっていうのは、絶対間違いないです。
ここで間違えてほしくないのが、レーザーやってればスキンケアいらないみたいな方もいらっしゃるんですけども、私も患者さんを見てて思うんですが、スキンケアが適当な人って、レーザー当てていてもあんまり綺麗にならないというか、よくはなるんですよ。でも、また来るときに、また、ちょっとマイナスになっていて、それをまたプラスに戻して、っていう、こういう感じにしかならないんですけど。
スキンケアちゃんとしている人は、レーザー当ててよくなって、それをキープしてくれるので、また当てるとよくなって、っていうような段階が踏めるんですよね。なので、スキンケアと美容医療って、本当に大事な両輪だなって、私も思っているので、これからもずっとやっていきたいなと思っています。
続いて5つ目。これはちょっとあれかもしれないんですけど、痩せないっていうことです。10代20代30代までは、もう本当に痩せたかったし、本当にモデルさんみたいになりたいみたいに思ったんですが、30代後半も、特に40代になってくると、ですね、痩せると首がゲッソリなったり、あと顔がゲッソリなったりするんですよね。私はもともと顔がパツパツなので、いつもヒアルロン酸とか入れてますか疑惑が、よくあるぐらい、パツパツなんですけれども、やっとようやく顔がちょっとね、いい感じに落ち着いてきたかなと思うんですけど。
やっぱり、忙しくて顔がゲッソリしちゃうと、それだけですごく老けて見えるので、痩せないようにする。水着着るとか、足すごい出すとか、そういう服も着ないので、もちろんね、見苦しくないぐらいの体重管理っていうことは大事だなとは思うんですけれども、必要以上に痩せない。息子が10歳になるので、実は一番最初に妊娠した時から、2,3キロ戻ってないんですけれども、1回に戻ったことがあるんですよ。そうするとすっごい、なんか、ゲッソリしましたね、みたいに言われたので、本当に痩せない。
特に、このYouTube始めてからは、本当に顔のゲッソリ感っていうのが、画面を通して分かるので、楽しく毎日おいしいものを食べております。もちろん、太りすぎてもダメだから、そこら辺はあれなんですけども、本当に必要以上に痩せないということも、ポイントかなと思っています。
6つ目。これはもう当たり前ですけど、日焼けをしないということです。先生も、いいよ、日焼けの話はしつこいよ、っていうぐらい聞いたと思うんですけど、どんなにスキンケアしても、どんなに美容医療しても、日焼けをしたら本当に意味がないんですよね。お金をかけた分、全くゼロになる、むしろマイナスになると思っていただいていいので、本当に日焼けだけはしないようにしてください。
日焼けが本当に光老化といって、肌の老化の7割を占めてますよ、ということを、4年前からずっと言ってきてるんですけれども、それに伴ってだんだんだんだん、日焼け止めっていうものの種類も増えたし、今年も本当に私もいろいろ紹介してるんですけれども、使い心地だったりとか、美容液みたいな日焼け止めもたくさん出てきて、日焼け止めってすごく大事なんだ、というふうに思っていただいたことが、大きくなったかなと思うので、それは私自身も嬉しいし。
私も選ぶね、チョイスがたくさんになったので、本当に嬉しいなと思います。なので、何回も言いますけれども、肌の老化の7割は日焼けだと思ってください。シミシワだけじゃなくて、たるみが起こったりとか、そういったこともあるので、4年前の自分と比べても、なんか、ちょっと顔がくすみしたなとか、なんか顔の輪郭が丸くなったなとか、いろんなことがエイジングサインとして出てくるので、本当に日焼けっていうのは大敵だということを、私自身も改めてすごく感じていますので、これから先も、ずっと永遠続けていきたいなと思っています。
そして、最後7番目。今までにも、いろいろ言ってきたことの逆説みたいに聞こえるんですけれども、頑張らないっていうことですかね。アラフィフになってくると、なんか、いろいろやりすぎちゃうと、うーんっていうところもあると思うんですよね。スキンケアとかも頑張りたいとか、レーザーとか美容医療とかも頑張りたい、みたいなところもあるんですけれども、やりすぎちゃうと、人工的になったりとか、疲れちゃうじゃないですか。
だから、まあ、いい意味で、エイジングっていうのも多少ね、ちょっとぐらい変化があっても、これって当たり前だと私は思っているので、やばい、なんかしなきゃ、みたいな感じに思わずに、これも人間だもの、みたいな感じで、受け入れるっていうことも、すごく大事だなと思っているので、そういうストレスから来るエイジングサインもたくさんあるので、あんまりそれに固執しないっていうことも大事かなと思って。
46にもなれば、たるみもあるし、もちろん笑ったらシワもできるし、シミもちょっとはできるし、くすみもできるし。でも、それでも楽しんでいけるかなっていうような、ちょっとは頑張らない気持ちも出てきているので、それが少し大人の余裕みたいに言ったらあれですけれども、そういったところも大事かなというふうに思います。
美容を突き詰めるがあまり楽しめなかったら、本末転倒だなと思って、美容っていうのは、自分もハッピーになって、それを見てる周りもハッピーになることだと思うので、頑張らないというのも大切かなというふうに思っております。

Hello everyone, this is Tomori Atarashi. Today, I, a 46-year-old dermatologist, would like to introduce 7 things I must do to stay beautiful. Yes, I'm now 46 years old, in my late 40s, and my skin is aging. I started this YouTube channel 4 years ago. Over these 4 years, my face has been uploaded to YouTube and I've aged by 4 years. Of course, I can see that I've climbed another step towards maturity, but since the videos remain permanently, I feel the need to try not to change too much. My concept is to look 3 to 4 years younger than my actual age, and to maintain a skin care routine and lifestyle so that people will say they don't see much change over time. I'm also doing cosmetic procedures at the clinic with this in mind. Today, I'd like to talk about these things in detail, so please watch until the end.
