皆さんこんにちは、とものりあらたです。今日は目の下のたるみの改善方法についてお話ししたいと思います。 目の下のたるみや目の下のぷっくり感はどうしたら改善されますか、というご相談を結構いただいています。スキンケアで何とかなりませんか、あるいはユーチューバーが紹介している目の周りのぷっくり感を治す体操はどうですか、といったご質問をよく受けるのですが、結論から言うと、化粧品だけで改善する、あるいはマッサージだけで改善するということは医学的にはありません。 いきなり残念なお話で申し訳ないのですが、そもそも目の下のぷっくりとは何かを説明します。骸骨を思い浮かべていただくと、目のところが開いていますよね。そこに眼球が入っていて、それをカバーするように眼窩脂肪というクッションのような脂肪が入っています。若いときはこの眼窩脂肪をしっかりと支える張りがあるので、脂肪が前に出てくることはありません。 目の下がぷっくりしている人は、その部分を押すと元の位置(上まぶたのくぼみ)に戻るという現象が起こります。つまり、まぶたの張りがなくなり、眼窩脂肪が下に落ちてきてしまっているのです。目の下がぷっくりしている人はまぶたがくぼんでいて、そのくぼんだ分が目の下に移動していることが多く、目の下のふくらみを押すとまぶたのくぼみが改善するのは、脂肪が移動しているからです。 この脂肪の移動が起こる一番の原因は、目の下の張りがなくなることです。眼輪筋などの筋肉の衰えも関係しますが、一番の原因はやはり皮膚の張りの低下です。一度移動してしまった脂肪を、化粧品や運動、マッサージで元に戻すことは不可能です。 化粧品でできることは、これ以上ぷっくり感が進まないように保湿をする、張りを持たせるといったことです。以前、脂性のテープでぷっくり感をなくすという製品をご紹介しましたが、あれは肌に貼ることで圧をかけて一時的に戻しているだけです。続けることである程度抑える効果は期待できますが、外せば元に戻ってしまいますし、劇的なビフォーアフターのような改善は望めません。最近広告で見かける「貼るだけでぷっくりが治る」という美容液は、正直ほとんど意味がないと思いますので、試すのはあまりお勧めしません。 マッサージについてもよく質問をいただきます。脂肪を移動させたり、筋肉を鍛えて脂肪を抑えたりという発想は理解できますが、その部分だけを鍛えたりマッサージしたりするだけで脂肪を移動させることは、正直難しいと思います。むしろ、間違ったマッサージ、特に強くグリグリするようなものは、目の周りに炎症を起こし、その炎症によって張りがなくなったり皮膚が薄くなったりして、逆に目の下のぷっくりを悪化させることがあります。 マッサージには血流を良くする効果はありますが、筋トレのように筋肉を発達させて脂肪を抑え込むということは難しいです。目をぎゅっと開けるような眼輪筋のトレーニング程度なら良いのですが、目の周りを強くこすったり摩擦したりするようなケアは、たるみを悪化させる可能性があるので注意が必要です。 では、一度できてしまったぷっくりはどうすればよいのか。残念ながら、手術以外の方法はありません。「手術」と聞くと怖いと感じる方もいらっしゃると思いますが、最近はマスク生活の影響もあってか、目の下のぷっくりを気にしてご相談にいらっしゃる方が非常に増えています。 私たち美容皮膚科で行うのは、目の下の脱脂という施術です。下に落ちてきた眼窩脂肪を、少しメスを入れて取り除きます。私自身が執刀するわけではありませんが、表参道スキンクリニックには外科の先生がいらっしゃるので、クマや目の下のぷっくりが気になって来院された患者さんを紹介し、手術をしていただくことがあります。やはり他の方法をあれこれ試すよりも、手術のほうがすぐに改善します。もちろん取れる脂肪の量には個人差があるため、全員に手術が最適というわけではありませんが、悩みながら意味のないケアを続けるよりは、一度美容外科にご相談いただくのがよいと思います。 手術といっても小さくメスを入れる程度で、ダウンタイムは1週間前後です。脂肪を取る際に内出血が起こることもあります。 ただし、目の下のぷっくりを放置しておくと、皮膚がどんどん脂肪に押されて伸びてしまいます。そうなると、脱脂だけでなく皮膚を切って縫う必要が出てきます。間違ったケアを続けたり放置しすぎたりすると、脂肪だけでなく皮膚まで伸びきってしまうのです。サーマクールなどで引き締める機械もありますが、伸びきってしまった皮膚は切除して縫うしかなくなります。ですので、ぷっくりが気になり始めたら、早めに美容外科医に相談することをお勧めします。 もちろん、アイクリームや、張りを持たせるレチノール、ニールワン、ナイアシンアミド配合のスキンケアで目元をケアすることは、とても良いことです。ただし、これはあくまで予防、そして現状以上に進行させないためのものであることを理解し、過度な期待はせず、しっかりとケアをしていただき、それでも気になる場合は皮膚が伸びきってしまう前に美容皮膚科にご相談いただければと思います。 SNS上ではさまざまなことが言われていて、「結局クリニックに来させたいだけでしょう」と思う方もいらっしゃるかもしれません。もちろん来院していただけるのは嬉しいのですが、一番避けたいのは間違ったケアで悪化させてしまうことです。できないことはできない、できることはできる、ときちんとお伝えしたいと思っています。手術をすれば完全に改善できるということを理解した上で、それでも手術まではせずスキンケアで頑張りたいという方はそれでも良いと思いますが、「治る」と思い込んでケアを続けるのは避けていただきたいです。全員に手術をお勧めするわけではありませんが、正しい知識を持った上で、自分がどの選択をするかを決めていただきたいと思います。 もちろん、中には「これで改善しました」という体験談もあるかもしれません。ですが、それはあくまでその方個人の結果であり、全員に当てはまるとは限りません。気になる方は、まず目の下のぷっくりがなぜ起こるのか(脂肪が下に落ちてきていること)、そしてそれがなぜ起こるのか(皮膚の張りがなくなること)を理解し、自分のスキンケアでそうした原因を助長していないか見直してみてください。そして張りを保つケアを行い、それでも悪化するようであれば、美容皮膚科や美容外科にご相談ください。 目元に張りを持たせるスキンケアをする、手術前にサーマクールやハイフなどで目の下のぷっくりを軽減・進行を抑える、という美容皮膚科的なアプローチもあります。それでもどんどん気になっていくという場合は、美容外科での脱脂手術を検討していただくとよいと思います。 いかがでしたでしょうか。今日は耳が痛い、あまり聞きたくなかった真実かもしれませんが、正しい知識を持った上で、スキンケアだけで様子を見るのか、美容皮膚科的な施術をするのか、それとも根本治療として美容外科的な手術をするのか、この3段階を理解しておけば、自分に合った選択ができ、無駄なケアをせずに済むと思います。よかったら参考にしてみてください。 このチャンネルでは、このように美容や健康について正しい情報をお届けしていますので、よろしければチャンネル登録をお願いします。また、「この治療はどうなの」「このスキンケアはどうなの」「この化粧品を深掘りしてほしい」といったリクエストがあれば、ぜひコメント欄に書いていただけると嬉しいです。 では、また次の動画でお会いしましょう。さようなら。

