長州力さんの娘婿さんを救いたい〜勝手に診察シリーズ〜

どういう内容かというと、ざっくり言うと長州力さんの娘婿さんが、結婚されてから長州さんのマネージャーもされているのですが、結婚した2年前から10キロもの体重減少があるということなんです。これは医師の目線から見るとかなり心配で、私たち医者がこの体重減少に対してどういうことを考えるかという観点からもお話ししたいと思います。また、その会話の中で長州さんが「内科に行け」とおっしゃっているのですが、内科の中にもいろんな科に分かれていますので、もし長州力さんの娘婿さんにこの動画が届くのであれば、参考にしていただきたいと思って動画を撮っております。
私たち医者はまず体重減少があると言われたら、「食欲はありますか?」と聞きます。なぜかというと、食欲があるのに体重が減る場合と、食欲が普通もしくは低下していて体重が減っている場合とで、考える疾患が異なるからです。
まず、食事をたくさん食べているにもかかわらず体重が減ってしまう場合、2つの病気が頭に浮かびます。一つ目が甲状腺機能亢進症です。甲状腺は首のあたりにあるホルモンを分泌する臓器で、甲状腺ホルモンは体の代謝を上げる働きがあります。そのため、動悸がしたり、汗をかいたり、暑がりになったり、食べても食べても太らなくなったりといったことが起こります。ただ、長州さんの動画を見ていただければわかると思いますが、長州さんは半袖・ハーフパンツなのに対し、娘婿さんはパーカーにジャケット、パンツを着用されています。甲状腺機能が亢進しているのであれば厚着はしないと思われますので、甲状腺機能亢進症は少し可能性が低いかなと思います。なお、甲状腺機能亢進症が疑われる場合は、内科の中でも内分泌内科を受診してください。
もう一つ、食欲が亢進しているにもかかわらず体重が減少する原因として、コントロール不良の糖尿病が考えられます。ただし、これは糖尿病を長期間患っている方に多いため、動画の印象からすると可能性は低いかなと思います。糖尿病が疑われる場合は、内分泌代謝内科または糖尿病内科を受診してください。
動画の中で「ちゃんと食事は食べている」とおっしゃっていましたが、食欲が普通もしくはない場合には、2つの怖い病気が頭に浮かびます。一つ目はやはり悪性腫瘍です。特に娘婿さんが「胃がたまに痛くなることがある」とおっしゃっていたので、胃がんや膵臓がんなど消化器系の悪性腫瘍が考えられます。ただ、2年間かけての体重減少であれば、悪性腫瘍であれば他の症状も出てきているはずなので、可能性は低いと思っています。しかし、もし便に血が混じるとか、胃痛が続くといった症状があれば、消化器内科を受診するようにしてください。
最後に、動画を見た多くの方が「長州さんの娘婿さんになったことや、マネージャーを務めるようになったことによるストレスが原因ではないか」とコメントされていますが、私たち医者もその可能性は頭に置いています。うつ病とまではいかなくても、気分が落ち込む「気分障害」が体重減少を引き起こすことがあります。少し心配だったのは、動画の中で長州さんが「大丈夫だよ」とおっしゃっていた点です。気分障害や抑うつ傾向のある方に「大丈夫」「なんとかなる」といった言葉をかけると、真面目な方ほど自分を追い込んでしまう場合がありますので、そういった声がけはご注意いただければと思います。
気分が優れない場合には、「心療内科」という選択肢があります。いきなり薬物治療というわけではなく、カウンセリングを受けながらお悩みを引き出し、気分を改善していくことができます。もしこの動画が長州さんの娘婿さんに届いているなら、心療内科の受診をぜひお勧めします。私は投入病(糖尿病)を専門としているため直接診ることはできませんが、ご相談いただければ適切な先生をご紹介することも可能です。
最後に、長州さんが「シーサーを飾れ」とおっしゃっていましたが、これは医者としては少し心配な一言です。原因不明の体調不良のとき、非科学的なものにすがりたくなる気持ちはわかりますが、それによって病気の発見が遅れてしまうことがあります。おまじないや非科学的なものに頼ることはやめていただきたいと思います。また、私は沖縄出身なのですが、シーサーはそもそも魔除けのもので、病気を治すためのものではありません。玄関に置いて災いを防ぐものですので、すでに起きている体調不良に対して効果があるものではないと、沖縄出身者としても申し上げたいと思います。

Hello everyone. I'm Arata Tomori, an internal medicine and dermatologist physician. As I mentioned in the thumbnail, I'm making this video because I'm concerned about the son-in-law of professional wrestler Riki Choshu. Have you all seen the video featuring him? If not, I'll leave a link in the description, so please check out Riki Choshu's YouTube channel and the video of his conversation with his son-in-law.