By the way, this is the very first video I made when I started YouTube, from around December or November 2019, which was uploaded in March 2020. It's been about 4 years plus a little. This is how I look now, 4 years later. Of course, the camera, lighting, and equipment have changed, but I don't think the change is very noticeable.
So, the first thing I've been doing to be told that I haven't changed much is: to normalize my skin turnover. Some of you might think "What? Isn't that too obvious?" When we talk about cosmetics, we often say to normalize skin turnover and brighten your skin. You've probably heard the term "turnover" many times, but it's incredibly important for your skin. What turnover is is that cells are born from the basal membrane at the bottom of the skin, grow and develop, and then become the stratum corneum, shed as dead skin, and fall off. That's skin turnover, which is essentially the life cycle of skin.
During this turnover process, skin cells don't just grow. As they develop, they produce intercellular lipids, ceramides, hyaluronic acid, and other moisturizing and radiance-giving components. When turnover slows down, your skin can't produce these radiance-boosting components on its own. Also, when turnover slows down, the stratum corneum thickens, causing dullness. Beyond just the thickness, the cells aren't being carried away, so cells that have received melanin don't leave, and the whole skin becomes dull.
Sadly, as we age, turnover slows down. So with each passing year, turnover gets slower, which leads to aging. That's why our skin loses radiance and develops dullness. Some people think, "Well, let's speed up turnover then," and do peeling to thin the stratum corneum and accelerate turnover. But if it's too fast, the cells don't have enough time to produce the moisturizing and radiance-giving components I mentioned, and they get pushed out prematurely. This causes peeling and redness from minor irritation. So turnover can't be too slow or too fast—it needs to be normal.
That's why using skincare that normalizes turnover is very important. You might ask, "But specifically, what is that?" The answer is "basal membrane care." Ingredients like Rice Power Extract No. 11, Shiseido's D-Amino Acid, and linoleic acid-derived ingredients are examples of products that approach basal membrane care and help normalize turnover. I make sure to use these kinds of products. That's how skin stays vibrant and can produce its own moisturizing components.
Next, the second thing is carbonated skincare. This connects to the first point. To normalize skin turnover and incorporate various skincare products, and also to reduce inflammation, I use carbonated skincare products. Specifically, high-concentration carbonated products. High-concentration carbonated treatments reduce skin inflammation, promote collagen and elastin production, prevent acne in people like me with oily skin, slightly exfoliate the stratum corneum to normalize turnover, care for collagen in the dermis, and also manage inflammation.
When thinking about carbonated products, the key point is whether the carbonic acid is properly ionized. Kao group products are most trustworthy for this—brands like Est, Sofina IP, and Alblanc make good carbonated cosmetics. Products that create carbonated mist and deliver ionized formulations are also good. This improves circulation, so complexion improves and puffiness is reduced. I'd like everyone to incorporate at least one carbonated cosmetic product. Carbonated cleansers are good for exfoliation and acne care, but for inflammation and collagen care, you need high-concentration carbonated products. Anyway, I incorporate high-concentration carbonated care every day.
The third thing is using vitamin C and retinol every day. I've been saying this for 4 years straight. Use vitamin C morning and night, and retinol is great for speeding up turnover and anti-aging. Now it's become common sense to use vitamin C in the morning and retinol at night, but I'm more in favor of using both morning and night.
