皮膚科医が自分のニキビをどう治療するか公開します。

みなさんこんにちは、ともり先生です。今日はですね、みなさんに残念なお知らせがありまして、なんと私、42歳にも関わらずニキビができてしまいました。 ちょうど今日は、尾野先生がいらっしゃいます。 尾野先生：こんにちは、尾野です。 ニキビを見てください。こんな大きいのが久しぶりにできたんですが、マスクの影響もあるかもしれませんね。今日はこれをどのように治していくか、美容皮膚科・一般皮膚科ではどのように治療するかをご紹介したいと思います。尾野先生に炭酸ガス（CO2）レーザーを当ててもらいますので、よろしくお願いします。 尾野先生：はい、では準備します。 結構大きいですよね。42歳でもニキビはできます。では、マスクを外して、消毒させていただきます。尾野先生、今日はどのような治療をしていただけますか？ 尾野先生：炭酸ガスレーザーで穴を開けて、中の膿を全部出します。黄色く膿がたまっているのがわかりますか？これを出して、治りを早くします。早く膿を出さないと、クレーター（ニキビ跡）になってしまいますので。 そうですよね。自分でニキビを潰してはいけない理由は何でしょうか？ 尾野先生：不潔な針で穴を開けたり、手で潰したりすると、穴が大きくなったり、感染を起こしてしまいます。ですので、必ずクリニックで処置を受けてください。自分で潰している方は、クレーターになってしまい、その後悩まれているケースが多いです。 クレーターを治す方が難しいですもんね。皆さん、朝起きてニキビを発見すると自分で潰したくなると思いますが、それは本当にやめてください。炎症物質が残ることで繰り返しできる原因にもなりますし、不潔な状態でつぶすと感染を起こしてクレーターになることが多いです。宣伝ではなく、本当にニキビは自分で潰さない方がいいです。 では、レーザーを当てていきます。目を保護しますね。消毒します。痛みは大丈夫ですか？ ちょっと熱いなという感じで、全然痛くないです。 尾野先生：では膿を出しますね。白いのがわかりますか？比較的浅めでしたが、しっかり膿が出ました。痛みは大丈夫ですか？ はい、全然大丈夫です。少し赤みはありますが、血は出ていません。 先生、この後はどのような治療をすればよいですか？ 尾野先生：雑菌がつかないよう、抗生物質の塗り薬をしっかり塗ってください。赤みは1〜2日で引いてくると思います。跡がなくなるまでは1週間ほどかかることがあります。 やはりニキビはすぐには治らず、時間をかけて治っていきます。できて2〜3日は赤みが続きます。一般の皮膚科では抗生剤の塗り薬を処方されることが多いですが、美容クリニックでは複数の抗生剤を混合したものを使うこともあります。 今回、マスクをつけていることが多かったため、蒸れてニキビができてしまったと思われます。代謝を上げたり角質を柔らかくするお薬も併用していきたいと思います。 --- 【翌日】 おはようございます。翌日になりました。ニキビがどうなっているかご報告します。眉毛は描いていますが、患部には何も塗っていません。赤みがはっきりありますが、コンシーラーで隠せば目立たなくなると思います。昨日、尾野先生にCO2レーザーで処置していただき、その後抗生剤の塗り薬を塗って一晩寝た状態がこちらです。 --- 【4日後】 CO2レーザーを当ててから4日が経ちました。かなり目立たなくなっています。盛り上がりはほとんどなく、少し赤みが残っている程度です。では、コンシーラーでどれくらい隠せるか試してみましょう。 使用するのは「IPSA ホワイトニング クリエイティブコンシーラー（部分用ファンデーション）」です。3色タイプのコンシーラーで、濃い色・中間・薄い色を混ぜて使います。 ニキビ部分を不潔にしたくないので、付属のブラシではなく使い捨てのチップを使っています。 まず濃い目の色を少量取り、ニキビの赤い部分に上からトントンと叩き込むように塗ります。周りとなじませることより、上から叩いて密着させることがポイントです。これだけで赤みがかなり目立たなくなります。 次に白い色を上からトントンと重ね、チップでなじませます。この「トントン」を何度か繰り返すことで自然になじんでいきます。 コンシーラーは引いた目で確認するのがポイントです。近くで見ると「まだ隠れていない」と深追いしてしまいがちですが、他人は腕を伸ばした程度の距離でしか顔を見ません。深追いしすぎないよう気をつけてください。 最後にパウダーファンデーションで周りをトントンとなじませれば完成です。近づいてもほとんどわかりません。引いて見ると全くわかりません。これがCO2レーザーを当てて4日目の状態です。 --- 私たち美容皮膚科医がニキビができたとき、ほとんどの場合CO2レーザーで圧出処置を行います。CO2レーザーの優れた点は、熱で穴を開けるため、熱によって組織が収縮し（創傷治癒）、通常の傷よりも治りが早いことです。また、レーザーの熱による止血効果で、炎症性物質が周囲に広がりにくく、炎症が悪化しにくいため、ニキビ跡（クレーター）になりにくいという利点もあります。 私自身、美容皮膚科医であってもニキビはできます。マスクによる蒸れや、もともとのニキビ体質により、顎周りにできやすい状態です。市販薬を塗ることも一つの選択肢ですが、今回は皮膚科医が実際にニキビができたときにどう対処するかをご紹介しました。 ニキビができたら、まずは皮膚科にご相談ください。すぐに受診できない場合は、抗生剤入りの塗り薬を使用するのも一つの方法です。ただし、自分で潰したり、圧出器具を使ったりすることは絶対にやめてください。 このチャンネルでは美容や健康についてわかりやすくお伝えしています。よかったらチャンネル登録をよろしくお願いします。それでは、また次の動画でお会いしましょう。さようなら。

Today, I'd like to show you how a dermatologist — and specifically a cosmetic dermatologist — treats a pimple like this. Dr. Ono is going to use a CO2 laser, so I'll be in good hands. Dr. Ono: Sure, let me get everything ready. It's quite big, isn't it? Pimples can happen at any age. Let me remove my mask. Dr. Ono, what will today's treatment involve? Dr. Ono: I'll use a CO2 laser to open the pimple and drain all the pus inside. You can see the yellowish pus here. By draining it, the healing process will be much faster. If we leave it too long, it could turn into a crater — a pimple scar. That's right. Why should people avoid popping pimples themselves? Dr. Ono: Using an unsterilized needle or squeezing with your fingers can cause the opening to enlarge and introduce bacteria, leading to infection. Please always have this done at a clinic. Many people who pop pimples at home end up developing craters and later seek treatment for those scars. And treating craters is much more difficult. Everyone is tempted to pop a pimple when they find one in the morning — but please resist that urge. The inflammatory substances inside can spread and cause recurring breakouts. Popping it yourself in an unclean way can cause infection and lead to scarring. I'm not just saying this to promote clinic visits — truly, please do not pop your pimples. Now, we'll