Hello everyone, I'm Arata Tomori, an internal medicine and dermatologist. Today I'd like to talk about a topic I get asked about quite often: dark spots in the intimate zone. A common question I receive during live sessions is, "What should I do to improve dark spots in my intimate area?" or "I'm really bothered by the darkening — please tell me how to make it lighter." I have two main things I want to address on this topic. First, let's clarify what we mean by the "intimate zone" or "delicate zone." When it comes to body darkening concerns, the most common areas people mention are the intimate zone, the groin area, places where underwear rubs, and areas like the elbows and knees. Among these, intimate zone darkening is something many people struggle with silently because it's hard to discuss with others. And unlike elbows or knees, you rarely get to compare your intimate area with someone else's, so most people have no idea whether their level of darkening is typical or not. I'm not an OB-GYN, so I don't examine this area all the time, but as a dermatologist I do see patients for itching or skin conditions in the intimate area. So I do have a sense of what's within the normal range of variation. What I want to convey to you is this: it is completely normal for the VIO and intimate zone to be dark. You might think, "But babies have such smooth, light skin — isn't that different?" And that brings us to the reason why the intimate area darkens in the first place: sex hormones. Once puberty begins and sex hormones start to be secreted, both men and women experience darkening in the intimate zone. Conversely, as we age and hormone levels decline, the darkening can actually fade somewhat. But for as long as sex hormones are being produced — from puberty until they significantly diminish — darkening in the intimate zone is a normal, physiological process. There are creams and treatments marketed to whiten the intimate area, but from my perspective, these are creating an unnatural state, they place a significant burden on the skin, and frankly, the area will not become as white or pink as people hope. As long as sex hormones continue to be secreted, the root cause of the darkening remains. No treatment can fully counteract that — you can achieve some lightening, but there will always be a gap between expectations and reality. If you're considering treatment, please make sure your expectations align with what's realistically achievable, so you don't end up disappointed. Clinics do offer laser treatments and whitening agents, and at my clinic we discuss these options if patients are interested. I always explain that while you might see one to two shades of improvement, it will never become fully white or pink — and only then do we proceed if the patient still wishes to. These treatments are not wrong, and clinics offering them are not doing anything improper, but I do want patients to go in with realistic expectations and minimize the gap between what they hope for and what can actually be achieved. Another point I want to make: many people feel ashamed or embarrassed about the darkening in their intimate area, and I really don't want you to feel that way. As I've said, this darkening is driven by hormones — it is a natural phenomenon. During pregnancy, for example, sex hormones surge dramatically, and this can cause not only the intimate area but also the nipples and underarms to darken. This is simply a physiological response. After delivery, as hormone levels settle, some of the darkening will fade on its own. Wanting to suppress or reverse something that is a normal bodily function seems, in my view, unnecessary. Everyone experiences some degree of darkening in these areas while their sex hormones are active. Worrying about it is a bit like worrying about why dogs bark or why the sky is blue — it's just how things are. Understanding which parts of your body naturally darken and which can realistically be lightened will save you from wasting time and money chasing something that isn't achievable. That said, I do believe in caring for these areas. When I ask patients who say they're bothered by intimate zone darkening to describe exactly where, I often find that what they're actually concerned about is the darkening where their underwear rubs — not the intimate area itself. Darkening of the actual intimate area due to hormones cannot be made fully white, but darkening caused by friction from underwear can realistically be improved. There are skincare products designed to address this kind of darkening. My first recommendation is to avoid tight underwear, since compression causes friction, which triggers inflammation in the skin — similar to what UV rays do. Second, use products with whitening ingredients that suppress melanin production. Third, apply products with anti-inflammatory ingredients to the VIO line or underwear contact areas. The product I personally use is the Bioderma Pigmentbio White Sensitive Cream, a moisturizing cream for the neck and body. Bioderma is a French brand well known among dermatologists in France. It's formulated for sensitive skin and is fragrance-free, colorant-free, and alcohol-free. It contains Laminaria ochroleuca extract for high moisturizing efficacy, as well as azelaic acid, an ingredient known for its anti-inflammatory properties — making it effective at calming inflammation caused by friction. The ingredient I find particularly impressive is glabridin, an extract derived from licorice root (known as kanzou in traditional Chinese medicine). Glabridin inhibits the activity of tyrosinase — the enzyme that signals the skin to produce melanin pigment in response to UV exposure or inflammation — thereby suppressing melanin production. It is recognized as a potent whitening ingredient that is also gentle enough for sensitive skin, and it is included in this cream. With both azelaic acid and glabridin, this cream can be beneficial for those experiencing darkening from underwear friction, darkening in the underarms from self-hair-removal, or darkening on the elbows and knees from repeated physical friction. By combining moisturization, anti-inflammation, and melanin suppression, there is a reasonable expectation of improvement. To reiterate: please let go of the idea of making the hormonally influenced intimate zone fully white. However, if the darkening you're truly concerned about is in areas like the underwear line, the underarms after shaving, or the elbows and knees — areas affected by physical friction — then reviewing your lifestyle habits and using a skincare product with anti-inflammatory and mild whitening ingredients can help prevent further darkening and may bring some improvement. The key is to first identify whether the area you're worried about is one where darkening is largely unavoidable due to hormones, or one where improvement is genuinely possible — and then take the appropriate approach for each.