To summarize briefly: Mr. Choshu's son-in-law, who has been serving as Mr. Choshu's manager since their marriage, has lost 10 kilograms over the past two years since the wedding. From a physician's perspective, this is quite concerning. I'd like to discuss what doctors consider when evaluating unexplained weight loss, and since Mr. Choshu suggested simply going to a 'general internal medicine' clinic, I want to clarify that internal medicine has many subspecialties — I'm sharing this hoping the video reaches Mr. Choshu's son-in-law.
When a patient reports weight loss, the first question we ask is: 'Do you have a normal or increased appetite?' This is because the list of possible diagnoses differs depending on whether appetite is preserved or reduced.
If someone is eating well but still losing weight, two conditions come to mind. The first is hyperthyroidism. The thyroid gland, located in the neck, produces hormones that regulate the body's metabolism. When overactive, it can cause palpitations, excessive sweating, heat intolerance, and weight loss despite a good appetite. However, looking at the video, Mr. Choshu is in a short-sleeve shirt and shorts, while his son-in-law is wearing a hoodie, jacket, and trousers. If he had hyperthyroidism, he would likely feel too warm to dress that way, so this condition seems less likely. If hyperthyroidism is suspected, please see an endocrinologist.
The second condition to consider when appetite is increased but weight is falling is poorly controlled diabetes mellitus. This typically occurs in people with a long history of diabetes, and based on the impression from the video, this also seems unlikely. If diabetes is suspected, please see an endocrinology/metabolism specialist or a diabetes specialist.
The son-in-law mentioned that he is eating properly. However, if appetite is normal or decreased, two more serious conditions come to mind. The first is malignancy — particularly gastrointestinal cancers such as stomach cancer or pancreatic cancer, especially since he mentioned occasional stomach pain. That said, given that the weight loss has occurred gradually over two years, other symptoms would likely have appeared by now if cancer were the cause, so this seems less probable. However, if there is blood in the stool or persistent stomach pain, please see a gastroenterologist.
Finally, many viewers commented that the weight loss might be due to stress — from marrying into the Choshu family and taking on the role of manager. This is absolutely something we physicians consider. Even without a full diagnosis of depression, a milder form of low mood known as a mood disorder or adjustment disorder can cause weight loss. One thing that concerned me was that Mr. Choshu said things like 'You'll be fine' in the video. When said to someone experiencing depressive symptoms, reassurances like 'You'll be fine' or 'It'll work out' can actually put more pressure on conscientious individuals who may internalize those words and push themselves harder. Please be mindful of this.
For someone experiencing persistent low mood, a psychosomatic medicine clinic (心療内科, shinryō naika) is a great option. Treatment doesn't necessarily mean medication right away — counseling and talking through concerns can help lift one's mood gradually. If this video reaches Mr. Choshu's son-in-law, I would strongly recommend visiting such a clinic. While my own specialty is in a different area of internal medicine and I cannot see him directly, I would be happy to introduce him to an appropriate physician if he wishes to reach out.
One last thing — Mr. Choshu mentioned that his son-in-law should 'put up a shisa.' As a doctor, I find this a little worrying. When the cause of an illness is unclear, it's natural to want to turn to something non-medical for comfort. However, relying on non-scientific remedies can delay the diagnosis of real medical conditions, so I strongly advise against it. Also, as someone from Okinawa myself, I want to clarify that shisa are traditional guardian figures meant to ward off evil spirits from the entrance of a home — they are not intended to cure illness. They are a preventive charm, not a remedy for existing problems, so I must say, as an Okinawan, that using a shisa for this purpose isn't quite right.
