みなさんこんにちは。内科・皮膚科医の友利新です。今日はマスクニキビについて、その原因と対策法、そしてレスキューアイテムについてを皆さんにシェアしていきたいと思いますので、最後までご覧ください。 私自身も思春期から20代前半ごろまでずっとニキビに悩んでいました。ニキビって青春のシンボルや若い人のイメージが多いのですが、マスクをつける時代になってから、マスクをつけている部分にニキビが増えたという患者さんがかなり増えています。もちろん思春期だけでなく、30代・40代、私自身もできることがあります。なぜマスクをするとニキビができるのかという原因を知れば対処法がわかりますし、対処法に合わせたスキンケアをすることで、なるべくニキビができないような習慣が作れると思い、皆さんにお話しします。 ただし、一つ注意点があります。ニキビは「ざ瘡(疾患)」という慢性疾患、つまり病気です。スキンケアだけで改善・完治するものではありません。マスクニキビでも思春期ニキビでも大人ニキビでも、ニキビはニキビ。必ず皮膚科で治療を受けてください。保険治療でも自費治療でも構いません。まず皮膚科を受診すること、そしてプラスアルファで毎日のスキンケアでニキビができにくい肌を目指すことがポイントです。 ■ なぜマスクをするとニキビが増えるのか ニキビの原因はざっくり言うと、毛穴の中に皮脂が溜まり、そこにニキビ菌が集まって炎症が起きることです。マスクはこのプロセスにどのように関係しているのでしょうか。 【原因①:皮脂分泌の増加】 マスクをしているとマスク内の皮膚温度が上がります。どんなに通気性の良いマスクをつけていても、マスク内の温度は上昇します。体温が1度上がると皮脂分泌量が10〜15%増えることが分かっています。もともとニキビ肌で皮脂分泌が多い方はさらに増加し、年齢とともに皮脂分泌が落ち着いていた方も再び過剰になることがあります。 また、皮脂分泌が増えると皮膚の常在菌バランスにも影響します。私たちの肌には腸内細菌のように「皮膚常在菌」がいて、ニキビの原因となるアクネ菌なども含まれています。皮脂分泌が増えるとアクネ菌がその皮脂を栄養にして増殖し、善玉菌(いわゆる美肌菌)とのバランスが崩れ、ニキビが増えやすくなります。 【原因②:水分量の低下(インナードライ)】 マスクをつけて話すと、呼気に含まれる水蒸気がマスク内にこもり、皮膚がふやけた状態になります。ふやけると皮膚のバリア機能が低下し、内側の水分が蒸発して乾燥が起こります。乾燥した肌は皮脂を余分に分泌して水分の蒸発を防ごうとするため、「乾燥しているのにベタつく」インナードライ状態に陥りやすくなります。 【原因③:摩擦による炎症】 マスクが肌に擦れることで炎症が起きます。肌は刺激から守ろうと角質を厚くしますが、角質が厚くなると毛穴の出口が塞がれやすくなり、皮脂が詰まってニキビができやすくなります。 これらの原因から、マスクニキビの対策としては次の3点が重要です。 ①皮脂分泌をコントロールする ②インナードライを解消するよう、油分ではなく水分を補う保湿をする ③摩擦をなるべく減らす ■ スキンケアの基本:ノンコメドジェニック処方 ニキビ肌のスキンケアには「ノンコメドジェニック処方(ニキビのもとになりにくい処方)」の製品を選ぶことが基本です。特に保湿剤は肌に残るため、ノンコメドジェニック処方のものを使用してください。 ■ おすすめアイテム 【1】ONE BY KOSÉ バランシングチューナー(薬用皮脂分泌抑制化粧水) 医薬部外品で、有効成分「ライスパワーNo.6」を配合。お米を発酵させた独自成分が皮脂栓に働きかけ、皮脂分泌が多い方には抑制し、少ない方には必要以上に抑えないよう調整してくれます。ビタミンC誘導体も配合されており、皮脂分泌の調整効果が期待できます。ノンコメドジェニック処方済み。テカリが気になる方、オイリー肌の方に向いています。 【2】キュレル 皮脂トラブルケア 保湿ジェル 医薬部外品で、有効成分「アラントイン」が抗炎症作用を発揮し、マスクの摩擦による炎症も抑えてくれます。キュレルならではのセラミドがインナードライを改善し、余分な皮脂分泌を抑制します。ノンコメドジェニック処方済み。皮脂は気になるが乾燥もある、インナードライ傾向の方に特におすすめです。ONE BY KOSÉがテカリ・オイリー肌向けとすれば、こちらは皮脂+乾燥の混合肌向けです。 【3】ETVOS バランシングVCクリアスポッツ(ジェル状美容液) 部分使いに適した美容液です。注目成分は「アゼライン酸誘導体」。アゼライン酸は海外ではニキビ治療薬として使用されており、皮脂分泌抑制・抗炎症効果が期待できます。日本では医薬品として未承認のためクリニック専売品が多いですが、このETVOSのアイテムはオンラインでも購入可能です。ビタミンC誘導体も配合されており、ニキビケアと美白ケアを同時に行いたい方にも向いています。ノンコメドジェニック処方済み。 ■ まとめ マスクニキビの原因は、①皮膚温上昇による皮脂分泌増加、②インナードライ、③摩擦による炎症の3つです。スキンケアでできる対策としては、皮脂のコントロール、水分補給を重視した保湿、抗炎症成分入りの製品の活用、そして摩擦レスな洗顔(泡洗顔やクレンジングオイルの活用など)を心がけましょう。 ニキビは1つできるだけで憂鬱になるものです。まずは皮膚科で治療を受け、日々のスキンケアで予防を心がけてください。

