目のクマの種類や治療法について解説します。

皆さんこんにちは。内科・皮膚科医の友利新です。今日は皆さんが悩んでいることが多い、目の下のクマについてお話しします。クマについて悩んでいる方は本当に多いですよね。美容皮膚科での相談でも上位に来る悩みです。ただ、クマといっても原因はさまざまで、原因によって治療法も変わります。スキンケアで対応できるものもあれば、クリニックでの治療、場合によっては手術が必要なものもあります。今日はクマの種類・原因・対処法についてお話しします。
クマはよく「茶グマ」「青グマ」「黒グマ」と分類されます。それぞれ原因によって色が異なります。
■茶グマ
茶グマには主に2つの原因が考えられます。
1つ目は炎症性の色素沈着です。摩擦や紫外線による炎症が目の下に起こることで茶色っぽく見えます。アイメイクをする際や、クレンジングの際に目の周りをゴシゴシこすることが原因になりやすいです。目の下の皮膚は非常に薄いため、日々の細かな摩擦でも炎症が起きてしまいます。また、花粉症やアトピー性皮膚炎で目をこする習慣がある方も茶グマになりやすいです。
茶グマはスキンケアで改善しやすいクマです。まず摩擦を減らすこと、つまりクレンジング方法を見直したり、花粉症やアトピー性皮膚炎をきちんと治療して目の周りを触らないようにすることが重要です。また、炎症性色素沈着はメラニン色素が活発になっているため、美白効果のあるアイクリームを使用することで改善が期待できます。
■黒グマ
黒グマの中には、「後天性メラノサイトーシス」というアザの一種が含まれている場合があります。これは皮膚の奥のメラニン色素が活性化したもので、表面から見ると黒っぽく、または濃い茶色に見えます。茶グマとの違いは色の深さや分布で、鼻の脇にも色素が現れることがあります。ただし自己判断は難しいため、スキンケアを続けても改善しない場合は皮膚科への相談をおすすめします。後天性メラノサイトーシスにはレーザー治療が有効です。
■青グマ
目の周りの皮膚は非常に薄いため、皮下の血管が透けて青っぽく見えることがあります。血流が悪くなると静脈血が滞り、青グマがより目立ちます。対策としては、血行促進効果のあるアイクリームの使用や、温熱アイマスクなどで目の周りの血流を改善することが効果的です。
■番外編:目の下のふくらみによるクマ
色素の問題ではなく、目の下がぽこっと膨らんでいることでクマのように見えるタイプもあります。これは「眼窩脂肪」が皮膚の弛みによって前に出てきたことで生じます。目の下のふくらみを押すと、まぶたのくぼみが改善するように見える場合は、このタイプの可能性があります。このタイプはスキンケアやレーザーでは対応が難しく、外科的に脂肪を取り除く手術が必要になることがあります。広告などで見かける貼るだけのケアグッズは効果が期待できませんので、気になる方はクリニックへの相談をおすすめします。遺伝的に目の下の脂肪が出やすい方は、早めにアイクリームで予防することが大切です。

Hello everyone! I'm Arata Tomori, an internal medicine and dermatologist. Today, I'd like to talk about dark circles under the eyes—a concern that many people struggle with. It's one of the most common topics that comes up in cosmetic dermatology consultations. The important thing to understand is that dark circles can have various causes, and the treatment approach differs depending on what's causing them. Some cases can be managed with skincare, while others require professional treatment or even surgery. Today, I'll explain the different types of dark circles, their causes, and how to address them.
Dark circles are commonly categorized into three types: brown circles, black circles, and blue circles. Each type has different underlying causes.
■ Brown Dark Circles
There are two main causes of brown dark circles.
The first is post-inflammatory hyperpigmentation. Inflammation caused by friction or UV exposure around the eyes can leave behind brownish discoloration. Common culprits include rubbing the eye area vigorously during makeup application or makeup removal. The skin under the eyes is extremely thin, so even minor daily friction can trigger inflammation. People who rub their eyes frequently due to hay fever or atopic dermatitis are also prone to this type.
Brown dark circles are the most manageable type and can often be improved with skincare. The key first step is to reduce friction—review your cleansing routine and treat any underlying conditions like allergies or eczema to avoid touching the eye area. Since post-inflammatory hyperpigmentation involves activated melanin production, using a brightening eye cream with whitening ingredients can also help improve the appearance.
■ Black Dark Circles
Some cases of black dark circles may be caused by a condition called acquired dermal melanocytosis, a type of birthmark-like pigmentation that occurs deeper in the skin. Because the pigment sits deeper than typical spots, it appears darker—either black or a deep brownish color. It may also appear on the sides of the nose. It can be difficult to distinguish this from brown circles on your own, so if your dark circles don't improve despite consistent skincare efforts, it's worth consulting a dermatologist. Laser treatment is an effective option for acquired dermal melanocytosis.
■ Blue Dark Circles
Because the skin around the eyes is very thin, blood vessels beneath the surface can show through, giving the area a bluish tint. Poor circulation causes venous blood to pool in the area, making blue dark circles more prominent. Using an eye cream with circulation-boosting ingredients and applying warm eye masks to improve blood flow in the area can be helpful.
■ Bonus: Puffiness-Induced Dark Circles
This type isn't caused by pigmentation, but rather by puffiness under the eyes that creates a shadow, making the area look dark. The cause is orbital fat—the fat that cushions the eyeball—pushing forward as the surrounding skin weakens. A telltale sign is that pressing on the puffiness temporarily makes the hollowness around the eyelid appear to improve. This type cannot be corrected with skincare or laser treatments. In many cases, surgical removal of the fat is required. Products advertised as patches or topical solutions for this issue are generally ineffective. If this is a concern, consulting a clinic directly is recommended. Those with a family history of under-eye puffiness should start using eye cream early as a preventive measure.
In summary, dark circles range from those that can be improved with skincare, to those that require clinical treatment, to those that need surgery. The foundation is daily prevention and care using appropriate products such as eye creams.
