皆さんこんにちは、内科・皮膚科医の友利新ともこです。最近YouTubeを見ていると、「関わってはいけない人」というテーマの動画が多いですよね。大体がビジネスや自己啓発の観点から、関わると人生を損するという内容が多いと思います。実は医学・医療の世界にも同様のことがあります。お子さんやご両親など、大切な家族のためにもなる内容ですので、ぜひ最後まで聞いていただければと思います。
【1人目:ステロイドを全否定している人】
「脱ステロイド(脱ステ)」という言葉で検索すると、全身が真っ赤になった赤ちゃんの写真などが出てきて、見るたびに心が痛くなります。なぜ脱ステがSNS上に多く上がるかというと、「ステロイド=悪」「ステロイドを塗ると健康被害が起こる」という考えが広まっているからです。
日々の診療でも、アトピー性皮膚炎や湿疹の患者さんにステロイドを処方しようとすると、「ステロイドは危ない薬なんでしょう?」と言われることがよくあります。その根拠を尋ねると、「みんなが言っているから」「ネットで見たから」という答えがほとんどで、「なんとなく怖い」という感覚が一人歩きしているのが現状です。
ステロイドは炎症を抑える薬であり、免疫を調整する薬でもあります。内服ステロイドは膠原病や新型コロナウイルス感染症による免疫暴走にも使われており、その有効性は確立されています。皮膚科の塗り薬としてのステロイドが「悪」とされているのは、使い方や種類を誤って使用し、悪化させてしまった事例が都市伝説のように広まったためだと思われます。この点については、私たち皮膚科医にも説明不足な面があったと反省しています。
ステロイド外用薬は、皮膚の炎症を抑えるための薬です。皮膚炎は何らかの原因でバリア機能が低下し、炎症が起きている状態、いわば「火事」の状態です。ステロイドはその消火剤の役割を果たします。ただし、表面の炎症を抑えただけでは、奥でくすぶっている炎症は残るため、再発してしまいます。
そのため、炎症の程度に応じてステロイドの強さ(ランク)を選び、症状が改善するにつれて徐々にランクを下げていく必要があります。また、使用期間も非常に重要です。2〜3日で表面の症状が改善しても、奥の炎症はまだ残っているため、医師に指示された期間(例えば2週間)はしっかりと塗り続けることが大切です。自己判断で中断してしまうと悪化する原因になります。
「ステロイドを塗ると皮膚が黒くなる・薄くなる」とおっしゃる方もいますが、これは半分正しく、半分誤りです。強すぎるステロイドを長期間使い続けると皮膚が薄くなることはありますが、それは誤った使い方をした場合です。皮膚の黒ずみについては、日焼けと同じ「炎症後色素沈着」が原因です。炎症が長引くほど色素沈着が起こるため、ステロイドを正しく使って炎症をしっかり抑えることで、皮膚の黒ずみは防ぐことができます。ステロイドが体に蓄積されるということも基本的にありません。肝臓や腎臓がしっかり機能していれば、代謝・排出されます。
赤ちゃんへのステロイド使用を怖がる方も多いですが、赤ちゃんの皮膚はバリア機能が未熟で乾燥しやすく、生後1歳までに7〜8割の赤ちゃんが何らかの皮膚疾患を起こすと言われています。また、日本では沐浴(洗う)習慣はあっても保湿が不十分なケースが多く、それが乳児湿疹につながることがあります。
脱ステにより「赤ちゃんの湿疹が治った」という声もありますが、これは1歳を過ぎると皮膚のバリア機能が発達し、自然に症状が改善するためです。ステロイドによる適切な治療を行えば、もっと早く、もっと楽に改善できた可能性があります。赤ちゃんだからこそ、適切な強さのステロイドを適切な期間使うことが大切です。それが将来のアトピー性皮膚炎の予防にもつながります。
アトピーに対する「ステロイドを使わない民間療法」は数多くありますが、医学的エビデンスのあるものはほとんどありません。まずは標準治療を行った上で、気休め程度に取り入れるのはよいですが、民間療法にのめり込むのはやめましょう。
【2人目:ワクチンは悪だという人】
SNSなどで「ワクチンは毒だ」と主張する方を見るたびに心が痛くなります。ワクチンはそもそも、命に関わる感染症や重篤な後遺症を予防・軽減するために開発されたものです。天然痘ウイルスはワクチンの普及によって世界から撲滅されました。
麻疹(はしか)・風疹・流行性耳下腺炎(おたふくかぜ)などは「少し熱が出るくらいだから、自然にかかった方がいい」と言う方もいますが、これらの疾患は重篤な後遺症や死亡例も多数あります。麻疹は脳炎を引き起こすことがあり、数十年後に麻痺が起こる「亜急性硬化性全脳炎(SSPE)」の原因にもなります。風疹は妊娠初期にかかると赤ちゃんの聴力・視力に障害を残すことがあります。おたふくかぜは髄膜炎や難聴の原因にもなります。「昔かかったけど何ともなかった」という方は、ラッキーだっただけです。
また、「ワクチンで自閉症になる」という話がありますが、これはかつて発表された論文が後に撤回されたものです。その後の訂正情報は広まらず、誤った情報だけが一人歩きしてしまっています。「ワクチンより感染した方が体が強くなる」というエビデンスもありません。国が定めたワクチンは、自分と家族を守るためにきちんと接種しましょう。