Today, I'd like to share information about mask-related acne (maskne): its causes, how to deal with it, and some recommended skincare products. Please watch until the end. I myself struggled with acne from my teenage years through my early twenties. Acne is often associated with youth and adolescence, but ever since mask-wearing became common, many of my patients have been reporting breakouts specifically in the areas covered by their masks — and this isn't limited to teenagers. It affects people in their 30s and 40s, myself included. Once you understand why masks cause acne, you can take the right steps, and with the right skincare routine, you can minimize breakouts. One important note: acne — including maskne — is a chronic skin condition known medically as acne vulgaris. It cannot be fully treated with skincare products alone. Always consult a dermatologist for proper treatment, whether through national health insurance or private care. Think of skincare as a complement to medical treatment, not a replacement. ■ Why Does Wearing a Mask Cause Acne? Acne is broadly caused by excess sebum accumulating in pores, where acne-causing bacteria (C. acnes) proliferate and trigger inflammation. Here's how masks contribute to this process: [Cause 1: Increased Sebum Production] Wearing a mask raises the skin temperature inside it. Even with breathable masks, the temperature increases. Research shows that a 1°C rise in body temperature increases sebum production by 10–15%. This means that people who already have oily or acne-prone skin will produce even more sebum, and even those who had outgrown teenage acne may find it returning. Excessive sebum also disrupts the skin microbiome. Just as our gut has beneficial and harmful bacteria, our skin has its own ecosystem of microorganisms — including C. acnes. When sebum increases, C. acnes (which feeds on sebum) multiplies, throwing the balance off and promoting acne formation. [Cause 2: Moisture Loss (Inner Dryness)] Breathing and talking into a mask causes water vapor to accumulate on the skin. This initially makes the skin feel damp, but it actually weakens the skin barrier, allowing internal moisture to evaporate. When the skin becomes dry, it compensates by producing more sebum — resulting in skin that is simultaneously dry and oily, a condition known as "inner dryness" or combination-dry skin. [Cause 3: Friction-Induced Inflammation] The constant rubbing of a mask against the skin causes inflammation. The skin responds by thickening the stratum corneum (outer skin layer) to protect itself. However, thicker skin can block pore openings, trapping sebum and leading to breakouts. Based on these three causes, the key strategies for preventing maskne are: ① Control sebum production ② Hydrate with water-based (not oil-based) moisturizers to address inner dryness ③ Minimize friction in your skincare routine ■ The Basics: Non-Comedogenic Formulas When choosing skincare products for acne-prone skin, always look for "non-comedogenic" formulas — products formulated to not clog pores.