Hello everyone, this is Tomonori Arata. Today I'd like to talk about how to properly improve under-eye sagging (bags under the eyes). I often get questions like, "How can I improve my under-eye sagging or puffiness?" People ask whether skincare products can help, or whether the facial exercises some YouTubers introduce to fix under-eye puffiness actually work. To cut to the conclusion first: medically speaking, there is no way to improve this condition through cosmetics alone or through massage alone. I'm sorry to disappoint you right from the start, but let me explain what under-eye puffiness actually is. If you picture a skull, you'll notice the eye sockets are open cavities. The eyeball sits inside, cushioned by a fat pad called orbital fat. When you're young, the skin and tissue around the eyes have enough firmness to hold this fat in place, so it doesn't bulge forward. In people with under-eye puffiness, if you press on the bulge, it actually goes back up into the hollow of the upper eyelid area. In other words, the firmness of the eyelid area has decreased, and the orbital fat has slid downward. People with under-eye puffiness often have a hollow upper eyelid, and that hollow volume has essentially relocated to below the eye. The fact that pressing on the under-eye bulge fills in the upper hollow shows that the fat has literally migrated. The main cause of this fat migration is loss of firmness in the skin around the eyes. Weakening of the orbicularis oculi muscle also plays a role, but the primary cause is decreased skin firmness. Once fat has migrated, it's impossible to move it back through cosmetics, exercise, or massage. What cosmetics can do is prevent further worsening of the puffiness through moisturizing and improving firmness. I previously introduced a product using an elastic tape to reduce puffiness — but that works simply by applying pressure to physically push things back temporarily. Continued use may provide some suppressive effect, but once removed, it returns to its original state, and you shouldn't expect a dramatic before-and-after transformation. As for the serums advertised recently that claim "just apply and puffiness disappears," I honestly think they're essentially meaningless, so I wouldn't recommend trying them. I also get a lot of questions about massage. I understand the idea behind trying to move the fat or strengthen the muscles to hold it in place, but realistically, training or massaging just that one area isn't enough to relocate fat. In fact, improper massage — especially aggressive rubbing — can cause inflammation around the eyes, which can further reduce firmness or thin the skin, making the puffiness worse rather than better. Massage can improve blood circulation, but it's unlikely to build up muscle like strength training would, to the point of holding back the fat. Something like gently opening your eyes wide to engage the orbicularis oculi muscle is fine, but vigorously rubbing or scrubbing the eye area could actually worsen sagging, so please be careful. So what can you do once the puffiness has already formed? Unfortunately, surgery is the only real option. I know the word "surgery" sounds scary, but perhaps due to increased mask-wearing, more and more people have been coming in concerned about under-eye puffiness lately. In cosmetic dermatology, the procedure we perform is called under-eye fat removal (transconjunctival fat removal). A small incision is made to remove the orbital fat that has sunk downward. I don't perform