ステロイドをしっかり塗って治療してくださいと動画でお話ししたところ、コメント欄に「ステロイドでちゃんと治療しているけれど、よくならない」というご意見をいただきました。 これを受けて、ステロイドに対する誤った認識がいまだに広まっている現状について、あらためて考えさせられました。かつてステロイドを悪者扱いするような報道があり、その誤解が30年近く経った今も払拭できていない背景には、私たち皮膚科医が十分な説明をできていなかったことも一因だと感じています。その結果、いわゆる「ステロイドビジネス」と呼ばれる誤った情報に患者さんが誘導されてしまっている可能性があります。 もちろん、私はステロイド治療が間違っているとは思っていません。アトピー性皮膚炎をはじめとした炎症性皮膚疾患において、ステロイドは基本的な治療薬です。 では、なぜ「使っているのに治らない」という状況が生まれるのか。主な原因として、以下の2点が考えられます。 **① 適切な量を塗れていない** 内服薬であれば「1日2錠を朝晩」のように量が明示されていますが、塗り薬は「1日2回」としか書かれていないことがほとんどです。実は、ステロイド軟膏の適切な量には「FTU(フィンガーチップユニット)」という基準があります。 1FTUとは、口径5mmのチューブから人差し指の先端〜第一関節まで絞り出した量で、約0.5グラムに相当します。この1FTUで、手のひら2枚分の面積の患部に塗るのが適切な量です。塗った後は皮膚がテカテカし、ティッシュをあてると一瞬くっついてからゆっくり落ちる程度が目安です。 具体的には、1日2回塗る場合、1日の使用量は約1グラムになります。一般的なステロイド軟膏のチューブは5グラム入りですので、約5日間でなくなるペースが適切です。「そんなにたっぷり塗るの?」と驚かれる方も多いと思いますが、量が少ないと効果が不十分になり、炎症が長引く原因になります。 **② 適切な期間、塗り続けていない** 最低でも2週間は同じステロイドを使い続けることが大切です。通常は1〜1.5か月程度塗り続けるのが一般的です。炎症が強い時期には強めのランクのステロイドを使い、2週間ほどで炎症が落ち着いてきたら、必ず皮膚科を受診してランクの見直しを行ってください。 「少ない量を1週間だけ塗って治らなかった」という状態で受診すると、医師との間で「適切な治療をしても効かなかった」という誤ったコミュニケーションが生まれてしまいます。受診の際には、「どれくらいの量を、何日間塗ったか」を医師に具体的に伝えるようにしてください。 **副作用について** ステロイドには副作用もありますが、少な過ぎる量を長期間ダラダラと塗り続ける方が、炎症性色素沈着や皮膚の菲薄化・多毛などの副作用が起こりやすくなります。1FTUの量を1日2回、最低2週間、適切なランクのもので治療すれば、炎症をしっかり抑えながら副作用のリスクも最小限に抑えられます。 もしこれまでステロイド治療がうまくいかなかったという方がいれば、まず「量」と「期間」を見直してみてください。そして、いわゆる「脱ステ」や「ステロイドビジネス」に頼る前に、ぜひ皮膚科医に相談していただきたいと思います。 私たち皮膚科医も、一人ひとりの患者さんにこうした情報をしっかりお伝えできるよう努めてまいります。ステロイドを悪者にせず、正しい知識で治療に取り組んでいただけることを願っています。 ご視聴ありがとうございました。

In a previous video, I talked about the importance of properly applying steroid medications for treatment. In response, I received comments from viewers saying, "I am using steroids correctly, but my condition isn't improving." This made me reflect seriously on the persistent misconceptions surrounding steroids. Decades ago, there were media reports that portrayed steroids as dangerous drugs, and those misunderstandings have not been fully corrected even after nearly 30 years. I believe one reason is that dermatologists, including myself, have not done enough to properly explain steroid treatment to patients. As a result, patients are sometimes misled toward what might be called "steroid alternative businesses" built on misinformation. To be clear: I do not believe steroid treatment is wrong. For inflammatory skin conditions such as atopic dermatitis, steroids remain a cornerstone of treatment. So why do some patients feel that steroids "aren't working"? I believe there are two main reasons: **① Not applying enough medication** With oral medications, the dosage is clearly stated—for example, "take 2 tablets twice a day." But with topical medications, the instructions often only say "apply twice a day," with no guidance on how much to use. In fact, there is a standard unit for measuring the correct amount of topical steroid: the FTU, or Fingertip Unit. One FTU is the amount of cream squeezed from a tube with a 5mm opening along the length of an adult index finger from fingertip to the first joint—approximately 0.5 grams. This amount should cover an area equivalent to two adult palms. After application, the skin should appear slightly shiny, and if you press a tissue against it, the tissue should lightly stick and then slowly fall away. In practical terms, applying twice a day means using about 1 gram per day. A standard 5-gram tube of steroid ointment should therefore last approximately 5 days. Many people are surprised by this—"Do I really need to apply that much?"—but using too little results in insufficient effect and prolonged inflammation. **② Not applying for a sufficient duration** It is important to continue using the same steroid for at least 2 weeks. In general, treatment typically lasts 1 to 1.5 months. During periods of intense inflammation, a stronger-ranked steroid may be appropriate, but after about 2 weeks, when the inflammation begins to settle, please revisit your dermatologist to reassess whether the current steroid rank is still appropriate. If a patient comes in having applied only a small amount for just one week and reports no improvement, it can lead to miscommunication—both the patient and doctor may mistakenly conclude that proper treatment has failed. When visiting your doctor, please be specific: describe how much you applied and for how many days. **Regarding side effects** Steroids do have potential side effects, but applying too little over a prolonged period actually increases the risk of side effects such as post-inflammatory hyperpigmentation, skin thinning, and localized increased hair growth. When used correctly—1 FTU twice daily for at least 2 weeks at an appropriate potency—inflammation can be effectively controlled while minimizing the risk of side effects. If steroid treatment has not worked well for you in the past, I encourage you to first reconsider the amount you are applying and the duration of treatment. Before turning to "steroid-free" alternatives or unverified treatment businesses, please consult a dermatologist. We dermatologists also need to do better—making sure every patient receives clear, thorough guidance on how to use these medications properly.