Vitamin C especially has strong antioxidant properties and brightening effects. For people with oily skin like me, it also controls sebum production. But the key mistake people make is thinking higher concentration is always better. Some people do fine with high concentrations, while others' skin is sensitive to even low concentrations. The point is to use it daily. Daily use means if high concentration is too irritating, you should use a lower concentration consistently. Also, if you're using it long-term, buying expensive products becomes difficult. So the key is consistency, not concentration. Choose a formulation you can use daily.
Vitamin C has strong antioxidative power and oxidizes easily. Rather than using vitamin C alone, I use products like Doctor K's K-Shield Essence. Vitamin C sacrifices itself to reduce oxidized components and restore them—a process called reduction. So vitamin C itself gets oxidized. To then reduce this oxidized vitamin C and restore it, we add glutathione and other ingredients in a vitamin cycle. So if you use products that maintain this antioxidative cycle of vitamin C, you can get vitamin C benefits even without high concentration.
With retinol too, it's not about concentration. High-percentage retinol products sadly do exist. Individual reactions to retinol vary greatly. Some people have no redness or peeling even with high percentages, while others get redness from low concentrations. Like vitamin C, the point is using it long-term even at low concentration. I use very gentle formulations and have barely any redness. There are retinol derivatives and pure retinol—any of these works. The key is continuing long-term, so don't think low concentration means it's useless. Consistency is the point.
The fourth thing is getting regular laser treatments. As a dermatologist, maybe this is obvious to me. I first got laser treatment for facial hair removal in my late 20s. Since then, lasers have evolved so much. Light therapy came, then picosecond lasers, then radiofrequency microneedling. So many revolutionary treatments have emerged. People always ask about new lasers. Honestly, I think all good lasers work. Basically, they apply heat to destroy melanin, eliminate redness, or increase collagen and elastin production through heating.
While specialized skin tightening lasers are great, other lasers also tighten skin when applied regularly over years because of the heat. I never go more than 3 months without some laser treatment. So I get some kind of laser every 1-3 months, and I've been doing this for these 4 years too. That's definitely why people say I look unchanged.
One important point: don't think laser treatment means you don't need skincare. From my experience with patients, people with poor skincare don't get as good results from lasers. They improve temporarily, but when they come back, things have regressed, and we're back to square one. People with proper skincare keep the results. When you add a new laser treatment, they get even better results. So skincare and cosmetic procedures are both important wheels. I'll continue both.
The fifth thing is not losing weight. This might sound odd. In my teens through 30s, I desperately wanted to be thin and model-like. But from late 30s, especially in my 40s, weight loss makes your neck and face look gaunt. I naturally have a full face and often get asked if I've had fillers. Finally, my face has settled into a nice fullness. When I'm busy and my face gets gaunt, I look so much older. So I try not to lose weight. I don't wear swimsuits or show my legs much. Of course, I maintain a healthy weight and don't look sloppy, but I don't try to be unnecessarily thin.
My son is 10 now. I haven't lost the 2-3 kilos I gained during my first pregnancy. One time I did lose it, and people said, "Wow, you look so gaunt!" So I don't lose weight. Especially since starting YouTube, facial thinness is visible on screen. So I enjoy eating delicious food daily. Of course, I don't overeat, but I avoid unnecessary weight loss.
The sixth thing—and I apologize for being repetitive—is no sun exposure. You've probably heard me repeat this so many times that you're tired of it. No matter how much you do skincare, no matter how much you do cosmetic procedures, if you get sun damage, it's meaningless. You lose all the money you invested and go backwards. I really urge you not to get sun damage. Sun damage causes photoaging, accounting for 70% of skin aging, which I've been saying for 4 years. Consequently, sunscreen options have increased, and this year especially, there are many options. I've noticed more people understanding how important sunscreen is, and there are now so many products, including beauty serum-like sunscreens. That makes me happy.
So I'll say it again: 70% of skin aging is from sun damage. It causes not just spots and wrinkles, but also sagging. Comparing myself to 4 years ago, my face is a bit dull, and my jawline is rounder. Various aging signs appear. So sun damage really is the enemy. I feel this strongly, so I'll continue forever.
Finally, the seventh thing might sound contradictory, but it's: don't overdo it. As you get to your late 40s, you can overdo things. You want to work hard on skincare, work hard on cosmetic procedures. But overdoing it makes things look artificial and tiring. Here's the thing: some aging signs are normal. I don't think you need to panic and do something about everything. Accepting things with "I'm human, after all" is important. Stress-related aging signs are real. Don't obsess over every sign.
