It's a scandal, Doctor! I witnessed something incredible — you were getting a huge number of impressions on X, weren't you? It's the first time in my life I've gone viral on X — or maybe slightly "went up in flames" is more accurate. That day I happened to have a rare dinner appointment. When I got home, I was like "what is this?!" and got a little scared. Even Nomori-kun said, "Doctor, this is a huge deal!" Right, right — you showed off your driver's license photo from 5 years ago next to your current one. Is that okay to do? (laughs) When I looked at them, I thought, "I really am making the exact same expression." True, there was definitely a set "signature face" expression. It's not like I took the photo myself beforehand — you know how they tell you "please take your license photo" and you just do it. I thought it was funny that I had the same expression both times, so I posted it, and got all these comments... Okay, that's enough setup. So what is it — what have you been doing these past 5 years? It's basically everything I talk about on YouTube. You do a lot of things, though. People who watch my videos know I do aesthetic medical treatments too, of course. But my main focus is really skincare — I love it. People ask "did you do something special?" but I'm always doing something. Lately I've been thinking less about what to do and more about what NOT to do. In the past I wanted to use all sorts of ingredients, tried every new product, but now the essentials get naturally selected. Rather than using something because someone else uses it or because it's trending, sticking consistently with what suits you is most important. The one thing that stands out is sunscreen — applying it properly, 365 days a year. That's the most important thing. What percentage of maintaining that license-photo face would you say is due to sunscreen? Since I also do aesthetic treatments, I'd say about half is sunscreen. Sunscreen can wear off or become uneven, so... When I first made this YouTube video, I talked about how many grams to apply. After that, everyone started saying "0.6 grams" as the standard amount — but I was the one who actually measured that, you know! Back then there wasn't even AI, so I researched anatomy books and worked out the formula myself. Basically, applying enough sunscreen and blocking light properly is most important. But since you can't always apply enough, things like parasols, hats, and stoles — physical sun protection along with sunscreen — make up about the other half. What about the remaining 50%? I overhauled my skincare over these 5 years. I used to talk passionately about adding more and more products, but eventually my skin started feeling dry, so a big shift was moving toward simpler skincare. At its simplest, it's just washing my face, applying toner, then cream, and going to sleep — that kind of simple care has become more common for me. If you overdo it, layering more and more cosmetics becomes "too much," and being overly diligent can actually weaken the skin barrier and lead to inner dryness. So instead of loading up on ingredients, the idea is more about balancing the skin. That's why I often use anti-inflammatory products, or things like rice power essence that rebuild the skin from its foundation — that simplification accounts for maybe 20-30%. Should people simplify from their 20s and 30s, or gradually reduce products while observing their own skin? You should watch whether your skin condition is good, whether your pores are staying tight, and so on. As I always say, if you over-cleanse thinking you have too much oil, areas like around the eyes can feel tight and stretched. So using retinol only in specific areas, doing partial application — that kind of "subtraction skincare" is important, I think. The third point is inner care. Glycation and oxidation are big factors, so eating in a way that keeps blood sugar stable — not eating something sweet first when you're hungry, for example. Also properly taking antioxidants like astaxanthin, vitamin C, vitamin D. And maintaining gut health is extremely important — when gut bacteria diversity is lost, the body tends toward inflammation, which weakens the skin barrier and can cause excess sebum again. So making sure to eat fermentable dietary fiber properly. Also, if your sleep schedule becomes irregular, your skin condition suffers, so sleeping and eating on a regular schedule matters. Also, I lost weight. I gave birth to my second daughter in 2019. Since the photo was taken in 2021, I still had some extra softness — I thought it was fine since I was wearing a mask anyway. But after that, I never really got back to my pre-pregnancy weight. I spent almost 10 years carrying an extra 2-3 kg. I wanted to get back to my pre-pregnancy weight. I've made a video about how I lost the weight, so please check that out — but getting back to my original self was probably a big factor too. The fifth point is aesthetic medical treatments. Aesthetic treatments are about boosting your skin, while skincare is about maintaining that boost. So it's about boosting while doing skincare in parallel. For me, I get laser treatments regularly. There's a device called BBL that removes pigmentation — that kind of light-based treatment. Another is something like a YAG laser — devices like Genesis also react to spots but are also good for regenerating dermal collagen. We have a device called XLV for these two treatments, and I do them once a month, or if I don't have time, once every two months or two and a half months at minimum. Also, rather than YAG, RF (radiofrequency) is what I use for tightening since my face tends to look puffy. Every six months I do things like Thermacool, InMode, or Potenza's diamond tip to reduce sagging in that area. What changed from 5 years ago is that I started doing RF regularly, and I started treating my neck too. Five years ago I thought it was a waste, but the difference between the neck and face becomes most noticeable, so I started doing that as well. Oh, I forgot something — I always get Botox injections too. And skin booster injections. I use something called Densifi — if lasers are like strength training, skin boosters are like protein. Before, I didn't need protein since strength training alone was enough, but now I want to add nutrients too. So the biggest changes over these 5 years are doing RF more frequently and adding skin booster injections. By the way, Doctor, do you not do hyaluronic acid fillers? Dr. Nakanishi — I made a video about this too — when I got a facelift, I insisted "it has to be this doctor," and he said "we need to dissolve the hyaluronic acid here." I said "I haven't had any injected," and he was like "What?! You haven't?" So it's all my own natural fat. Why don't I use fillers? Because my face was already puffy to begin with, so I wanted to tighten it — I'm doing RF and such, so I don't add filler. But I do want filler in my temples. I actually got it done once, in my temples. It's probably been absorbed already, though — with this kind of hairstyle, my temples aren't even visible anyway. So I don't do anything unnecessary. But putting the right amount in the right place where it's needed is a good thing — I think that's totally fine to do. This might not have been that entertaining, but — choosing only what's necessary. The one thing I absolutely recommend is sunscreen. And weight management. Repeatedly gaining and losing weight isn't good, so keeping your weight relatively stable matters. When you lose weight, sometimes you end up with excess skin, so having lost weight, I made sure to properly do treatments (to address that). I see — so that's the truth behind the driver's license photo. Yeah, it was really just day-to-day effort. I'd love to say "I did it with some amazing amount," but there's no such magic. So over these 5 years, that's basically it. From age 43 to 48, it's been about maintenance. How to maintain, maintain, maintain — maintaining really is hard work. But in another 5 years, maybe I'll have gotten a facelift by then. If so, it'll surpass this — like, "wow, I was so young back then." But once I turn 50, I really do want to get a facelift, so I'll say I did it around that time. Alright, so today we talked about the truth behind the driver's license photo. I actually do post on X occasionally, even if rarely, so please follow me if you'd like. Thank you very much! Please subscribe to the channel and hit the like button.