【3人目:すぐに陰謀論を持ち出す人】
ステロイドやワクチンについて医学的な説明をすると、「製薬会社からお金をもらっているんでしょう」「国家的なプロジェクトで人口操作をしている」などと言い出す方がいます。しかし、実際には製薬会社から何かを言わされたことは一度もありません。むしろ、ワクチン接種はクリニックの収益にほとんど貢献しません。病気が増えた方が患者さんが来院するため、経済的には病気を増やした方が「儲かる」ということになりますが、それは本末転倒です。私たちは医学的エビデンスに基づいて発言しています。
陰謀論を強く信じている人は、科学的なエビデンスを示しても「その科学が嘘だ」と言うため、議論が噛み合いません。こうした方々への説得は、健康被害を防げないだけでなく、こちらの時間も無駄になります。そのため、無理に反論せず、「そうですね」「大変ですね」と言って、適切な距離を置くことをお勧めします。

Hello everyone! I'm Arata Tomori, an internal medicine and dermatologist physician. If you've been watching YouTube lately, you've probably noticed a lot of videos about 'people you should avoid.' Most of them focus on business or self-improvement — the idea that certain people will cost you time, money, or happiness. As it turns out, the same concept applies in medicine. Today I'd like to share a doctor's perspective on this topic, because it's relevant not only for yourself but also for your children and other family members. Please watch until the end.
[Person #1 to Avoid: People Who Completely Reject Steroids]
If you search for 'datsute' (steroid withdrawal) online, you'll find heartbreaking photos of babies covered in red, inflamed skin. The reason this movement has spread so widely on social media is the belief that 'steroids = evil' and that applying steroids causes health damage.
In my daily practice, when I try to prescribe topical steroids for patients with atopic dermatitis or eczema, I'm often told: 'Please don't give me steroids — aren't they dangerous?' When I ask why they think so, the answer is almost always: 'Everyone says so' or 'I read it online.' It's a vague, unfounded fear that has taken on a life of its own.
Steroids are medications that suppress inflammation and modulate the immune system. Oral steroids are used for autoimmune diseases and even for COVID-19, where they help control immune overreaction — their effectiveness is well established. The negative reputation of topical steroids in dermatology seems to stem from cases where they were used incorrectly — wrong strength, wrong duration — leading to worsening symptoms that then spread as urban legends. I'll admit that we dermatologists share some of the blame for not explaining their use more clearly.