the surgery myself, but at Omotesando Skin Clinic, we have surgeons on staff, and I've referred several patients who came in concerned about dark circles or under-eye puffiness for this procedure. Compared to trying various other remedies, surgery tends to bring about improvement much faster. Of course, not everyone is an ideal candidate, as the amount of fat that can be removed varies by individual, but rather than continuing with ineffective treatments while worrying, I genuinely think it's worth consulting a cosmetic surgeon. Even though it's called "surgery," it only involves a small incision, and recovery time is typically about a week. Bruising can occur when the fat is removed. However, if you leave under-eye puffiness untreated, the skin continues to be pushed and stretched by the fat over time. Once that happens, it's no longer just a matter of removing fat — the excess skin will need to be surgically cut and stitched as well. If you continue improper care or leave it unaddressed for too long, not just the fat but the skin itself can become overstretched. While there are tightening devices like Thermage, severely stretched skin ultimately requires excision and suturing. So if you're starting to notice puffiness, I'd recommend consulting a cosmetic surgeon sooner rather than later. Of course, using eye creams or skincare containing retinol, DNA repair enzymes (Nyle-One), or niacinamide to maintain firmness around the eyes is genuinely beneficial. But please understand this is purely for prevention and to keep things from worsening — don't expect too much, take proper care of the area, and if it still concerns you, consult a cosmetic dermatologist or surgeon before the skin becomes overstretched. There's a lot of information floating around on social media, and some of you might think, "They just want to get people to come into the clinic." Of course, I'm happy when people visit, but what I really want to avoid is people making their condition worse through improper care. I want to clearly communicate what is and isn't possible. Understanding that surgery is the only way to completely resolve the issue, if you'd still like to try managing it with skincare without going as far as surgery, that's perfectly fine — just don't fall into the trap of believing it will actually "cure" the condition. I'm not saying everyone needs surgery, but I want people to have accurate knowledge so they can make their own informed choice. Of course, there may be testimonials out there saying, "This worked for me!" But that's simply one individual's experience and doesn't necessarily apply to everyone. If you're concerned, first understand why under-eye puffiness occurs (fat sinking downward) and why that happens (loss of skin firmness), then check whether your current skincare routine might be contributing to that cause. Try to maintain firmness through proper care, and if it still worsens, consult a cosmetic dermatologist or surgeon. There are also cosmetic dermatology approaches such as skincare to maintain firmness around the eyes, or treatments like Thermage or HIFU before surgery to reduce puffiness or slow its progression. If, despite all this, it continues to bother you more and more, then I'd suggest considering under-eye fat removal surgery with a cosmetic surgeon. So, what did you think?