apply the laser. Let me protect your eyes first, and then we'll disinfect the area. Are you okay with the pain? It just feels a little warm — no real pain at all. Dr. Ono: Now I'll drain the pus. Can you see the white material? It was relatively shallow, but we got all the pus out cleanly. How's the pain? No pain at all. There's a little redness but no bleeding. Dr. Ono, what should be done after this? Dr. Ono: Please apply an antibiotic ointment to prevent infection. The redness should subside within one to two days. Complete healing and fading may take about a week. Pimples don't heal overnight — they take time. The redness typically lasts two to three days. At a general dermatology clinic, you'd typically receive a simple antibiotic ointment, while cosmetic clinics may use a compound ointment with multiple antibiotics. Since I've been wearing a non-woven mask frequently lately, moisture build-up is likely the cause. I'll also use a medication to promote cell turnover and soften the skin around that area. --- 【The Next Day】 Good morning! Here's my update from the next day. My eyebrows are done since I'm filming, but I have nothing on the affected area. There's visible redness, but I believe it can be covered with concealer. This is what it looks like after Dr. Ono performed the CO2 laser treatment yesterday, followed by applying antibiotic ointment overnight. --- 【Four Days Later】 It's now been four days since the CO2 laser treatment. It's much less noticeable. There's almost no raised texture — just a faint redness remaining. Let me show you how well concealer can cover it. I'm using the IPSA Whitening Creative Concealer (spot foundation). It's a three-shade concealer palette with a deeper tone, medium tone, and lighter tone that you mix together. Since I want to keep the area clean, I'm using disposable applicator tips instead of the included brush. First, I take a small amount of the darker shade and gently pat it onto the red area using a tapping motion — not rubbing, just tapping from above. The key is to press it in rather than blend it outward. Even this step alone significantly reduces the visible redness. Next, I layer the lighter shade on top with the same tapping motion and gently blend with the tip. Repeating this "tap and blend" technique multiple times creates a natural, seamless finish. An important tip for applying concealer: always assess from a distance. When you look too closely, you'll keep thinking it's not covered enough and over-apply. In reality, people typically see your face from about arm's length away. Don't over-analyze from up close. Finally, blend the surrounding area with a powder foundation to finish. Even up close, you can barely notice anything. Viewed from a normal distance, it's completely invisible. This is four days post CO2 laser treatment. --- When cosmetic dermatologists develop a pimple, most of us use CO2 laser drainage. The key advantage of CO2 laser is that the heat not only makes the opening but also causes tissue contraction (wound healing response), which helps the area heal faster than a conventional puncture wound. Additionally, the laser's heat cauterizes the area, preventing inflammatory substances from spreading to surrounding tissue. This reduces the risk of worsening inflammation and the development of post-acne scarring (craters). Even as a cosmetic dermatologist, I still get pimples. Moisture trapped under masks combined with my acne-prone skin type means I tend to break out around the chin area. While over-the-counter topical treatments are a valid option, I wanted to share how a dermatologist actually handles a breakout. If you develop a pimple, please consult a dermatologist first. If you can't get to a clinic right away, applying an antibiotic topical cream is a reasonable short-term measure.
