皆さんこんにちは。内科・皮膚科医の友利新です。今日は前回の毛穴ケアについてのお話の続きです。前回は詰まり毛穴とたるみ毛穴についてお話ししました。皮脂分泌が過剰になって毛穴に皮脂がたまる「詰まり毛穴」、そして加齢によって皮膚がたるんでしまう「たるみ毛穴」についてお話ししたのですが、実はもう一つ、もっと怖い毛穴があるんです。なぜ怖いかというと、この毛穴は一度起こしてしまうとなかなか改善が難しくなるからです。ですから予防とケアが非常に大切です。今日はその毛穴についてしっかりと説明しますので、最後までご覧ください。
一度なると怖い毛穴の正体、それが「すり鉢毛穴」です。すり鉢毛穴とは何かというと、皮膚の表面が削られた状態の毛穴のことです。
なぜすり鉢毛穴になるのかというと、まず毛穴が目立つ・開いていく原因として、皮脂の分泌が活発になり、毛穴の中に皮脂がたまります。通常であればその皮脂は表面に出ていくのですが、余分な角質や汚れがたまって蓋をするような状態になります。これがいわゆる「黒ずみ毛穴」です。これがケアされないまま放置されるとどうなるかというと、本来皮脂は毛穴から出て肌全体をコーティングし、バリア機能を担っています。肌は皮脂をたくさん分泌しているのに肌をコーティングするものがない状態を察知して、「もっと出さなければ」と働くと同時に、詰まった塊を分解しようと分解酵素を分泌します。この分解酵素によってたまった皮脂が分解され、「遊離脂肪酸」が生成されます。この遊離脂肪酸が毛穴の出口付近に集まると、毛穴周囲の皮膚を溶かしてしまうのです。その結果、通常はまっすぐな毛穴が、すり鉢のように広がった形になってしまいます。つまり、皮膚が溶けて削れ、へこんでしまうのです。
このすり鉢毛穴は、一度なってしまうとスキンケアだけで改善するのは非常に難しく、美容医療に頼ることになってしまいます。ですから、予防とケアが非常に重要です。
では、どのような予防が必要なのかをお話しします。ポイントは3つです。
1つ目は、毛穴の詰まりを角栓としてガチガチに固めないことです。角栓が硬く固まってしまうと、皮脂が全体に行き渡らなくなり、それを溶かそうとする働きが起こり、すり鉢毛穴につながります。ですから、酵素洗顔や角質ケア商品を使って角栓を少しずつ溶かし、皮脂が皮膚全体に行き渡るようにしてあげることが大切です。
2つ目は、炎症を抑える成分を取り入れることです。遊離脂肪酸が発生すると肌に炎症が起こり、それがすり鉢状の毛穴につながります。この炎症を抑える成分としては、ビタミンCやグリチルリチン酸などが挙げられます。そういった炎症を抑える成分を含む美容液を取り入れることをおすすめします。
3つ目は、日焼けと摩擦を避けることです。遊離脂肪酸による炎症に加えて、摩擦や紫外線による炎症が重なると、すり鉢毛穴の進行が加速してしまいます。毛穴が気になるからといって触ることは絶対に避けてください。また、毛穴が詰まりそうだからといって日焼け止めを塗らないのもNG行為です。注意してみてください。

Hello everyone! I'm Toa, a physician specializing in internal medicine and dermatology. Today I'd like to continue our previous discussion on pore care. Last time, I talked about clogged pores and sagging pores — clogged pores caused by excess sebum accumulating in the pores, and sagging pores caused by skin losing its firmness with age. However, there is actually one more type of pore that is even more concerning. The reason it's so worrying is that once it develops, it becomes very difficult to improve. That's why prevention and care are extremely important. Please watch until the end as I explain this type of pore in detail.
The pore type that is so hard to reverse is called the "mortar pore" (suribachi-ke ana). A mortar pore is essentially a pore where the surrounding skin surface has been worn away, creating a crater-like depression.
Here's how mortar pores develop: First, as sebum production becomes more active, sebum accumulates inside the pores. Under normal circumstances, this sebum would travel to the surface, but excess dead skin cells and impurities pile up and act like a lid, trapping the sebum inside. This is what we commonly call "blackhead pores." If left without proper care, problems begin to arise. Sebum is supposed to coat the entire skin surface and maintain the skin's barrier function. When the skin senses that sebum is being produced but isn't reaching the surface, it responds by producing even more sebum — and also by releasing digestive enzymes to break down the blockage. These enzymes break down the accumulated sebum into substances called free fatty acids. When these free fatty acids gather near the pore opening, they begin to dissolve the surrounding skin tissue. As a result, what was once a straight, normal pore gradually widens into a mortar or funnel shape. In other words, the skin is literally dissolved and worn away, leaving a sunken depression.
Once mortar pores develop, they are extremely difficult to improve with skincare products alone, and professional cosmetic treatments are often required. This is precisely why prevention and care are so critical.
So, what can you do to prevent mortar pores? There are three key points.
The first is to prevent sebum plugs from hardening into rigid comedones inside your pores. When plugs harden, sebum can no longer flow freely across the skin, triggering the enzyme response that leads to mortar pores. To prevent this, use enzyme cleansers or exfoliating products to gradually dissolve plugs and allow sebum to circulate properly across the skin.
The second is to incorporate anti-inflammatory ingredients into your skincare routine. Free fatty acids trigger inflammation in the skin, which contributes to the formation of mortar pores. Ingredients that help suppress this inflammation include Vitamin C and glycyrrhizic acid (also known as glycyrrhizin). Look for serums containing these anti-inflammatory ingredients.
The third — and perhaps the most surprising — is to avoid sun exposure and friction. When inflammation caused by free fatty acids is compounded by additional inflammation from UV rays and physical friction, the progression of mortar pores is accelerated. Resist the urge to touch or squeeze your pores, even when they bother you — this is an absolute must-avoid. Additionally, skipping sunscreen because you're worried about clogging your pores is also a harmful habit. Please be mindful of these behaviors.