Topical steroids work by suppressing skin inflammation. Think of skin inflammation as a fire. Steroids are the fire extinguisher. But just as putting out surface flames doesn't mean the fire is completely out — it may still smolder underneath — treating only the surface of the skin doesn't resolve deeper inflammation. That's why it comes back.
This is why the strength (rank) of the steroid must be matched to the severity of inflammation, and then gradually reduced as symptoms improve. The duration of use is equally important. Even if the surface looks better after two or three days, the deeper inflammation is still present. You should continue applying the steroid for the full prescribed period — for example, two weeks — and then return to your doctor to reassess. Stopping early on your own is one of the most common causes of relapse.
Some people claim that steroids darken or thin the skin. This is partly true and partly false. Using an overly strong steroid for an extended period can thin the skin — but only with improper use. As for darkening, this is actually caused by post-inflammatory hyperpigmentation, the same process that happens with a sunburn. The longer inflammation persists, the more pigmentation accumulates. By properly controlling inflammation with steroids, you prevent the skin from darkening. Steroids also do not accumulate in the body — the liver and kidneys metabolize and excrete them, provided they are functioning normally.
Many parents are afraid to use steroids on babies, but infant skin has an immature barrier function, is prone to dryness, and it is estimated that 70–80% of babies develop some form of skin condition before their first birthday. In Japan, bathing (washing) babies is standard practice, but moisturizing afterward is often overlooked. Leaving skin exposed after washing leads to dryness, barrier breakdown, and ultimately infantile eczema.
Some parents claim their baby's eczema resolved without steroids, but this is largely because skin barrier function matures significantly around age one to two, and symptoms often improve naturally at that point. Proper steroid treatment would have resolved the symptoms faster and with less suffering. Because babies are still developing, it is especially important — not less important — to use steroids of appropriate strength for an appropriate duration. This also helps prevent the development of atopic dermatitis later in childhood.
There are many alternative therapies marketed for atopic dermatitis that avoid steroids, but very few have medical evidence behind them. It's fine to use them as a supplement after receiving standard evidence-based treatment, but relying on them instead of proper treatment is dangerous.
[Person #2 to Avoid: People Who Say Vaccines Are Evil]
It's deeply saddening to see people online calling vaccines 'poison.' Vaccines were developed specifically to prevent death and serious complications from infectious diseases. Smallpox, for instance, was eradicated worldwide thanks to vaccination.
People sometimes say about measles, rubella, or mumps: 'It's just a fever or a rash — isn't it better to get it naturally than to get a scary vaccine?' But historically, these diseases killed and permanently disabled many children. Measles can cause encephalitis and, decades later, a fatal neurological condition called SSPE (subacute sclerosing panencephalitis). Rubella contracted in early pregnancy can cause deafness and vision problems in the baby. Mumps can lead to meningitis and permanent hearing loss. Those who say 'I had it as a kid and was fine' were simply lucky.
The claim that vaccines cause autism originates from a paper that was later retracted after being found to be fraudulent. Unfortunately, the original claim spread widely while the correction did not. There is also no evidence that contracting an infectious disease naturally makes you stronger than vaccination does. Please follow your country's recommended vaccination schedule — it protects both you and your family.
[Person #3 to Avoid: People Who Immediately Resort to Conspiracy Theories]
Whenever I recommend steroids or vaccines based on medical evidence, some people respond with: 'You're getting paid by pharmaceutical companies' or 'This is a government population-control plot.' I have never once been told by a pharmaceutical company to promote their products. And frankly, vaccines generate very little revenue for clinics. If anything, more disease means more patient visits — economically speaking, keeping people sick would be more 'profitable.' But that is obviously not how we think. Our recommendations are based on scientific evidence.
The problem with people deeply committed to conspiracy theories is that presenting scientific evidence doesn't help — they simply say 'that science is fake.' The conversation goes nowhere. Engaging with them doesn't prevent health harm; it just wastes your time. My advice: don't argue. Say something like 'I see' or 'That sounds tough,' and quietly keep your distance.
